Full mouth rehabilitation in edentulous patient with unstable mandibular position using flat table treatment dentures and CAD-CAM technology (치료의치와 CAD-CAM 기술을 이용한 불안정한 하악위를 가진 완전 무치악 환자의 치료 증례)

  • For edentulous patients with unstable mandibular movements and abnormal facial features due to condylar fractures and morphological abnormalities, it is important to find a stable mandibular position. In this case, the patient's facial improvement, mandibular movement stability, and denture stability were improved by using flat table treatment dentures. In addition, computer-aided design/manufacturing (CAD-CAM) technology was used in denture fabrication to maintain the vertical dimension and lip support of flat table treatment dentures, we report good results in improving the patient's discomfort.

  • In the oral cavity, the teeth undergo wear and corrosion throughout their lives. Progressive and constant tooth wear is a natural phenomenon of aging, but wear and corrosion due to specific factors are pathological factors. It can cause pathological damage of the occlusal surface, aesthetic problems, dimensional loss and jaw joint disorders. This case is a 26-year-old female patient with general tooth abrasion and erosion on the entire dentition. Diagnostic wax-up was fabricated based on the information including digital facial analysis, physiological stabilization, and evaluation of anterior crown length. Through the digital analysis, the necessary guides for crown lengthening were prepared and the mastication function and esthetics were evaluated by using temporary crowns. Definitive prosthesis was fabricated with the zirconia restorations. The results were satisfactory when they were observed 3 months of follow-up.

The Distribution of Pediatric Patients According to Accessibility to Dental Facilities in Pusan National University Dental Hospital (치과의료기관 접근성에 따른 부산대학교 치과병원 소아치과의 신환 분포)

  • The purpose of this study was to analyze the accessibility to dental facilities and the differences in chief complaints of new patients who visited Pusan National University Dental Hospital. We collected information from electronic medical records for 1,820 new patients. The accessibility was analyzed by measuring the distance from the patient's house to the dental facilities. The distance was categorized into 10 km sections, and the presence or absence of a local pediatric dental clinic within 10 km of the patient's house was also categorized. As the distance between the house and Pusan National University Dental Hospital increased, the proportion of patients who visited for dental caries decreased, and orthodontic treatment increased. Dental caries accounts for 27.9% of less than 10 km and 20.5% over 30 km. Orthodontic treatment accounts for 25.4% within 10 km and 27.3% more than 30 km away. The presence or absence of a local pediatric dental clinic within 10 km did not significantly affect the distribution of chief complaints. This study can be used as basic research data to establish effective treatment measures that can improve the physical and geographical accessibility of patients visiting pediatric dentistry.

  • Recently, an advanced workflow has been introduced to finish implant surgery and prosthetic treatment in one-day. However, Because of 1. Patient's physical condition, 2. Surgical technique, 3. Digital technical limitations, the complete completion of one-day implant treatment is practically difficult. Therefore, this paper proposes a "two-days implant digital workflow" that short-time implant surgery and restores prosthetics the next day. Even though it takes more than one day, this workflow is a realistic implant treatment protocol that can reduce the chair time in the clinic.

  • In edentulous patient, not only mastication and pronunciation but also esthetics are compromised. By restoring an edentulous patient with complete dentures, masticatory function and esthetics can be rehabilitated. The advantages of the dentures using CAD/CAM are that they can be reconstructed ubiquitously with stored data, and the strength and density of CAD/CAM material are clinically acceptable. In this clinical report, interocclusal record was taken in a conventional manner and then two lab processing procedures were performed: conventional method and CAD/CAM method. Two types of processing procedures were compared and evaluated. Fair clinical results were obtained in both ways, satisfying the patient.

  • Black staining of the teeth in children and adolescents does not cause pain or serious illness, but it can be socially debilitating for esthetic reasons. Black staining is easily removed through periodic professional mechanical tooth cleaning and ultrasonic scaling, but it can easily recur within few months. Using essential oil-containing mouth rinses diluted at 50% twice per day could prevent the black staining from returning after it is removed, reducing the need for mechanical treatments and improving esthetics.

  • Application of digital technology in dental treatments can simplify the treatment process and reduce patient discomfort. In regards of digital complete dentures, several commercial systems are available. However, these systems are not being routinely applied in Korea. In this case report, comparative analyses were performed regarding the two sets of complete dentures fabricated in digital and conventional workflow in a single patient. Complete dentures made with digital workflow were able to form an ideal occlusion because customized teeth were used. Compared to that, there were also limitations such as lower retention of mandibular dentures, so we would like to report this.

  • Interim immediate denture is fabricated to minimize the period of edentulousness after removal of the patient's remaining teeth and before delivery of final prosthesis. In the case of using the CAD/CAM system, there is an advantage in that the manufacturing process in the clinic and laboratory can be simplified by overcoming some of the limitations of manufacturing interim immediate dentures in the traditional way. However, there are also disadvantages in that errors occur in the process of transmitting information about the patient's intermaxillary relationship to the digital network of the laboratory, resulting in unstable occlusal relationships or non-esthetic prostheses. To overcome this problem, using the simple and accurate POP BOW system's occlusal plane digital transfer method, it was possible to fabricate an esthetic and functional 3D printed interim immediate denture after removal of the remaining upper and lower anterior teeth.

National Patterns and Characteristics in Pediatric Dental Emergency Visits for Dental Conditions (치과를 주소로 응급실에 내원한 소아청소년의 특성과 경향성)

  • The purpose of this study was to investigate pediatric emergency department visits patterns and characteristics of children and adolescents under the age of 20 in South Korea from 2002 to 2015 due to dental conditions. This study used a stratified sample of approximately 1 million people from the Health Insurance Review and Assessment Service Database. The age, region, household income, and treatment cost were included for the patient characteristic analysis. Pediatric patients were compared to the adult group (over 20 years old). In children and adolescents, the rate of emergency department visits due to dental conditions was higher for traumatic conditions than for non-traumatic conditions. Children and adolescents with higher household income visited the emergency department more often than those with lower household income. The region with the highest number of children and adolescents visiting the emergency department for dental conditions was Busan (per 100,000 population). Although this study could not confirm the annual trend of children and adolescents' dental emergency visits due to the sample size limitation, the characteristics of children and adolescents' dental emergency visits were compared with those of adults using a stratified sample.

  • Currently, digital technology is being used in various fields of dental treatment. In particular, in the case of anterior esthetic restoration, the traditional restoration method cannot contain facial information and it is difficult for the patient to predict the treatment result. However, in the case of esthetic restoration through digital design, the visualization of the prosthesis design and the ease of reflecting patient feedback, and expecting the treatment result is available. In this case, the patient confirmed the results of restoration treatment using a digital method before treatment and obtained consent for treatment in an anterior tooth trauma patient. In addition, since the conventional digital smile design method uses only the patient's facial and smile information, the design was made on a two-dimensional plane, and its application was somewhat limited. However, in this case, a three-dimensional virtual patient was created and thus the designed restoration was viewed from various angles. Through this case, it was possible to obtain a high degree of satisfaction with the ease of communication with the patient and the technician during the esthetic restoration using the digital method, the simplicity of the procedure, and the treatment result.
