Complete denture rehabilitation utilizing digital process: A case report (디지털 방식을 활용한 양악 총의치 수복 증례)

  • Complete dentures are one of the most basic treatment methods for the treatment of edentulous patients. The manufacturing process of traditional complete dentures goes through the steps of taking primary impressions, secondary impressions, jaw relation record, trying in wax denture, and final denture insertion. Multiple visits and complex manufacturing procedures are required, and errors may occur in each step. With the development of digital technology, manufacturing steps have been reduced by introducing digital technology to the denture treatment process. In the process of manufacturing dentures by introducing a digital process, a more precise work is possible using Computer-Aided Design, and it is possible to shorten the period of labor and reduce the number of visits. In this case, the anterior teeth arrangement of the patient's existing dentures was transferred to the final dentures using a digital method. After taking impression, try-in dentures were digitally fabricated and tried in the oral cavity to evaluate their retention in the oral cavity. Final dentures were manufactured by milling process. The number of visits was reduced, satisfactory retention and stability of dentures were obtained, and aesthetic recovery was achieved.

  • Recently, there are cases where anterior esthetic prostheses are fabricated for better esthetics, but biologic, mechanical factors could be overlooked, too focusing on esthetic factor. This leads to changes in neutral zone, dentition, position of tongue and lips, occlusion and anterior guidance causing inaccurate pronunciation. Therefore, consideration of systematic diagnosis and treatment procedure are required. In this case, prosthesis was refabricated through a systematic diagnosis and treatment procedure using four factor (acoustic analysis, esthetic analysis, occlusion, neutral zone) for the patient who complained of inaccurate pronunciation and esthetics of the fixed prosthesis fabricated 10 years ago. Thus, by promoting functional, esthetic recovery, this case report demonstrates satisfying results to both the patient and dentist.

  • For a full-mouth fixed prosthetic treatment of the edentulous patient, it is essential to confirm the proper tooth position and thorough evaluation of the remaining alveolar bone and soft tissue before surgery. CAD-CAM dentistry and guided implant surgery have such advantages of providing simultaneous planning of surgery and prosthetic treatment to ensure pre-knowledge of the treatment. In this clinical case, using the digital technology, digital temporary denture fabrication, esthetic evaluation before fixed prostheses treatment, and guided surgery planning was possible. After the surgery, previously obtained data was used for fabricating fixed temporary prostheses. Definitive zirconia prostheses transferred from the provisional prostheses were fabricated and functionally and esthetically satisfying results were obtained.

  • Periodontal tissue destroyed by inflammation is difficult to achieve regeneration of the tissue and esthetic restorations only by surgical methods. In particular, improvement of esthetics is more difficult if the problem is related to the implant. A 23 year old woman suffered from unesthetic anterior implant prosthesis. According to her dental history, a repeated bone graft and soft tissue graft failed at a local dental clinic. It was needed to resolve the inflammation and to improve the esthetics. A free gingival graft and ridge augmentation accompanied by guided bone regeneration and a vascularized interpositional periosteal connective tissue graft was performed. Instead of implant prosthesis, a conventional fixed bridge was adopted for better esthetic result. The patient was satisfied with the esthetic conventional fixed prosthesis. This case report introduces esthetic rehabilitation of unesthetic implant prosthetics in the maxillary anterior dentition by a combination of surgical and prosthetic approaches.

  • The patient was a 26-year old female who was not satisfied with the aesthetic approach of her maxillary incisors. The treatment goal throughout the process was to manage the risk and recreate the original and youthful appearance of the smile. The biomechanical risk was managed by providing coverage of the exposed dentin and rebuilding the eroded structures with minimal tooth reduction throughout the treatment. Functional risk and prognosis for this patient were both improved by achieving acceptable function. The patient was satisfied with the full-ceramic restorations and the symmetry, harmony gingival architecture.

  • The aesthetic appearance of the incisors is essential for a natural and healthy smile, and it is also an important factor for confidence in interpersonal relationships. If there is crowding or protrusion of the anterior teeth, root canal treatment and excessive tooth removal are often required to resolve it in a short period of time. However, it is difficult to achieve sustainable aesthetic results. In this case, orthodontic treatment will be necessary to preserve the remaining tooth structure as much as possible. Middle-aged and elderly patients are often aware of the need for orthodontic treatment, but they are hesitant to use the traditional orthodontic treatment method using brackets because of the social gaze. In this case, clear aligners can be a good option. So, in this article, a case in which aesthetic needs were resolved with clear aligners alone and another case in which satisfactory results were obtained through restorative treatment following clear aligners application are reported.

  • In case of endodontic treatment and extensive restoration of severe dental caries, trauma, and developmental defect, esthetic restoration of primary incisors and permanent anterior teeth for children and adolescents period is delicate matter for pediatric dentists. Existing restorative methods for anterior teeth have retentive and esthetic limitations for badly damaged teeth, especially for the adolescent anterior teeth. Therefore, the preparative stage for setting the permanent prosthesis as well as the retention and esthetics have to be considered. In this case, esthetic restoration for badly destroyed anterior teeth was tried with fiber-reinforced post and the result was satisfactory.

Prosthetic rehabilitation of an oligodontia patient with atrophic maxilla (위축된 상악골을 가진 부분무치증 환자의 보철수복)

  • Congenital tooth agenesis is the most common developmental dental anomaly, of which oligodontia is defined as the absence of six or more permanent teeth, except the third molars. Tooth agenesis causes malocclusion, alveolar atrophy, aesthetic and psychosocial problems. This clinical report describes a multidisciplinary treatment for a patient diagnosed as oligodontia, who exhibited absence of 14 permanent teeth, atrophy of maxillary alveolar bone, and mandibular protrusion. Restoration space was secured and tooth axis was improved by the extraction of deciduous teeth and orthodontic treatment. However, edge-to-edge bite of posterior teeth and arch dimension discrepancy due to atrophic maxilla was remained. To restore the aesthetics and functionality, implant retained prosthesis was planned. Considering minimal bone grafts, location and number of dental implants and prostheses design were determined. Through the gradual adjustment of provisional restoration, the appropriate centric and eccentric occlusion was reflected into a definitive prosthesis. Currently, stable functional results were attained, however, regular follow up and maintenance care over lifetimes should be performed.
