  • The purpose of this study was to investigate the effects of pelvic adjustment on low back pain and spino-pelvic parameters in middle-aged women. Thirty-eight middle-aged women with chronic low back pain were randomly assigned to the pelvic adjustment (n = 20) oruntreated control (n = 18) group. Pelvic adjustment interventionswereperformed four times a week for 8 weeksin the former group. At baseline and after 8 weeks, the back pain and back function were evaluated using the visual analogue scale (VAS), Oswestry disability index (ODI), and back flexibility. Additionally, the spino-pelvic radiographic parameters and serum C-reactive protein (CRP) levels were assessed. After 8 weeks, the VAS, ODI, and back flexibility significantly improved in the pelvic adjustment group compared with the control group. It was found that the changes from baseline in the lumbar lordotic angle, sacral slope, pelvic crest unleveling, and femoral head height inequality were significantly greater in the pelvic adjustment group than in the control group. There were no significant changes in the pelvic incidence or serum CRP levels in either group. In conclusion, pelvic adjustment has beneficial effects on chronic low back pain and back function, suggesting that the effects of pelvic adjustment on back pain may at least in part result from changes in the spino-pelvic alignment.

  • Purpose: SAPHO syndrome is well known to various disease entities including synovitis, acne, pustulosis, hyperostosis and polyarthritis. The purpose of this study is to evaluate sicntigraphic findings and to compare with radiologic findings in SAPHO syndrome. Materials and Methods: Five patients (M:F=5.0, Age $22.8{\pm}4.78$ yrs) with SAPHO syndrome were enrolled in our study. All patients underwent whole-body bone scintigraphy with intravenous administration of 740 MBq of Tc-99m MDP. Among them, two patients were additionally perfomed SPECT of the spine to evaluate the location and extort of spinal lesion. Results: All patients were demonstrated abnormal increased uptakes in sternoclavicular joint (SC), sacroiliac joint (SI), and small joints of both hands. Among them, three patients were bilateral involvement (3/5) and two were unilateral (2/5) in SC. involvement of SI showed bilateral in four patients (4/5) and unilateral in one (1/5). SPECT images demonstrate that the lesion sites of the lumbar spine are more likely facet joints than vertebral bodies or pedicles. Conclusion: As SAPHO syndrome is the disease entity involved polyarticular joints with various dermatologic manifestations, the bone scintigraphy may be a very useful method to evaluate the location and extent of joint involvement, and to avoid inadequate surgical management or ineffective antibiotic treatment.

  • The purpose of this study was to investigate the effect of applying whole body vibration exercise at 30 Hz on the thickness and mechanical properties of the erector spinae. The study population included 24 adults (11 male and 13 female) in the N region. The subjects were divided into two groups, a 30Hz whole body vibration exercise(WG, n=12) and an LSG(12 subjects). Changes in the thickness and mechanical properties of the erector spinae were analyzed before the exercises and after four and eight weeks of exercises using a two-way repeated analysis of variance with a statistical significance level of α=0.05. When there was an interaction between the groups and measurement times, post hoc analyses were conducted at a statistical significance level of α=0.01. The thickness, frequency, stiffness and strain of the erector spinae muscles were significantly different in each time period and in the interaction between time period and the group (P<0.05), and recovery was significantly different only in each time period (P<0.05). Thus, 30Hz whole body vibration exercise had a positive effect on the thickness and mechanical properties of the erector spinae. The results of this preliminary study suggest that whole body vibration exercise may find a place as a lumbar stabilization exercise in clinical practice in the future.

  • In 2020, the diversity of benthic macroinvertebrate communities was investigated in the Sohan stream, an ecological and landscape conservation area, and the results were compared with the previous research conducted in 2011. In total, 42 species (two phyla, three classes, and seven orders) were found in the Sohan stream. Species richness and abundance sharply decreased at all sampling sites because of Typhoons Haishen and Maysak in 2020, which had a direct impact on the stream. In the functional feeding group, the ratio of collector-gatherers was the highest at all sampling sites. However, during the autumn season, the shredder ratio increased from 13.4% to 42.4% in the uppermost stream site. Compared with the diversity of benthic macroinvertebrates surveyed in 2011, a total of 53 species (two phyla, three classes, and eight orders) were found. The percentage of species richness and abundance of Ephemeroptera, Plecoptera, and Trichoptera was more than 50% both in 2011 and 2020. Only the richness of Ephemeroptera was significantly different between them (2011: 2.9 and 2020: 6.7). In this study, the abrupt changes of species richness and abundance in benthic macroinvertebrate were not observed before and after the designation of an ecological and landscape conservation area. However, it is necessary to monitor benthic macroinvertebrates in order to confirm that biodiversity is continuously maintained long after the designation of the ecological and landscape conservation area.

  • The purpose of this study was to confirm the reduction of pain and symptom relief of Parkinson's disease by vertically stimulating the spine through the application of a mechanical bed capable of thermal and massage stimulation. For this purpose, after confirming the segmental motion of the spine due to the use of a medical combination stimulation bed for Parkinson's disease patients, VAS, ODI, gait ability, and spiral drawing tests were performed, and the relationship between the variables was identified. In the 10-day visual analog scale and evaluation of low back pain dysfunction, the average trend of decreasing after bed use was confirmed. For walking ability, a decrease in the moving time and an increase in the moving distance were observed. In the spiral drawing test, the mean test time after using bed was significantly lower than before. As a result, it suggested the possibility of using it as an auxiliary method for recovery and pain relief of Parkinson's disease patients due to spinal segmental movement with mechanical heating and massage. However, this study is a preliminary study, and there is a small number of subjects, so additional research is needed that considers the number and condition of future subjects in detail.

  • Background: Paraplegia is a serious complication of thoracic or thoracoabdominal aortic operations, which is related to ischemic injury of the spinal cord induced by low perfusion pressure during cross clamping of the aorta. Ischemic preconditioning of heart or brain with reversible sublethal ischemic injury induces resistance to subsequent lethal ischemia. The aim of this study is to investigate whether ischemic tolerance could be induced by the preconditioning of the spinal cord using swine model. Material and Method: The animals were randomly assigned to three groups: sham group(n=3), control group(n=6) and pre-conditioning group(n=8). In the sham group, we performed the left thoracotomy only without any ischemic injury. In the preconditioning group, the swine received reversible spinal cord ischemic injury by aortic clamping for 20 minutes, whereas control group had no previous aortic cross- clamping. Forty-eight hours later, the aorta was clamped for 30 minutes in both groups. Neurological examination was done 24 hours later, then the animals were euthanized for histopathology and malonedialdehyde(MDA) spectrophotometry assay of the spinal cord. Result: Statistically significant difference in neurological outcome was observed between the control and preconditioning groups at 24 hours after ischemic injury. The incidence of paraplegia and severe paresis was 100% in the control group, and 62.5% in the preconditing group(p=0.028). There was no statistically significant difference in histopathology and MDA assay of the ischemic spinal cord between these two groups with borderline statistical difference in MDA assay(p=0.0745). Conclusion: In the present swine study, ischemic preconditioning could induce tolerance against 30 minute ischemic insult of the spinal cord, although the animals did not completely recover(stand-up or walk). We expect that combining this preconditioning with other currently existing protection methods might lead to a synergistic effect, which warrants further investigation.

  • Background : The prone position is often used for operations involving the spine and provides excellent surgical access. The complications associated with the prone position include ocular and auricular injuries, and musculoskeletal injuries. In particular, the prone position during general anesthesia causes hemodynamic changes. To evaluate the cardiovascular effects of the prone position in surgical patients during general anesthesia, we investigated the effects on hemodynamic change of the prone position with the Jackson spinal surgery table. Materials and Methods : Thirty patients undergoing spine surgery in the prone position were randomly selected. After induction of general anesthesia, intra-arterial and central venous pressures (CVP) were monitored and cardiac output was measured by $NICO^{(R)}$. We measured stroke volume, cardiac index, cardiac output, mean arterial pressure, heart rate, CVP and systemic vascular resistance (SVR) before changing the position. The same measurements were performed after changing to the prone position with the patient on the Jackson spinal surgery table. Results : In the prone position, there was a significant reduction in stroke volume, cardiac index and cardiac output. The heart rate, mean arterial pressure and CVP were also decreased in the prone position but not significantly. However, the SVR was increased significantly. Conclusion : The degree of a reduced cardiac index was less on the Jackson spinal surgery table than other conditions of the prone position. The reduced epidural pressure caused by free abdominal movement may decrease intraoperative blood loss. Therefore, the Jackson spinal surgery table provides a convenient and stable method for maintaining patients in the prone position during spinal surgery.

  • The aim of this study was to quantify the sedative effects of high spinal anesthesia, according to patient age, by also evaluating respiratory and hemodynamic changes. A prospective study was performed in 60 patients who were electively scheduled for lower limb or lower abdominal surgery. They were allocated into one of 2 groups according to their age: older age (51-80 years) and young age (20-50 years). In all groups, the lowest intraoperative bispectral index (BIS) values significantly lower than preoperative values. Patients in the older age group had significantly lower intraoperative BIS values than younger patients. The sedative effect of high spinal anesthesia is greater in older patients than in young patients, with an increase in respiratory instability. Our study showed a negative correlation between age and lowest BIS value; the lowest BIS value was reduced in older patients.

  • This study was to find out the effect of cervical and thoracic stretching and strengthening exercise program on forward head posture. 32 adults had forward head posture were selected as the subjects for the study. The training program of the exercise group(n=16) consist of postural education with exercise program, subjects in the control group(n=16) participated in a postural education only. Postural measurements of Craniovertebral Angle(CVA) and Cranial Rotation Angle(CRA) were obtained from the using x-ray and PACS(Picture Archiving and Communications System) ${\pi}view^{TM}$. The results were compared with change of CVA and CRA exercise group were significant(p<.05) compared to the control group. However CRA of contol group was significant(P<.05). Also, there were no significant difference(p>.05) between groups on any pre-test measure. however there were significant difference(p<.05) between groups on CVA and CRA post-test measure. This study showed that cervical and thoracic stretching and strengthening exercise program can improve spinal posture alignment for decreasing forward head posture in subjects. These results can be fundamental data for further development of exercise program in interventions of postural improvement.
