Relationship between intraarch space discrepancy and craniofacial morphology (치열 공간 부조화와 두개안면골격 형태의 상관성)

  • The Purpose of this study was to Investigate the relationship between the space discrepancy of the mandibular dentition and craiofacial morphology in adults with good Angle class I occlusion. Dental casts of normal group, crowded group and spaced group were selected on the basis of dental crowding and spacing. Subjects with excessive space to accomodate the lower teeth were classified as spaced group(39). Subjects with a space discrepancy of more than 4mm were classified as crowded group(45). Normal subjects had little or no dental crowding and spacing(40). Various skeletodental measurements in lateral cephalograms were evaluated and compared by ANOVA, Pearson correlation analysis and multiple stepwise regression analysis. The results were as follows; 1. ANB angle was larger in crowded group than in spaced group. 2. Maxilla and mandible in crowded group were inclined more downward and forward than in spaced group, so crowded group was found to have vortical tendency. 3. Anterior cranial base length and mandibular body length were longer in spaced group than in crowded group. 4. According to the multiple stepwise regression analysis with space discrepancy as dependent variable, 40% of variance of space discrepancy could be explained by ANB angle, anterior facial height and ramus height. Tooth size and arch parameters of normal occlusion in a Iarge Korean sample (한국인 정상교합자의 치아크키와 치열궁 계측치에 관한 연구)

  • The aim of this study was to establish normative data on the tooth size and arch parameters of Koreans with normal occlusion, This study employed the dental casts of 296 (male: 179; female 117) normal occlusion samples, who were selected from 15,836 adults through a community dental health survey. The mesiodistal diameters of teeth, arch width, and arch length were measured by digital electronic calipers(accurate to 0.01 mm) and Bolton's indices as well as intermaxillary arch width ratios were calculated. In order to ensure reliability, intra- and inter-examiner error were evaluated. Although our dimensional data showed overt differences between genders, the indices and intermaxillary ratios evaluated were the same. The resultant data obtained were compared with the previous data to reveal whether any changes have occurred over the time. The clinical implication of the present findings was also discussed. This biometric study seemed to provide a clinically applicable diagnostic criteria for an individual malocclusion patient.

Mandibular Clinical Arch Forms in Koreans with Normal Occlusions (한국인 정상교합자의 하악 치열궁 형태)

  • The purpose of this study was to clarify morphologic characteristics between mandibular clinical arch forms in Koreans with normal occlusions. The study included data from 102 Koreans. The most facial portion of 13 proximal contact areas was digitized from photocopied images of the mandibular dental arches. Clinical bracket points were calculated for each tooth based on mandibular tooth thickness data. Four linear and two proportional measurements were taken. The dental arches were classified into ovoid, square and tapered forms. The frequency distributions of the three mandibular arch form classifications were determined and compared between male and female subjects. No significant differences in arch form size were found between the sexes. However, there were a few differences in molar width. It was useful to classify mandibular clinical arch forms present in normal occlusion samples into ovoid, square and tapered categories. The frequency of the ovoid form was the highest, and that of the square form was the second highest. The tapered arch form was found in less than 10 percent of subjects. No significant differences in their frequency distributions and dimensions were shown between males and females.

Effects on Osteoclast in Periodontal Ligament Space by Denerveation of Inferior Alveolar Nerve in Young and Adult Rats (하치조 신경 절단이 치주인대공간에서 파골세포에 미치는 영향)

  • Osteoclast action is necessary for alveolar bone remodeling in orthodontic tooth movement. The nervous system has also been reported to be associated with bone remodeling. This study was aimed to investigate the changes of osteoclasts in the periodontal ligament (PDL) space after surgical resection of the inferior alveolar nerve (IAN). Experimental rats were divided into young and adult groups. A surgical resection procedure of the IAN was carried out in the left side of the mandible and a sham operation in the right side of the mandible. The number of osteoclasts on the bundle bone surface and the resorption activity of the osteoclasts were histomorphometrically measured. The changes in distribution of substance P (SP) immunoreactive (IR) nerve fiber were evaluated in the PDL and pulp. SP-IR nerve fiber was depleted in both the PDL and pulp of the IAN resection side in both groups, which confirmed the resection of IAN to be successfully conducted. The number of osteoclasts in the IAN resection side was significantly reduced in both the young and adult groups (p<0.01 and p<0.05), whereas the resorption activity of osteoclasts did not show any significant difference between the IAN resection side and the sham operation side in both groups (p>0.05 and p<0.05). The adult group showed that the number of osteoclasts reduced significantly (p<0.01) and the resorption activity didn't change in comparison with the young group (p>0.05). These results suggest that surgical resection of the IAN and aging reduce the population of the recruited osteoclasts within the PDL, but don't affect on the osteoclastic resorption activity.

Effects of nicotine on the formation of osteocalcin and osteoprotegerin and synthesis of its mRNA in MG63 osteoblast-like cell (니코틴이MG63 조골세포주의 오스테오칼신과오스테오프로 테제린의 생성 및 mRNA 발현에 미치는 영향)

  • The purpose of this study was to evaluate the correlation between nicotine and the activity of bone forming cell. MG63 osteoblast-like cells were used for this study. Several factors were examined including the proliferation of cell, alkaline phosphatase activity, the formation of osteocalcin and osteoprotegerin. and the synthesis of its mRNA. MG63 osteoblast-like cells were incubated for 1, 2, 3 and 6 days with nicotine added to the culture medium in 1.0 ${\mu}M$, 1.0mM, 2.5mM, 5.0mM, 7.5mM, and 10.0mM concentrations. The proliferation of MG63 osteoblast-like cells was temporarily activated at the low nicotine concentrations. At high concentrations (>5.0 mM), however. it was suppressed. Alkaline phosphatase activity was suppressed in a dose-dependent manner as the concentration of nicotine increased. Osteocalcin decreased in a dose-dependent manner at high nicotine concentrations of more than 7.5mM and the same result was show when the osteoblasts were treated with low concentrations for longer than 3 days. There was a difference in the influence of nicotine on the synthesis of osteocalcin mRNA and formation of osteocalcin itself at 1 and 3 days. Generally, osteoprotegrin notably declined in all experimental groups. However, the level of its mRNA increased at high nicotine concentrations of more than 7.5mM after 3 days and more than 5.0mM after 6days.

The effect of NaF on bone and tooth resorption around an anchor tooth during a rapid maxillary expansion procedure (급속상악확대술 시행 후 지대치와 지지골 표면에 나타나는 치근흡수의 불화나트륨 단독투여를 통한 예방에 대하여)

  • This study was undertaken to determine the effect of a 2.2mg/Kg/day intraoral administration of NaF on the amount of root resorption and osteoclastic activity during or after a rapid maxillary expansion procedure. Ten puerile female dogs were divided into two groups: a control group and a NaF-treated group. A fixed type maxillary expansion device was delivered to all dogs. The appliance was activated twice daily throughout a 20-day period, causing a 5-mm expansion of maxillary bone. After the expansion procedure, the animals were sacrificed at days 0, 15, 30, 45, and 60 of the retention period. The buccal surface of the root of each maxillary canine was examined by means of a surface electron microscope (SEM). Using SEM, web-like resorption lacunae were observed on the bone or the tooth surface at the site of osteoclastic activity; these observations were verified by histological methods. No peculiar resorption lacunae were found in the apical tip of the roots of either the control group or the NaF-treated group animals. The NaF-treated retention group was found to have less resorption lacunae formation on day 45 and day 60. The preventative effect of NaF on resorption lacunae formation on the surface of the bone covering the anchor tooth was confirmed. Larger areas of resorption lacunae were found on the surface of the bone covering the canines in the control group animals, as compared to those of the NaF-treated group, especially on day 30 and day 60. Using SEM, the present study revealed a difference between the control group and the NaF-treated group in the prevalence and the size of the resorption lacunae formation on the cemental root surface. The preventative effect of NaF on bone resorption was confirmed. Further studies concerned with the optimum concentration of NaF that has an effect in vivo are necessary.

Comparison of histologic observation and insertional and removal torque values between titanium grade 2 and 4 microimplants (Grade 2, 4 티타늄 마이크로 임플랜트의 식립 및 제거 토크와 식립 후 조직학적 반응의 비교)

  • The purpose of this study was to evaluate the light microscopic features and the maximum insertional and removal torque value of microimplants, made from titanium grade 2 or 4, in the tibia of 6 rabbits. First, the maximum torque values of microimplants at implantation were measured. After 2, 8, and 12 weeks of healing time, the microimplant-containing segments of tibia of 2 rabbits were removed and the maximum removal torque of each microimplant were measured. Comparisons of histologic examination and insertional and removal torque values were carried out for the two groups of microimplants. Removal torque values were significantly increased in both groups after 8 and 12 weeks as compared to 2 weeks after implantation. Other values measured did not show any statistically significant differences and there were no histological differences between grade 2 and 4 titanium. Based on these results, this study showed that there were no significant differences between grade 2 and 4 titanium. It seems better to use grade 4 titanium for making microimplants because grade 4 titanium is mechanically harder than grade 2 titanium and has similar retention.

A study on the correlations between facial biotype, submentovertex cephalometric measurements and surface EMG activity in patients with facial asymmetry (안면 비대칭 환자의 수직 안모 유형과 이하두정 두부방사선 규격사진 계측치 및 표면 근전도의 상관성에 대한 연구)

  • Although the submentovertex radiograph and surface EMG are not often used due to the difficulty of interpretation, they are accepted as useful diagnostic and analytic aids for skeletal asymmetry. There have been reports which state that they were also useful for the evaluation of vertical skeletal relations. The purpose of this study was to evaluate the correlations between EMG data, measurements from submentovertex radiographs, facial types and facial asymmetry following examination of 60 asymmetric patients. The radiographic corpus length were greater in the nonaffected sides (p<0.001), gonion to interspinosum axis were greater in the affected sides and the mandibular condyle and gonion were located more anteriorly in the non-affected sides than in the affected sides but not significant (p=0.07). The activity of the anterior temporal muscle in rest position was higher in the affected sides than in the non-affected sides (p<0.01). The activity of the masseter muscle at maximum clenching was found to be nonsignificant but it was higher in the affected sides than in the non-affected sides (p=0.09). There was positive correlation between facial index and the intercondylar axes angle (p<0.01). There was positive correlation between masseter muscle activity in maximum occlusion and facial index in the affected and non-affected sides (p<0.05). The results demonstrate that the submentovertex radiograph and EMG can provide useful information for the evaluation of horizontal and vertical skeletal relations.

A cephalometric and dental cast study of obstructive sleep apnea patients (폐쇄성 수면 무호흡 환자의 측모 두부방사선계측사진 및 치아모형 연구)

  • To evaluate the cephalometric and dental characteristics of obstructive sleep apnea (OSA) patients, 23 OSA patients and 15 control, non-OSA, patients who visited the Sleep Disorder Clinic Center, Keimyung University were investigated. Patients who suffered from apnea-hypopnea episodes over 10times per hour were diagnosed as having OSA after polysomnograph testing, Impressions were taken with alginate. Cephalometric radiographs were taken at maximum intercuspation. The dental cast measurements, including transpalatal width, intercanine width, intermolar width and palatal depth did not differ between the control and OAS groups and did not have a positive correlation with the apnea-hypopnea index (AHI). Upper airway width was statistically narrower than the control group. Upper airway width had a low negative correlation with AHI, but, lower airway width had a low positive correlation, and, the higher the AHI score, the longer the mandibular border to hyoid distance.

A RADIOGRAPHIC STUDY ON THE MANDIBULAR CONDYLE POSITION IN KOREAN MALOCCLUSION (부정교합자의 하악과두 위치에 관한 방사선적 연구)

  • Temporomandibular joint is a major structure to play an important role in the function & stability of the occlusion as well as the stomatognathic system. Therefore, the TMJ is the structure that requires the complete analysis for diagnosing and planning treatment of pathologic changes by TMJ dysfunction and malocclusion. So, in this study, to evaluate TMJ situation in Korean malocclusion, based on the previous accomplishments, students of the dental college of Won-Kwang Univ. are surveyed and selected in terms of Angle's classification of malocclusion, whose TMJ radiographs were taken in the centric occlusion and centric relation. In each maiocclusion groups, the mean and standard deviation of anterior, posterior and superior joint space of the right, left and both side in CO & CR are evaluated and also those of the fossa height and the articular eminence angle of the right, left and both sides are evaluated. The obtained results were as follows: 1. In the correlation coefficient between the malocclusion groups, no other items except the posterior joint space of the right side in CR between in class I and class III are significant. 2. In the correlation coefficient between the right and left side, the each joint space in class I malocclusion group and class II malocclusion group are significant. 3. In the change of each joint space during the transmit from CO to CR, there is a tendency of increasing anterior joint space and decreasing posterior, superior joint spaces in class I, II malocclusion and increasing superior joint space and decreasing anterior, posterior joint space in class III malocclusion, which is significant in the correlation coefficient, but not significant in the T-test. 4. In each malocclusion group, the correlation coefficient between the posterior joint space and the superior joint space in C.R is highly significant. 5. The fossa height of class II malocclusion group is lesser than that of class I or class III, which is not significant in T-test. 6. In the correlation coefficient between Rt. and Lt. side in the fossa height, it is not significant in class I and class III group, but significant in class II malocclusion group. 7. The articular eminence angle of class II malocclusion group is larger then that of class I or class III groups, which is fairly significant. 8. In the correlation coefficient between Rt. and Lt. side in the articular eminence angle, it is significant in each malocclusion group.
