The Effect of Direct Swallowing Treatment on the Swallowing Function and Quality of Life of the Stroke Patient (뇌졸중 환자의 직접 삼킴 치료가 삼킴 기능과 삶의 질에 미치는 효과)

  • The purpose of this study was to investigate the effects of direct swallowing treatment on recovering swallowing functions and quality of life. The study was performed over three weeks from December 3 to 21, 2012 for one dysphagia patient who was diagnosed with a stroke. The study design was carried out using a reversal design (AB) in single-subject experimental research. The direct swallowing treatment were implemented by a total of 10 times, five times a week and 20 minutes for each treatment, and the results of the treatment were compared and analyzed based on the differences in scores during the treatment period of the baseline and swallowing treatment. After completing the direct swallowing treatment, the swallowing function in stroke patients was improved and the improved swallowing function affected the quality of life positively. Based on the results of this study, it is necessary to make efforts to improve the swallowing function and quality of life in patients through applying such early direct swallowing treatment with oral intakes for the subjects who can take direct swallowing treatment in clinics.

Effect of Direct Feeding Intervention on Swallowing Function and Life Quality of Stroke Patients (직접 삼킴 치료가 뇌졸중 환자의 삼킴 기능과 삶의 질에 미치는 영향)

  • The purpose of this study was to investigate the effect of dysphagia intervention function and life quality of stroke patients. each 5 patients were randomly assigned in two groups, control group performing indirect feeding and experimental group performing direct feeding. Evaluation of the swallowing function was performed before intervention, after 4weeks and 8 weeks using CDS, PAS with VFSS and quality of life was performed twice, before and after intervention. Two groups showed the improved swallowing function, control group showed the significant difference only in CDS(p<.05). but experimental group showed the significant differenc in CDS and PAS(p<.01). both groups showed the significant improvement in quality of life(p<.05). but in comparison between group, no significant statistical difference were found(p>.05). Therefore, this study showed that both the swallowing function and quality of life were improved as time passed, but swallowing function was much more improved in experimental group.

A Survey About Awareness and Necessity of Community Based Dysphagia Therapy of Community Dwelling Older Adults (지역사회 거주 노인들의 연하장애 인식과 중재 필요성)

  • Objective : This study aimed to investigate the awareness and experience of community-based dysphagia therapy and related education in community-dwelling older adults. Methods : A total of 89 older adults were recruited from a public health center in Gyeonggi-do. Awareness, experience, and related education regarding community-based dysphagia therapy were analyzed using descriptive statistics. Results : We analyzed 89 questionnaires. Awareness, treatment experience, and education regarding dysphagia were low; however, the importance and intention to participate were high. Respondents wanted education about proper chewing and safe swallowing, oral health, oral motor exercise, and participation in community-based dysphagia programs in public health centers. The reason for the lack of experience in dysphagia education and therapy is insufficient information and opportunities. The respondents had a good understanding of dysphagia symptoms. Conclusion : Dysphagia therapy maintains swallowing and eating functions as a life-long Activity of Daily Living, and is a very important area in community rehabilitation. Based on the results of this survey, the necessity and importance of community-based dysphagia were identified. It is time to provide correct information and develop a systematic education program for community-based dysphagia therapy. Occupational therapists need to play an active role in improving quality of life by early detection and providing proper intervention.

Analyses of Studies on Swallowing Rehabilitation Program for Stroke patients in Korea (뇌졸중 환자 대상의 연하재활프로그램에 대한 국내 연구 논문 분석)

  • The purpose of this study was to investigate the status of studies on swallowing rehabilitation interventions for stroke patients that had been conducted in the past 5 years (2010-2014) in Korea and to examine their findings. A literature search was conducted through RISS, National Assembly Library, and KISS, and 12 papers were analyzed. Four articles were from nursing science, and 8 from other disciplines. Qualitative evaluation of the articles was conducted by using an evaluation framework that was designed by the researcher, which took into account the checklist presented by the Scottish Intercollegiate Guideline Network (2013). In the results of the qualitative evaluation on the papers, 4 studies scored 7 out of 10 points, 6 studies scored between 5 and 6 points, and 2 studies scored 4 points. Intervention sessions were conducted for 25 minutes, on average, each at a frequency of 4.1 sessions per week, for a total of 33.7 sessions. This study found that direct swallowing training and comprehensive intervention of respiratory and oral, including pharyngeal were effective in relieving difficulties in swallowing. In the future, such findings are expected to be used in nursing interventions for establishment of the basis for evidence-based nursing practice.
