  • Purpose: This paper describes the basic principles and the feasibility of the field of dental diagnosis of optical coherence tomography (OCT) used in diverse field of medical diagnosis. Materials and methods: In this review, the research data of OCT were searched from PubMed, medical journal and related papers. Results: Generally, OCT is noninvasive and is possible to secure an excellent spatial resolution and real-time images of biological microstructure. Conclusion: This review discusses not only the basic principles of operation, types, advantages, disadvantages of OCT but also the future applications of OCT technology and their potential in the field of dental diagnosis.

Treatment of a tooth with severe periodontal involvement using intentional replantation: case report (중도 치주염에 이환된 치아의 보존을 위한 의도적 재식술을 통한 치료: 증례보고)

  • Although intentional replantation is frequently used as a treatment modality for endodontic problems, severe periodontal involvement has usually been regarded as a contraindication. However, there are some studies suggesting that intentional replantation could be a successful treatment alternative for periodontally involved teeth. This paper reports the treatment of a tooth with severe periodontal involvement using intentional replantation. The tooth, which had had root canal therapy due to endodontic-periodontal combined lesion but showed extensive bone loss, was gently extracted and replanted after thorough debridement of the root surface. By intentional replantation, a tooth with severe periodontal involvement in this case could be preserved, without extraction, over the course of a 3-year follow-up period.

The Effect of Occlusal Splint on the Masseteric Silent Period in Patients with TM disorders (교합안전장치가 측두하악장애 환자의 교근의 휴지기에 미치는 영향)

  • The purpose of this study was to evaluate the effect of occlusal splint on the electromyographic silent period duration of masseter muscle in patients with TM disorders. The author measured the Helkimo dysfunction index and masseteric silent period duration before and after 4 weeks splint therapy in 15 patients with TM disorders. The results were as follow: Before splint therapy, the mean durations of masseteric silent period in 15 patients with TM disorders were 50.7msec in left and 50.5msec in right. After 4 weeks splint therapy, the mean durations of masseteric silent period were 25.4msec in left and 26.5msec in right. In the correlation between Helkimo dysfunction index and masseteric silent period duration in before treatment, correlation coefficient is 0.675, p.<0.01. There is statistically significant correlation between Helkimo dysfunction index and masseteric silent period duration. Helkimo dysfunction index was significantly decreased after 4 weeks splint therapy (P<0.01) The masseteric silent period duration was significantly decreased after 4 weeks splint therapy (p<0.01).

  • Since its introduction to the dentistry, the CAD/CAM technology has been used over fifteen years. The application of CAD/CAM started from coping fabrication, expanded to the posterior full crown, and anterior restoration manufacture has also become widespread recently. While the development of machine was rapid, the advance of zirconia block was the fastest. Although some differences exist among individuals, the various natural tooth color can be expressed through the zirconia coloring itself without porcelain build up. And the texture characteristics can be reproduced just by CAD design. In this article, the development history of digital dentistry was summarized.

Pain Occurrence after Partial Pulpotomy by using Endocem MTA and ProRoot MTA: a Clinical Study (Endocem MTA와 ProRoot MTA를 이용한 부분치수절단술 후 통증 발생에 관한 임상 연구)

  • Objectives: This study aimed to compare the postoperative pain and clinical performance after partial pulpotomy by using ProRoot MTA and Endocem MTA. Materials and Methods: Twenty-eight teeth requiring partial pulpotomy due to deep dental caries or traumatic injury were included in this study. After 2mm removal of exposed pulp and bleeding control, the ProRoot MTA or Endocem MTA was randomly adjusted to the exposed site. 1, 2, 4, and 12 weeks after the final restoration, the patients were recalled to check the postoperative pain or another unfavorable signs. Pearson's chi-square test was used for statistical analysis to evaluate any differences among tested materials. Results: 3 of 28 teeth showed postoperative pain and cold positive during follow-up period (10.7%). There were no statistically differences in pain occurrence between two tested materials (P > 0.05). Conclusions: In the limitations of this study, partial pulpotomy by using Endocem MTA showed the advantages of short setting time and lower postoperative pain incidence, allowing one visit treatment.

Effect of needle tip design and position, and irrigant flow rate on apical pressure (주사침 말단의 형상과 위치, 세척액 주입속도가 치근단에 작용하는 압력에 미치는 영향)

  • The purpose of this study was to evaluate the effect of needle tip design and position, and irrigant flow rate on apical pressure (AP) during root canal irrigation. Five human mandibular premolars were instrumented up to #35 (0.06 taper) using nickel-titanium rotary instruments. Three different needles according to change of needle tip design (notched, side-vented, and flat) were positioned at the point of 1, 3, and 5 mm from the apical constriction (needle tip position). For each needle tip design and position, APs were measured with varying flow rates of 0.05, 0.1, 0.2, and 0.3 ml/s. When the other conditions were controlled, AP increased with decreasing needle tip position or increasing irrigant flow rate (p<0.05). The AP of flat needle was the highest, followed by notched, side-vented needle for the same needle tip position and irrigant flow rate. The APs at needle tip position of 1 mm or with more than 0.1 ml/s flow rate were higher than central venous pressure (5.88 mmHg) for all conditions. Flat needle was not recommended for clinical use due to sharp increase of AP with changing needle tip position and irrigant flow rate. For safe and effective root canal irrigation, irrigant should be applied with the needle tip position of 3 mm and flow rate of less than 0.05 ml/s.

Effect of cooling rate on precipitation hardening of a Pd-Cu-Ga-Zn metal-ceramic alloy during porcelain firing simulation (금속-세라믹용 Pd-Cu-Ga-Zn계 합금의 모의 소성 시 냉각 속도가 석출 경화에 미치는 영향)

  • The effect of cooling rate on precipitation hardening of a Pd-Cu-Ga-Zn metal-ceramic alloy during porcelain firing simulation was investigated and the following results were obtained. When the cooling rate was fast (Stage 0), the hardness of the alloy increased at each firing step and the high hardness value was maintained. When the cooling rate was slow (Stage 3), the hardness was the highest at the first stage of the firing, but the final hardness of the alloy after complete firing was lower. The increase in hardness of the specimens cooled at the cooling rate of Stage 0 after each firing step was caused by precipitation hardening. The decrease in hardness of the specimens cooled at the cooling rate of Stage 3 after each firing step was attributed to the coarsening of the spot-like precipitates formed in the matrix and plate-like precipitates. The matrix and the plate-like precipitates were composed of the $Pd_2(Cu,Ga,Zn)$ phase of CsCl-type, and the particle-like structure was composed of the Pd-rich ${\alpha}$-phase of face-centered cubic structure. Through the porcelain firing process, Cu, Ga, and Zn, which were dissolved in Pd-rich ${\alpha}$ particles, precipitated with Pd, resulting in the phase separation of the Pd-rich ${\alpha}$ particles into the Pd-rich ${\alpha}^{\prime}$ particles and ${\beta}^{\prime}$ precipitates composed of $Pd_2(Cu,Ga,Zn)$. These results suggested that the durability of the final prosthesis made of the Pd-Cu-Ga-Zn alloy can be improved when the cooling rate is fast during porcelain firing simulation.

Effect of layer combinations with nanocomposite and low-shrinkage composite resins on their color and mechanical properties (나노복합레진과 저수축 복합레진의 복합 층으로 이룬 시편이 색과 물리적 성질에 미치는 영향)

  • This study investigated the colors and mechanical properties of layered dental composites. Four nanocomposite resins (Aelite LS, Grandio, Tetric EvoCeram, Filtek Z350XT) and a silorane-based composite resin (P90) were used for overlying and underlying materials, respectively, with different thickness combinations. Colors, translucency parameter (TP), flexural and compressive properties were evaluated. All tested specimens had different color coordinates, although all were of A3 shade. Color coordinates and TP values of layered specimens better matched those of the corresponding overlying product as the thickness of the overlying product was increased. High TP values were related with high $b^*$ value differences between specimens (p<0.05). Both flexural strength and modulus, compressive strength and modulus of layered specimens with different thickness combinations were mostly lower than those of the corresponding overlying products, respectively, in their non-layered state.

Characteristics of Antibacterial Chlorhexidine-Containing Hydroxyapatite Coated on Titanium (타이타늄 상에 코팅된 클로르헥시딘 항균제를 함유한 수산화인회석의 특성)

  • In this study, antibacterial chlorhexidine (CHX)-containing hydroxyapatite (HAp) was coated on titanium and investigated its characteristics. Ti-mSBF-CHX group was prepared by soaking titanium disks in the modified simulated body fluid (mSBF) mixed with CHX. Ti-mSBF group was coated using mSBF without CHX. Ti-mSBF-adCHX group was prepared by soaking Ti-mSBF specimen in CHX-containing solution. The crystallines clusters composed with nano-shaped crystallites were coated on the surface of the Ti-mSBF specimen. The ribbon-shaped crystallites were observed with the crystalline clusters on the Ti-mSBF-CHX specimen. The content of CHX chemical compositions was high in ribbon-shaped crystallites. HAp crystalline structure was dominant for all prepared specimens, and ${\beta}-TCP$ (tricalcium phosphate) and OCP (octacalcium phosphate) crystalline structures were observed in the Ti-mSBF-CHX specimen. FT-IR spectra showed the strong peaks of CHX in Ti-mSBF-adCHX and Ti-mSBF-CHX groups. However, after immersing in a phosphate buffered saline (PBS), CHX was rapidly released in Ti-mSBF-adCHX group, while it was slowly released in Ti-mSBF-CHX. We expect that the coating method of Ti-mSBF-CHX group could be used for protecting inflammation of titanium implant by incorporating antibacterial agent CHX into HAp layer.
