  • 개업치과의사들에 있어서 근관치료시 발생되는 가장 골치아픈 문제중 하나가 바로 Endodontic Flare-up이다. 미국 근관치료학회에서는 이러한 문제에 대한 일반 개업의들의 이해를 돕고자 미국내에서 개업하고 있는 일반 치과의사 70,000여명에게 다음 내용의 카세트테잎을 보낸바 있다. 역시 우리들에게도 많은 도움이 될 것같아 소개해 본다.

  • LINGUAL ARCH APPLIANCE는 1908년 Mershon이 처음 창시한 교정장치로 band를 사용하는 교정장치중 가장 간단하면서도 여러가지 장점을 지니고 있다. 이 장치는 특히 성장발육기의 소아교정에 최적하여 치아의 기능을 제한하지 않으며 악골 및 치아의 자연적인 성장발육을 허용하고 자연적인 치아이동을 도모하게 한다. 또한 이 장치의 특징은 지속적인 교정치료보다는 간헐적인 치료를 한다는 사실이다.예를 들면 Angle씨 2 급부정교합 즉 상악전돌의 치료시는 상악에 Libial arch wire, 하악에 Lingual arch wire를 장착하고 여기에 악간고무를 이용하여 2급의 교합상태를 개선한후 수개월간 장치를 제거하여 치아의 자연적인 적응을 허용하고 다시 장치를 장착하여 치료를 계속하는 것이다.

  • 2000년대에 있어서 세계의학계는 인공심장의 완성과 인공신장, 인공 간, 인공수족과 인공골, 관절, 혈관 등의 인공장기이식의 눈부신 연구, 발전에 의하여 인간의 수명은 더욱 연장될 것이며 사망률을 낮추는 치료의학은 한없이 발전하게 될 것이다. 치과의학의 과제는 상실된 치아와 악골, 구강내외 장기의 수복을 위한 인공치아 및 장기이식의 치료요법의 발전이 될 것이다. 인공치아 치료요법의 한가지인 nonsubmerged and one stage operation type인 I.T.I. 임프란트 시스템은 골성접착과 결합이 이루어지는 인공치아 이식술로서 그 단순한 시술과 높은 성공률에 의해 전세계에 각광을 받는 치료방법의 하나이다.

Analysis of Factors that Affect the Follow-up Pattern after Dental Treatment under Conscious Sedation in Children (어린이의 진정법 하 치과치료 후 정기검진에 영향을 미치는 요인 분석)

  • Children need regularly dental check-ups after dental treatment because there are a lot of changes of oral conditions such as tooth exfoliation and eruption, occlusion, and recurrence of dental caries. The purpose of this study is to determine the factors that affect the follow-up pattern after dental treatment under conscious sedation. Patients who were treated under sedation at the department of pediatric dentistry of Ewha Womans University Mokdong Hospital from 2009 to 2013 were included in this study. The final samples comprised of 562 patients that were classified into 4 groups according to the number of visits during 3 years. The chi-square test was used to determine the factors affecting regular follow-up. There were statistically significant differences between each treatment groups and factors such as the change of doctor, overall treatment time, the number of treated teeth under sedation, restorative treatment, minor operation, and preventive treatment (p < 0.05). This study identified factors that affect regular follow-up after dental treatment under conscious sedation. It is recommended to educate the importance of regular check-ups to parents who are less likely to return.

  • Pediatric dentistry is differentiated from other fields of dentistry in that it provides comprehensive dental care for children and adolescents. In early days, pediatric dentistry used to be confined to the caries treatment and extraction of primary teeth However, the practice spectrum of pediatric dentistry has broadened to the orthodontic, surgical, esthetic and preventive treatments A survey that contained questions about practice patterns were mailed to 50 pediatric dentists, and 21 surveys were returned. Results were as follows: 1. The average number of patients per week was 82.4, and the average number of treated teeth per patient was 2.35. 2. Preventive treatments comprised 15.7%, restorative treatments 55.7%, endodontic treatments 15.6%, surgical treatments 10.5%, and orthodontic treatments 2.4%. 3. In restorative treatments, amalgam restoration comprised 3.8%, glass ionomer 5.5%, composite resin 63.0%, and stainless steel crown 27.7%.

  • General anesthesia has been researched and developed in dentistry as on type of management technique to sedate patients who may be uncontrollable or require medical consideration. There has been continuous research into this area, but analysis of large set of patients over a sustained period of time is lacking. Thus, this study analyzes the records of patients who received general anesthesia at the Yonsei University Dental Hospital Department of Pediatric and Clinic for the Disabled. 1. Patient's age ranged from 1 to 66, with under 5 being the largest group with 410 members(38.5%). The study included more men than women, with 695 male members(65.3%). 2. Type of dental procedure performed were as follows(per person) : 5.6 Dental restoration; 2.3 Endodontic treatment of deciduous and primary teeth; 2.5 preformed crowning; and 1.6 extractions. Procedures took an average of 100 minutes. 3. 1022 patients(95.9%) received dental care under general anesthesia once and 43 patients(4.1%) received dental care under general anesthesia two or more times. Dentistry under general anesthesia has the many benefit. However, without appropriate post-treatment care, it is difficult to maintain good oral health. Therefore, it is important to improve the efficiency and safety of general anesthesia through future research.

DENTAL ANXIETY OF SECOND GRADE ELEMENTARY SCHOOL CHILDREN PROJECTED IN THE DRAWING OF DENTAL TREATMENT SITUATION (초등학교 2학년 어린이의 치과치료장면 그림에 투사된 치과불안도)

  • The purpose of this study was to observe dental anxiety of second grade elementary school children projected in the drawing of dental treatment situation. The subjects of study were 213 school children. Questionnaire survey was done regarding their experience of pain and fear during the dental treatment and children's attitude toward the dental treatment and dentist. Drawings were analyzed using criteria modified from Dental Anxiety Scale(DAS) by Sheskin et al. The mean anxiety score was 2.00 and the range was from 0 to 6. The mean anxiety score was 2.82 in untreated children and 1.92 in treated children (P<0.05). The mean anxiety score was 2.00 in boys and 1.99 in girls. The rates of children who showed severe fear and pain of the subjects were 8.2% and 5.6%. The rates of children who showed slightly or definitely negative attitude for dental treatment and dentists of the subjects were 19.9% and 4.6%.
