Rehabilitation of severely worn dentition using Monolithic surveyed restoration and electronic surveying in RPD metal framework fabrication: A case report (심한 마모를 가진 환자에서 전자 서베잉을 이용한 금관 및 국소의치 수복 증례)

  • Excessive tooth wear results in unacceptable damage to the occlusal surface and can cause pulpitis, occlusal disharmony, dysfunction, and unesthetic result. Patients with severe attrition have to be classified as several types relative to the vertical dimension of occlusion (VDO) and the interocclusal distance for the prosthetic space. The patient in this case was a 80 - year - old woman who lost support of posterior occlusion and collapsed of the occlusal plane due to confrontation of the opposing teeth, accompanied by an increase in the number of remaining bristles, resulting in a loss of intermaxillary space for prosthesis. In this case, treatment with increased vertical dimension may have stability if the increase in vertical occlusal height is minimized within the required range, and a stable occlusal contact is provided after an increased vertical occlusal height stabilization period. After the new VDO had been confirmed under interim fixed restorations, definitive fixed restorations were produced. Through these treatment processes, we obtain satisfactory results that are functional and aesthetically pleasing.

  • The purposes of this study were to examine the effect of 35% hydrogen peroxide (HP) bleaching agent on the changes in physical and chemical characteristics of tooth. The bleached teeth showed an apparent color changes. The whiteness increased linearly within the tested period as the period of bleaching increased. The microhardness between bleached groups after bleaching showed any statistically significant difference according to the paried t-test. The bleached enamel surface showed any apparent morphological changes compared to the enamel which was stored in distilled water only. The difference of the total mineral contents for the distilled water and hydrogen peroxide did not show statistical significance. These results demonstrated that bleaching using 35% hydrogen peroxide were adversely affects application time of experimental group and may confirm the safety of using these agents for a short time in dentist-monitored bleaching.

  • Topical fluoride application for children is a widely performed procedure in the field of Pediatric Dentistry for its dental caries prevention effects However, it is recently recognized as having some unwanted effects on several esthetic restorative materials, the author immersed glass ionomer cement, compomer and composite resin specimens in APF gel and measured the surface roughness and also, examined the specimens under the scanning eletron microscope. The followings are the results: 1. In the specimens of glass ionomer cement and compomer, APF gel 4 minute immersion group and pH cycling group show statistically significant increased surface roughness than artificial saliva immersion group (p<0.01). 2. There was no statistically significant surface roughness for composite resin in all group (p>0.05). 3. When the specimens were examined under scanning electron microscope, the surface change were in the order of glass ionomer cement, compomer, composite resin and also in the order of pH cycling group, APF gel 4 minutes immersion group, artificial saliva immersion group.

Anticariogenic activity of acanthoic acid isolated from Acanthopanax koreanum $N_{AKAI}$ against oral pathogens (섬오가피로부터 분리된 acanthoic acid의 항치아우식효과)

  • This research was aimed to search for natural antimicrobial agents that are sefe for humans and specific for oral pathogens. Acanthoic was isolated from the chloroform fraction of methanol extract of Acanthopanax koreanum $N_{AKAI}$ and its structure were elucidated by 13-NMR, 1H-NNR and ESIMS. Antibacterial activity of acanthoic acid was investigated by the minimum inhibitory (MIC) and minimun bactericidal (MBC) concentration. MIC/MBC of acanthoic acid against Streptococcus mutans $N_{AKAI}$ causing dental caries was determined to be $2/4\;{\mu}g/mL$, which was much lower than these of other natural antimicrobial agents such as $8/16\;{\mu}g/mL$ of sangurinarine and $250/500\;{\mu}/mL$ of green tea extract, $500/600\;{\mu}g/mL$ of thymol and borneol. Acanthoic and significantly inhibited the growth of other cariogenic bacteria such as Streptococcus sobrinus $N_{AKAI}$ and Streptococcus sanguis $N_{AKAI}$, and Streptococcus gordonii $N_{AKAI}$ in the MIC range of $4{\sim}32\;{\mu}g/mL$. My finding suggests that acanthoic acid could be employed as a potential antibacterial agent for preventing dental caries.

The Bleaching Effect of Plasma Are and 35% Carbamaide Peroxide and its Influence on the Enamel Surface (Plasma Arc와 35% Carbamaide Peroxide의 미백효과와 법랑질 표면에 미치는 영향)

  • The purpose of this study was to evaluate color change and enamel surface changes using the plasma arc light source during tooth bleaching treatments. Twenty-four extracted bovine incisors were selected and embedded in the resin blocks. All the specimens were highly polished and discolored with commercial $COCK^{(R)}$. High concentration carbamide peroxide with and without plasma arc were used for bleaching. Specimens were bleached for 1 hour per week during 3 weeks. Color and enamel surface changes were determined with colorimeter (TC-8600A), microhardness tester(MXT-a7), scanning electron microscope(S-4200). All the collected data analyzed with paired t-test, t-test and one-way ANOVA. After the bleaching, both groups showed the color changes(${\Delta}E^*$). Microhardness of two group decreased after tooth bleaching. The SEM evaluation of enamel surface of both group showed a similar morphology of decalcification after tooth bleaching. Office bleaching using the plasma arc application with 35% carbamide peroxide can increase the color change. Office bleaching using the high concentration of carbamide peroxide and plasma arc also detract the outer surface of enamel. It is recommended that careful procedures are needed during office bleaching with high concentration of carbamide peroxide and light source.

Case study on Dental Hygiene Care Program with the Focus on Non-surgical Periodontal Treatments with the Maternal and Child Health Center (일부 보건소 모자보건실 연계 비외과적 치주처치 중심의 치위생관리 프로그램 운영 사례연구)

  • This research was conducted by targeting pregnant women in the trial operation of dental hygiene care program with the focus on non-surgical periodontal treatments at the Maternal and Child Health center in the public health center. This study was conducted to provide basic data to dental hygienists who operate oral health promotion programs by targeting pregnant women of the local community through the evaluation on structure, the assessment process and results from the pilot program at the public health center. The targets are nine pregnant women who have been involved in the Dental hygiene care program three times. An evaluation on the structure, process and results according to the Indicators of each program were conducted. The results of dental hygiene care program found that the retention rate decreased. The retention of the probing pocket depth larger than 4mm was 40.4% to 14.0%(p<0.05). The retention rate of the gingival bleeding teeth was 4.6% to 2.7% (p=0.180). PHP Index improved. 1.7 points in the 1st to 1.4 points in the 3rd in the case of the second pregnant woman and 2.4 points in the 1st to 1.3 points (p<0.05) in the 3rd in the case of the third pregnant woman. The applicable and expandable possibility of the dental hygiene care program with the focus on non-surgical periodontal treatments at the Community Health Center is significant in the result of this study. In the future, long-term operation and ongoing monitoring of this program is needed.

FINE STRUCTURES OF PHYSIOLOGIC AND PATHOLOGIC ROOT RESORPTION SURFACES OF DECIDUOUS TEETH (생리적 및 염증성 유치 치근 흡수면의 미세구조)

  • Deciduous teeth can be extracted for two reasons, one due to the physiologic resorption and the other by the inflammation at the apex after traumatic injury. Physiologic resorption may be different from pathologic resorption in timing and mechanism. Therefore we resumed the different features of physiologic and pathologic resorption root surfaces. Many previous studies showed micromorphology of resorbed surface of roots of deciduous teeth. But, few studies compared physiological and pathological root resorption surfaces. In this study, we carefully observed microscopic morphologies of those two different root surfaces by scanning electron microscope and histologic features by light microscope. The resultant differences between physiologic and pathologic resorption surfaces of deciduous teeth were as follows: 1. The morphology of pathologic resorption lacunae due to inflammation varied in size and shape with irregular boundaries compared with the physiologic areas from scanning electron microscope observations. 2. From light microscope observations, several large resorption fossae containing numerous resorption lacunae were found, whereas the resorption lacunae were irregular in shape with pathologic resorption surface. 3. Numerous multinucleated giant cells were closely attached to the physiologic resorption lacunae, whereas several kinds of mesenchymal cells with numerous inflammatory cells were found in the areas adjacent to the pathologic resorption surface. 4. Light microscope findings showed that compensating cementum formation took place along some of the areas of inflammatory dentinal resorption. In conclusion, several morphological differences were present between physiologic and pathologic root resorption surfaces of human deciduous teeth. The future studies should include cytochemistry to clarify the cellular roles in resorption process observations of pulpal surfaces of coronal and radicular dentin to and the changes that occur in each phase of human deciduous tooth resorption.

Experimental Research on the Surface Changes and the Abrasion Resistance of Pit and Fissure Sealant by Fluoride (불소도포제에 의한 치면열구전색제의 마모저항성과 표면 변화에 관한 실험연구)

  • To test the effects of representative fluoride vanishes-1.23% APF gel and 5% NaF Fluoride Varnish-on the surface structure of pit and fissure sealant, this study classified samples of pit and fissure sealant into five groups: Group I deposited in distilled water for ten minutes, Group II treated with APF gel for one minute, Group III treated with APF gel for four minutes, Group IV treated with Fluoride Varnish for one minute, and Group V treated with Fluoride Varnish for four minutes. An abrasion test was carried out to measure changes in weight, along with observation of the surface structure by using an optical microscope, consequently drawing the following conclusions. 1. The results of the abrasion test using pit and fissure sealant, Concise, showed that Group III had the reduction in weight more increased than Group I and that Group V had less reduction in weight (p<0.05); the results of the abrasion test using Eco-S showed Group III had the reduction in weight more increased than Group I and that Group V had less reduction in weight (p<0.05). 2. The results of observation using an optical microscope showed that application of APF gel made the filler remarkable due to loss of substrate and that Group III treated with APF gel for four minutes had the toughest surface, followed by Group II treated with APF gel for one minute, Group I deposited in distilled water for ten minutes, Group IV treated with Fluoride Varnish for one minute, and Group V treated with Fluoride Varnish for four minutes.

DOUBLE TEETH IN MAXILLARY PERMANENT INCISORS : CASE REPORTS (상악 전치부에 발생한 이중치: 증례보고)

  • Fusion and gemination are irregularities in tooth development. It is often difficult to differentiate between gemination and fusion and it is common to refer to these anomalies as 'double teeth'. The deciduous teeth are most commonly involved, but in 0.1% of cases permanent teeth are affected, in which case aesthetic, funtional and periodontal problems can result. Double teeth present great difficulties in management and required a multidisciplinary approach. The central groove on the labial and palatal surfaces of a double tooth is extremely prone to caries, therefore early 'fissure sealing' is essential. In permanent dentition, surgical separation of fused teeth may be possible with subsequent orthodontic alignment and restorative treatment as needed to reshape the crown. Reshaping or reduction of a double tooth with a single canal may be attempted by modifying the appearance of the labial groove and the use of composite tints but is often impossible and extraction may be the only alternative. Orthodontic treatment and prosthetic replacement is then required. Implants may be an option for adolescents. The present study describes three clinical cases of double teeth in the position of the maxillary permanent incisors. The first case demonstrates an example of multidisciplinary care including surgical intraoral hemisection, root canal therapy, restorative and orthodontic treatment. The second and third cases describe the external and internal morphology of the two fused teeth by means of three dimensional dental computer tomography.

Full mouth rehabilitation for a patient with vertical dimension loss using digital diagnostic analysis: A clinical report (수직고경이 감소된 환자의 디지털 진단 분석을 이용한 완전 구강 회복 증례)

  • Full mouth rehabilitation is re-organizing the occlusion of the remaining teeth and missing teeth considering the functions, esthetics, and neuromuscular harmony. With the loss of multiple teeth, the patient's occlusal plane gradually collapses and the vertical dimension can be reduced. Since reduced vertical dimension can be a potential etiology of the temporomandibular joint and masticatory muscles, prosthetic restoration with increased vertical dimension is required. This case report is about a 68 years old patient with vertical dimension loss due to worn dentition and multiple loss of teeth. In this case, the loss of vertical dimension is assessed carefully using the digital dentistry technology. Using CAD software in digital analysis step, the occlusal plane was established and evaluated using several criteria. Orienting the position of the bone and teeth using CBCT image, patient's condition was visualized in 3 dimension and treatment planning was possible virtually. The information that matches the patient's condylar position with the articulator, which is the virtual face bow, is reproduced on the actual articulator, and evaluated again. After the evaluation, provisional prosthesis was fabricated and it was confirmed that the patient adapts without any abnormality. This was implemented as a final prosthesis. As a result, the patient obtained satisfying results, utilizing the benefits of digital dentistry technology and traditional methods.
