Clinical Study on the Dental Abnomalities of Number and Morphology in Cleft Alveolus Patients (치조열 환자에서 치아의 선천결손과 형태이상에 관한 연구)

  • 구순구개열은 선천성 악안면 기형중에서 발생률이 가장 높은 질환이며 특히 동양인에게 높은 빈도로 발생된다. 그리고 순구개열에서는 파열부 조직의 선천적 상실과 파열부 봉합 수술 후 형성된 반흔에 의한 상악골의 열성장과 상악궁의 협착, 코의 형태 이상, 등과 함께 치아의 수, 크기와 형태 및 맹출 이상도 높은 빈도로 동반된다. 선천성 치아 발육 이상이 구순열 또는 구개열 가진 환자에게서 종종 보고되고 있다. 이런 치아 이상은 과잉치, 결손치, 크기, 형태, 맹출시기, 법랑질 광화 등의 많은 특징을 포함한다. 이번 연구의 목적은 다음과 같다. 1. 구순열 또는 구개열을 가진 환자의 선천성 치아 결손 발병률을 결정하여 정상인과 비교하는 것이다. 2. 상하악에서 파열이 있는 부위와 없는 부위의 제2소구치 결손가능성을 비교 하는 것이다. 구강악안면외과에서 구순구개열로 진단한 환자 중 2005년 1월~2009년 8월까지 전북대학교 구강악안면외과에서 치조열부위 자가골이식수술을 받은 32명의 환자로 초진시의 교정 chart, 구강악안면외과 chart, x-ray(파노라마, 치근단사진, 교합사진(상악), 석고 모델, 구강내 외 임상사진을 사용하여 순구개열군 별로 매복치, 과잉치, 결손치, 왜소치의 유무와 위치를 조사하여 다음과 같은 결론을 얻었다. 1. 결손치는 비교적 높은 발생빈도(53.1%)를 보였으며, 치아별 발생빈도는 상악 측절치와 상악 제2소구치, 하악 제2소구치 순이었다. 구순구개열군이 구순치조열군에 비해 발생률이 높게 나타났다. 그리고 구순구개열군 내에서 양측성이 편측성에 비해 결손치의 발생률이 높게 나타났다. 2. 왜소치는 71.6%에서 발견되었으며, 치아별 발생빈도는 상악측절치에서 가장 많았다. 구순치조열군이 구순구개열군에 비해 발생률이 높게 나타났다. 3. 치조열을 가진 환자에 있어 상/하악간 제2소구치의 선천적 결손에 유의한 차이가 있었다. 4. 구순구개열이 인접한 상악측절치의 발생중인 미성숙 조직에 영향을 미치며 파열부위와 좀 더 떨어져 있는 상악 제2소구치에도 일정한 영향을 미침을 이번 연구를 통해 다시 확인할 수 있었다.

  • Anchorage control in orthodontic treatment is an important factor affecting treatment results. In the conventional approach, intra-oral anchorage such as application of differential force and moment, Nance holding arch and lingual arch, as well as extra-oral anchorage such as head gear were used for anchorage reinforcement. However, these anchorages may result in undesired tooth movement and require patient cooperation. To overcome these disadvantages, skeletal anchorage system was introduced as orthodontic anchorage. Types of skeletal anchorage include implant, onplant, miniplate and miniscrew. Especially, miniscrew has many advantages such as reduced patient cooperation, low cost and easy placement. Recently, it is successfully used in orthodontic treatment. This cases were treated using orthodontic miniscrews for retraction of ectopically erupting maxillary canine and impacted mandibular canine and intrusion of maxillary incisors.

Prognostic Factors Influencing Survival and Success Rates of Autotransplanted Maxillary Canine (자가이식한 상악 견치의 생존율과 성공률에 영향을 주는 예후 인자 연구)

  • The aim of this study was to investigate the survival and success rates of autotransplanted maxillary canine at the artificially formed recipient socket and evaluate the factors that influence survival and success rates. A total of 28 children who had 37 impacted canines were reviewed over the mean observation time of 25.1 months. Tooth mobility, pathologic root resorption, vertical bone loss, periapical lesion and anklyosis of donor tooth were investigated clinically and radiographically. The survival rate was 100%. The success rate was 64.9%, because 13 teeth were evaluated as unsuccessful. Significant parameter in determining the success rate of autotransplantation was enlarged follicle size of donor tooth according to Cox proportional hazard regression model. The results provide information necessary for clinician to make a better treatment decision about autotransplantation at the artificially formed recipient socket.

  • Transposition has been described as an interchange of position of permanent tooth and is a relatively rare dental anomaly. Transposition of teeth may occur both in the maxillary and mandibular arches. but it appears more often in the maxilla of individual teeth, the maxillary canine is the most often involved. A canine transposes most often with a first premolar and less frequently transposes with a lateral incisor. Incomplete transposition is a condition describing an interchange in the position of the crowns of two permanent teeth, while the root apices remain in their relative position. Complete transposition is a situation in which both the crowns and entire root structure are transposed. The etiologic factors of transposition are tooth buds interchange, retained deciduous canines, migration of the erupting canine, trauma to deciduous teeth etc. This report describes a case of a transposition between a maxillary left canine and a lateral incisor and impaction of a maxillary left central incisor due to trauma to deciduous dentition.

  • The pulp infection of primary tooth is often caused by dental caries or trauma. But, if it is not managed properly, it can be produce the periapical lesion. The periapical lesion can cause some complications such as enamel hypoplasia, displacement, root dilaceration and impaction. Treatment options of displaced permanent successor are clinical and radiological follow-up after extraction of primary teeth, surgical opening, orthodontic traction, transplantation and extraction. In these cases, the premolars with abnormal eruption path caused by periapical lesion of the primary tooth have shown successful spontaneous eruption just as a result of extraction of infected primary tooth and space maintenance.

  • Distal displacement of the tooth germ of the mandibular second premolar (MnP2) leads to its impaction and obturation of the eruption path of the mandibular first molar delaying its eruption. The present case report describes the treatment of 2 cases of eruption guidance for distally displaced developing MnP2 that caused delayed eruption of the mandibular first molar. Intentional extraction of primary predecessor results in the mesial shift of the displaced MnP2. However, unfavorable distal ectopic eruption of the mandibular first premolar after the premature loss of primary second molar has been previously reported. Hemisection and sequential extraction of the mandibular primary second molar were performed to mesially shift the distally displaced MnP2, while preventing unfavorable distal ectopic eruption of the mandibular first premolar.

  • Autotransplantation of tooth has recently become a combined method with orthodontic therapy. With the use of autotransplantation, it has become possible for orthodontists to solve complicated problems in the dental arches. Autotransplanted teeth showed the same development as their contralaterals with respect to both eruption and root growth. This indicates that autotransplantation and orthodontic treatment is a method that should be considered in case of impacted, missing, or malformed teeth. In this report two cases are presented to demonstrate that orthodontic treatment with autotransplantation can contribute to more satisfactory solution to the problem for impacted teeth.

  • The odontoma is relatively a common benign odontogenic tumor and caused by overgrowth of odontogenic tissues. The recent classification by World Health Organization divides odontoma into 2 groups such as compound odontoma and complex odontoma. Compound odontoma comprises dental tissues, resembling the morphology of a tooth and has predilection for the anterior maxilla. In contrast, complex odontoma has unorganized mass, not resembling the normal tooth and has predilection for the posterior mandible. The etiology of odontoma is unknown and almost asymptomatic. So, it usually is found in routine radiographic examination, and most common presenting symptom is impacted or unerupted permanent teeth and retained primary teeth. It can occurs almost anywhere in jaws. It is desirable that odontoma should be removed by surgical enucleation including follicle and surrounding soft tissues. Considering the age and behavioral cooperation of patient, the development of permanent dentition, the location of odontoma in jaw, the need for the concomitant operative dentistry, operation is performed in outpatient department with/without sedation or under general anesthesia with endotracheal intubation. In this case report, 2 patients with compound odontoma were treated by surgical enucleation including follicle and surrounding soft tissues. One patient, about 5 years old, was treated under general anesthesia and concomitant operative dentistry was performed. The other patient, about 11 years old, was treated under local anesthesia in outpatient department. In 2 cases, after 4 months, surgical defects were filled with new bone and normalization of eruption path of impacted permanent teeth was observed.

  • A dentigerous cyst is an epithelium-lined sac that surrounds the crown of an unerupted tooth or odontoma. And the most common sites of this cyst are the mandibular and maxillary third molar and maxillary cuspid a areas. Clinically, expansion of bone with subsequent facial asymmetry, extreme displacement of teeth, severe root resorption of adjacent teeth and pain are all possible sequelae of this cyst. The standard treatment for a dentigerous cyst is enucleation and extraction of the involved tooth. But in large cysts, this can lead to functional, cosmetic and psychologic consequences to the patients. So recently, more conservative methods are used. We report 5 cases of dentigerous cysts in pediatric patient which were treated by a conservative approach, By this methods, we can preserve teeth and guide eruption of the teeth which are involved in cystic area.

INTRAOSSEOUS TOOTH MIGRATION OF IMPACTED MESIODENS IN THE INVERTED POSITION (상악 정중부에 역위 매복된 과잉치의 악골 내 이동)

  • Supernumerary teeth are frequently found in the anterior portion of the maxilla and develop as a result of abnormal proliferation of the dental lamina during tooth germ formation, caused by genetic or environmental factors. They may result in various complications, such as eruption interference, displacement, rotation of adjacent teeth, diastema, eruption into the nasal cavity, and development of dentigerous cyst. The optimal time for surgical extraction of supernumerary teeth has been a controversial issue. Someone prefer early surgical extraction because supernumerary teeth can cause eruption interference and displacement of adjacent teeth, eventually altering occlusion. Others prefer to delay surgical extraction until $8{\sim}10$ years of age in consideration of root maturation of the adjacent teeth and also patient's behavior. When surgical extraction of supernumerary teeth is postponed, there is possibility that impacted supernumerary teeth in the inverted or horizontal position move toward the nasal cavity, hard palate, or premolar area. When such intraosseous tooth migration is combined with the vertical growth of the maxilla, surgical approach becomes even harder. Therefore, possibility of intraosseous tooth migration should be considered as an important factor when deciding appropriate time for surgical extraction. We are presenting cases of mesiodens which showed intraosseous migration during $6{\sim}7$ years of follow-up period since the first diagnosis had been made at the $2{\sim}3$ years of age.
