3D analysis of soft tissue around implant after flap folding suture (Flap folding suture를 활용한 판막의 고정에 따른 임플란트 주변 연조직 3차원 부피 변화 관찰)

  • Purpose: The various suture techniques can be utilized in order to maximize the keratinized tissue healing around dental implants. The aim of this study is to compare the soft tissue healing pattern between two different suture techniques after implant placement. Materials and Methods: 15 patients with 18 implants were enrolled in this study. Simple implant placement without any additional bone graft was performed. Two different suture techniques were used to tug in the mobilized flap near the healing abutment after paramarginal flap design. Digital intraoral scan was performed at baseline, post-operation, stitch out, and 3 months after operation. The scan data were aligned using multiple points such as cusp, fossa of adjacent teeth, and/or healing abutment. After subtracting scan data at baseline with other time-point results, closed space indicating volume increment of peri-implant mucosa was selected. The volume of the close space was measured in mm3. The volume between two suture techniques at three time-points was compared using nonparametric rank-based analysis. Results: Healing was uneventful in both groups. Both suture technique groups showed increased soft tissue volume immediately after surgery. The amount of volume increment significantly decreased after 3 months (P < 0.001). Flap folding suture group showed higher median of volume increment than interrupted suture group after 3 months without any statistical significance (P > 0.05). Conclusion: After paramarginal flap reflection, the raised flaps stabilized by flap folding suture showed relatively higher volume maintenance after 3-month healing period. However, further studies are warranted.

Discussion of Neurologic Factor Influencing on Chewing Ability of Implant (임플란트 저작능에 영향을 주는 신경학적 원인에 대한 고찰)

  • Many researches have been published about the numerous factors related to the chewing ability of implant prosthesis. Most respective studies have concluded that the chewing ability of implant prosthesis is mostly fine compared to other type of prosthesis. However, some patients are not satisfied with their chewing ability of implant prosthesis. Therefore the neurologic factor, one of the factors related to dissatisfaction of chewing ability was reviewed in this study to understand the mechanism of action of mastication. Data was searched using the keywords; 'implant chewing ability, masticatory ability' in Pubmed database and reviewed. Definitions of chewing ability, factors of chewing ability are reviewed and the neurologic factor, one of the factors influencing on chewing ability, is reviewed. Mechanoreceptor of Periodontal ligament(PDL) is providing the mastication information to brainstem. Due to the absence of mechanoreceptions of PDL in implant, masticatory ability is decreased especially when chewing hard food. Masticatory muscles and mechanoreceptor in TMD may compensate the lack of mechanoreceptor of PDL in implants. Furthermore sensitivity of nerve fiber around peri-implant tissues may support the mechanoreception and sensory reaction in the implant mastication. However, further studies should be conducted to prove the relationships between neurologic factors and mastication.

Change in Periodontal Status by Professional Toothbrushing, and Patients' Satisfaction (전문가잇솔질에 의한 치주상태 변화와 환자만족도)

  • The purpose of this study was to examine the effects of the toothpick method, one of professional toothbrushing methods, on the prevention of periodontal diseases and the satisfaction level of patients with that. The subjects in this study were 33 patients who included 16 men and 17 women. After the toothpick method was applied to the selected patients from January 9 to February 28, 2008, the collected data were analyzed with SPSS (Statistical Package for the Social Science) 12.0 program. The findings of the study were as follows: First, the patients investigated showed a 0.71 reduction in PI than in the past. The index of bleeding upon probing stood at 5.12, which dropped from 8.09 in the beginning. Second, the patients gave a mean of 4.20 to the professional toothbrushing out of possible five points, which showed that they expressed satisfaction with that. Third, as for the relationship between general characteristics and satisfaction level, the women were more satisfied with that ($4.25{\pm}0.33$). By age group, those who were in their 60s and up found that more satisfactory ($4.31{\pm}0.30$). Fourth, as to changes in oral symptoms by gender, the largest number of the men and women considered their gums to become healthier after the professional toothbrushing was applied. The above-mentioned findings suggested that dental institutions should be equipped with well-educated oral health care personnels who are responsible for the periodontal health of patients, and the development of oral health promotion programs is urgently required as well.

Evaluation of Periodontal Status according to Sociodemographic and Health Behavior Characteristics (인구사회학적 특성 및 건강행위 특성에 따른 치주상태 평가)

  • This study was carried out in order to offer basic data available for being helpful to promoting periodontal health, by analyzing effects in socio-demographical and health-behavior characteristics on index of evaluating periodontal status. The study conducted a survey for the 151 adults aged 30 to 59 from October. 1, 2006 to March. 30, in 2007. The collected data were analyzed with t-test and one-way of variance using SPSS 12.0. Then, the following result were obtain. 1. sociodemographic and health-behavior characteristics is not related to O'Leary plaque index, Loe & Silness gingival index. 2. The number of sextants possessing periodontal pocket was higher in men(3.34) than women(2.43), also statistically significant difference in the occupational fields. Smoker(3.38) was higher than non-smoker(2.72), and it was higher over 3 times a week than almost no drinking as for frequence alcohol intake. 3. The amount of loss for inter-dental alveolar crest was higher in men(71.79) than women(42.24), was highest of the 'professional' in the occupational fields, and smoker(67.23) was higher than non-smoker(48.22). 4. There was significantly difference in the number of missing tooth between age and frequence of alcohol intake. These results that occupational fields, smoking and alcohol intake were related to the number of sextants possessing periodontal pocket and amount of loss for inter-dental alveolar crest.

  • The multifaceted role of chronic inflammation in multistep carcinogenesis has been extensively investigated and well documented. Periodontal diseases are associated with multifactorial agents, including bacterial endotoxins and the generation of an inflammatory response, indicating that poor oral health is associated with a variety of systemic diseases. The association between poor oral health, chronic inflammation, smoking, and increased alcohol consumption as risk factors for tumorogenesis is well established. More recently, associations between oral health and tooth loss and gastric, lung, and pancreatic cancers have been explored, with some studies pointing to smoking and oral health as a common link with an increased risk for malignant disease. In addition, epidemiological studies consistently indicate increased risks of various cancers with periodontal disease or poor oral condition caused by oral bacteria, which may activate alcohol- and smoking-related carcinogens locally or act through chronic inflammation. Appropriate oral care is vital in preventing cancer, as well as many other diseases. Thus, research on the correlation between oral care and periodontal inflammation and cancer is required. This review highlights the association between oral health and the risk of certain malignancies, such as periodontal disease-associated chemoprevention of inflammation" in this sentence.

A Study of Oral Health Status in Handicapped Students Comparing with Normal Students (장애학생과 일반학생의 구강건강상태 조사연구)

  • This study was carried out to evaluate the status of oral health in physically or mentally handicapped students. The status of oral health in 597 handicapped students of six special education institutes and 731 normal students in Chonbuk province were inspected by a dental hygienist from August to November of 1999. 1) The DT index(Decayed Teeth index) was significantly higher in handicapped students comparing with those of normal students. The proportions of children having gingivitis and malocclusion were also significantly higher in handicapped group (p<0.01). 2) The incidences of dental caries, gingivitis and malocclusion were significantly higher in mentally retarded students comparing with those of physical or sensory impairment groups(p<0.01). 3) Handicapped students participating in the regular school teeth-control program showed significantly low incidences of gingivitis and malocclusion comparing with non-participants. However, dental caries had no relationships with teeth control program. There were no differences between commuting and domitory-resident students in the incidences of dental caries, gingivitis and malocclusion.

  • Patients who have a moderate periodontitis with pathologic tooth migration of maxillary incisors, it is necessary not only periodontal treatment for reduce periodontal inflammation, but also orthodontic treatment to teeth repositioning. For orthodontic treatment, it is necessary to apply less force and careful considerations of the center of resistance of the tooth and optimal force of tooth movement. At this time, the segmental arch applied only to the target teeth, is more effective and predictable, because applied force and direction can be controlled. In addition, to design the orthodontic appliance that can prevent the unwanted tooth movement that used as an anchorage is important. In recent years, various types of skeletal anchorage system have been used for preventing loss of the anchorage. We reported the patient who had extruded maxillary central incisor due to pathologic tooth migration, treated by a successful periodontal-orthodontic multidisciplinary treatment using an orthodontic appliance designed to apply less traumatic force and reduce an anchorage loss.

Study on the relevance of dietary behavior and oral management by periodontal assessment and survey (치주검사와 설문조사를 통한 성인의 구강건강관리행태 및 식이행태와 치주질환과의 관련성)

  • Objectives: This study investigated the relevance of dietary behavior and oral management for oral heath in adults to extend the recognition of the importance of food intake in oral health and to provide basic information for oral health management. Methods: A survey and clinical assessment were conducted for 119 adults over 20 years of age. The chi-square test and logistic regression analysis were carried out using SPSS Statistics 20.0. Results: Gingivitis was influenced less when ham and sausage in the sub-area of meat, fish, egg, and bean were consumed once a week rather than the cases when consumed rarely (3.47 times) or when consumed two times or more a week (7.87 times) (p<0.05). Periodontitis was influenced more in the groups of 30s and 40s age than in that of over 50 years of age (14.97 times, 7.73 times) and in the group without drinking experience than in group with that (4.17 times) (p<0.05). Conclusions: The results verified the close relationship between food intake frequency and oral health in patients with periodontitis and gingivitis. Thus, it would be necessary to study the relationship between dietary behavior and oral health continuously.

  • 우리는 한국동굴학회의 요청과 사업계획에 따라, 동북-중앙아시아지역 동굴과학정보망 구축 사업에 동의하고 이를 기원하는 뜻으로 중국 길림성 연변조선족자치주 화룡시 근교의 충천계곡(골)자연동굴을 탐사하였다. 이를 위하여 1년 전 어느 날 화룡시 충천 골에 있는 동굴을 찾아 연길에서 차를 타고 장인촌으로 간 적이 있었다. 그곳에 가서 수소문하여 동굴에 대해 잘 알고 계신다는 김창현 노인을 찾았다. 당시 우리가 그 노인을 찾았을 때는 이미 점심때가 다 되었다. 우리가 동굴을 찾아가 보려고 김창현 노인을 찾아왔다고 하자, 그 노인은 동굴이 이곳에서 멀리 떨어져 있어서 아침 일찍 떠나야 하기에 오늘은 갈 수 없다고 하였다. 그리하여 우리는 김 노인으로부터 동굴에 대한 이야기만 좀 듣고 돌아와서 후에 약속한 날에 다시 탐사방문을 추진하기로 결정하였고, 바로 이때부터 우리는 국제교류와 협력을 통하여 사실상 국제동굴과학정보망 구축 사업에 동참을 시작한 셈이 되었으며, 이를 적극 지지한다.

A multidisciplinary approach to restore crown-root fractured maxillary central incisors: orthodontic extrusion and surgical extrusion (치관-치근 파절이 발생한 상악 중절치를 수복하기 위한 다각적 접근법: 교정적 정출술과 외과적 정출술)

  • To restore a tooth with a fracture line extending below the marginal bone level, a surgical crown lengthening procedure accompanied by ostectomy could be considered to expose the fracture line and reestablish the biologic width. However, this procedure could lead to esthetic failure, especially in the anterior teeth. Therefore, orthodontic extrusion, which elevates the fracture line from within the alveolar socket without sacrificing the supporting bone and gingiva, is recommended. This technique allows for the proper placement of the crown on a sound tooth structure, with the reestablishment of the biologic width. Alternatively, surgical extrusion is an one-step procedure that is simpler and less time-consuming than orthodontic extrusion; placing and adjusting the orthodontic appliance does not require multiple visits. This study presents successful restoration in 2 cases with a crown-tooth root fracture of the maxillary central incisor treated using a multidisciplinary approach through orthodontic extrusion or surgical extrusion followed by successful restoration.
