Utilization of digital technology in fabricating mandibular implant overdenture for skeletal class II edentulous patient: A case report (2급 악간관계를 보이는 하악 무치악 환자에서 디지털 진단기술을 이용한 임플란트 피개의치 수복증례)

  • Unlike class I patients, skeletal class II patients have unstable occlusion thus leading to instability of mandibular complete denture. Therefore, mandibular implant overdenture has been the standard of care due to its advantages in stability and retention. The types of attachments can be divided into two categories: solitary and bar type. The indications vary between two categories. In this clinical report, digital technology was utilized from the implant planning to the choice of appropriate attachment. Implants were placed at the desired location as previously planned in terms of angle and depth. Maxillary removable partial denture and mandibular implant overdenture are expected to have fair prognosis.

  • Traditional removable partial denture is a successful treatment for partial edentulous patients. When the abutment is formed with unilateral minority teeth, satisfaction with the use of removable partial denture may be lowered due to patient's discomfort and damage of the abutment. Recently, it has been reported that the unilateral posterior extension partial denture is used as a bilateral posterior extension partial denture using implant fixed prostheses as abutments. In this case, by using implant surveyed crown prostheses, bilateral posterior extension partial denture is fabricated in site that is predicted to have a poor prognosis. This resulted in improved support, maintenance, and stabilization of the removable partial denture, which were economically beneficial to patient and satisfied with use.

  • The use of implants could improve the support, retention, and stability of removable prosthetic restoration for fully edentulous patients with severe alveolar bone resorption. When the prosthesis is manufactured without accurate diagnosis and evaluation, this may lead to unfavorable treatment result. The patient in this case had the treatment of implant overdentures at a private dental clinic, but visited the hospital for retreatment due to soft tissue pain and decreasing retention of existing dentures. The vertical dimension was raised compared to the existing dentures, and overdentures were manufactured using milled bars and additional attachments. The clinical results were satisfactory in the aspect of improved oral hygiene maintenance, function and esthetics.

  • There are many problems in evaluating study results by p value in null hypothesis testing for dental research. It is a logical fallacy to conclude that the null hypothesis is true when the it is not rejected. There are much serious misunderstanding about p value, and researchers should be cautious about interpreting p value in writing papers. As alternatives to complement or replace the null hypothesis significance testing, effect size, confidence interval, and Bayesian statistics are introduced.

Changes in bisphenol A concentrations in urine according to sealant filling in children (치면열구전색 전후의 어린이 소변 내 Bisphenol-A의 농도 변화)

  • Objectives: The objectives of this study were to investigate the changes in bisphenol A (BPA) concentrations in urine from before to after sealant filling and the relationship between BPA and the number of teeth with sealed surfaces. Methods: Thirty-one children aged 6 and 7 years from three elementary schools in Daegu city who did not have any sealant and resin filling were selected as subjects. Urine samples were collected before and after sealant filling until 24 hours, with informed consent from their caregivers. The BPA concentration in all the collected urine samples was analyzed at Seegene Medical. Statistical analysis was performed using the Friedman test, Scheirer-Ray-Hope test, and the repeated-measures generalized linear mixed model of SPSS version 22.0. Results: The BPA concentrations increased from $3.49-{\mu}g/g$ creatinine before to $4.91-{\mu}g/g$ creatinine 2-3 hours later and to $4.15-{\mu}g/g$ creatinine after 24 hours. The more teeth with sealed surfaces, the higher the BPA concentration in children, but the difference was not statistically significant (P>0.05). Conclusions: The BPA concentrations were highest at 2-3 hours after sealant filling and decreased at 24 hours. Exposure to the sealant appears to have a meaningful correlation with the concentration of BPA in the urine of children.

Relationship between cancer and oral health in Korean adults determined using data from the 6th (2013-2014) Korea National Health and Nutritional Examination Survey (제 6기(2013-2014) 국민건강영양조사 자료를 활용한 한국성인의 구강건강과 암의 관련성)

  • Objectives: Dental caries and periodontitis are major oral bacterial infections associated with dental plaque. Infection and inflammation play a role in carcinogenesis, and a significant link has been found between some highly prevalent oral and dental diseases and some types of cancer. Therefore, the aim of this study was to evaluate the relationship between cancer and oral health among Korean adults. Methods: Data from the 6th Korea National Health and Nutrition Examination Survey were used to analyze the incidence of cancer according to oral health. Demographic and socioeconomic statuses and oral behavior of the participants was analyzed using complex chi-square tests. Logistic regression was used to analyze the relationship between a high DMFT index and cancer by calculating the 95% confidence intervals. Results: In total, 6,450 case-control subjects were included. Significant difference was observed in the DMFT index, but not in periodontal diseases, between the cases and controls. The odds ratio for cancer was 1.80 (95% confidence interval: 1.18 to 2.73) in the high-risk group. However, no significant difference was observed after adjusting for age, education level, and income. Conclusions: The results provide potential evidence of a significant association between cancer and oral disease.

  • 우리나라 치의학 역사의 산증인이며 구강외과학의 개척자인 이춘근 박사는 새벽 4시면 어김없이 일어나 집 가까이에 있는 교회에서 5시 새벽기도를 시작으로 하루의 일과를 연다. 올해 희수를 맞은 이박사는 1918년 평북 구성군 방현면 남시에서 태어났다. 이박사는 1983년 2월 말 정년퇴직후 한남동에 치과의원을 개원, 아침 10시부터 환자를 돌보고 있다. 서울대 명예교수로 요즘도 매 금요일 서울대 치대에 나가 대학원 학생들의 세미나를 주재한다.
