Stellate Ganglion Block and Electrical Acupuncture for Treatment of Allergic Rhinitis (성상신경절 차단과 전기침 자극에 의한 알레르기성 비염의 치료)

  • Allergy is one of the most common intractable disease, and also allergic rhinitis is the common symptom of the allergic diseases encountered in the otolaryngologic department. There are number of treatment such as: allergen avoidance by environmental control, pharmacotherapy, allergen immunotherapy and more. But choosing the exact reguired treatment is very difficult and troublesome. During the past 3 years, we have provided treatments to 49 allergic rhinitis patients with the combined therapy of stellate ganglion block(SGB) and eletrical acupuncture(E-AP) with very good results. From our experiences we highly recommend the combined therapy with SGB and E-AP for allergic rhinitis.

  • 스트레스로 인한 인체의 반응은 중추신경계, 시상하부, 변연계 및 기타 표적기관으로부터 시작되는데, 자율신경계 반응, 내분비계 반응, 면역계 반응 등을 통하여 복합적인 신체 증상으로 발현되며, 스트레스 상황에서는 시상하부-뇌하수체-부신 축 (HPA axis)과 교감신경계의 작용으로 여러 신경전달물질 방출에 변화가 생기며 이러한 변화는 면역기전에 중요한 역할을 하고 일부는 면역세포의 활성에 직접적으로 영향을 미쳐 신체 각종 질병의 원인이 될 것으로 추정된다. 한의학에서는 천인상응(天人相應)의 관점에서 육기(六氣)를 생체자극의 외적 요인으로 간주하고, 생체내적 현상인 정신이 외적 자극을 통하여 나타나는 생체반응을 칠정(七情)으로 보았으며, 이러한 관점에서 스트레스는 신체에 오장(五臟)의 허실(虛實), 혈허(血虛), 정손(精損), 기역(氣逆), 기(氣)의 순환장애, 담연(痰涎), 화(火) 등의 병적인 요인을 만들어 준다. 본 연구에서 재료로 사용된 호초 (Piper nigrum Linne)는 후추나무의 과실을 말린 것으로서 세계적으로 널리 사용되는 향신료이며, 한의학에서는 온중제한하기(溫中除寒下氣), 쾌격소담(快膈消痰), 해독(解毒)등의 효능으로 한담식적(寒痰食積) 완복냉통(脘腹冷通) 곽란(癨亂) 토사(吐瀉)등의 치료에 활용되어 왔다. 특히 쾌격소담(快膈消痰)하는 작용은 정신적 스트레스에 유효할 것으로 생각되므로 본 연구에 이용하게 되었다. 실험동물은 ICR계 생쥐를 이용하였으며, 심리적 스트레스는 옆쪽 cage에서 다른 마우스의 신체에 가해지는 전기 충격을 하루 1시간 동안 지켜보게 하는 것으로 유발하였으며, 이 상태에서 약물을 투여한 그룹을 실험군, 그렇지 않은 그룹을 대조군으로 하였다. 정상군은 아무런 자극 없이 하루 1시간 동안 일정 공간에 가두어 두는 것으로 하였다. 실험 결과, 호초(胡椒) 추출물을 100mg/kg/day 용량으로 5일간 투여한 실험군은 아무런 처치를 하지 않은 대조군에 비해 혈장 중 corticosterone 함량이 유의하게 감소되었고, 뇌에서의 noradrenalin 분비량이 유의하게 증가되었으며, plus maze test에서의 머무름 시간이 연장되는 것으로 나타나 호초(胡椒)가 심리적 스트레스를 효과적으로 억제하고 진정작용이 있는 것으로 사료되나 구체적인 작용기전 및 인체에서의 효과에 대해서는 향후의 보다 자세한 연구가 필요할 것으로 생각된다.

Effects of Functional Electrical Stimulation Gait Training with Rhythmic Auditory Stimulation on Balance and Gait Ability of Stroke Patients (리듬청각자극을 동반한 기능적 전기자극 보행훈련이 뇌졸중 환자의 균형과 보행능력에 미치는 영향)

  • PURPOSE: The present study was conducted to investigate the effects of functional electrical stimulation gait training with rhythmic auditory stimulation on balance and gait ability in stroke patients. METHODS: In this blinded randomized controlled study, 26 stroke patients were assigned to either experimental group (n=13) consisting of 30 min of gait training 5 days per week for 4 weeks while performing functional electrical stimulation gait training with rhythmic auditory simulation, or a control group (n=13) performing the same gait training program, also consisting of 30 minutes 5 days a week and lasting for 4 weeks, but without functional electrical stimulation and rhythmic auditory stimulation. At baseline and after the 4 week intervention, balance was measured using the timed up and go test (TUG). Gait velocity was measured using the 10-meter walk test (10 MWT) and gait ability was assessed using the functional gait assessment (FGA). RESULTS: After the intervention, the experimental group showed statistically significant differences in gait velocity and ability (10 MWT, FGA) (p<.05). Between-group differences were statistically significant in gait velocity and ability (10 MWT, FGA) (p<.05). CONCLUSION: The findings suggest that functional electrical stimulation gait training with a rhythmic auditory stimulation gait training program may help improve gait ability in stroke patients.

  • Carbondioxide($CO_2$) laser is one of the most widely employed lasers in oral soft tissue surgery because of its excellent affinity for water based soft tissues. It has some inherent advantages such as hemostasis, less postoperative swelling, reduction of bacterial population at surgical site, less need for suturing, less scarring, and less postoperative pain compared to conventional surgical therapies including the use of scalpel, diathermy, cryotherapy and electrosurgery. A 30-years-old male was presented with gingival swelling. Clinical examination revealed a well?defined pedunculated fibrotic mass on the buccal gingiva near right maxillary 1st molar. In radiographic examination, no remarkable abnormality was seen. Excisional biopsy was performed with $CO_2$ laser (continuous wave mode, 4.0W). Histological diagnosis was "Irritation fibroma". $CO_2$ laser has advantages those are suitable for surgical treatment of intraoral lesion. If appropriate training and experience are provided, the dentist would be able to manage intraoral lesions more efficiently and successfully with the use of the $CO_2$ laser.

  • Attainment of the goals of functional electrical stimulation(FES) applications such as strengthening of muscle, increasing muscular endurance, improvement in joint range of motion or the reduction of spasticity does not insure that patients will be able to produce voluntary muscle contraction sufficient to maintain posture or produce purposeful movements.   • A case of severe complications following intrathecal administration of 45 ml of hypertonic saline solution for the treatment of postherpetic neuralgia was presented. Transient immediate complications included were tachycardia, hypertension, neck stiffness and muscle twitch. Pulmonary edema, paralytic intestinal obstruction, and the cauda equina syndrome including sphincter disorder with atonic urinary bladder developed shortly after the injection. Tenesmus and sensory abnormality around perineum and soles were the longlasting complications.

  • This study was to investigate the effects of a functional electrical stimulation (FES) on temporal parameters (stride length, step width and cadence) of gait patterns in a patient with right hemiparesis. A single-subject reversal (ABAB) design was used. The subject was a 25-year-old male who had foot drop and circumduction gait pattern. An ink foot-print method was used to assess the temporal parameters of gait between the baseline phase and the intervention phase. FES was applied at 8 m walkway, three times a week for 5 weeks. The results showed that stride length was increased by 4.04 cm and that step width was decreased by 3.93 cm in the intervention phase. There was no difference in cadence between the baseline phase and intervention phase.

  • The purpose of this single subject care study was to evaluate the effectiveness of a functional electrical stimulation(FES) treatment program designed to prevent glenohumeral pint stretching and subsequent subluxation and shoulder pain in hemiplegic patients. The subject was a 43-year-old male who had left side hemiparesis with shoulder pain and subluxation on affected side. He received conventional physical therapy and additional FES therapy where two flaccid shoulder muscles, supraspinatus and posterior deltoid. were induced to contract repetitively up to 20 minutes for 2 weeks. As a result. the patient showed improvements in hemiplegic arm function, reduction in subluxation(as indicated by Jig test) and range of motion(SLROM). But it was difficult to generalize. We concluded that the FES program was effective in reducing the severity of shoulder subluxation and pain. Further study must be evaluated its statistical significance.

  • The purpose of this case study was to introduce functional electrical stimulation(FES) for paraplegic patients. FES provides the ability to rise from sitting to standing, maintenance of a standing position, and the ability to walk with a reciprocal gait. Six channels of electrical stimulation are sufficient for synthesis of a simple reciprocal gait pattern in these patients. During the double-stance phase, knee extensor muscles of both knees are stimulated, providing sufficient support for the body. Only one knee extensor muscle group is excited during the single-stance phase. The swing phase of the contralateral lower extremity is accomplished by eliciting the synergic flexor muscle response through electrical stimulation of afferent nerves. The transition from the double-stance phase to the swing phase is controlled by two hand switches used by the therapist or built into the handles of the walking frame for using by the patient. A twenty-five years old male was with a T9/T9 spinal cord injury due to a traffic accident and admitted to Yonsei Rehabilitation Hospital for comprehensive treatment. After 30 days of training using the Parastep(R) he was able to stand for 10 minutes. After 43 days, he was able to walk and at discharged he could walk for 100 meters.

Effects of Transcutaneous Electrical Nerve Stimulation and Laser at Auricular Points on Experimental Cutaneous Pain Threshold (외이에 대한 경피신경 전기자극과 레이저가 실험적 피부 통증역치에 미치는 영향)

  • The purposes of this study were 1) to determine the changes between pre-treatment and post-treatment of four groups of 15 persons each and 2) to compare the effect of conventional transcutaneous electrical nerve stimulation(TENS) and laser at auricular acupuncture points on experimental pain threshold measured at the wrist. Sixty healthy adult men and women(M:32, F:28), aged 20 to 28 years, were assigned randomly to one of four groups. Group 1 received TENS to the appropriate auricular point for wrist pain, Group 2 received laser to the appropriate auricular point for wrist pain, Group 3 received placebo TENS to the appropriate auricular point for wrist pain, Group 4 received no treatment and served as controls. Experimental pain threshold at the wrist was determined with a painful electrical stimulus before and 20 minutes after ear stimulation. Group 1 was the only group that showed a stastically significant increase (p<0.05) in pain threshold after treatment whereas the Group 2,3 and 4 did not. These results suggest that TENS has the capability to higher pain threshold but laser does not.
