The Development and Effect of a Dementia Care Support Program among Community Dwelling Older Adult Men (지역사회 거주 남성노인을 위한 치매 돌봄 프로그램 효과)

  • Purpose: We developed and tested the effects of a care program for dementia patients among community- dwelling older adult men. Methods: This study used a non-equivalent control group pre-and post-test design. To verify the effectiveness of the care program for dementia patients, 26 community dwelling older adult men participated in this study. We used multiple intervention strategies including improving understanding of dementia through education, activities for dementia prevention, and promoting psychological change. In particular, based on the current understanding of the care of the men older adults, we used strategies to promote motivation and reinforce strengths. Results: After completing the 6-week intervention program, when compared with the control group, older adult men in the care program intervention group showed significant differences in scores for the following: cognitive functions (p=.035), attitude toward dementia (p=.026), preventive behavior (p=.007), geriatric depression (p=.013), caring confidence (p=.018), and self-esteem (p=.013). Conclusion: These results indicate that the care program for dementia patients has positive effects on increasing their cognitive function, attitude toward dementia, preventive behavior, caring confidence, self-efficacy, and on decreasing depression rates in this population. Based on this, we can recommend this program to men caregivers for the improved care of dementia in community centers.

  • Rapid Aging Society demands the transformation of medical paradigm of diagnosis and treatment towards prevention and management. This paper explores the norm and development of digital health care, focusing on Busan Metropolitan City. Digital health care which combines new ICT technology and medical technology is predictive, preventive, personalized and participatory; and suggests alternative to solve the problem of demographic changes and increasing social cost of medical welfare. Community Health Center in Busan is unique one based in the minimum community of collecting data from self-leading health management. Digital transformation using basic health data and social information can build preventive care system in the community. Easy access leads community center to test bed of developing new technology, as a living lab. In order to use the newly developed goods and service effectively, user-participatory test is nicessary. Finally community nurse and activists can specify health-welfare converged service through digital transformation empowerment training.

  • The aim of this study is to explore fathers' own expectations and experiences of fatherhood along with their views of what it meant to be a 'good' father. Qualitative methodologies were used; semi-structured in-depth interviews with 10 dual-earner fathers with young children. Interviewees overwhelmingly welcomed the opportunities offered to them by the new fatherhood model and supported a perceived cultural shift towards men and fathers being involved in, rather than detached from, family life. However tension and difficulty in living the ideal were also reported: gaps between perceptions and behaviors; struggle for traditional breadwinning role and caring; conflicts between selflessness and career and uninvolved in family decisions. Political and practical considerations are discussed, and the implications of this study for future research are identified.

Factors Influencing Burnout in Primary Family Caregivers of Hospital-based Home Care Patients (의료기관 가정간호 환자 주돌봄자의 소진에 영향을 미치는 요인)

  • Purpose: The purpose of this study is to identify factors influencing burnout in primary family caregivers of Home Health Care Patients. Methods: Data were collected from 121 primary family caregivers of home health care patients in three different hospitals in 'D' metropolitan city and the study was conducted from August 10, 2016 to January 17, 2017. The data were analyzed using descriptive statistics, t-test, ANOVA, Pearson's Correlation Coefficient, Stepwise Multiple Linear Regression. Results: Mean scores for the nursing needs of the participants were $3.54{\pm}0.79$, the family functions were $1.24{\pm}0.58$, the burnouts were $2.74{\pm}0.49$. The burnouts were positively correlated with the nursing needs but inversely correlated with the family function. The factor that had the greatest influence on the burnouts of primary family caregivers of Home Health Care was family function (${\beta}=-.245$, p=.001), followed by patients' daily activity (${\beta}=-.213$, p=.014), age (${\beta}=.208$, p=.032), monthly nursing services cost (${\beta}=-.196$, p=.044) and nursing needs (${\beta}=.129$, p=.014). The Explanatory Power of Models was 23%. Conclusion: Individually customized home care nursing intervention programs are required to be provided in accordance with patient's family function and daily activity, monthly home care nursing service cost, nursing needs and general characteristics of primary caregivers of Home Health Care Patients such as their age, the number of family members living together, sex and the name of disease.

The Impact of Professional Self-Concept on the Organizational Socialization of Hospital Nurses (전문직 자아개념이 병원간호사의 조직사회화에 미치는 영향)

  • The purpose of this study wa to analyze the impact of nurses' perceptions of care and professional self-concept on their organizational socialization. The participants were 193 nurses working in university hospitals and general hospitals in regions C and D. Data collection took place from July to August 2016, and the data were analyzed using descriptive statistics and multiple regression analysis with IBM SPSS 22 software. The results indicated that educational level and professional self-concept are determining factors influencing organizational socialization. Nurses with an associate degree showed higher levels of organizational socialization compared to those with a bachelor's degree or higher, and higher levels of professional self-concept were associated with higher organizational socialization. Therefore, it is necessary for hospitals to implement effective human resource management that enables nurses of each educational level to quickly socialize within the organization, providing differentiated support and programs based on educational level. Additionally, continuous research is required to enhance nurses' professional self-concept and establish their work identity.

Analysis of Characteristics and Symptoms in Home-Based Hospice-Palliative Care Patients Registered at Local Public Health Centers (일 지역 보건소 등록 호스피스 완화돌봄 대상자의 특성 및 증상 분석)

  • Purpose: This study was aimed at analyzing the characteristics and symptoms in home-based hospice-palliative care (HBHPC) patients registered at local public health centers. Methods: A retrospective study was performed; Data of 144 HBHPC patients registered at six public health centers in Pusan City were analyzed, including their initial visit records (registration cards, initial pain evaluation and symptom evaluation). Results: The average age of the patients was 67.7 years old. Among all, 46.2% of the patient lived alone, and 65% had middle school education or lower. The most popular (36.3%) religion was Buddhism, and 47.5% received medical assistance from the government. The most frequent diagnosis was lung cancer followed by stomach cancer and liver cancer in that order. Of all, 48.9% were functionally too weak to lead a daily life, 39.6% were under cancer treatment when registered at the public health center, and 84.5% were aware of the fact that they have reached the terminal phase. Moreover, 83.6% complained about pain, and the pain level was moderate or severe in 36.5% of them. Besides pain, fatigue was the most complained symptom (84.7%), and 49.3% of them rated their fatigue as moderate or severe. Conclusion: Most of the HBHPC patients were socio-economically underprivileged and complained about moderate or worse pain and symptoms. Therefore, it appears necessary to develop an integrated strategy that is tailored for each patient reflecting their characteristics.

  • This study aims at investigating the major symptoms that help family carers detect mental illness in elderly patients. Another purpose of this study is to empirically verify the major factors determining the utilization of mental health services with a focus on family carers. The results of this study are as follows. First, the most commonly detected symptoms that caused the family carers to suspect mental illness in the elderly patients were memory decline and other forms of cognitive function decline. Second, the determinants of the elderly's utilization of mental health services included the patient's long-term care insurance level, the age of the family carer, the period of care, the level stress associated with the provision of care felt by the carer, his understanding of geriatric mental illness, and the level of perception about community mental health services. Based on these findings, this study suggests policies and practical implications for the early detection of and response to elderly mental health problems and the utilization of related services from the viewpoint of the family carers of the elderly.

Perception of Community Family-Friendliness among Parents with Young Children in Urban and Rural Areas (도시화 규모에 따른 미취학자녀 부모의 지역사회 가족친화성 인식 : 대도시, 중소도시, 농어촌의 비교)

  • This study aims to find the association between community scales and perceived community family-friendliness among parents with young children. Communities were identified into three groups: metropolitan, small and medium-sized cities and rural, according to the degree of urbanization. The data were drawn from a sample of 1,038 parents who had at least one pre-school child. The data were analyzed by frequencies, means, GIS, ANOVA, and multiple regressions. This study compared the level of community family-friendliness both at the 'dong' and 'si' levels after controlling for the individual and household characteristics of the respondents. The findings of this study are as follows. First, after controlling for individual and household factors, the level of perceived community family-friendliness differed by the community scales. The parents in the small and medium-sized cities had higher scores on community family-friendliness than their counterparts in metropolitan communities. And parents in rural communities received had lower scores in community family-friendliness than those in metropolitan communities. The results also showed similar patterns for the 'dong' and 'si' models. While job and housing type were associated with the perceived family-friendliness of the community, other characteristics such as sex, age, educational level, monthly household income and home ownership were not.

Impact Of Convergence on the psychosocial well-being levels : On Some Care Service Providers (직무만족이 정신사회적 안녕수준에 미치는 융합적 영향 : 돌봄 사회 서비스)

  • This study was conducted to identify the Convergence influence of job satisfaction of care service providers on their psycho-social well-being status. A study subjects were 308 care service providers employed in local self-support centers for Social Service in Gwangju, Jeonam Jeonbuk province, from whom data was collected using self-administrated structured questionnaire after regular holiday education. In consequence hierarchical multiple regression analysis, research findings; adjusting general and occupational characteristics, the higher they perceived sub-dimensions of job satisfaction such as relationship with job demands(${\beta}=0.393$, p=.063), clients(${\beta}=0.669$, p=.000), relationship with peers and supervisors(${\beta}=0.229$, p=.068) the better their psychosocial well-being status was. And psychosocial well-being status of those who held certificate of care provider(${\beta}=4.815$, p=.003), helpers for neonate and mother(${\beta}=4.348$, p=.011) were higher than that of those who did not, which reflexed difference in their clients. As a results, promote mental health of care providers, it would be necessary to provide them training programs for facilitating human relationships with clients, peers and supervisors.
