A CLINICAL EVALUATION OF SAFETY OF AN OFFICE BLEACHING GEL CONTAINING 30% HYDROGEN PEROXIDE (30% 과산화수소를 함유한 전문가 미백제의 안전성 평가)

  • This study evaluated the safety of an office bleaching gel (RemeWhite, Remedent Inc., Deurle, Belgium) containing 30% hydrogen peroxide. 37 volunteers were received office bleaching with the RemeWhite for 3 times at one visit, total 2 visits. As control group, the same gel in which hydrogen peroxide was not included was applied to 34 volunteers with the same protocol. There was no difference between experimental group and control group using electric pulp test. In the result of gingival inflammation index and tooth sensitivity test, there was mild pain response in experimental group but it disappeared as time went by. Therefore, safety of the office bleaching gel containing 30% hydrogen peroxide was confirmed.

Efficacy of a self - applied paint - on whitening gel combined with wrap (Wrap을 사용하는 자가 도포 미백젤의 치아 미백 효과)

  • Purpose: The aim of this clinical study was to evaluate the efficacy and safety of a self - applied paint - on whitening gel combined with wrap to increase the effect of a whitening gel and minimize gingival irritation. Materials and Methods: Ninety adult volunteers were randomly treated to a control group and two experimental groups using whitening gel containing 2.8% and 3.0% hydrogen peroxide for 30 persons each. They had used the wrap and whitening gel on maxillary 4 anterior teeth for 30 minutes per day during 2 weeks. Whitening tooth color response was measured by VITA shade guide and ShadeEye $NCC^{(R)}$. And side effects were assessed from interview and intraoral examination. The efficacy and safety evaluations were statistically analyzed. Results: In the evaluation with VITA shade guide, there was significantly the whitening effect in experimental groups compared with the control group. In the evaluation with ShadeEye $NCC^{(R)}$, the 3.0% experimental group showed significantly the whitening effect compared to the control group and the 2.8% experimental group (P < 0.05). There were some complaints of minor side effects, but there did not find abnormal symptoms of the gingival stimulation in all groups. Conclusion: A self - applied paint - on whitening gel combined with wrap can be used as a useful self-whitening material because the whitening effect increases as the concentration of hydrogen peroxide from 2.8% to 3.0% and also no significant side effects are observed.

  • Purpose: Gingival whitening is one of dental treatment purposes which is close to treating aesthetic disorders. Initial gingival whitening treatment was done by dermabrasion using a high power Diode Laser. However, this treatment method cannot be free from any infection or pain after the treatment. Therefore, we have decided to progress gingival whitening treatment using a low power LED laser. Materials and Methods: The laser was irradiated on pork meat then the safety of output power, temperature change and skin denaturalization was measured. Bison 365 nm LED laser was irradiated on oral mucosal pigment of a 15 - 20 kg beagle for 15 min for 1 - 2 weeks, one or two times each. Any pigment loss was checked through Hematoxyline-Eosin staining. Results: The melanin pigments at the area of 365 nm LED Laser irradiation were decreased. Conclusion: The 365 nm LED Laser proposed in this study is considered to compensate the bleaching effect achieved by either using Diode laser or surgical methods.

Isolation of Melanogenesis Inhibitors from Cnidii Rhizoma (천궁으로부터 멜라닌 생성억제 물질 분리)

  • Gingival hyperpigmentation may cause esthetic problems and embarrassment. Especially in patients with a gummy smile. Melanin pigmentation is related to estiologic factor such as hormon, systemic factor, drug, smoking and gingival inflamation. During our search for new inhibitory components on melanogenesis from natural resources, MeOH extracts of more than 100 higher plants were tested for the inhibitory effect on melanogenesis in cultured B-16 mouse melanoma cell lines, and methylene chloride soluble part extract of Cnidii Rhizoma MeoH extraction was found to have potent activity. Cnidii Rhizoma, the root of Cnidium officinale Makino (Umbelliferae), is used for the treatment of abdominal pain, arthralgia, headache, hypertension, intestinal colic and for menstrual disorders and uterine cramps for its anti-blood stagnation effect. Two compounds were isolated and their chemical structures were determined as linoleic acid methyl ester(1), 1,3-dilinoleoyl-2-stearoyl glycerol(2), on the basis of physical and spectral data.

  • The attractiveness of the gingiva is determined by its color, shape, and the shape and location of the boundary between the teeth and the gingival tissue. The standards beauty, balance, and health of the gingiva are all different, but the general public would agree that a coral pink gingiva is more beautiful than black or brown gingiva. Hence, one would be able to smile more confidently in public if he or she receives a gingival pigmentation removal surgery that changes the color of black or brown gums to a beautiful pink color with relative simplicity. The color of one's gingiva varies from pale pink to deep bluish purple, depending on many health components. The most prominent among these include the vascular supply, epithelial thickness, the degree of keratinization, and the presence of pigment in the epithelium. Melanin, carotene, reduced hemoglobulin, and oxyhemoglobulin are the main pigments contributing to the normal color of the oral mucosa. The health of one's gingival tissue are essential for an attractive smile. Excessive melanin deposits in the basal and early basal layers of the epithelium stored in the form of melanosomes frequently cause pigmentation.   • This study evaluated the effectiveness and safety of an experimental bleaching strip (Medison dental whitening strip. Samsung medical Co., Anyang, Korea) containing 2.9% hydrogen peroxide. Twenty-three volunteers used the bleaching strips for one and a half hour daily for 2 weeks. As control group. the same strips in which hydrogen peroxide was not included were used by 24 volunteers with the same protocol. The shade change (${\Delta}E^{*}$, color difference) of twelve anterior teeth was measured using Shade Vision (X-Rite Inc., S.W. Grandville, MI, USA). Chroma Meter (Minolta Co., Ltd. Osaka. Japan) and Vitapan classical shade guide (Vita Zahnfabrik, Germany). The shade change of overall teeth in the experimental group was significantly greater than that in the control group (p < 0.05) and was easily perceivable. The change resulted from the increase of lightness (CIE $L^{*}$ value) and the decrease of redness (CIE $a^{*}$ value) and yellowness (CIE $b^{*}$ value). The shade change of individual tooth was greatest in canine. and smallest in central incisor. The safety of the bleaching strip was also confirmed.

  • This clinical study evaluated the whitening effect and safety of polymer based-pen type BlancTis Forte (NIBEC) containing 8.3% carbamide peroxide. Twenty volunteers used the BlancTis Forte whitening agent for 2 hours twice a day for 4 weeks. As a control. Whitening Effect Pen (LG) containing 3% hydrogen peroxide was used by 20 volunteers using the same protocol. The change in shade (${\Delta}E^*$, color difference) was measured using $Shadepilot^{TM}$ (DeguDent) before, during, and after bleaching (2 weeks, 4 weeks, and post-bleaching 4 weeks). A clinical examination for any side effects (tooth hypersensitivity or soft tissue complications) was also performed at each check-up. The following results were obtained. 1. Both the experimental and control groups displayed a noticeable change in shade (${\Delta}E$) of over 2. No significant differences were found between the two groups (p > 0.05), implying that the two agents have a similar whitening effect.2. The whitening effect was mainly due to changes in a and b values rather than in L value (brightness). The experimental group showed a significantly higher change in b value, thus yellow shade, than the control (p < 0.05). 3. None of the participants complained of tooth hypersensitivity or soft tissue complications, confirming the safety of both whitening agents.

  • The patient was a 26-year old female who was not satisfied with the aesthetic approach of her maxillary incisors. The treatment goal throughout the process was to manage the risk and recreate the original and youthful appearance of the smile. The biomechanical risk was managed by providing coverage of the exposed dentin and rebuilding the eroded structures with minimal tooth reduction throughout the treatment. Functional risk and prognosis for this patient were both improved by achieving acceptable function. The patient was satisfied with the full-ceramic restorations and the symmetry, harmony gingival architecture.
