The Effect of Topical Application of Lidocaine Cream before Arteriovenous(AV) Fistula Puncture on Pain and Anxiety Among Hemodialysis Patients (리도카인 연고도포가 혈액투석 환자의 동·정맥루 천자 통증과 불안에 미치는 효과)

  • Purpose: The purpose of this study was to examine the effect of topical lidocaine cream on pain and anxiety during the AV fistula puncture among hemodialysis patients. Methods: The study employed one group repeated measurement design. The data were collected from 50 hemodialysis patients who received AV fistula puncture. The topical lidocaine cream was applied 30 minutes before the puncture. The data were measured total 3 times (T1=without lidocaine, T2=2% lidocaine, T3=5% lidocaine). Pain was measured by VAS and a behavioral checklist. Anxiety was measured by Korean manual of SCL-90-R. Results: Patients with 5% lidocaine cream reported significantly lower of VAS pain score than those with 2% lidocaine and without lidocaine. Patients with 2% lidocaine cream reported significantly lower of behavioral pain scores than those without lidocaine, but less effective than 5% lidocaine cream. Patients with 2% lidocaine cream reported significantly lower of anxiety scores than those without lidocaine, but less effective than 5% lidocaine cream. Conclusion: Topical application of lidocaine cream for 30 minutes before AV fistula puncture significantly decreased pain and anxiety among hemodialysis patients. Specifically 5% lidocaine was more effective than 2% lidocaine for both pain and anxiety.

Effects of Local Anesthetic Cream on Pain Relief in Newborns During Venipuncture (신생아의 정맥천자시 통증 완화를 위한 국소마취 크림 적용 효과)

  • Purpose: Newborns routinely experience pain associated with invasive procedures such as blood sampling, venipuncture, heelstick, or venous cannulation. This study was done to provide data for a nursing intervention to alleviate newborn pain clinically by investigating the effect of local anesthetic cream during venipuncture. Methods: Participants were 70 newborns hospitalized in the nursery. Informed Consent was obtained from parents of the newborns. Venipuncture for regular blood sampling was carried out for a test on 2 groups; the experimental, placebo group. The neonatal infant pain scale (NIPS), and duration of crying were measured to assess pain reaction. All neonatal behaviors were recorded on videotape. Results: There were significant differences in pain behavior during venipuncture (t=-4.752, p<.001), immediately after sampling (t=-5.591, p<.001), 3 minutes after puncture (t=-2.469, p=.017), and in duration of crying (t=-3.005, p=.004). Conclusion: Results show that local administration of EMLA cream before venipuncture causes a reduction in neonatal pain response, indicating that the EMLA cream has the effect of pain relief.

Effect of EMLA Cream to Decrease Venipuncture Related Pain and Anxiety in Children (피부국소마취제가 아동의 정맥천자시 통증경감과 불안감소에 미치는 효과)

  • Purposes: Venipuncture is one of the most painful and frequently performed invasive procedure for children by nurses. The purpose of this study was to investigate the effect of EMLA cream(eutectic mixture of local anesthetics: 2.5% lidocaine and 2.5% prilocaine) to decrease venipuncture related pain and anxiety in children. Methods: A randomized, double blinded, placebo-controlled study was conducted. Sixty children scheduled venipuncture were recruited, and randomly allocated to receive either EMLA or a placebo cream 1 hour before venipuncture. Venipuncture was carried out by one nurse investigator. Pain and anxiety was measured by parent, nurse investigator, and nurse observer. Results: Pain in the EMLA group was significantly lower than that of placebo group. However, there was no significant difference in anxiety score between the EMLA and placebo groups. Local side effect of EMLA cream were negligible. Conclusions: EMLA cream was found to be an effective local anesthetic for pediatric venipuncture pain. Further studies should continue to evaluate the facilitators and barriers of EMLA cream application in pediatric nursing practice.

  • Neurolysis of the celiac plexus has been performed to relieve intractable pain caused by carcinoma of the pancreas, liver, gall bladder or stomach. It is also occasionally effective in controlling the pain of chronic pancreatitis. In practice, however celiac plexus block is not a simple procedure to the beginner. The results and complications are variable. In order to correctly inject neurolytic agents into or near the celiac plexus and to reduce the time consumed to perform celiac plexus block, we used CT scanner for 7 cases of alcohol celiac plexus block. The effects will be described. The purpose of this article is to improve the technical aspect of celiac plexus block to the beginner.

  • 삼출성 중이염은 소아연령군의 이비인후과질환중 유병율이 가장 높은 질환의 하나로 1867년 Politzer가 최초로 기술하고 치료법을 소개한 이후 그 병인 및 치료법이 다양하게 연구 개발되어 왔으며 1950년대 및 1960년대 electroacoustic impedance unit의 발전에 따라 진단법에 많은 변혁이 이루어졌다. 연자들은 1981년 1월1일 부터 1986년 12월31일까지 삼출성 중이염으로 중이내 통기관유치술 및 고막천자를 시행한 15세 이하의 290명의 환자를 대상으로 tympanogram의 양상 및 술전 기골도차를 조사하여 중이내 삼출액의 유무 및 점성도의 예측도에 대하여 살펴 보았다. Tympanometric peak pressure (T. P. P.) 가 -50mm$H_2$O 미만이며 compliance가 0.4 c.c.미만인 영역을 B영역으로. T.P.P.가 -50mm$H_2$O 미만이며 compliance가 0.4 c.c. 이상인 영역을 C영역으로 정하였을 때 중이내 삼출액 유무의 예측도는 B영역이 87.5%. C영역은 81.48%였으며 삼출액이 장액성일 경우의 예측도는 B영역이 24.51%, C영역이 22.72%이었고, 삼출액이 점액성일 경우의 예측도는 B영역이 70.39%, C영역이 54.54%이었다. 연령을 15세 - 7세군과 6세이하의 군으로 세분하였을 경우에도 비슷한 결과를 얻었다.

Difference of Pain Depending on the Bed Rest Period after Lumbar Puncture in Patients with Leukemia (백혈병환자의 요추천자 후 침상안정시간에 따른 두통과 요통의 차이)

  • Purpose: This study was conducted for an evidence-based nursing intervention on bed rest period after puncture, by confirming whether there was a difference in scores of headache and back pain between the 1-hour bed rest group and the 6-hour bed rest group following lumbar puncture in patients with leukemia. Method: The subjects were 45 inpatients of the department of hematology at a general hospital in Seoul, from January 1 to March 31, 2005. There were 21 patients in the 1-hour bed rest group and 24 patients in the 6-hour bed rest group. A graphic rating scale was used to measure headaches and back pains. Collected data were analyzed with SPSS 12.0. Non- parametric statistics were used to analyze the data. Results: There was no significant difference between the 1-hour bed rest group and the 6-hour bed rest group in the headache level and back pain level. Conclusion: Therefore, it is considered that a long period of bed rest over 6 hours after lumbar puncture is not a necessary nursing intervention, and repeated studies are required to confirm the result.

  • A 60-year old male patient admitted with complaints of dyspnea and pleuritic chest pain. The chest X-ray demonstrated right pleural effusion. We planed to do the conventional thoracentesis to evaluate the characteristics of pleural effusion and to relieve the symptom of the patient. Focal reexpansion pulmonary edema was seen on the follow-up chest X-ray. After the 5-day conservative management, the patient recovered without any complications.

A Study on the Safety of Continuous Hemostasis after Arterial Puncture Intervention (동맥천자 인터벤션 시술 후 지속지혈 안전성에 대한 연구)

  • Most of the vascular procedures performed for various diagnoses and treatments of various abdominal intervention procedures performed by the Department of Radiology and Angiography are performed by puncture of the femoral artery. For this reason, patients should undergo blood-related tests such as prothrombin time (PT) and partial thromboplatin time (PTT). Therefore, many patients are instructed to take precautions such as putting a sandbag on the puncture site to prevent delayed hemorrhage after hemostasis of the femoral artery puncture site, and not to bend the leg of the treated area for about 3 hours. Because of this, many patients have complained of pain during the procedure and inconvenience during the absolute bed rest time in the ward. The purpose of this study was to compare the safety of balloon ancillary devices with sandbags placed on the hemostasis site to prevent delayed hemorrhage after arterial puncture. We compared the safety of each patient with the results of medical records in consideration of the problem that the patient could not press with the focus, the position of the patient was changed depending on the patient's body shape, and the problem of falling down according to the location of the puncture site. As a result, the use of a balloon type ancillary device improves the effect of continuous hemostasis, reduces discomfort during the patient's absolute stabilization time, increases the patient's satisfaction, and is a good alternative to the existing sandbag.

Comparison of Skin Stimulation Method and Topical Anesthetic Cream on Pain and Heart Rate Variability during Arteriovenous Fistula Puncture in Hemodialysis Patients (혈액투석 환자의 동정맥루 천자 시 피부자극법과 국소마취크림 도포에 따른 통증과 심박변이도 비교)

  • Purpose: The purpose of this was to compare effects of application of the skin stimulation method and topical anesthetic cream on pain, heart rate variability and satisfaction according to nursing intervention methods during arteriovenous fistula puncture in chronic renal failure hemodialysis patients. Methods: This study was a crossover design. Participants were 36 patients with chronic renal failure receiving hemodialysis treatment. Two forms of intervention were applied to participants, and then pain and heart rate variability were measured during the puncture. Results: There were no statistically significant differences according to each treatment in vein pain and artery pain. Also, there were no statistically significant differences according to each treatment in stress index, sympathetic activity (LF), parasympathetic activity (HF) and sympathetic activity/parasympathetic activity (LF/HF ratio). Satisfaction with application of skin stimulation method was statistically higher than that of topical anesthetic cream application. Conclusion: This suggests that application of the skin stimulation method complements disadvantages of topical anesthetic cream application and demonstrates possibility of application as a nursing intervention method which can be conveniently used by nurses in clinical practice.

Effects of Ethyl Chloride Spray before Arteriovenous Fistula Puncture on Pain, Depression, and Noncompliance of Hemodialysis (혈액투석 환자의 동정맥루 천자 시 에틸클로라이드 스프레이 적용이 통증, 우울 및 치료 지시 불이행에 미치는 효과)

  • Purpose: This study was conducted to examine the effects of ethyl chloride spray during arteriovenous fistula puncture on pain, depression, and noncompliance in hemodialysis patients. Methods: This study used a randomized controlled trial design on adults with chronic renal failure who received hemodialysis treatment through arteriovenous fistula. Ethyl chloride spray was applied to 20 subjects in the experimental group during arteriovenous fistula puncture, and a placebo spray was applied to 20 subjects in the control group; the intervention was conducted 12 times for 4 weeks. A total of 33 participants were used in the final analysis. The outcomes were measured using the face pain rating scale, Center for Epidemiological Studies Depression Scale (CES-D), and modified United States Renal Data System tools. Results: As a result of the pre-homogeneity test between the two groups, it was confirmed that the sexes were not homogeneous. Therefore, sex was treated as a covariate and analyzed. Puncture pain was significantly reduced in the experimental group (p<.001). However there was no significant difference between the groups in depression and noncompliance. Conclusion: Ethyl chloride spray was convenient and effective pain management intervention for both hemodialysis patients and medical staff.
