연구목적 치매는 노인인구의 가장 심각한 정신건강문제 중 하나이다. 치매환자를 돌보는 부양자 역시 신체적, 정신적, 감정적 스트레스를 경험한다. 따라서 본 연구는 부양자를 위한 프로그램(치매가족교실)을 개발하고, 이 프로그램이 부양부담을 줄여주는 효과가 있는지에 대해 알아보고자 하였다. 방 법 5회기로 구성된 치매 부양자 지원 프로그램을 30명의 치매환자를 집에서 돌보고 있는 부양자들에게 제공하였다. 프로그램은 천안시 치매지원센터에서 시행되었다. 프로그램 시행 전후 부양부담의 차이를 확인하기 위해 단축형 자릿 부양부담평가척도를 프로그램 시행 전후에 시행하였으며 프로그램의 전반적인 평가를 위해 만족도 조사를 하였다. 결 과 대상자의 평균연령은 61.9세였다. 40.0% (n=12)의 부양자는 배우자, 53.3%(n=16)의 부양자는 자녀였다. 프로그램 시행 이후 단축형 자릿 부양부담평가 총점이 시행 전 25.73 (±8.6)에서 시행 후에는 22.07 (±8.0)으로 유의하게 감소하였다(p<0.001). 각 항목 별로는 4개의 항목(7, 10, 11, 12)이 프로그램 시행 후 유의하게 감소하였다(p=0.036, p=0.018, p=0.01, p=0.024). 프로그램 완료 후 만족도 조사에서 10개 세부항목 모두 평균 3점 이상으로 '그렇다' 이상의 긍정적 평가였다. 결 론 연구결과 치매환자 부양자 지원 프로그램은 부양부담을 감소시키는 효과가 있으며 프로그램 자체에 대한 만족도도 높았다. 따라서 향후 치매환자 치료 뿐만 아니라 부양자 지원 프로그램의 중요성에 관심을 갖고 프로그램의 개발 및 효과적인 제공에 대해 추가적인 연구가 필요할 것으로 생각된다.
우리나라는 경제발전, 출산율 저하, 보건의료의 발달로 65세 이상 노인 인구의 비율이 2018년 14% 고령사회, 2026년 20% 초고령사회로 예상되어 빠른 인구구조의 변화를 겪고 있다. 고령화에 따라 치매, 중풍 등 일상생활이 어려운 노인들의 수는 날로 증가하고, 핵가족화, 여성의 사회참여 증가 등으로 장기요양이 필요한 노인들을 가정에서 돌보는 것이 어려워졌다. 이에 정부는 이러한 사회적 구조 및 노인문제를 해결하기 위해 장기요양대상 노인의 삶의 질 향상과 가족부양 부담의 완화 등을 목적으로 노인장기요양보험법을 제정하여 시행하고 있다. 따라서 본 연구는 시설의 민영화와 난립으로 서비스의 공적 책임과 시설의 질 제고에 대한 요청이 확산되고 있는 부분을 살펴보고자 한다. 이 밖에도 저임금 요양인력의 양산, 재원조달 방식에 관한 문제 등의 대비가 다각도로 부족한 부분을 도출하고자 하였다. 본 연구를 통하여 노인장기요양보험제도가 초고령화 사회를 대비하여 장기 지속적으로 유지 발전하기 위해서는 법 제도적 측면의 개선방안의 모색과 더불어 노인복지서비스가 신체적 건강 뿐만 아니라 노후 생활의 안정을 위한 정신적 심리적 조치를 강구하는 방안을 검토하고자 한다.
본 연구의 목적은 노인요양시설 입소노인의 인지장애 및 그와 관련된 요인을 파악하여 조기 개입을 통해 인지장애 악화방지 및 개선을 도모하기 위함이다. 서울과 경기도 소재 노인요양시설 5개소에서 생활하는 65세 이상의 노인 229명에 대한 자료를 수집하여 SPSS statistic 18.0과 Clementine 11.1 프로그램을 이용하여 로지스틱 회귀분석을 시행하였다. MMSE-K 도구를 사용하여 인지장애 여부를 선별하였으며 노인요양시설 입소대상자의 인지장애의 관련요인으로 성별(OR=2.41, p=.035), 연령(75-84세: OR=3.26, p=.002, 85세 이상: OR=4.46, p<.001), 입소전 동거여부(OR=2.17, p=.015), 청력(OR=8.88, p=.004), 치매가족력(OR=4.39, p=.009), 일상생활 수행능력(OR=0.82, p<.001), 고혈압(OR=4.07, p<.001), 당뇨병(OR=3.42, p=.001) 등이 최종 확인되었다. 따라서 본 연구에서 도출된 인지장애 관련요인 중 조절 가능한 요인을 중심으로 노인요양시설 대상자에게 지속적인 중재를 제공한다면 건강의 유지, 증진으로 이어져 삶의 질 향상을 도모할 수 있을 것이다.
The purpose of this study was to describe about elders with dementia, their families, and their living environment by visiting the households where demented elders resided. The findings will be used as a basis to develop future individualized adjustment programs for demented elders and their families living in communities. The study participants were 64 demented elders and their families who were registered to a dementia counseling center at Nam-Gu community health center located in Inchon, Korea. Data were collected for two months, from May to June 2001. The length of data collection for each home visit ranged from 1.5 to 2.5 hours. Conclusion are as follows based on these study findings: Those demented elders had more than one chronic health problem in addition to their pre-existing dementia condition. Two thirds of the demented elders were not receiving any specific treatment for dementia. They showed a moderate level of independence in basic ADL, but were mostly residing at home because of lack of ability to perform more delicate and complicate routine daily activities by themselves. In addition, the primary caregivers were not well adjusted to the care-giving activities for their demented family members due to the lack of knowledge and information about dementia. The caregivers were mostly women including daughters-in-law, woman spouses and daughters, over a half of whom perceived their physical and mental health status as poor. Their image toward the demented elders was considerably negative. while their level of knowledge on dementia was moderate. The burden for the care-giving was high, whereas their coping method was passive. As the difference in image toward elderly before and after the onset of dementia in their family member increases, the caregiver burden also increased. The main resource of social support for the caregivers was their children. The caregivers showed high level of needs for knowledge and information on dementia, and day care service was the most preferred type of service by the caregivers. There was lack of safety in the living environments for the demented elders and their families, and in the surrounding environments to prevent dementia-related symptoms. Considering that home-based family care-giving is the most culturally appropriate model of providing care for the demented elders in Korea, we need to develop and apply an individualized adjustment program for the demented elders and their families.
이 연구의 목적은 기능손상 노인을 가정에서 돌보고 있는 주보호자의 자기 돌봄 활동이 주보호자 자신의 신체 정신건강에 미치는 영향을 탐색하고 사회복지 차원의 개입 방안을 모색하는데 있다. 조사대상자는 치매, 뇌졸중, 파킨스병 등의 진단을 받은 60세 이상의 부모 또는 배우자를 6개월 이상 가정에서 돌보고 있는 주보호자이며, 일대일 대인면접의 서베이 방식으로총 185명의 자료가 수집되었다. SPSS 21.0을 이용하여 빈도분석, 기술통계, 상관관계 분석, 위계적 다중회귀분석을 실시하였다. 분석 결과 첫째, 주보호자의 주관적인 신체건강은 5점 만점에 평균 2.81점(SD=.93)의 부정적 수준으로 나타났으며, 자기 돌봄 활동 요인 중 건강 책임(${\beta}=-.244$, p<.01), 신체적 활동(${\beta}=-.198$, p<.05)이 유의미한 영향 요인으로 나타났다. 둘째, CES-D로 측정된 주보호자의 정신건강은 평균 26.38점(SD=10.53)으로서 임상적으로 매우 심한우울 수준으로 확인되었으며, 자기 돌봄 활동의 영적 성장(${\beta}=-.409$, p<.001)이 유의미한 영향요인으로 나타났다. 마지막으로 주보호자의 신체 정신건강 증진을 위한 자기 돌봄에 대한 인식 개선, 적극적인 자기 돌봄 여건 마련 등을 강조하는 실천적, 정책적인 제언을 제시하였다.
본 연구는 노인장기요양보험제도 2차 시범사업 결과를 바탕으로 우리나라 노인의 요양시설 서비스 이용에 영향을 미치는 요인을 탐색해 보고자 하였다. 이론적 모형으로는 앤더선과 뉴만(Andersen and Newman)의 행동주의 모델을 토대로 하여 영향요인을 구분하였으며, 연구방법은 의사결정분석을 이용하였다. 분석결과 서비스 이용에 가장 큰 영향을 미치는 것은 대상 노인의 서비스 이용의향이었으며, 이러한 선행요인 다음 중요한 요인으로 자원 요인(수발자 있음 및 동거가족 여부)으로 나타났다. 욕구요인으로는 인지장애가 있는 경우, 치매로 인한 문제행동이 있는 경우, 기본적 일상생활수행능력에 있어 완전도움을 받아야 하는 경우로 나타났다. 이러한 분석결과는 앤더슨과 뉴만의 행동주의 이론의 과정을 그대로 보여주고 있음을 알 수 있는데, 즉 선행요인과 자원요인에 의해 서비스 이용형태가 달라지며, 이러한 선행요인과 자원요인은 욕구요인과 결합하여 실제 서비스 이용에 영향을 주는 것으로 나타났다. 이를 통해 본 연구는 시설 서비스 이용 대상자의 욕구요인을 정확히 파악하여 이에 적절한 서비스 내용을 구축하고 지역사회 내 시설서비스 이용노인을 선별하는데 기초 자료를 제공하는데 기여하고자 한다.
Purpose: This study was designed to examine the relationship between perceived entrapment to depression and subjective well-being of women as family caregivers caring for elderly dementia patients. Methods: One hundred and sixty-nine women family caregivers were recruited from two high schools located in Seoul, Korea for this descriptive study. The instruments used were The Entrapment Scale, The Center for Epidemiology Studies Depression Scale (CES-D) and Subjective well-being scale. Results: The score of perceived entrapment significantly correlated with depression and subjective well-being. The significant predictor of depression in women caregivers was perceived entrapment, 50.3% of the variance in depression. Also, perceived entrapment was predictor of subjective well-being in women caregivers, explaining 41.4% of the variance in depression. Conclusion: This study showed that perceived entrapment is an important predictor for depression and subjective well-being. Therefore, in order to reduce depression in women caregivers, it is necessary to design an intervention program that helps with coping and reduces perceived entrapment.
Purpose: The purpose of the study was to understand and describe the overcoming experiences of family members caring for elderly patients with dementia at home. Methods: Data came from autobiographies on the overcoming experiences of caregiving from 31 participants, who had submitted the autobiographies to a public contest held by the Seoul Metropolitan Center for Dementia in 2012. Data were analyzed using qualitative content analysis. Results: Four overcoming stages emerged from the analysis: confronting stage; challenging stage; integrating stage; and transcendental stage, representing transformation of experiences from frustration and suffering to happiness and new hope in life. The confronting stage illustrates severe negative feelings and exhaustion occurring after the diagnosis of dementia. The challenging stage signifies major driving forces in taking good care of their patients. It includes tender loving memories about the patients as well as family and social supports. The integrating stage shows genuine empathy for the patients' situation and the happiness of 'here and now', while the transcendental stage represents new hope in the future. Conclusion: Health professionals need to support caregivers to find true meaning of caring and happiness in everyday life, while providing specific information on dementia care and relieving various negative feelings.
Objectives: The purpose of this study was to investigate the present conditions and problems of oral health care in senior citizens with dementia using a qualitative research method, through focused group interviews. Methods: Data was collected for approximately one month from May 2019. The subjects were divided into two groups: care workers and family caregivers. Fifteen participants were included in the study. Results: In-depth interviews with the care workers revealed the following three categories: characteristics of senile dementia patients, oral health care in senile dementia patients, and oral health care education. In-depth interviews with the family caregivers revealed the following four categories: characteristics of senile dementia patients, oral health care in senile dementia patients, oral health care education, and burden of care. The central themes common to both the care workers and family caregivers were the challenges owing to the characteristics of senile dementia patients, poor health condition of the senile dementia patients, difficulty in oral health care of the senile dementia patients, the desire to receive oral health care education and related information, and to access the information more easily. Additional central themes specific to the care workers were, the applicability of the intervention programs, variability between the facilities, and the problems of oral health care education. An additional central theme specific to the family caregivers was the burden of care. Conclusions: It is necessary to provide oral health care education and information to care workers and family caregivers of senile dementia patients, and to manage and support the dental health professionals ready to care for senile dementia patients. In addition, support to the family caregivers should not be limited only to the financial aspects, but also consider the psychological and emotional difficulties.
The focus of this study was on the self-differentiation level of families with an elderly member suffering from dementia. Based on 340 questionnaires collected in Seoul and Gyeonggi, the study intended to explain the adaptation issues of those families. The variables of interest, which might be related to the self-differentiation, were chronic anxiety(i.e. stress), the family function, psychosomatic symptoms and the family's adaptation to care-giving. As a result of analyzing the effects of the variables potentially related to the self-differentiation level of care-giving families, the findings from this study were as follow. With decreasing self-differentiation level of the family caring for an elderly member suffering from dementia, the levels of stress and psychosomatic symptoms were significantly increased, but the family function and the adaptation to care-giving tended to decrease. Conversely, with increasing self-differentiation, the levels of stress and psychosomatic symptoms were significantly decreased, while the family function and adaptation to care-giving were promoted.
