Surgically assisted orthodontic treatment of ankylosed maxillary incisor (유착된 상악 절치의 외과적 처치를 동반한 교정 치료)

  • If dental ankylosis occurs in maxillary incisors of a growing child, the ankylosed tooth can not move vertically with the subsequent disturbance in vertical growth of the alveolar process. Because ankylosed tooth does not respond orthodontic force, extraction was recommended in the past. But the loss of tooth and accompaning alveolar bone loss incur compromised esthetic situation. And it is very hard to replace by prosthetics. So intentional surgical luxation and orthodontic movement was attempted, but usually this approach is followed by recurrence of the ankylosis. Nowadays the unitooth subapical osteotomy and rapid movement of block bone was reported. Two cases we presented, one is treated by intentional luxation and the other is by unitooth subapical osteotomy following application of light continuous force soon.

The Effect of EGF, T3 and HB-EGF on Human Periodontal Fibroblasts (EGF, T3, HB-EGF 가 치주인대섬유모세포에 미치는 영향)

  • Viable cells of periodontal ligament would be an important factor for the successful replantation of an avulsed tooth. Therefore, it is critical to choose the storage medium for the preservation of traumatically avulsed teeth. Growth factors and hormones could be considered for the therapeutic application of the maintenance of viable periodontal ligament fibroblasts (PDLFs). Epidermal growth factor (EGF) has been suggested as an important player for the regeneration and wound healing process on other tissues. Therefore, EGF was evaluated for the therapeutic application on avulsed teeth. In addition, the synergic effect of EGF with tri-iodothyronine (T3) and heparin-binding epidermal growth factor-like growth factor (HB-EGF). The cell proliferation of PDLFs was determined by MTT assay and increased dose-dependently up to 10 ng/ml in the presence of EGF. Maximum cellular growth was shown at the concentration of 10 ng/ml EGF. Also, EGF promoted the wound healing of PDLFs examined by in vitro wound healing assay. Combined effects of EGF with T3 or HB-EGF on the proliferation of PDLFs were also studied. Interestingly, EGF showed the synergic effect on the proliferation of PDLFs with T3 and HB-EGF. To find out the mechanism of the synergic effect of EGF and T3, the effect of T3 on the expression of endogenous EGF receptor was determined by RT-PCR. The result was that T3 enhanced the expression of EGF receptor in PDLFs. It suggested that EGF might be a good choice for a therapeutic application, which can be used as combination with T3 and HB-EGF.

EFFECT OF TOPICAL ALENDRONATE APPLICATION ON INFLAMMATION OF REPLANTED RAT MOLAR (탈구치의 alendronate 도포가 재식 후 염증반응에 미치는 효과)

  • This study histologically assessed the effect of topical alendronate application on periodontal healing in replanted teeth in fifty four SD Rats. Upper first molars in rat were extracted and replanted after dried during 15 minutes or 60 minutes in the air. In Group I, all teeth were replanted after 15 minutes of dry storage without any other treatment. In Group II and III, the pulps were removed and all teeth were replanted after soaking 10 min in Hank's balanced salt solution with/without alendronate, followed by 60 minutes of dry storage. the rats were sacrificed after 7, 15 and 30 days. The histological parameters studied were healed PDL, surface inflammatory and replacement resorption, and inflammatory severity. The following conclusions could be drawn from the present investigation. 1. Group I showed lower inflammatory root resorption and inflammation severity rate, compared to Group II and Group III. In Group I there showed effective for reattachment and regeneration of PDL. 2. In Group II, inflammatory root resorption were more severe and faster than other groups. There were extensive root resorption in the rats sacrificed after 30 days. 3 In Group III, there were localized inflammatory resorption in several areas, but extensive resorption did not occur Group III showed increase in root resorption rate, compared to Group I. However this difference was not statistically significant. 4. There were no difference between sacrificed days in replacement root resorption in all groups.

DIVERSE DENIAL TRAUMA OF MAXILLARY ANTERIORS : CASE REPORT (상악 전치부에 발생한 여러 유형의 치아 외상)

  • It is rare that the complete avulsion, intrusive luxation, and extrusive luxation occurred by trauma at the same time. In July, 2003, 10-year and 2 month-old-girl was referred to Department of Pediatric dentistry, Chonnam National University Hospital due to complete avulsion of upper right lateral incisor, intrusive luxation of upper right cental incisor and extrusive luxation of upper left central incisor. Traumatized teeth were surgically repositioned and fixated with resin-wire splint at the same day. Endodontic treatment of avulsed tooth was performed 2 weeks after trauma. A radiograph was taken 3 months after trauma, which demonstrated root resorption of both upper central incisors. External root resorption was arrested by the root canal therapy with calcium hydroxide. It showed good results for 1 year and 4 month but further follow-up is needed to check root resorption and ankylosis.

  • Dentofacial trauma can result in tooth fracture, avulsion, facial bone fracture. The Unites states and Japan mandated the use of mouthguards for contact sports. But, Korean didn't. Mouthguards divided into ready-made type and custom-made type. Mouthguards protect the lips, intraoral soft tissues, teeth and provide the mandible with resilient support to prevent jaw fracture and dislocations. Sports-related accidents have been reported to be one of the most common causes of dentofacial trauma. Sports trauma of involving teeth with incomplete root formation cause long chair time, multiple visit, economic considerations, additional dental services. So, mouthguards can offer considerable protection against sports-related trauma.
