국가적 수준의 치과의사 역량을 위한 치과마취과학의 교육역량과 치과의사국가고시 제고

  • 치과마취학의 교육목표는 치과치료 시 적절한 환자평가와 함께 동통 및 불안을 관리하기 위한 약품선택과 치과치료를 위한 흡입법과 정맥주사를 이용한 의식하진정법과 언제, 어떻게, 어디로 전신마취를 의뢰하는지를 알아야 할 사항과 특히 의학적 병력을 가진 환자와 특별한 관리가 필요한 소아 노인 장애인의 전반적 이해와 치료 전 전신적 환자평가를 할 수 있는 능력과 치과진료 중 일어날 수 있는 의료적 응급 상황과 치과응급상황을 식별하고 인공소생술을 포함한 치과 치료 과정에서 발생할 수 있는 의학적 응급상황들에 적절한 관리를 할 수 있어야 한다. 치의학교육의 최소한의 질 보장을 위한 국가적 수준의 치과의사 역량체계를 구축함으로써 치의학교육의 질 향상을 위한 치과대학(원)의 변화를 자극하고, 치과의료와 의료인력의 질적 증진 및 치과의료행위의 최소수준을 보장하는데 있다. 하지만 현행 국가고시에 있어 치과마취과학은 구강외과 과목에 포함되어 위 기술한 치과마취과학의 교육목표를 달성하는 데 어려움이 있다. 그러므로 국가수준의 치의학교육의 질에 대해 제고하고 학교의 교육과정 변화와 개선에 적용하고 국시원의 새로운 과목의 도입과 실기시험을 통하여 치과마취과학의 교육목표를 달성하는 데 목적이 있다.

  • To provide high-quality oral health services, the concept of measurable oral health quality must first be defined. Owing to reports of significant preventable adverse events in the healthcare service provision process, the recognition and consensus of patient safety and patient-centered health service provision was formed. Six areas that are important medical care have been identified, including the quality along with existing concepts such as timeliness, effectiveness, equity, and efficiency of medical services. While there is ongoing research on the quality of oral health, there is not yet any internationally accepted definition. The purpose of this study is to introduce the ideas of oral healthcare quality and quality indicators that have been developed and announced in the field of dentistry.

Analysis of Factors affecting the Patient's Service Satisfaction in Kimhae Dental Hospital (김해 치과 의료기관의 치과진료 만족도 요인분석)

  • In order to meet the medical demand according to the changing medical environment and to provide patients with quality dental treatment and improve treating environment by establishing reasonable management plan, with 149 patients for whom continuous dental health program has been applied, the degree of satisfaction with the dental treatment, and the influence factors on the satisfaction were investigated. Using the SPSS Ver. 13.0, mean value analysis were performed on the satisfactions with dental care service and the quality of dental care service according to general characteristics, and regression analysis were performed on the influence of general characteristics and the degree of satisfaction with dental care service on the satisfaction of dental treatment. The result of the multiple regression analysis revealed the human (staff), environment, and the procedure of treatment as the influence factors of the satisfaction with dental treatment. Systematic management and training should be implemented to improve the quality of dental care service and enhance the satisfaction.

Conditions and Performance of Quality Improvement Activity in Korean Dental Hospitals (치과의료 질 향상활동 현황과 개선활동 만족도)

  • This study aimed to evaluate conditions and performance of quality improvement (QI) activity in Korean dental hospitals. Twenty one representative dental hospitals in Korea were selected by the selection criteria. A questionnaire was implemented to measure awareness, perceived needs, educations, conferences, works, activating/obstructive factors and satisfaction related to QI activities. Satisfaction score range from 0 (lowest) to 5 (highest) among 329 dental hygienists who worked in the selected 21 dental hospitals. Implementation rate of programs on external customer satisfaction was the highest 59.9% in QI activities, and reduction of expenses (59.4%), Improvement in health care quality (58.7), risk management (52.8) were followed, while improvement in work efficiency (46.5) showed the lowest implementation rate. The most influencing factor accelerating QI activities was 'active participation of hospital workers' (54.7%), and 'effort to improve customer satisfaction' (44.7%) followed. The most influencing QI related obstructive factor was 'lack of QI-related manpower and support' (47.1%). A balanced development including QI programs with lower implementation rates is necessary. Encouraging participation of workers may be the most important in developing dental QI activities further.

The relationship between oral health and dental care usage behavior and health-related quality of life (EQ-5D) in women who have experienced pregnancy and childbirth (임신과 출산 경험이 있는 여성의 구강건강 및 치과의료 이용 행태와 건강 관련 삶의 질(EQ-5D)과의 관련성)

  • Objectives: This study analyzed raw data from the 8th National Health and Nutrition Examination Survey (2019-2021) to assess the impact of oral health problems and dental care usage patterns of women who have experienced pregnancy and childbirth. Methods: Out of the 2,389 participants with pregnancy and childbirth experience, 1,301 were included in the final analysis. Multiple regression analysis was conducted using to determine the influencing factors on EQ-5D. Results: Factors influencing EQ5D include general characteristics (age, family income (5 quintile), status of basic living subsistence, education level), oral health problems (chewing problems, complaints of chewing discomfort, speaking problems) (p<0.001). Conclusions: The oral health problems and dental care usage patterns of women who experience pregnancy and childbirth have a significant impact on their health-related quality of life. This study provides basic data for women's ongoing oral care.

Service Quality of Dental Institutes Measured by SERVQUAL Model and Intention to Re-visit (SERVQUAL 모형으로 측정한 일부 치과의원의 서비스 질과 재이용의도)

  • The main purpose of this study was to identify the service quality of some dental clinics located in Gwangju and find an association between service quality and patient's intention to re-visit the same dental clinic. The search objects were 197 patients from five dental clinics, and data collected by self-administered questionnaire which was composed general characteristics, characteristics relating to visit of clinics, quality of service, patients satisfaction and intentions to re-visit. The used statistical analysis to find the association between quality of service and intention to re-visit was multiple regression analysis. This study looked at relations between quality of dental service, service satisfaction and intentions to visit again, and found that quality of service including six service areas had a statistically significant positive correlations with satisfaction and intentions of re-visit. When this study conducted a multiple regression analysis to identify variables that influence intentions of re-visit, it found that patients revisited the dental clinic only when quality of service was included. In conclusion, quality of service evaluated by patients influenced on their intention of re-visit and especially. Therefore, this study concludes that improvement in satisfaction and intentions of re-visit relies on Accessibility, that is, providing of medical information, consultant telephone, how delivery the dental medical system is. Problems to Solve and Job Enlargement on the Inclusion of Dental Hygienists in the Category of Medical Personnel (치과위생사 의료인화에 대한 현안과제와 직무확충 방안)

  • The purpose of this study was to examine the perceptions, precedent tasks, positive and negative effects, and expandable professional tasks regarding the inclusion of dental hygienists (DHs) in the category of medical personnel. This study involved a survey of 259 DHs and 128 dentists. The findings were as follows: 94.2% of DHs and 46.9% of dentists were aware of inclusion in the category of medical personnel; 95.0% of DHs and 64.1% of dentists supported the idea; and 84.9% of DHs and 51.6% of dentists recognized its legitimacy. As for precedent tasks for inclusion in the category of medical personnel, both DHs and dentists scored high points in professional consciousness in the area of occupation. Both DHs and dentists scored the highest points in the quality management of DH education and the lowest points in the unification of school systems in the area of institution. In the area of society, DHs scored high points in the persuasion of the central government, whereas dentists scored high points in collaboration among concerned organizations. Regarding the positive effects of inclusion in the category of medical personnel, both DHs and dentists scored the highest points in the expanded perception of DHs. As for its negative effects, DHs scored high points in the aggravation of salary increase, whereas dentists scored high points in the aggravation of salary increase. Regarding expandable professional tasks after the inclusion of DHs in the category of medical personnel, the management of independent periodontal care programs recorded the highest percentage both in DHs and dentists. These findings highlight the need for adequate discussions about the meanings of the inclusion of DHs in the category of medical personnel and will hopefully contribute to the rational adjustment and legalization of DHs' works with regard to their inclusion in the category of medical personnel.

A convergence study on dental infection management awareness and experience of dental users (치과이용자의 치과 감염관리 인식과 경험에 관한 융복합 연구)

  • The study investigated the perception and experience of infection control targeting dental users. During July-August 2020, 198 adults over the age of 20 were surveyed on general characteristics, infection control awareness and experience, and improvement. Analysis was performed using PASW Statistics ver 18.0. The research results, 91% of dental users recognized that infection control was important. In the recognition of infection control were highly investigated oral treatment equipment sterilization, hand hygiene and glove replacement before and after treatment by dental staff. And dental users was relatively low the replacement of disposable gowns and safety glasses for each patient by medical staff. The dental staff are doing well in personal protection and instrument sterilization. Surface disinfection and water quality management needed improvement. It was meaningful to suggest improvement in infection control based on the perception and experience from the perspective of dental users. It is expected to be used as basic data necessary for high-quality medical services through infection control in dental medical institutions.

Analysis of Impact Factors of Affective Commitment in Organization of Dental Hygienist in Susan Area -Focused on Quality of Working Life(QWL)- (부산권 치과위생사의 정서적 조직몰입 영향 요인 분석 -근로생활의 질을 중심으로-)

  • This research is for dental hygienists to pursue affection and commitment in organization together with improvement of quality of working life by analyzing factors of working life affecting affective commitment in organization of dental hygienists, to find out plans to increase sense of accomplishments of working life and ultimate satisfaction of life, and to suggest direction for human resources management in management of dental clinics. Objects of investigation were dental hygienists working at dental medical institutions in Busan area with questionnaire survey and 197 sheets of questionnaires were used in the study. As the result of the study, high correlation was shown between composition elements of quality of working life and affective commitment per factor and as the result of quality factors of working life affecting affective commitment in organization, work environment, inter-personal relations, and social meaningfulness provided meaningful impacts on affective commitment in organization. It is believed to be important that dental medical institutions need to promote pride and love of companies in the future by providing regular social services or services for local residents so that employees can form smooth inter-personal relationships among employees with improvement of working environment and welfare in personnel management dimension. And for dental hygienists, they shall improve their work performances and enhance vocational values for development of their institutions and them with positive thinking through ceaseless efforts.

Dental implant cost estimation using the Activity-Based Costing approach (활동기준원가(Activity Based Cost)를 적용한 치과 임플란트 원가산정)

  • Purpose: There is a growing concern for the cost management of medical institutions. The purpose of this study was to estimate Activity-Based Costing (ABC) for dental implant cost. ABC refers to allocating resources or cost based on the activities of services. Materials and methods: A dental institution located in the metropolitan area was selected in this study. The tax accounting data of the institution were utilized to confirm total cost, and the institution was asked to make out clinical activities to figure out what activities were carried out. The direct cost and indirect cost for dental implant were separately estimated, and cost driver was analyzed to estimate the indirect cost accurately. Results: The rates of the direct and indirect cost respectively stood at 35.8 and 49.5 percent. The cost for a dental implant was found to be approximately 1,579 won, and the cost of prosthetic surgery and treatment that included implant surgery accounted for the largest portion of the cost, which was 470 thousand won (30%). And the weight of training and education on dentistry was relatively higher than that of the other kinds of treatment. Conclusion: In order to ensure accurate and scientific costing for dental implant, not only direct medical procedure but every pre- and post-procedure activity should fully be taken into account. Pre-activities, post-activities, education and training are included in the indirect cost, but all these activities are mandatory and associated with the quality of treatment and the satisfaction level of patients.
