The stage for Tadao Ando's architectural works is not only in Japan but extends to many countries around the world. Japanese traditional elements used in the his works makes it so. Tadao Ando argued intuition and insanity which were deprived by modernism could recover with locality and it's historic elements laid importance to the self. To realize that, it is necessary the regeneration of landscape which was based on the site and self-reflection must accomplished with the nature. So as to regeneration of landscape, he used Japanese Landscape-gardening and make the nature condensed. With the su-ki, he made the architectural elements simple. And with the Japanese detoured circulation, he made deep and various space. In that way, he intended the nature which is changed with his idea experienced the way of yu-gen.
Reported are the heat transfer characteristics of a two-phase loop thermosyphon (TLT) with nanofluids consisted of nano-size silver particles and distilled water as the working fluid. The nanofluids used in the present study are dispersed solutions with various amount of silver nanoparticle in distilled water. It is seen from the present study that the heat transfer performance of the test TLT with nanofluids increased as much as about 2 times higher than that of a TLT with pure water as the working fluid based on same heat flux. The study also showed that there was no deterioration of the TLT performance with time, up to a period of 8 days of continuous operation which implies that there was no coagulation of nanoparticles within the working nanofluid during the operation of the test TLT.
Purpose: Many patients are injured by trauma. And some of them expire due to severity of trauma. Various scoring systems have been introduced in grading severity and predicting mortality of trauma patients. This study is to evaluation the usefulness of factors for determining the severity and predicting the prognosis of the trauma victims. Methods: Data on the patients who visited our Emergency departments from January 2010 to December 2011 were retrospectively reviewed using electronic medical records. The patients were activated severe trauma team calling system. The patients were categorized as survivors and non-survivors. Univariated associations were calculated, and a multiple logistic regression analysis was used to determine variables associated with hospital mortality. Results: Two hundred sixty two(262) patients were enrolled, and the mortality rate was 25.6%. By multivariate analysis, lower respiration rate, lower Glasgow Coma Score, higher International Normalized Ratio and emergency transfusion within 6 hours were expected as severity and prognosis predict factors (each of odds ratio were 24.907, 14.282, 2.667 and 16.144). Conclusion: As predict factors, respiration rate, Glasgow Coma Score, International Normalized Ratio and emergency transfusion, are useful determining the severity and predicting prognosis of trauma victims.
서울시가 추진해온 대규모 공원 조성 사업의 일환으로 강북지역의 생활권 공원조성을 위하여 드림랜드 부지를 거대한 녹지의 숲과 공원이 어우러진 공간으로 계획하였다. 공간 구성기법으로 채움보다는 비움의 미학을 통하여 불확실한 도시의 변화에 대응하기 위한 잠재성을 부여하였고, 경관구성을 위하여 전통적인 서울의 산악경관 요소를 도입하고 차경하여 조망축을 형성하였는데, 유려하게 펼쳐진 한폭의 산수화 같은 한 그루의 나무, 하늘과 바람, 그리고 물과 흙의 심상이 표현되고 있다. 이것은 도시화의 계획에 배제되어 왔던 자연을 중요한 고려의 대상으로 삼음으로서, 서구도시의 건축적 랜드마크와는 차별화된 우리의 산과 구릉지 모습으로 도시경관의 지배적 랜드마크를 만드는 것이다. 그러므로 본 연구는 전통의 경관개념과 한국적 공간의 유형에서 추출한 시설배치 기법과 요소를 설계에 적용하여 기존의 도시가로와 마당과 같은 공원의 공간이 전통적 위계를 가지도록 하였으며, 도시의 확장된 오픈 필드(open field)를 통하여 공간경험의 다양성과 잠재성을 추구하여 북서울꿈의숲을 설계하였다.
본 연구는 15~16세기에 형성된 영남지방 상류주택을 대상으로 입지별 상류주택의 사랑채 내부 공간, 누마루의 기능, 규모, 유형 등을 통해 사랑채 누마루의 입지별 특성을 비교하고 또한 입지별 사랑채 누마루와 사랑마당 및 외부차경과의 시각적 관계를 분석하여 조선 초기 사랑채 누마루의 입지별 내 외부 공간 특색의 차이점을 파악하였다. 그 결과 구릉지형의 경우 좌향과 상관없이 사랑마당과 접하는 누마루의 면이 단변을 이루고, 평지형의 경우 구릉지형과 달리 사랑마당과 접하는 누마루 면이 장변을 이루었다. 둘째, 구릉지형은 사랑마당이 주조망의 대상이 아니며 주변 산세를 조망하는 보조적인 역할을 하며, 평지형은 사랑마당이 조망의 대상이면서 구릉지형과 달리 사랑마당 안에 내부정원을 형성하고 개방된 입면을 형성한다. 셋째, 구릉지형은 조산을 외부차경으로 활용하며 평지형은 사랑마당을 조망대상으로 활용하고 사랑마당의 기능이 다양화되면서 내부정원을 형성하는 차이가 나타난 것을 알 수 있었다.
The effect of glossopharyngeal nerve transection on the taste buds of the rat vallate papilla was examined by using the method of DNA nick-end labeling (TUNEL) and standard electron microscopic technique at 1, 3, 5, 7, 9 days after denervation. In general, the number and size of taste buds decreased as more days passed after denervation. They started decreasing on day 3 post denervation and virtually all taste buds were disappeared on day 9 post denervation. In studies using TUNEL method, TUNEL postive cells markedly increased in their numbers one day post denervation, as compared with controls. The number of apoptotic taste bud cells per taste bud profile was averaged to be 0.64 and 0.44 for day 1 and 3 post denervation, respectively, whereas it was 0.10 in controls. In electron microscopy, apoptotic cells were identified by the presence of condensed and fragmentary nuclei in a cytoplasm, which resulted in increased density. In control rats, only few apoptotic cells were found. On days 1 and 3 post denervation, nerve fibers almost disappeared from the taste buds and some apoptotic cells were apparent. On days 7 and 9 post denervation, a few taste bud cells were still present in the epithelium of the bottom of the trench wall of the vallate papilla and most of them showed apoptotic changes. The results indicate that the death of taste bud cells in normal conditions is controlled by apoptosis and the decrease and disappearance of taste buds after denervation is also caused by apoptosis of taste bud cells.
Purpose: Aspiration pneumonia is an important complication of drug intoxication with decreased mental status. The purpose of the study is to investigate the risk factors of aspiration pneumonia in the patients of benzodiazepine overdose with or without co-ingestion of other drugs. Methods: A retrospective chart review of patients who visited the emergency department between January 2012 and December 2014 was conducted. Demographic data, time from ingestion to visit, initial vital signs, symptoms, mental status, medical history, laboratory results, chest radiological findings and co-ingested medications were recorded. Multiple logistic analyses were performed to verify the association between variables and the development of aspiration pneumonia. Results: A total of 249 patients presented to the emergency department with benzodiazepine overdose. Aspiration pneumonia had developed in 24 patients (9.6%). Univariate analysis revealed time from ingestion to visit was longer, Glasgow coma scale score was lower, hypoxia was presented, leukocytosis was shown, types of ingested drugs was high, less activated charcoal was applied and tricyclic antidepressants was taken in patients that developed aspiration pneumonia. Time from ingestion to visit (odds ratio (OR) 1.121, 95% confidence interval (CI), 1.057-1.189, p=0.000), GCS score (OR 0.724. 95% CI, 0.624-0.839, p=0.000), oxygen saturation (OR 0.895, 95% CI, 0.835-0.959, p=0.002), and co-ingestion of TCA (OR 4.595, 95% CI, 1.169-18.063, p=0.029) were identified as risk factors of morbidity of aspiration pneumonia upon multiple logistic regression analysis. Conclusion: Time from ingestion to visit, low GCS score, low oxygen saturation and co-ingestion of TCA were risk factors of the development of aspiration pneumonia in benzodiazepine overdose patients.
Purpose: This study examined the clinical characteristics of severe elderly poisoning patients to determine the factors that can prevent them. Methods: Data were collected from patients over 65 years of age presenting to the emergency center with poisoning from 2013 to 2018. Their medical records were analyzed retrospectively, and patients with a poisoning severity score of three or more were defined as the severe poisoning group. The risk factors were evaluated by univariate and multivariate analysis. Results: This study analyzed 292 patients, of whom 37 (12.7%) belonged to the severe poisoning group. The severe poisoning group showed a significantly higher association with pesticide poisoning and intentional suicide attempts. Loneliness and somatization were the cause of the suicidal ideas. No significant differences in age, sex, drinking, ingestion time, poisonous materials other than pesticides, and neuropsychological consultation were observed between the two groups. Conclusion: The severe elderly poisoning patients were the result of intentional poisoning for suicide. Loneliness and somatization were the most influential causes of suicidal poisoning. Therefore, psychiatric screening and frequent medical treatment for elderly people are required to prevent severe poisoning in elderly patients.
Purpose: This study estimated the incidence of delirium and associated risk factors and outcomes in ICU patients with acute poisoning. Methods: Data were collected from ICU patients over 18 years of age that were admitted via the emergency center after presenting with poisoning from 2010 to 2015. Delirium was assessed retrospectively using the Intensive Care Delirium Screening Checklist (ICDSC). Risk factors were evaluated by univariate and multivariate analysis. Results: A total of 199 patients participated in this study and 68 (34.2%) were diagnosed with delirium based on the ICDSC score. The delirium group showed a significantly higher association with prolonged length of stay in the hospital and ICU in comparison with the non-delirium group. The delirium group was associated with greater use of physical restraint. A statistically greater number of patients with pharmaceutical substance poisoning developed delirium over a short period of time than those with non-pharmaceutical substance poisoning. There was no significant difference between the two groups with respect to age, sex, past history, GCS score, vital signs, application of ventilator care and renal replacement therapy. Conclusion: The finding that the delirium group had a greater length of stay in both the hospital and the ICU is consistent with the results of previous worldwide studies of the effects of delirium on the prognosis of patients who were admitted to the ICU, suggesting the possibility for domestic application. Additionally, use of physical restraint was positively related to the incidence of delirium. Thus, interventions for minimizing the use of physical restraints and considering alternatives are needed.
Purpose: The Prescott nomogram has been utilized to forecast hepatotoxicity from acute acetaminophen poisoning. In developing countries, emergency medical centers lack the resources to report acetaminophen concentrations; thus, the commencement and cessation of treatment are based on the reported dose. This study investigated risk factors that can predict acetaminophen detection after 15 hours for safe treatment termination. Methods: Data were collected from an urban emergency medical center from 2010 to 2020. The study included patients ≥14 years of age with acute acetaminophen poisoning within 15 hours. The correlation between risk factors and detection of acetaminophen 15 hours after ingestion was evaluated using logistic regression, and the area under the curve (AUC) was calculated. Results: In total, 181 patients were included in the primary analysis; the median dose was 150.9 mg/kg and 35 patients (19.3%) had acetaminophen detected 15 hours after ingestion. The dose per weight and the time to visit were significant predictors for acetaminophen detection after 15 hours (odds ratio, 1.020 and 1.030, respectively). The AUCs were 0.628 for a 135 mg/kg cut-off value and 0.658 for a cut-off 450 minutes, and that of the combined model was 0.714 (sensitivity: 45.7%, specificity: 91.8%). Conclusion: Where acetaminophen concentrations are not reported during treatment following the UK guidelines, it is safe to start N-acetylcysteine immediately for patients who are ≥14 years old, visit within 15 hours after acute poisoning, and report having ingested ≥135 mg/kg. Additional N-acetylcysteine doses should be considered for patients visiting after 8 hours.
