Oral rehabilitation with magnetic overdentures for an old patient with complex systemic diseases (복합적인 전신질환이 있는 안면마비 환자의 양악 자성 피개의치 수복 증례)

  • As dental implant treatment becomes popular for both partial and complete edentulous patients, old people with complex systemic diseases also tend to prefer implant-assisted-overdenture or implant-supported-fixed prostheses to conventional complete denture. In this case, 77-year-old female who was on medication for hypertension and osteoporosis and paralyzed on right side due to stroke visited for implant-assisted-overdenture on lower jaw. After oral and radiographic examination, root-assisted magnet overdenture on upper jaw and implant-assisted magnet overdenture on lower jaw are planned. Consequently, overdentures using self-adjusting magnetic attachment(SA) system on both jaws resulted in recovering satisfying function and retention, which is enable to insert and remove with only one hand.

LESCH-NYHAN SYNDROME: A CASE REPORT (Lesch-Nyhan syndrome: 증례보고)

  • Lesch-Nyhan syndrome is a disease caused by metabolic disorder of purine. General muscle stiffness and hyposomia are shown from infancy and symptoms can include involuntary or irregular movements of arms and legs, mental retardation, and compulsive self-mutilating behaviors. Self-mutilating behaviors begin at approximately the first year or sometimes at late teens. The patients bite their lips, especially lower lip, tongue, buccal mucosa, hands and fingers. Tongue and lips can be injured or mutilated in severe cases. As the patient gets older, self-mutilating behaviors become more serious and extensive and secondary infection of injured areas is possible. Periodic soft tissue damage due to self-mutilating may evolve to cancer. Medical treatment, appliance treatment, extraction of tooth and surgical operation was attempted to control self-mutilaing behaviors. We hereby report the case of child Lesch-Nyhan syndrome patient who has self-inflicted labial damage as chief complaint. When patient was treated with conservate therapy, such as removable or fixed appliance, the frequency of labial damage could be subdued and yielded favorable results.

Reestablishing the occlusal plane in full mouth rehabilitation patient, using Shilla system (전악수복환자에서 Shilla system을 이용한 교합평면 재구성 증례)

  • Occlusal plane is a sagittal expression of dental arch form, and it composes the shape of occlusion, which is one of the most important elements of Maxillo-oral system. In this case, vertical, horizontal coordinates of bionic-median-sagittal plane was produced in articulator, and to achieve relation of left and right position of upper, lower teeth and deficits in alveola, Shilla system was used to reconstruct occlusal plane. In this case, a 41 year-old male patient visited for fracture of 10 unit metal-ceramic fixed partial denture of upper anterior teeth and for overall treatment. Clinical, radiographical, model examination was held, full mouth rehabilitation was achieved by placing dental implant. Maxillo-oral relation was recorded using Gothic arch Tracer complex and were mounted. And for the next step, we estimated original occlusal plane using Shilla system. After analysis we produced diagnosis wax pattern. On the basis of this, radiography stent was manufactured and dental implant was placed, and temporary prosthesis was made by using diagnosis wax pattern. Cross mounting and anterior guiding table were performed in order to reproduce temporary restoration morphology and bite pattern, followed by final restoration made of all ceramic crown with zirconia coping. As stated above, appropriately esthetic and functional results can be seen in using Shilla system in diagnosis and treatment procedure of full mouth rehabilitation patient.

TREATMENT OF CLASS I CROWDING WITH EXTRACTION OF THE SECOND PERMANENT MOLAR (제2대구치 발치를 이용한 Class I crowding의 치험례)

  • Since Non-Extraction treatment in some orthodontic case shows unstable result and unfavorable profile, extraction is selected as inevitable treatment option for the harmonious profile, facial skeleton, and the stable dentition on both arches. For the achievement of proper goal, premolars, molars, sometimes incisors or canines are selected to be extracted. The first Premolar is usually extracted for relieving the crowded dentition with which the molar relations are class I to gain stable dentition and proper profile, but often results in the depression of profile or the loss of vortical dimension. On the contrary, the extracton of the second molar helps maintaining the fullness of profile and the vertical dimension, prevents additional space closural procedures which often make the procedures complicated, relieves both anterior and posterior crowding, and substitutes the extraction fossae for newly erupting 3rd molars. From the point of recurrence, the second molar extraction procedure, therefore, is more beneficial. This cases showed the good results of second molar extraction procedures in the patients with class I crowding.

Effect of Presurgical Nasoalveolar Molding in Unilateral Cleft Lip and Palate Infants (편측성 구순구개열 신생아에 대한 술전비치조정형장치의 효과)

  • Cleft lip and palate, the most common craniofacial anomalies, are severe congenital defects that have an incidence of 0.28 to 3.74 per 1000 live births. Although there has been great improvement in the field of cleft surgery, surgical approach cannot be the single solution to resolve the various problems encountered in patients with cleft lip and palate. The concept of presurgical infant orthopedics (PSIO) for gradual closure of the cleft gap and simplified surgical performance was first introduced by McNeil in 1950. Recently, there are many attempts not only to approximate the alveolar segments but also to reshape the nasal cartilage. Three infants with unilateral cleft lip and palate were referred from the department of Plastic Surgery for presurgical nasoalveolar molding (PNAM). Maxillary appliances using resin with orthodontic wire were fabricated. Then these appliance was applied until patients underwent lip surgery. In all cases, the patients could wear the appliance all day since they were able to eat even with the appliance on, This resulted in significant improvements in the nasal symmetry were found. Our appliance, namely K-NAM, extends the wearing time within the limited period and as a result it is expected to maximize the treatment effects. Used properly, this appliance would play a major role in enhancing nasal symmetry with satisfactory results.

The Diagnosis and Treatment of Anterior Openbite Malocclusion (전치부 개방교합의 진단과 치료)

  • There are varieties of severe malocclusions, which can be treated orthodontically, but with a great deal of effort. Anterior openbite, in particular, is one malocclusion thought to be more difficult to treat, and therefore, most of them have to be corrected by means of surgical intervention. To solve these problems, numerous studies pertinent to treatment modalities have been introduced with controversies on the effectiveness of treatment. Suggested treatment modalities for anterior openbite are based directly or indirectly on the neuromuscular and morphological features and on the etiologic and/or the environmental factors. Even though the vertical relationship of the face is increased due to the growth variation, the normal occlusal relationship can be achieved by the adequate dentoalveolar compensatory mechanism, but in the case of inadequate or negative dentoalveolar compensation, openbite is likely to be present. If the skeletal dysplasia is too severe to be solved by orthodontic treatment alone, combined treatment with surgery should be done to restore the function and the esthetics of the orofacial complex. In many cases, however, orthodontic alteration of the dentition pertinent to the given skeletal pattern with the proper diagnosis and treatment planning can bring satisfactory results. The treatment changes with the Multiloop Edgewise Archwire(MEAW) therapy occurred mainly in the dentoalveolar region and showed a considerable similarity to the natural dentoalveolar compensatory mechanism. In other words, the MEAW technique allows orthodontists to produce the natural dentoalveolar compensation orthodontically. Even if an openbite is corrected by the orthodontic dentoalveolar compensation suitable for the skeletal pattern, relapse may still occur by the persisting etiologic factors which originally prohibited the natural dentoalveolar compensation. The etiologic factors should be determined at the time of initial diagnosis and should be controlled during treatment and retention.

THE STUDY OF FRACTURE STRENGTH OF PORCELAIN LAMINATE VENEER WITH VARIOUS LINGUAL EXTENTION LENGTH WHEN LNCISAL RESTORING (Porcelain Laminate veneer 절연 피복시 설측 연장 길이에 따른 파절강도의 연구)

  • The purpose of this study was to evaluate the fracture strength of porcelain laminate veneer with various lingual extention length when incisal restoring. Sixty recently extracted, intact maxillary incisors were used and stored in a physiologic saline solution from the time of extraction. Seating form was preparation at the labial surface of each tooth with a water-cooled round diamond bur. Standard block was formed with 32 gauge and 24 gauge wax at tooth labial and lingual surface. Lingual extention length differed according to each group. (group I : 0.5mm, group II : 1 mm, group III : 2mm, group IV : 0mm) All tooth specimens were impressioned with examix(GC Inc., Japan). Refractory cast were maked with refractory die material(Ceramco Inc., U.S.A.) Laminate porcelain (Ceramco II Veneer porcelain, Ceramco Inc., U.S.A.) was condensed in refractory die cast and baked according to the manufacturer's recommendations. Each surface was contoured with low speed diamond bur according to guide block. All porcelain specimens were sandblasted and ultrasonically cleaned in distlled water for 3 minutes. Then, all porcelain specimen were etched with 8% hydrofluoric acid for 5 minutes. Sixty specimens were bonded with composite resin cement(Choice Porcelain Veneer System, Bisco Inc., U.S.A.) according to manufacture's directions. The fracture loads of the specimen were measured by Instron universal testing machine. The mean values of fracture loads for the groups were statistically compared by Duncan's multiful range test. The result were as follows : Mean fracture strengths of each group were 86.95Mpa in no lingual extention group, 44.98Mpa in 0.5mm lingual extention group, 27.47Mpa in 1mm lingual extention group, 19.61Mpa in 2mm lingual extention group. There was a statistically significant difference between all group(p<0.01).

  • In an intra-alveolar root fracture (IARF) of a primary tooth with severe mobility and displacement, extraction and periodic-follow-up is the choice of recommended treatments because of the fear of aspiration of the mobile tooth and the possibility of damage in the permanent succeeding tooth. However, repositioning and splinting are presented as a fresh proposal recently. In case of extracting a primary incisor, many problems occur; esthetic problems; functional problems such as pronunciation and mastication; space loss; and psychological and social problems. Therefore, the best treatment is conservation of the primary tooth. The aim of this report was to suggest the conservative treatment of an Intra-alveolar root fracture of the primary central incisors with severe mobility and displacement based on two cases that describe the diagnoses, treatments and follow-ups (mean period: 27-month). All cases have been treated by reduction and immobilization by resin wire splint (RWS) (mean period: 6-week). Both cases were followed up until the successors were erupted. There have been no complications such as pain, pulp necrosis, periapical lesion, displacement of permanent tooth germ, eruption disturbance and etc.

  • This study was undertaken to assess the effect of growth on the lips. Not only does lip growth influence the stability of such orthodontic treatment; it also directly influence facial profile, in which the lips have an important part. An understanding of the growth of lips is thus central to a consideration of profile change in orthodontics. By analyzing the serial lateral cephalograms of 15 male and 15 female of 8 years old to 16 yaers old who have normal occlusion. The result of this study were summerized as follows; 1. The largest growth increments in the length of the lips was mod age of 14 in both sexes. 2. The thickness of lips showed lager value for the male than that of the female in the most age group. The lagest growth increments at A point was occured age of 14, while Ls, Li, B point decreased after the age of 10-11. 3. The largest increase in the interval between crest of lower lip and edge of upper incisors was occured between ages 9 and 11 in males. The interval decreased slightly from 8 to 16 years in females. 4. The nasolabial angle decreased slightly from 8 to 16 years in both sexes. 5. The mentolabial angle showed large variation.

Level of perception of changed lip protrusion and asymmetry of the lower facial height (하안면부에서 입술의 돌출 정도와 안면 비대칭의 인지도에 관한 연구)

  • Objective: While one of the most prevailing motivations for seeking orthodontic treatment is to achieve good facial esthetics, understanding the level of a person's perception to the changes that have occurred on the face after orthodontic treatment is critical to the process of orthodontic diagnosis and treatment planning. Methods: 40 students attending art school participated in determining the level of their perception of changed lip position and facial asymmetry. Computer-graphic frontal face and facial profile photographs with balanced proportions were used to evaluate the level of a participant's perception of the changes in facia! asymmetry and in lip position. Results: Change of lip position over 2 mm and over a 3 mm change of facial asymmetry was perceived significantly. Conclusion: The results indicated that at least a 2 mm change of lip position was needed to be perceived after orthodontic treatment. The level of perception of the change in facial asymmetry was lower than that of the change in lip position. Information about facial changes given prior to the evaluation enhanced the level of perception.
