MULTIPLE CONGENITAL MISSING TEETH : CASE REPORT (선천성 다수 영구치 결손 환아의 증례보고)

  • Congenital Missing Teeth(hypodontia, oligodontia) is the developmental absence of one or more teeth. It has been reported as being the most common anomaly of dental development in human, relatively common in the permanent dentition. In a recent review, Vastardis has quoted incidence ranges of $1.6%{\sim}9.6%$ in the permanent dentition. Brook has quoted a prevalence of $3.5%{\sim}6.5%$ in most populations, with severe hypodontia, defined as the absence of six or more teeth, having a prevalence of $0.3{\sim}0.4%$. The most commonly affected teeth are third molars, followed by maxillary lateral incisor, and second premolars. The etiology is unknown, several hypotheses include trauma, nutritional deficiency, infection, metabolic abnormalities, systemic disease and genetic influence. The multiple congenital missing is commonly associated with specific syndrome or severe systemic abnormalities such as cleft lip & palate and Down's syndrome. These cases present that children have multiple congenital missing teeth in the permanent dentition, without any systemic disease. Management of this condition must be considered orthodontic and prosthodontic treatment comprehensively. In these cases, children were treated by space maintainer or orthodontic appliance and follow-up checked.

CARIES-RELATED MICROBIOLOGICAL SCREENING IN CHILDREN UNDER THREE YEARS OF AGE (3세이하의 어린이의 치아우식증과 관련된 미생물학적 선별검사)

  • To evaluate microbial data and salivary measurements from clinically compatible, culture-based screening procedures employed with children younger than 36 months old. Plaque and stimulated saliva specimens were collected from 87 children. The pH of each saliva sample was measured before and after 0.94% lactic acid was added. Specimens were diluted and plated on selective media and non-selective media. Data collected were counts of mutans streptococci (MS) and lactobacilli (LB). In addition, total viable counts (TVC) of specimens, salivary pH and buffering capacity were also assessed. Each variable was compared to caries status of subjects. According to this study, the results were as followed: 1. Highly significant correlation with caries rates were found for counts of MS and LB. 2. The specific counts/ml saliva or plaque above which caries is predicted, or below which caries is not predicted were as follows: 1) Saliva MS; $10^5$ 2) Plaque MS; $2{\times}10^5$ 3) Saliva LB; $10^3$ 4) Plaque LB; $10^3$. 3. Salivary pH and buffering capacity versus caries status were not significant. 4. Microbial screening methods based on mutans streptococci had higher predictive values and odds ratios than methods for lactobacilli. 5. MS counts were clearly the best indicators of caries status in young children. This measurement can easily be obtained in a dental clinical setting both by conventional culture techniques, or commercial kits for MS recovery.

  • Hypohidrotic ectodermal dysplasia refers to a group of disorders with the following common features : thin, sparse or absent hair, missing or peg-shaped teeth and inability to sweat adequately. Both the primary and secondary dentition are affected. Teeth may be absent (anodontia) or reduced in number (oligodontia) and abnormally shaped. In case, A 5-year-old man presented with hypohidrosis and dry skin. He had no teeth in mouth. We report a case of hypohidrotic ectodermal dysplasia with anodontia.

  • Odontoma, hamartomas of odontogenic orgin, are composed of all the structures that make up teeth, which may lead to interference with the eruption of its associated tooth. On the basis of gross, radiographic, and microscopic features, two types of odontoma are recognized: compound and complex. The etilogy of odontomas is unknown, although local trauma, infection, and gentic factors have been suggested. Odontomas occur central in bone between the roots of teeth and the mauority are asymptomatic. Although these tumors occur frequently and constitute 22% of all odontogenic tumors, are very rare. Erupted odontoma are defined as tumors that it occurs a calcifed mass may be defected on the ridge. The treatment of chico is the surgical removal of the lesion followed by a biopsy. This report presents a case of 8-year-old girl with the delayed eruption of the mandibular first molar by the calcified mass within the operculum covered the central fossa. And it was diagnosed as erupted complex odontoma by excisional biopsy.

A Study of Congenitally Missing Permanent Teeth in Wonju Severance Christian Hospital (원주세브란스기독병원의 선천성 영구치 치아결손에 관한 연구)

  • Congenitally missing teeth are one of the most prevalent dental anomalies in the oral cavity. In this study, the prevalence, distribution, and symmetry of congenitally missing permanent teeth among 1,865 patients aged 7 to 15 years who visited the Pediatric Dentistry Department of Wonju Severance Christian Hospital from March 2011 to May 2021 and took panoramic radiographs were investigated and analyzed. Most of the patients had one or two congenitally missing teeth, mainly in the second premolars and lateral incisors. Congenitally missing teeth occurred more in the mandible than in the maxilla, and there was no significant difference in prevalence between the left and right sides. Congenitally missing teeth tend to occur symmetrically on the left and right sides and in the maxilla and mandible, depending on the tooth. Early oral examination and radiological examination are required to prevent complications due to congenitally missing teeth, and appropriate interdisciplinary treatment is required.

A STUDY ON THE RELIABILITY OF THE OPTICAL CARIES ACTIVITY TEST (광학적 치아우식활성 검사법의 신뢰도에 관한 연구)

  • The purpose of this study was to evaluate the specificity, sensitivity, and diagnostic power of caries activity test using LED fluorescence. The subjects of this study were 55 children of $6{\sim}7$ years old. LED light were irradiated to labial or buccal surface of all teeth. Fluorescence from initial carious lesion of teeth illuminated by an LED light was observed through barrier filter and the number of teeth showing lesion, size and position of lesion were counted. Streptococcus mutans colony counting and dDfFtT rate test were also done and their correlation was compared. And then specificity, sensitivity, diagnostic power of optical caries activity test using LED light were evaluated. 1. There was positive $correlation({\gamma}=0.43)$ between LED fluorescence test and Streptococcus mutans count(P<0.05). 2. When visual examination was defined to standard testing method, the specificity, sensitivity, diagnostic power of LED fluorescence test were 100%, 76.1%, and 100%. 3. When dDfFtT rate was defined to standard testing method, the specificity, sensitivity, diagnostic power of LED fluorescence test were 88.9%, 47.8%, and 95.7%. 4. When S. mutans colony counting was defined to standard testing method, the specificity, sensitivity, diagnostic power of LED fluorescence test were 100%, 58.7%, and 100%. Considering the above results, optical caries activity test using LED light could be regarded as a practical method because of its close relationship with microbiological caries activity test.

Serum 25-hydroxyvitamin D levels are associated with dental caries experience in Korean adolescents: the 2010~ 2014 Korean National Health and Nutrition Examination Surveys (청소년에서 혈중 비타민 D 농도와 치아우식 경험 간의 연관성 : 2010~ 2014 국민 건강영양조사)

  • Purpose: This study was conducted to evaluate the association between serum 25-hydroxyvitamin D (25(OH)D) levels and dental caries experience in Korean adolescents based on the 2010 ~ 2014 Korean National Health and Nutrition Examination Surveys. Methods: The study subjects were 2,655 Korean adolescents aged 10 to 18 years. Subjects were classified into four groups according to their serum 25(OH)D levels. We used logistic regression to evaluate the relationship between vitamin D and for dental caries experience after adjusting for age, household income level, recipient of basic livelihood, tooth brushing and visiting dental clinics. Result: Multiple logistic regression analysis showed that serum 25(OH)D insufficiency (20 ng/mL ${\leq}25(OH)D$ < 30 ng/mL) was associated with increased odd ratios (ORs) for dental caries experience in boys (OR = 2.577, 95% CI = 1.013-6.557), compared with serum 25(OH)D sufficiency (25(OH)D ${\geq}30ng/mL$). Conclusion: The serum 25(OH)D levels were found to be related to risk of dental caries experience in Korean adolescent boys.

  • 본 논문에서 기존의 치아가공기는 회전하는 모터를 사용하여 구성하였으나 이러한 모터는 정밀도, 반복정밀도가 50um 이하로 가공물 가공시에 치기공사나 치과의사가 사람에 맞추어 다시 작업을 해야하는 불편함과 시간적, 작업자의 피로도를 높일수 있는데 이러한 모터에 스크류나 밸트를 연결하여 선형적으로 움직일 수 있는 리니어모듈과 리니어모터를 적용하게되면 20um수준의 고정밀의 위치제어가 가능한 5축 치아가공기를 만들 수 있었다. 또한 MEMS센서를 이용하여 스핀들의 상태를 모니터링 하고 임계값을 지정하여 이상 신호 발생시 모터를 멈추어 위험상황에 대해서 인공지능기법을 이용하여 정지하거나 관리자에게 알림을 주어 효과적으로 5축치아가공기를 운영할 수 있도록 하였다.

MANAGEMENT OF IMPACTED TEETH BY AUTOTRANSPLANTATION IN CHILDREN (소아에서 자가치아이식에의한 매복치의 처치)

  • We decided among extraction, orthodontic traction and autotransplantation such as direction and position of unerupted tooth, degree of developing root apex, eruption space, being of supernumerary tooth or odontoma or cyst when tooth impacted. Autotransplantation is considered when orthodontic traction is unrealistic or when tooth movement can absorb root of neighbor tooth. The prognosis for successful autotransplantation is dependent on a number of factors such as root development, surgical technique, patient's age, endodontic treatment, time and type of splinting, preservation of periodontal ligament and storage medium. Especially when severe osseous defect is being, bone graft considered for reducing of mobility and for assisting recovery. In all cases, chief complaint is unerupted tooth and various causing factors were supernumerary, odontoma, ectopia and so on. Before autotransplantation, space regaining was done if needed and demineralized freezed dried bone and autogenous bone graft was done when there is severe osseous defect by extraction of supernumerary tooth or odontoma. Splinting was removed after 2-3weeks At 3-4weeks after autotransplantation, endodontic treatment was decided. At follow up check, normal recovery was done and there was no inflammatory or replacement root resorption in periapical radiograph.
