  • Purpose : Bartter syndrome is a renal tubular defect in electrolyte transport characterized by hypokalemia, metabolic alkalosis and other clinical signs and symptoms. The aims of this study were to analyze the clinical manifestations and the short- and long-term outcomes of Bartter syndrome. Methods : We retrospectively reviewed clinical history, laboratory finding of blood and urine, renal ultrasonography, and hearing tests of five patients who were diagnosed and managed with Bartter syndrome at Asan Medical Center from April 1992 to May 2007. We also evaluated height and body weight periodically after institution of therapy. Results : All patients had poor oral intake, failure to thrive and polyuria. Three of them had maternal history of polyhydramnios and premature delivery. The mean age at diagnosis was 11.8 months. All children presented with hypokalemia, metabolic alkalosis, hyperreninemia. Their blood pressures were normal. One patient had nephrocalcinosis on renal ultrasonography and all of them had normal result in hearing tests. After treatment with indomethacin or other prostaglandin inhibitors and potassium supplementation, their clinical features improved with catch-up growth and improvement in the development during long-term follow-up. Conclusion : We emphasize that early diagnosis and proper treatment in patient with Bartter syndrome are related to better prognosis.

  • The authors conducted a cross-setional study to evaluate the differences of screening test results between clerks and laborers in an occupational health center in Taegu, 1992. A total of 10, 207 workers was included in the study. Of these, male were 6,597 and female 3,610. Constitutional variables included were items of health examination and some confounding variables(sex, age, body weight and work duration). All analyses were conducted separately for each sex through the use of multiple logistic regression analysis on occupation, controlling for age, work duration, and body weight. Laborers showed abnormal hearing test more often than clerks in both sexes. The blood pressure that showed statistical significance in univariate analysis in both sexes lost its significance after controlling covariates. Liver function and urine protein had statistically significant differences between two occupational groups in males and visual correction did in females. The results suggest that there are differences clerks and laborers in some of screening tests and analytic studies are needed to identify the causes of the differences.

  • It is well known that speech signals are the most riliable materials for the hearing test and there are various difficult problems in the selection of these materials. Because of these difficulties, there is not a confirmed test material of Korean speech sound up to date. For the basis of the test materials, author had studied articulation scores and confusion patterns of 100 monosyllable korean speech sounds in normal listners, in normal listners under various noisy (white noise and speech noise) circumstances, and in patients with hearing loss, The results reveal as follows. 1. Except for perceptive deafness with poor articulation score, Confusion was occured among initial consonants, vowels and final consonants respectively according to their distinctive features under above various test conditions. 2. There is remarkable differences in articulation scores between different kindes of noise under some intensity levels.

  • This study aimed to examine the types and characteristics of otorhinolaryngology examinations performed by clinical laboratory technologists and to investigate the working environment and scope of work of otorhinolaryngology laboratory personnel using online questionnaires. Based on the research results, constructive policy measures for the University-Association-Society were also presented. Most of the survey respondents were in their 40s (34.1%). Females accounted for 80.2% of respondents and 30.8% of them had a career spanning 15 years or more. We found that laboratory personnel had a wide scope of work, high work stress and frequently suffered occupational diseases. We observed that, to reduce stress and increase satisfaction, an expansion of the workforce was necessary. Compared to other occupations, 72% of clinical laboratory technologists occupied more senior positions, occupational distribution depended on senior positions (P<0.001). Clinical laboratory technologists performed about 26 types of otorhinolaryngology examinations. The most frequent test performed daily was pure tone audiometry, and polysomnography took the longest test time, with an average of 8 hours. In conclusion, clinical laboratory technologists were in charge of various specialized otorhinolaryngology examinations. Considering the importance of clinical laboratory technologists in otorhinolaryngology, the University-Association-Society should put in additional effort into nurturing otorhinolaryngology examination experts.

  • As using recruitment phenomenon, SISI test and Bekesy audiometry are employed to differentially diagnose both the cochlear hearing loss and the retrocochlear hearing loss. The main purpose of this study is to find out the sensitivity and specificity of the SISI test within the scope of 1 KHz to 4 KHz in diagnosing various types of hearing losses. In this study, the Bekesy audiometry was conducted for Bekesy audiograms of certain groups according to Jerger's classification, which included persons with normal hearing and patients with hearing impairment. Subject persons of the SISI test were categorized into the several groups with normal hearing, mild hearing loss, moderate hearing loss, moderate-severe hearing loss and severe hearing loss, respectively. The sensitivity and specificity of certain groups were evaluated according to the SISI scores of 20 % below, 25 ${\sim}$ 65 %, and more than 70%. Also, the Bekesy audiometry is conducted on the several subject groups with normal hearing, mixed hearing loss, and sensori-neural hearing loss.

  • Speech discrimination is affected by the speed of speech. The speed of speech can be adjusted at the pause duration, the pause duration can take the resting time to avoid in overloading information. The study will be examine the effects of aging and audiological rehabilitation, and the auditory processing as basic research to investigate the normative data. 7 boys and 8 girls were participated. They have no problem with speech language pathologically and audiologically. There are 4 sets of test implement, and each test set was made out with 20 3-syllable words. Pause duration of all of these words are adjusted in normal(250 ms), slow(500 ms) and very slow(1000 ms). There are 4 words for a multiple-choice that including one word with written correctly and three words with written 1 phoneme wrong. Participant hear the word, and then have to choose one. Speech discrimination score in 250, 500, 1,000 ms of pause duration were $73{\pm}19.4%$, $84{\pm}12.2%$, $88{\pm}8.8%$, respectively.

  • Speech consists of the sum of complex sine-waves. This study investigated the perception of electrical hearing by applying the sinusoidal model to cochlear implant simulation. Fourteen adults with normal hearing participated in this study. The sentence recognition tests were implemented using the sentence lists processed by the sinusoidal model which extracts 2, 4, 6, 8 sine-wave components and sentence lists processed by the same sinusoidal model along with cochlear implant simulation (8 channel vocoders). The results showed lower speech recognition for the sentence lists processed by the sinusoidal model and cochlear implant simulation compared to those by the sinusoidal model alone. Notably, the lower the number of sine-wave components (2), the larger the difference was. This study provides the perceptual pattern of sine-wave speech for electrical hearing by cochlear implant listeners, and basic data for development of speech processing algorithms in cochlear implants.

  • Recently, a method for detecting animal tinnitus objectively, gap prepulse inhibition of acoustic startling (GPIAS) were reported. However, the GPIAS method is difficult to be directly applied to human tinnitus research because measuring repeatedly startling responses evoked by more than 110 dB SPL acoustic stimuli for human is not easy. In this paper, the auditory late latency response (ALLR) measurement system which can measure conveniently evoked potentials involving the information about the brain cortical activity related with auditory psychologic phenomena such as a tinnitus has been implemented. By using the implemented system, 8 persons with normal hearing sense have been experimented to measure N1-P2 amplitudes of ALLRs evoked by gap prepulse based acoustic stimuli. Through the experimental results, the prepulse inhibitions of all the participants' N1-P2 responses have been observed and their characteristics were evaluated. And it is verified that our implemented system can be utilized as a device for researching and evaluating the objective tinnitus detection method in the future study.
