Purpose : Although air enema reduction has been known as a good method of diagnosis and treatment of intussusception, it could develop colon perforation. However, there have been few studies about this complication. So we analyzed the risk factors of colon perforation during air enema reduction in patients with intussusception. Methods : We reviewed the charts of 12 colon perforation patients during air enema reduction of intussusception, who were admitted to Gil Medical Center from Jan. 1990 to Dec. 2001. Their age, sex, major symptoms, length of time till hospital visit, types of intussusception, operative findings and pathologic reports were reviewed. Results : Among 657 cases, 596 patients(90.7%) were successfully treated, but 12 patients(1.83%) failed in air enema reduction and had colon perforation. In patients with colon perforation the male to female ratio was 11 : 1, and average age was 5.3 months. The most common symptom at the time of hospital visit was vomiting(91.7%). Cyclic irritability(75.0%), bloody stool(75.0%) and abdominal mass(41.7%) were also noted. The average length of time between symptom onset and hospital visit was 44.7 hours. Types of intussusception were predominantly ileocolic, ileocecal, and ileoileocolic. The site of perforation was most commonly found at the proximal part of intussusception including ascending colon(50%) and transverse colon(50%). Most cases were uncomplicated, and had a single perforation. Pathologic reports showed hemorrhagic necrosis and mesenteric laceration at the site of colon perforation. Complications of colon perforation were tension pneumoperitonium(58.3%), requiring immediate decompression. Conclusion : The chance of colon perforation during air enema reduction increases in cases with small bowel obstruction on simple abdominal x-ray of a patient younger than 6 months, delay in time till hospital visit and higher air pressure during reduction. Therefore more careful investigation is needed in these cases.
Domestic explosive demolition techniques have been developed and applied for low-rise structures up to now. However, the demand for the development of those techniques that can be applied economically, safely and environment-friendly rapidly increases because the old high-rise RC rahmen structures that were built since around 1970s are now required to rebuild. As a result, element technologies of explosive demolition for low-rise structures were applied to take advantage of technology in high-rise structures that performed application testing at Chungang Department demolition field in Daejeon city. It could judge elements technology establishment for high-rise structure demolition and field application and suggest the improvements when the problems occurred to develop High-rise building demolition techniques for method of protection a field test and the dust reduction test. The water cannon test was applied to reduce the dust site and the drilling tests are performed to select the best components for explosives demolition elements techniques of the reliability. This paper shows that we have the ability to remove a high-rise building using environmentally friendly safe and economical explosives demolition method. It would contribute to prevent a foreign company from entering the domestic market and should contribute to acquire competitiveness of domestic demolition industry.
Building tunnels means dealing with what rock is encountered. Relocation of the site of the underground structure is rarely possible. Tunneling engineers and miners have to cope with the quality of the rock mass as it is. Different tunneling philosophies and different rock classification methods have been developed in various countries. Most of the rock classification methods are based on the response of the rock mass to the excavation. Tunnel support requirements could be assessed analytically, supplemented by rock mass classification predictions, and verified by measurements during construction. Rock mass classifications on their own should only be used for preliminary, planning purposes and not for final tunnel support. Design of blast pattern in tunneling projects in Korea is also mostly prepared according to the general rock classification methods such as RMR or Q. They, however, do not take into account the blast performance, and as a consequence, produce poor blasting results. In this paper, the methods of general rock classification and blast design for tunnel excavation in Korea are reviewed, and efforts to develop a new classification method, reflecting the blasting performance, are presented.

Dental implants are universal restorative method on edentulous site in oral cavity and generally recognized by patients as well as clinicians. Rapid bone resorption of labial portion of maxillary anterior area is performed due to dental trauma, chronic periodontitis, and so on. Accordingly, Implants on maxillary anterior alveolar ridge with narrow labiopalatal width would lead to bony defects of dehiscence or fenestration. In this case, guided bone regeneration procedure is used to augment maxillary anterior alveolar ridge. It can have mechanical and biological advantages to mix tissue adhesive with bone graft materials in guided bone regeneration procedure. In these cases, when the dehiscence or fenestration defects was occurred by dental implants on maxillary anterior alveolar ridge with narrow labiopalatal width, guided bone regeneration procedures were performed with various combination of particle bone graft materials(allograft, xenograft, and alloplast) mixed with fibrin glue, excepting autogerous bone. We reported that all of 4 cases showed favorable alveolar ridge augmentations.
Recently, the incidence of pressure sore has been increased, due to increased number of patients with central nervous system injuries after traffic and industrial accidents or with long term loss of consciousness due to drug intoxication. The management of sacral pressure sore has been improved through the development of myocutaneous flap. However, sacrifice of the deep muscle cause some problems such as intraoperative bleeding, functional disabilities of donor site and further limitation of reconstruction for recurrent pressure sores in paraplegic patients. The development of perforator-based island skin flap introduce a new treatment modality for sacral pressure sores. We used perforator-based island skin flap in 15 cases with reasonable result from January 1998 to February 2000. This flap has a many advantages such as no significant sacrifice of the gluteus maximus muscle, wide rotation arc, reliable blood flow of the perforator, short elevation time for the flap, and no post-operative hindrance to walking in patients who are not paraplegic. There was no significant complication without significant sequelae and donor sites could be repaired primarily.
Gastrointestinal complications, especially duodenal complication after cardiopulmonary bypass are rare, but often fatal. We experienced 1 case of duodenal ulcer bleeding and 2 cases of duodenal ulcer perforation developing after cardiopulmonary bypass from August 1994 to April 1996. In the case of duodenal ulcer bleeding, palpitation, dizziness, tachycardia and melena were the clues leading to diagnosis, and in the cases of perforation, abdominal distension with pain, tachycardia, hypotension, oliguria were the clues. Duodenal perforations were diagnosed by abdominal paracentesis. The patient with duodenal bleeding was treated by H-2 receptor antagonist, antacids and transfusion. And emergency laparotomy was required for the patients with duodenal perforation. In addition to ulcer prophylaxis including H-2 receptor antagonist and antacids, a high index of suspicion and timely surgery are necessary for early diagnosis and appropriate treatment of duodenal complication developing af er cardiopulmonary bypass.

The injection of cement stabilizer is used as one of ways to stabilize soft sea-bottom. However, this method makes an argument for the possibility of releasing heavy metals from the cement stabilizer to the seawater. To investigate the effect of the cement stabilizer on the seawater, field measurements were carried out in Ocheon harbor of the Chunsoo Bay. Although the highest concentrations of dissolved heavy metals were found in the surface seawater of the injection point and the metal concentrations decreased with distances, the levels of heavy metals in all the seawater met the demand of environmental criteria of Korea, US and UK.
Purpose: The purpose of this study is to evaluate the clinical, radiological and histopathological results after microfracture surgery for degenerative arthritis of the knee. Materials and Methods: From Oct. 1997 to Dec. 1998, 48 knees in 46 patients were treated by microfracture technique. Their mean age at the time of operation was 56 years(range, 40-75 years) and mean period of follow-up study was one year(range, 7-20 months). For 24 knees in 22 patients, 'second-look' arthroscopies and biopsies were performed at 6 months following microfracture. At the last follow up clinical results were evaluated with Baumgaertner's scale. The specimens of 24 cases were stained with H-E, Safranin-O, and Masson's trichrome. Eighteen of 24 cases were stained immunohistochemically and the Western blotting test was performed on 12 cases for type II collagen. We analyzed the relationship of the Western blotting for type II collagen with clinical score, preoperative varus deformity, joint space widening in radiological result, extent of repaired articular cartilage in '2nd-look' arthroscopic findings, patient's age and weight. Results: Clinical results were excellent in 90% and good in 10%. Among the 24 knees, more than 80% of areas of chondral defect were covered with regenerated cartilage in 21 knees Histologically, the repaired tissue appears to be a hybrid of hyaline cartilage and fibrocartilage. Repaired cartilage contains variable amounts of type II collagen with immunohistochemical staining. The results of the Western blotting test were similar. The amounts of type II collagen formation had positive correlation with the extent of repaired cartilage and preoperative varus deformity. Conclusion: 'Second-look' showed that the chondral defect areas were covered with newly grown grayish white tissue. Articular cartilage repair was confirmed with histological and immunohisto-chemical study qualitatively, and the amount of type II collagen was calculated with the Western blotting test quantitatively. The exact nature and fate of repaired cartilagenous tissues need further long term follow-up study. The results of this study provide the rationale to select osteoarthritic patients indicated for microfracture surgery.
In order to clarify an effect of the cutoff grouting, a series of field experiments were performed during construction of the waterway tunnel from the River Gilancheon(Andong) to the Youngcheon dam. The experiments were conducted in three different ways based on the grouting time in the construction sequence, i.e., the pre-grouting, after-grouting and consolidation grouting tests. And those were also planned to compare the efficiency of grouting in relation to the material types of grout, base rock types and other geologic factors such as discontinuities, depth and direction of grouting holes, and number of grouting stages. Among the materials of grout employed in the experiments, such as a common Portland cement, a micro-cement, a micro-cement with sodium silicate, and a urethane, the urethane was the most effective as the cutoff grouting. And for the same grout material, the pre-grouting was more effective to cutoff the water inflow comparing to the after-grouting and the consolidation grouting. For the rock types, the grouting efficiency in the sedimentary rocks as a base rock was less than the other rocks such as granite and volcanic rocks, which is believed due to the smaller separation of joints and the abundance of infilling materials in the joints developed in the sedimentary rocks. There was no direct relationship between the total RMR value of the rock mass and the grouting efficiency, however, the joint separation which is one of the RMR criteria is believed to have positive relation to the grouting efficiency. And the direction of the grouting holes might not so much affect on the grouting efficiency while increasing the number of grouting stage showed the better results.

A 4-year-old castrated male Shih-Tzu was presented to the Chonbuk National University Animal Medical Center with a history of melting ulcer in the right eye (OD). Upon ophthalmologic examination, severe keratomalacia of approximately 70% of the entire surface area with a full thickness corneal defect was found in OD. In addition, iris and fibrin clots were observed on the center of the corneal defect. The menace response and dazzle reflex were normal in OS. As the size and damage of corneal perforation was so severe, amniotic membrane (AM) transplantation was considered to repair the cornea instead of direct suture technique, flap methods or corneal transplantation. Equine AM was sutured to the limbus to cover the entire cornea in a single interrupted pattern using 9-0 nylon suture material. On day 79, mild scarring and pigmentation, with almost no vasculature, remained. The menace response and dazzle reflex were normal of OD. Although pigmentation and scarring remained on the cornea, equine AM transplantation can be useful for reconstruction of severe corneal perforation with keratomalacia.
