The purpose of this study is to understand using of antithrombotic agents in clinical settings. The number of patients receiving antithrombotic therapy is increasing in the long term due to the increased prevalence of cardiovascular disease in the elderly. There is a guideline for discontinuation of antithrombotic treatment before dental treatment, but the present study shows that there is a difference from the actual clinical situation. We used an online survey methodology to involve 1000 participants. The questionnaires were asked about general items and Clinical experience such as history of antithrombotic history, experience of complications due to antithrombotics, discontinuation of use, reasons for discontinuing use. The result is as follows. The incidence of delayed bleeding was found to be 55.1%, and discontinuation of antithrombotic therapy before withdrawal was 87.8%. Among respondents 98.3% responded that they stopped antithrombotics because of delayed bleeding and 57.4% responded that they would stop for three to five days. Korean dentists maybe cope with delayed bleeding considering the emotional problem of the patient, the relationship with the patient's physician, and the additional socioeconomic problems.

Ectopic eruption and impaction of canines is a frequently encountered clinical problems. The incidence of impaction ranges between 1-3%. The cause of canine impaction can be the result of localized factors or can be a polygenic multifactorial inheritance and associated with other dental anomalies. The general dentist and pediatric dentist should know how to properly diagnose and manage potential disturbances in the eruption of maxillary canine. Diagnosis of impacted canine at age 8-10 years can significantly reduce serious ramifications, including surgical exposure and orthodontic traction as well as root resorption of the lateral incisors. Extraction of primary canine would be one of the method to prevent the impaction. The surgical procedure should be designed to minimize the destruction of periodontal tissue of impacted canine. Closed eruption technique is thought to be optimal method of surgical exposure compared with other methods. An overview of the incidence, sequela as well as the surgical periodontal, and orthodontic consideration in the management of impacted canine was presented.

Chemotherapy and radiotherapy used on pediatric oncology patients often causes dentofacial anomalies. Defects noted include tooth and root agenesis, root thinning, root shortening, localized enamel defect and maxillofacial underdevelopment. The effect of radiotherapy usually is confined to the radiation site but the effect of chemotherapy may be more wide spread becuase of its systemic distribution. Many pediatric cancers are treated with a combination of radiation and multiagent chemotherapy. Dental treatment affected by chemotherapy and radiation therapy damage to developing teeth and maxilloface includes retention of teeth, space maintenance, prosthetic considerations, requirements for oral hygiene. The lasers have been used in dentistry for more than 30 years and the application of lasers for drilling dental hard tissue has been investigated since the early developement of lasers. Recently, the Er:YAG laser was invented for hard tissue ablation. The Er:YAG laser, having a wavelength of 2.94um, is highly absorbed in both water and hydroxiapatite, leading to a very effective material for hard tissue removal by bursting off the solid tissue component that is, enamel and dentin are removed by the Er :YAG laser by water vaporization and microexplosion, without any melting of inorganic tissues. Therefore, the Er:YAG laser produced round craters with well defined margins and the surrounding tissues had no cracks and no charring. When used for cavity preparation, pulpal damage should not occur if hear buildup is minimized by careful selection of exposure parameters and by use of a water spray. The present study demonstrated that the Er:YAG laser cut the tooth substance adequately for composite resin restoration, without having undesirable side effects such as harmful effects on the pulp, discoloration or cracking etc. Also, the child patients were well cooperative during laser treatment mainly because of little noise, lesser vibration and minimal pain compared to conventional means of cavity preparation.

Pulp canal obliteration(PCO) is seen commonly in dental pulp after traumatic tooth injuries and is recognized clinically as early as 3 monthly after injury. Pulp canal obliteration is characterized by deposition of hard tissue within the root canal space and yellow discoloration of the clinical crown. Opinion differs among practitioners as to whether to treat these cases upon early detection of PCO or to observe them until symptoms or radiographic signs of pulpal necrosis are detected. PCO may make root canal treatment necessary because of the development of apical periodotitis or for cosmetic reasons. If carefully executed, root canal treatment in teeth with an PCO is hightly successful and may act as a basis for internal bleaching. During a game, a 12-year-old girl was hit in the face. At that time, she was diagnosis a subluxation of the maxillary right central incisor. At the 24-month recall examination, a root canal of the tooth had been calcified and discolored gradually. We performed endodontic treatment to prevent perfect pulp canal obliteration and internal bleaching.
This study is designed to present basic materials necessary for offering the more effective way of dental hygienists, or those who are in charge of the education of Oral health, the treatment and prevention of dental problems, educating Oral health by stage by dint of determining the actual condition of the knowledge and management of dental hygiene, or prevention of the basic dental disease except the professional methods of dental management and evaluating their abilities to manage dental hygienes. In terms of the real conditions of the management of dental hygienes, the majority of the subjects said, "not bad" or "healthy". Most of them (62.1%) said that they brush their teeth three times a day, and most of them (85.2%) depend on rolling methods. 69.2% of them used brushes that are neither hard nor soft, and 28.4% of them use soft brushes and 49.7% of them brush their teeth for about 3 minutes. In terms of brushing time, 27.8% of them brush their teeth after lunch, while 23.8% of them brush their teeth after breakfast. 66.3% of them use fluoric tooth pastes while 19.5 percent of them said, "I have no idea." and 14.2% of them said, "No." In terms of complementary dental hygiene goods, dental goggles are used by 23% of them, and dental threads are used by 78.1% and tooth brushes are used by 78.1% of them, and 42.4% of the subjects use the instruments one to twice a day, and 53.8% of them use them for less than one minute a time. In terms of dental health care, 17.2% of them have a chew of gum and 8.3% of them pay a regular visit to dental clinics and 5.3% of them don't smoke.

Porcelain fused to metal crown has been used mostly over the last 50 years for restorations in dentistry. However, the patients' awareness of aesthetic aspect, biocompatibility and the problems such as an allergy to metals led to the growing interest in the 'metal free restoration'. In particular, the price of the precious metals that have been mainly used to date has risen drastically, which made them impossible to play their role as oral restorative materials anymore, and in addition, the PFM restoration has intrinsic problems of chipping and fracture. Therefore, the CAD/CAM has been drawing more attention than ever due to the popular needs for the material that is more aesthetic and stronger for restoration of the molar implant. Considerations in carrying out the full zirconia restoration are as follows: 1) strength, 2) combination work, 3) light penetrability, 4) treatment of cracks, 5) the color reproducibility of the block, 6) the abrasivity of antagonistic tooth, 7) low temperature degradation. In this presentation, the color reproducibility of the block will be discussed. One of the biggest reasons for avoiding the full zirconia restoration is that it is difficult to reproduce the natural color compared to the conventional PFM restoration. Thus, many clinicians show reluctance due to the exposure of the ugly block when the coloring on the surface is removed after occlusal adjustment. From the experience of using blocks by Zirkonzahn for more than 4 years, it is considered that these problems can be addressed to some degrees. Accordingly, how to make restorations that are well in harmony with surrounding prosthesis or natural teeth will be discussed.

The relationship between occlusion and periodontal health has been extensively studied. However, reports on the effects of passive eruption using occlusal reduction has not been sufficient. The purpose of the present randomized clinical trial was to assess the influence of passive eruption using occlusal reduction on the clinical periodontal parameters consisting of attachment level, pocket depth, tooth mobility, width of keratinized gingiva and osseous defect. The study was performed on 40 teeth of 16 subjects who have been treated for the moderate periodontitis at the Department of Periodontology, Pusan National University Hospital. At the baseline examination, after hygienic-phase and after 6 month from passive eruption using occlusal reduction, clinical parameters were monitored and radiographs were taken. The 20 teeth in the test group received passive eruption using occlusal reduction while the 20 control teeth did not receive any occlusal reduction. The results were as follows; 1) Degree of inflammation of periodontium was improved by initial therapy 2) Teeth received passive eruption using occlusal reduction demonstrated significantly greater reduction in pocket depth, tooth mobility and amount of bone loss, and increase in the width of keratinized gingiva, but no significant changes in the attachment level compared to the control teeth 3) There was significantly greater reduction in pocket depth, mobility, amount of bone loss and attachment level in the test teeth after initial hygienic phase when compared with baseline data. Taken together, these results suggest that the passive eruption using occlusal reduction would be helpful to improve periodontal health.

There are a number of situations where the oral mucosa can be sucked or pressed to produce relatively banal but clinical distinctive changes. The labial and buccal mucosa and tongue may develop protuberances in areas where a tooth is missing or extra space is present. The mucosa is pressed and sucked into these spaces, thus leading to the development of a fibrous hyperplasia. This case report describes the management of fibrous hyperplasia in oral mucosa. Fibrous hyperplasia can be formed by habitual pressure or suction in oral mucosa. Treatment of fibrous hyperplasia consists of simple excision and, if feasible, elimination of the cause. And habit control is a important factor for preventing recurrence.

Exfoliation of the primary teeth and eruption of the permanent teeth is a normal and predictable physiologic process. The accurate prediction of tooth development is of major importance in treatment planning for children. Norms that correspond to the child's age and development have been determined by many investigators. However, there are a variety of factors that may hasten, retard, or otherwise affect the normal developmental process. Among the factors that influence tooth eruption is the premature loss of primary teeth. An abscess of a primary molar, followed by bone resorption and early extraction or exfoliation may lead to premature eruprion of the permanent succesor, and a variety of clinical sequelae. I, hereby present the patients who have prematurely erupted permanent successor in Seoul National University Hospital Pediatric Dentistry, and report the clinical and radiologic characteristics, its sequelae, proper managements, and followed up findings.
