Preparation and Characterization of Ipriflavone-Loaded Poly(L-lactide-co-glycolide) Scaffold for Tissue Engineered Bone (조직공학적 골을 위한 애프리플라본을 함유한 다공성 지지체의 제조 및 그 특성)

  • Ipriflavone (IP), a non-hormonal isoflavone derivative, has been shown to interfere with bone remodeling by inhibiting bone resorption and stimulating bone formation. IP consistently increased the amount of Ca incorporated into the cell layer by mesenchymal stem cells (MSCs). In this study, we developed the novel IP loaded poly(L-lactide-co-glycolide) (PLGA) scaffolds for the possibility of the application of the tissue engineered bone. IP/PLGA scaffo1ds were prepared by solvent casting/salt leaching method and were characterized by porosimeter, scanning electron microscopy, determination of residual salt amount, differential scanning calorimetry, and X-ray diffractometer, respectively. IP/PLGA scaffolds were implanted into the back of athymic nude mouse to observe the effect of IP on the osteoinduction compared with control PLGA scaffo1ds. Thin sections were cut from paraffin embedded tissues and histological sections were stained H&E, von Kossa, and immunohistochemical staining for Type I collagen and osteocalcin. It can be observed that the porosity was above 91.7% and the pore size was above 101 $\mu\textrm{m}$. Control scaffo1d and IP/PLGA scaffo1ds of 50% IP were implanted on the back of athymic nude mouse to observe the effect of IP on the induction of cells proliferation for 9 weeks. The evidence of calcification, osteoblast, and osteoid from the undifferentiated stem cells in the subcutaneous sites and other soft connective tissue sites having a preponderance of stem cells has been observed. From these results, it seems that IP plays an important role for bone induction in IP/PLCA scaffolds.

  • Hyperventilation is produced by several distinct causes: anxiety, respiratory alkalosis, increased blood catecholamine levels, and a decrease in the level of the ionized calcium in the blood. The dental fears about acute pain, bleeding, needle, drill and dental surgery lead to the severe anxiety and increased blood catecholamine level. Therefore, the most dental patient should be cared gently as the stress reduction protocol. In spite of the gentle care, a hyperventilation were occurred during the surgical extraction of impacted third molar with pericoronitis. We suggest that the dental patients with anxiety disorder must be attention for the manifestation of hyperventilation, especially in the psychologic disabled patient.

  • Dental professions recommend that mandibular impacted third molar be extracted for the prevention of osteomyelities crneoplasm or for the treatment of halitosis. Even the possibility of emergence of unexpected halitosis after the preventive extraction exists, there are few studies describing the unexpected halitosis. This study intended to measure the emergence of halitosis after extraction of mandibular impacted third molar. Thirty-eight patients who visited Daejin Medical Center participated in this study. Halitosis was objectively measured using Halimeter in 3 intervals-before, after I day, after 7 days. Third factors, scaling and gender, which may influence the halitosis were analyzed with stratification. This study finds that in general halitosis decreased after 1 day but increased after 7 days. Scaling was helpful in decreasing halitosis and women have less severe halitosis than men. The effects of time interval(p<0.05), gender(p=0.836), and scaling(p=0.7929) on the severity of halitosis were not statistically significant. However, since this study's patients are neither a representative sample nor a random sample, all data interpretation was focused on clinical significance instead of conventional statistical significance. Clinical significance of this study's findings are: 1) scaling should be conducted in advance, 2) men should be notified of the higher possibility of halitosis.

Complications of impacted third molar extraction: retrospective study (매복지치 발치 시 발생한 합병증 양상에 대한 후향적 연구)

  • Introduction: Surgical extraction of third molar is one of the most frequently performed procedures in oral and maxillofacial surgery unit as the impacted wisdom teeth could cause various complications. Even though, however, extraction of the impacted wisdom teeth is an obligation for the reason of possible complication even for general practitioners, it has been avoided. Various factors concerning surgical extraction of impacted third molar are considered: general condition of patients, relationship with relative anatomies, aspects of impaction, surgeon's skill. Materials and Methods: The consideration and crossing analysis of these factors with 2,463 patients who visit Dankook University dental hospital to extract those impacted third molar. Results: 1. Gender doesn't affect. 2. Medical problems have more complications. 3. There are more complications in high difficulty index (DI) impacted teeth. 4. When inferior alveolar canal overlap mandibular third molar, complication rate is 26.92%. 5. The most common complication was swelling and pain. 6. There is no statistical difference between the incidence of complication and surgeon's experience. Conclusion: In regard to these results, it seems that clinical or radiological examination can predict potential complications of wisdom teeth, and it is helpful to bear in mind the fact.

Propofol and Fentanyl Compared with Midazolam and Morphine for Conscious Sedation During Surgical Extraction of Bilateral Mandibular Impacted Third Molars (양측 하악 매복지치 동시 발치를 위한 의식하 진정의 비교 연구: Propofol, Fentanyl과 Diazepam, Morphine)

  • Background: The purpose of this study was to compare the availability of propofol and fentanyl (P + F) with diazepam and morphine (D + M) for intravenous conscious sedation during third molar surgery. Methods: Forty patients without systemic disease were operated under IV conscious sedation administered by either of the two techniques. Monitoring consisted of continuous observation of pulse rate, blood pressure, oxygen saturation, and the respiratory rate and were recorded every 15 minutes. Cooperation score was measured 5 and 15 minutes after induction of IV sedation. Following the operation, the surgeon and patients completed questionnaires including pain visual analog scale, amnesia, and side effects. Results: The P + F group was significantly more cooperative than the D + M group. The side effects of D + M group included pain on injection, nausea/vomiting and abdominal pain. The side effects of P + F group included talkativeness, nausea/vomiting, temporary apnea, pain on injection and vertigo. Conclusions: In this study, there were following benefits in the P + F group; more cooperative status and less pain perception. But respiratory depression developed in some patients.

  • Actinomycosis is chronic, granulomatous, suppurative and fibrosing disease caused by Actinomyces. Actinomyces are anaerobic, G(+), non-acid-fast, branched, filamentous bacteria. The most commonly found microorganism is Actinomyces israelii. Common site for isolation of actinomyces are dental plaque, dental caries, calculus, and tonsillar crypt. A breach in the integrity of the mucosa by direct trauma or following a fracture, tooth extraction, root canal therapy or some intraoral surgical procedure is thought to be the most likely portal of entry. This is a case report of 23 years old male with cervicofacial actinomycosis developed after extraction and treated with surgical excision and antibiotics.

Effects of Low Dose Gamma Radiation on the Formation of Shikonin Derivatives on Callus Cultures of Lithospermum erythrorhizon (지치(Lithospermum erythrorhizon S.)의 캘러스배양에서 Shikonin 유도체 생산에 미치는 저선량γ선의 효과)

  • The effects of low dosage ${\gamma}$-radiation on the cell growth and the formation of shikonin derivatives were investigated in callus cultures of Lithospermum erythrorhizon under different medium and light conditions. Gamma radiation significantly affected the cell growed and formation of shikonin derivatives, depending on the culture conditions. In the cell cultures grown on M-9 medium, 2Gy and 16Gy of ${\gamma}$-radiation increased the calli growth and the formation of shikonin derivatives, respectively under 16hr day light condition. When calli were cultured for 60 days in the dark after irradiation of ${\gamma}$-radiation, cell growth was increased at low dosage of 1Gy and 2Gy in LS medium containing BA 2mg/L and IAA 0.2mg/L. Interestingly, calli grown in M-9 medium by 2Gy irradiation for 60 days significantly stimulated the formation of shikonin derivatives(13.21mg/g cell fresh wt), which was approximately 6 times higher than untreated cells.

Identification of Fungal Pathogen Causing Seedling Rot of Lithospermum erythrorhizon and Study on the Optimum Growing Temperature for Decreasing of the Seedling Rot (지치 육묘시 발생하는 유묘썩음증상 원인균 분리·동정 및 경감 육묘환경에 관한 연구)

  • This study was conducted to isolate and identify the fungal pathogen causing seedling rot of Lithospermum erythrorhizon Siebold & Zuccarini, and to know the optimum growing temperature for decreasing seedling rot of Lithospermum erythrorhizon. On the basis of morphological characteristics, EF-1a sequence analysis, and pathogenecity to host plant, the fungi isolated from seedling rot and seeds of Lithospermum erythrorhizon were identified as Fusarium fujikuroi, indicating that disease causing fungus is seed-borne pathogen. Optimum temperature for germination of seeds of Lithospermum erythrorhizon was $15{\sim}20^{\circ}C$, but pathogenicity of Fusarium fujikuroi was shown more readily at $25{\sim}30^{\circ}C$. These results suggested that seedling culture of Lithospermum erythrorhizon between $15^{\circ}C$ and $20^{\circ}C$ might reduce seedling rot of Lithospermum erythrorhizon caused by seed-borne pathogen Fusarium fujikuroi.

  • The surgical removal of the wisdom teeth is obligate when forceps extraction fails or when the wisdom teeth are impacted. The surgical removal of impacted maxillary third molars is a commonly performed procedure usually associated with few complications & little morbidity. The most frequent complications are tooth root fracture, maxillary tuberosity fracture, tooth displacement into the maxillary sinus & oroantral fistula formation. A rarely reported complication is the displacement of a tooth into the infratemporal fossa. The method of prevention of this complication is by the placement of either a finger or periosteal elevator posterior to the tooth during extraction. To remove the displaced upper third molar is very difficult & has many complications, e.g., persistent bleeding & nerve damage. When the wisdom teeth is displaced, it is initially necessary to gain access to bone by developing a mucoperiosteal path of delivery is developed by additional bone removal or, preferably planned sectioning of the tooth. There are many approaching techniques to remove the displaced upper third molar. This following report describes the surgical technique of displaced upper third molar in the pterygopalatine fassa by the midpalatal &transpharyngeal approach.

Resilient Modulus of Weathered Granite Soil in the Central Part of Korea (화강암풍화토의 동탄성계수에 관한 연구 -중부지역을 중심으로-)

  • Over the years, most pavement designs based on soil strength and permanent strain are almost independent of soil elasticity. However, it was found that plasticity and elasticity of soil have both effected on the failure of pavement structures. The elasticity of soil, hence, using the resilient modulus is reflected for recent pavement design. Although the current AASHTO specifications(1986) for pavement design had changed the soil support value to the resilient modulus, triaxial devices conducting the resilient modulus test have not been fully equipped in a great majority of laboratories. Thus, in the present work, such a resilient modulus is usually derived(from CBR, K values, etc.) by estimating equations. The purpose of this study is to evaluate the resilient modulus of weathered granite soils sampled from 4 points of the central region of Korea by means of AASHTO T 274-82. According to this, some empirical equations for predicting that of the weathered granite soil are proposed and then, the relationship to convert CBR into the resilient modulus is developed.
