Factors Related to the Unmet Dental Care Needs of Adults with Dental Pain (구강 통증을 경험한 성인의 미충족 치과의료 관련 요인)

  • The purpose of this study was to examine unmet dental care needs and related factors among adults in Korea. The study included a nationally representative sample of Koreans (Korea National Health and Nutrition Examination Survey 2104). The dependent variable was unmet dental care need and the independent variables were socioeconomic status and oral health status. The chi-square test and logistic regression analysis were performed to identify the associations between explanatory variables and unmet dental care needs. The major causes of unmet dental care needs included economic burden, work life, and academic reasons; in addition, dental treatment was considered to have lower priority. The factors that had statistically significant relationships with unmet dental care needs were sex, age, self-rated oral health status, and difficulty in mastication. The findings of the study, suggest a need for lower dental insurance copayments in keeping with the policies and principles aimed at strengthening the national health insurance system. In addition, groups with limited access to dental services should be identified, and effective health care policies and services should be established for these individuals.

Analysis of the positive word-of-mouth, Quality of medical service and customer satisfaction of patients in dental clinics (치과의원 내원환자들의 긍정적 구전과 치과 의료서비스 품질 및 고객만족도 분석)

  • Dental health care in modern society has gradually been developing with patients'expectation, desire, and medical knowledge according to the changes in the medical industry. Dental marketing strategy is necessary to construct a better dental health environment in order to cope with customers' changes and desire. The purpose of this study was to evaluate the positive comments by word-of-mouth experience, quality of dental clinic service and customer satisfaction among dental patients. The study surveyed 5oo subjects being treated at 6 dental clinics in Seoul and Gyeonggi province. It was conducted from March 22 to April 9, 2010. The subject groups with a positive experience through word - of - mouth who had a perception about the quality of dental clinic service (p<.001) and customer satisfaction (p<.001) were higher than other groups who had no comments on their dental treatment and clinics. The results showed that there was a correlation(r=. 852, p<.001) between the positive comments by word-of- mouthabout the quality of dental clinics service and customer satisfaction. The perception of dental clinic service and patients' satisfaction with positive comments by word of mouth was highly recognized by older and more experienced patients of dental treatment. Therefore, the results suggest that empirical information on important factors about strategic marketing lead to positive comments by word of mouth experience for patients in dental clinics.

Awareness and Purchase of the Private Dental Insurance among Dental Patients in the Capital Region (수도권 지역 치과의료기관 이용자의 민영치과의료보험에 대한 인식과 가입 현황)

  • This study investigated the awareness and purchase status of the private dental insurance. Self-reported survey was conducted with patients over the age of twenties who visited dental institutions located in Seoul Metropolitan City and Gyeonggi Province. The demographic and health related characteristics of respondents were analyzed, and logistic regression was conducted to examine factors affecting the awareness and purchase of private dental insurance. Because only four years had been passed since the introduction of private dental insurance, the awareness and purchase rate was found to be low. However, the number of people considering subscription due to the economic burden of dental care service was relatively high. Factors affecting awareness were satisfaction of the national health insurance, purchase of private health insurance and private dental insurance, self-perceived dental health status, and smoking. The variables affecting purchase of dental insurance were age, awareness, purchase of private health insurance, smoking, number of visits to dental institution. Because qualitative and quantitative change would be made in the dental care utilization due to the rapidly growing dental insurance subscribers, further studies regarding the trend of purchase rate of private dental insurance and the effect of dental insurance on use of dental institution are needed.

Choosing Patterns for Dental Clinic among Community People (지역사회주민의 치과의료기관선택에 관한 연구)

  • This study was conducted to find out the factors affecting community people choice of dental clinic so that the results could be served as basic information to formulate marketing strategies for dental clinic. 238 people in Ulsan metropolitan city were surveyed through a self administered questionnaire. 1. All the Cronbach's ${\alpha}$ values related with dental clinic's internal factor, external factor and access were more than 0.6. 2. The most important factor of dental clinic choice was dentist's faithfulness. 3. On the occasion of choosing dental clinic, internal factor was more important than external factor. 4. The most important factor of dental clinic choice was dental clinic's access on more than 46 years old.

Estimation of lifetime dental expenditures for periodontitis (치주질환 유병자의 생애 치과의료비 추정)

  • Objectives: The objective of this paper was to estimate a South Korean's lifetime dental expenditures for periodontitis. Methods: For our study, we collected data from the Korea Health Panel Survey (KHPS), from 2010 to 2016. The outpatient dental care data of 1,919,608 samples of periodontitis were extracted using R version 3.0 and estimations of lifetime dental expenditures for them were generated using Excel. Results: Over 50% of the lifetime dental expenditure of South Koreans was spent for periodontitis, and incurred after the age of 40. The results showed that an estimate of average per capita lifetime dental expenditure for men (approximately 13 million won) was greater than that for women (approximately 8.8 million won) for periodontitis. Conclusions: Efficient methods for the prevention and management of periodontitis are necessary, and a new paradigm of health care system is required to reduce dental expenditure through its prevention.

Measure of Unmet Dental Care Needs among Korean Adolescent (우리나라 청소년의 미충족 치과의료 수준 측정)

  • The purpose of this study was to examine any possible differences among adolescents in unmet dental care needs according to their demographic and socioeconomic characteristics and oral health status and influential factors on the basis of the fifth national health and nutrition examination survey data. The findings of the study were as follows: Unmet dental care needs were accounted for 19.84 percent. As for the reason, many replied that they couldn't afford to leave school during school hours, and economic factors. As a result of analyzing what affected their unmet dental care needs, this experience was more common in the older age groups than in the 6~12 years age group, and the experience was less common in the respondents with a larger income. As for the subjective oral health status, the experience was 0.61-fold and 0.72-fold less in the respondents who perceived their health as fair or good than who thought their health was poor respectively. And there was a tendency that getting regular dental checkups led to 0.22-fold less experience. Given the above-mentioned findings, the kind of policy that makes dental care services more accessible to youth in consideration of socioeconomic circumstances should be carried out. As many of the adolescents couldn't leave school during school hours, schools should be equipped with dental clinics to improve accessibility to dental care services.

Perceptional Gap Analysis on Business satisfaction between Dentist and Dental Hygienist (치과의사와 치과위생사의 업무만족에 대한 인식 차이분석)

  • The purpose of the study was to explore the perception of dentist and dental hygienist of Interprofessional cooperation, to identify factors influencing dentist and dental hygienist satisfaction with their cooperation. The data were collected with a questionnaire for this study from 6 April, 2009 to 27 April 2009. Data were analyzed using SPSS(Statistical Package for Social Science) WIN 12.0 Program to determine the perception of dentist and dental hygienist of Interprofessional cooperation and predicting factors on their satisfaction. The result shows that dental hygienist is recognized that degree of depression which is caused by dentist's condemnation and by dental hygienist's condemnation rather than dentist. On the other hands, dentist is recognized that the importance of interprofessional cooperation and mutual trust between dentist and dental hygienist.

Satisfaction of the aged with their dental treatments and consideration of the necessity for dental hygienic education about the aged (노인환자의 치과진료만족도 및 노인에 대한 치위생교육의 필요성에 대한 고찰)

  • Objectives : 노인 환자들이 제공받고 있는 치과 의료서비스 만족도를 평가하여 노인 환자에게 양질의 구강보건진료 서비스를 제공하고 노인의 구강보건향상에 기여하기 위하여 실시하였다. 이와 같은 연구목적을 달성하기 위하여 노인관련 일반적 특성을 알아보고, 치과 병 의원을 이용하는 노인 환자의 만족도에 미치는 요인 및 병 의원 선택이유를 조사하였다. Methods : 서울특별시와 경기도, 경상도, 전라도 일부지역의 치과병원 및 치과의원을 이용하는 노인 환자 300명을 편의추출 하였으며, 2009년 4월 1일부터 5월 8일까지 면접 및 자기기입법으로 설문조사를 실시하였다. 설문지는 4개영역으로 구분하여 치과 의료서비스 만족도 22문항, 치과위생사의 의료서비스 만족도 14문항, 재이용 권유의사 4문항 및 일반적 특성과 치과이용에 관한 8문항으로 구성하였다. 노인환자의 일반적인 특성에 따른 치과 의료서비스 만족도, 연구대상자의 병원인지경로, 치과선택이유에 따른 치과의료서비스 만족도는 일원배치분산분석(One-way ANOVA)을 실시하였다. 치과의료서비스 만족도 영향인자, 만족도에 미치는 치과위생사 요인 등은 다중회귀분석을 실시하였다. 연구대상자의 성별과 병원내원기간에 따른 치과선택이유는 카이제곱 (${\chi}^2$) 분포를 따르는 정확검정(exact test)을 실시하였다. Results : 1. 노인환자들이 병원을 선택할 때 가족, 친척으로부터 추천을 받는 경우가 가장 많으며 이 경우 다른 인지경로에 비하여 치과에 대한 만족도가 가장 높았다. 2. 치과선택 이유에 따른 만족도를 비교하였을 때 첫째, 의사의 높은 진료수준, 둘째, 친절한 서비스에 만족도가 높았다. 또한 치과선택이유가 의사의 높은 진료수준과 친절함을 포함한 의사요인이 가장 중요한 것으로 나타났다. 3. 치과위생사의 높은 진료수준과 친절도는 치과의료서비스 만족도에 유의한 차이를 나타내었다. 노인환자들의 만족도를 높이기 위해 노인에 대한 치위생교육이 필요하고 노인에 대한 지식뿐만 아니라 태도에 대한 교육도 실시되어야 한다. 4. 동일한 치과를 계속적으로 다니는 이유는 첫 번째 의사의 높은 진료수준, 두 번째 친절도로 의사요인과 관련이 가장 높은 것으로 나타났다. 또한 3-4년 이상의 장기내원환자는 의사의 높은 진료수준 다음으로 친절도를 중요한 이유로 선택하였다. Conclusions : 이상의 결과로 노인 환자가 만족하는 치과는 치과의사와 치과위생사의 진료수준이 높아야 하겠고, 다음으로 친절한 서비스가 중요함을 알 수 있었다. 또한 노인환자들의 만족도를 높이기 위해 노인에 대한 치위생교육이 필요하고 노인에 대한 지식뿐만 아니라 태도에 대한 교육도 실시되어야 한다.

Some Adults' Opinions about Private Dental Insurance and National Dental Insurance according to Stress of Dental Treatment Cost (일부 성인의 치과진료비 부담에 따른 치과 건강보험 확대 및 민영치과보험 가입 의사)

  • The high dental treatment cost has been considered as one of the major reasons for inappropriate dental treatment in Korea. The aim of this study was to investigate the opinions of expansion of National Health Insurance (NHI) in dental treatment fields not covered with NHI and buying private dental insurance. This survey was carried out targeting 266 adults from their 30s to 50s with convenience sampling. The higher orders in stress of dental treatment cost were orthodontic treatment, dental implant, prosthodontic treatment, and caries treatment not covered with NHI. The higher orders in needs of expansion of NHI were caries treatment not covered with NHI, prosthodontic treatment, and orthodontic treatment. The agreement percentages with age restriction in NHI were scaling 57.1%, denture 23.3%, implant 14.3%, respectively. The subjective oral health had impact on the stress of dental treamtent cost and expansion on NHI. Although only 8.3% subjects had private dental insurance, 68.4% of the subjects had positive opinions buying private dental insurance. The correlation coefficients between stress of dental treatment cost and buying private dental insurance were implant 0.408, caries treatment not covered with NHI 0.404, denture 0.394, crown and bridge 0.375, and orthodontic treatment 0.313. Expansion of NHI in dental treatment and development of private dental insurance was suggested to treat dental disease in appropriate time.

Dental implant cost estimation using the Activity-Based Costing approach (활동기준원가(Activity Based Cost)를 적용한 치과 임플란트 원가산정)

  • Purpose: There is a growing concern for the cost management of medical institutions. The purpose of this study was to estimate Activity-Based Costing (ABC) for dental implant cost. ABC refers to allocating resources or cost based on the activities of services. Materials and methods: A dental institution located in the metropolitan area was selected in this study. The tax accounting data of the institution were utilized to confirm total cost, and the institution was asked to make out clinical activities to figure out what activities were carried out. The direct cost and indirect cost for dental implant were separately estimated, and cost driver was analyzed to estimate the indirect cost accurately. Results: The rates of the direct and indirect cost respectively stood at 35.8 and 49.5 percent. The cost for a dental implant was found to be approximately 1,579 won, and the cost of prosthetic surgery and treatment that included implant surgery accounted for the largest portion of the cost, which was 470 thousand won (30%). And the weight of training and education on dentistry was relatively higher than that of the other kinds of treatment. Conclusion: In order to ensure accurate and scientific costing for dental implant, not only direct medical procedure but every pre- and post-procedure activity should fully be taken into account. Pre-activities, post-activities, education and training are included in the indirect cost, but all these activities are mandatory and associated with the quality of treatment and the satisfaction level of patients.
