Relationships between Respiratory Diseases and Safety of Pediatric Dental Sedation (소아의 호흡기 질환과 안전한 치과진정법의 연관성)

  • The safety and success of dental sedation for children depend mainly on respiratory status of patients. A special condition, that is, nasal breathing in supine position with their oral airway blocked by rubber dam, should be considered. Therefore, irrespective of medical consultation, pediatric dentists themselves should do respiratory assessment especially adenotonsillar hypertrophy, nasal obstruction, posterior nasal drainage and airway hypersensitivity. Patients with sinusitis, allergic rhinitis, asthma, snoring and OSAS(obstructive sleep apnea syndrome) can induce the sedation failure and complete management of these can improve the safety of dental sedation.

EFFECTS OF CHILDREN'S TONSIL SIZE ON THE BEHAVIOR DURING PEDIATRIC DENTAL SEDATION (진정법을 이용한 치과치료 시 편도선의 크기가 행동에 미치는 영향)

  • The purpose of this study was to evaluate the effect of children's tonsil size on the behavior during pediatric dental sedation. The sedation records of thirty five children were examined in this study. All the sedations had been performed using chloral hydrate/hydroxyzine, nitrous oxide/oxygen or/and submucosal midazolam, and the subjects were divided into 4 groups according to their tonsil size using Brodsky's tonsil classification. The results were as follows: 1. The patients with enlarged tonsils showed negative behaviors in all behavior rating aspects(sleep, movement, crying, overall behavior) during sedation than those with normal tonsils(p<0.05). 2. Enlarged tonsils were correlated with mouth breathing(p<0.05) but not with snoring and coughing(p>0.05) during sedation. 3. For the patients with enlarged tonsils, significantly higher doses of midazolam were administered during sedation than for those with normal tonsils(p<0.05).

  • Dentists who want to perform sedation must have abilities to control any emergent situation that may occur during treatment procedures. In the 2010 guideline for the use of sedation by dentists, he/she must has valid certifications for basic life support (BLS) and for advanced cardiovascular life support (ACLS). The Korean Association of Cardiopulmonary Resuscitation (KACPR) has made a contract with the American Heart Association for education and certification. From 2004, they have held many courses for BLS and ACLS providers and instructors. The author of this research participated in the above mentioned courses and qualified as a BLS provider, BLS instructor and ACLS provider. The present paper was intended as an introduction to and arrangement of BLS and ACLS courses for health care providers through a year of experience. However, the previous course of the ACLS provider was focused more toward medical doctors. It is necessary to develop a new advanced course for dentists who employ sedation in their medical practices.

  • The aim of this study was to recognize guardians' desire and awareness on sedation, and to guide guardians' satisfaction and cooperation. Followings are the conclusion : 1. Information supply about conscious sedation were restrictive and insufficient. 2. Both first-visit guardians and guardians of sedated patients were interested in decrease of dental fear. 3. Both first-visit guardians and guardians of sedated patients were satisfied with psychological improvement better than behavior improvement. Therefore, decrease of patients'ear and anxiety was essential. 4. Both first-visit guardians and guardians of sedated patients didn't recognize accurate safety of conscious sedation.

  • 근래 환자의 불안을 조절하는 것이 성공적인 치료에 매우 중요하다는 인식이 확산되면서 의과, 치과, 영상의학과, 응급실 및 외래 수술센터 등에서 진정요법에 대한 필요가 현저하게 늘어났다. 응급상황이거나 아니거나 진정요법의 필요성은 점차 증가하고 있고 대한 소아치과학회는 이러한 진정요법에 대한 지침을 출간한 바 있다. 이번 개정은 진정요법에 필요한 적절한 환자감시(monitoring)의 필요에 관한 최신지견을 반영한 것이나 그럼에도 불구하고 이 지침에 충실하였다고 해서 특정환자의 결과를 보장하지는 못한다. 그러 나 이번에 소개하는 진정요법 protocol은 널리 받아들여지는 것으로 부작용 역시 최소화한 것이다. 소아 환자의 진정요법은 원하는 수준의 진정이나 투여 방법과는 상관없이 호흡기능 저하와 환자의 방어 반응이 소실될 수도 있다는 인식하에 작성되었다. 이번 호에서는 최근 2006년에 미국소아과학회와 미국소아치과학회가 공동으로 개정된 지침(Guideline for Monitoring and Management of Pediatric Patients During and After Sedation for Diagnostic and Therapeutic Procedures developed and endorsed by AAP a and AAPD)이 어떻게 변경되는지에 중점적으로 알아보며 새로 변경된 지침에 따라 소아치과의의 진정요 법의 진료와 교육도 영향을 미칠 것이다. 이러한 영향으로 다양한 약 투여방법(Alternative routes of d drug administration)과 다양한 약물의 선택에 대해 알아 보기로 한다.

A Retrospective Statistical Study on Sedation Cases in Department of Pediatric Dentistry at Dankook University Dental Hospital for 5 Years (단국대학교 치과병원 소아치과에서 5년간 시행된 진정법에 대한 후향적 통계 연구)

  • Recently, sedation is routinely used in children who lacked cooperation in pediatric dental clinics. The purpose of this study is to analyze dental treatment cases in department of pediatric dentistry at Dankook University Dental Hospital associating with sedation during 5 years. From January 2011 to December 2015, information about dental treatment cases in department of pediatric dentistry was collected and reviewed. The information gathered included patient gender, age, sedation type, sedative agents, date of operation, and type of procedure. The proportion of treatment cases with conscious sedation for total treatment cases continued to decline until 2014. On the other hand, the incidence of general anesthesia increased gradually. The frequency of midazolam administration and nitrous oxide inhalation sedation increased recently. The use of conscious sedation was significantly decreased when the age of patient was 3 years or older. The frequency of general anesthesia was similar in all age groups. There was a tendency to perform treatment with deeper sedation when the patient was male than female, and when the pulp treatment was performed together with the restorative treatment than not performed. The frequency of re-treatment within 90 days after restoration treatment decreased as the depth of sedation increased.
