Association of Lifestyle Factors With the Risk of Frailty and Depressive Symptoms: Results From the National Survey of Older Adults (노인의 라이프스타일 요인이 허약 및 우울 위험도에 미치는 영향: 노인실태조사 자료를 바탕으로)

  • Objective : This study aimed to investigate the association between lifestyle factors and risk of frailty and depressive symptoms among older South Korean adults. Methods : This study included 10,072 individuals aged 65 or older from the 2017 National Survey of Older Koreans, a cohort of community-dwelling older South Koreans. The following lifestyle factors were assessed: physical activity, nutrition management (NM), and leisure/social activity participation (AP). Frailty was measured using the frail scale and depressive symptoms were measured using the Geriatric Depression Scale. Logistic regression analyses were performed to determine the odds ratios. Results : All lifestyle factors were associated with the risk of frailty and depressive symptoms in the study population. Regular exercise (≥3 times/wk, odds ratio [OR] = 0.59, 95% confidence interval [95% CI] = 0.52~0.91; OR = 0.66, 95% CI = 0.59~0.75), active NM (OR = 0.86, 95% CI = 0.80~0.91; OR = 0.81, 95% CI = 0.76~0.86), leisure AP (OR = 0.79, 95% CI = 0.74~0.84; OR = 0.71, 95% CI = 0.66~0.77) and social AP (OR = 0.92, 95% CI = 0.88~0.96; OR = 0.82, 95% CI = 0.78~0.87) were correlated with lower odds ratios of frailty and depressive symptoms. Conclusion : Adopting a healthier lifestyle characterized by regular exercise, balanced nutrition, and active engagement in various activities can effectively reduce the risk of frailty and depressive symptoms among the older population. Ultimately, this study emphasized the essential role of lifestyle choices in promoting the physical and mental well-being of older adults.

A Recognition for community Based Rehabilitation by Public Health Center Physical Therapists in Seoul Province (서울시 보건소 물리치료사들의 지역사회중심재활에 대한 인식)

  • To survey the recognition of Community Based Rehabilitation(CBR) by Public Health Center(PHC) Physical Therapists in Seoul Province. This study analysed general characteristic of Physical Therapists and cognition, participation, education, problems and requirements for CBR. The subjects of this study were 31 therapists who work at PHC in Seoul Province and Seoul City Childerns Municipal Hospital. Data was collected for 18 days from April 13 to 31, 1998 and results computed from 25 responses. First, Female therapists outnumber males. The age category 30 to 35 old represented 44.0% of the total number studied. 48.0% of therapists had worked more than 7 years and therapists usually 84.0% worked from 3 to 4 years at the PHC. Junior colleage graduates were 72.0%, and respondents with family responsibility were 52.0%. Second, Recently pain control and therapeutic exercises has become the major requirement of patients at PHC(56.0%). If the offered expanded services, a rehabilitation programe at PHC(36.0%) was highly. When asked about a working guide book, nobody had one. Third, 64.0% of therapists were aware of the present level of CBR, 28.0% of them had gotten an information from newspaper and mass media. Fourth, For CBR, 72.0% had positive ideas in which they could participate. 56.0% wished to compose a team of various rehabilitation specialists. 52.0% wished for the meeting day to be flexible and 64.0% desired to meet once or twice a week. Fifth, Almost all(92.0%) desired more educational opportunity about CBR. Example, once or twice a year(48.0%) and training period of 4 to 7 day(44.0%). They wanted to learn theory and parctice together(88.0%) and 48.0% wanted national mutirehabilitation center as a educational managing organization. Sixth, The most important requirement for accomplishing CBR by Physical Therapists was job security and adequate number of therapists(48.0%). Required number of physical therapists in the PHC was 3(52.0%). The cooperative agency should be the distict govement(32.0%) and community welfare center of the disabled persons(32.0%). Factors inhibiting the execution of CBR were lack of physical therapists(56.0%), and equipments for pain control and for therapeutic exercises(68.0%).

The Effects of Functional Task Training on Activities of Daily Living in the Fall Experienced Elderly (기능적 과제훈련이 낙상경험노인의 일상생활수행능력에 미치는 영향: 단일대상연구)

  • Objective : The purpose of this study was to investigate the effects of occupational therapeutic intervention including functional task training on activities daily living of falls experienced elderly. Methods : This study used a single subject experimental design with alternating treatment and reversal design. One participant who have had falls experience among the community-dwelling elderly completed. This study a total of 18 sessions were conducted. Fall Efficiency Scale(FES), Berg Balance Scale(BBS) and task modification scale were used to assess the activities of daily living during each session, and Canadian Occupational Performance Measure(COPM) and Assessment of Home-based Activities(AHA) were used to evaluate the activities of daily living at pre-and post-test. Results : The participant showed significant improvements in the FES, BBS scores. However, the improvement was greater when progressive resistance exercise was practised along with functional task training. COPM and AHA scores improved at post-test. Conclusions : These findings provide evidence for occupational therapeutic intervention for falls-experienced elderly.

The objects of this study were 37 therapists who work at 27 of 39 PHC in Kyoungki province and data was collected for 20days from Jan. 12 to 31. 1998 and the results computed from 32 responses. First, Female therapists outnumber males. The age category 30 to 35 old represneted $56.2\%$ of the total number studied. $59.4\%$ of therapists had worked more than 7 years and therapists usually$(46.9\%)$ worked from 6 month to 2 years at the PHC. Junior college graduates were $84.4\%$, and respondents with family responsibilirt were $65.6\%$. Second, Recently pain control has become the major requirment of patiants at PH$(53.1\%)$. If they offered expaneded services - a rehabilitation program $(53.1\%)$, home vistation $(31.3\%)$. when asked about a working guide book, nobody had one. Third, $68.7\%$ of therapists were aware of the present level of CBR, $40.9\%$ of them had gotten an information from educated colleagues, and $68.7\%$ of therapists were aware of the present level of CBR, $86.4\%$ of therapists felt responsible to provide rehabilitation Fourth, For CBR, $93.7\%$ had positive ideas in which they could participate. $73.3\%$ wished to composed a team of various rehabilitational specialists. $73.3\%$ wished for the meeting day to be flexible and $86.6\%$ desired to meet once or twice a week. Fifth, Almost all$(96.9\%)$ desired more educational opportunity about CBR. Example, once or twice a year$(56.2\%)$ and training period of 4-7 days $(43.7\%)$. They wanted to learn theory and practice together$(74.2\%)$ and $74.2\%$ wanted the mational multi-rehabilitation center as a educational managing organization. Sixth, The most important requirment for accomplishing CBR by physical therapists was job security and an adequate number of therapists$(84.4\%)$ Required number of physical therapists in the PHC was 3 $(43.7\%)$. The cooperative agency should be the district goverment organization $(56.2\%)$. Factors inhibiting the execution of CBR were lack of administrative support, physical therapists, and equipments for pain control and for therapeutic exercise.

Reliability and Validity of the Korean Version of Melbourne Low-Vision ADL Index (한국판 맬버른 저시력 일상생활지수(Melbourne Low-vision ADL Index: MLVAI)의 신뢰도 및 타당도)

  • Objective: The purpose of this study was performed to modify the performance-based assessment tool, Melbourne Low-Vision ADL Index (MLVAI) had been developed in Australia to suit the Korea culture to verify the reliability and validity. Methods: The subjects were only over the age of 20 living in communities, 26 with low-vision and 42 normal persons. The Korean MLVAI was completed through the expert translation verification and validation of the configuration tool. The validity of the Korean MLVAI was established through the content, discriminant, and convergent validity. Also, the reliability was analyzed through internal consistency reliability for the items, test-retest, and interrater reliability. Results: The Content Validity Index(CVI) was more than .78. There was a statically significant low in low-vision. Also, there was a statically significant low in low-vision. The convergent validity was determined the correlation coefficient of .751 analyzing LVQOL and Korean MLVAI total score, had a significant correlation(p<.05). Cronbach's ${\alpha}$ coefficient indicated an internal consistency of .983(p<.05). Test-retest reliability had a high, significant correlation of .976 and interrater reliability had a high, an intraclass correlation coefficient of .91(p<.05). Conclusion: The results of this study mean that the Korean MLVAI which was modified to fit the Korean is the ADL assessment tool with both validity and reliability. Based on this study, the Korean MLVAI can be used as a useful ADL assessment for OT interventions in low vision.

Analysis the of User's Needs for Developing a Mobile Health Based Lifestyle Care Application for Health Promotion among the Elderly (장노년층 건강증진을 위한 모바일 헬스 기반 라이프스타일 케어 앱의 사용자 요구도 분석)

  • Objective : Recently, the demand for wearable mobile devices for the monitoring and improvement of one's health and quality of life is increasing. Therefore, the purpose of this study is to analyze the need of potential users in order to develop mobile health based and lifestyle care application for the elderly. Methods : Participants who lived in their community and used a mobile phone were recruited. Finally, a total of 84 participants completed the survey. Data were analyzed using descriptive statistics and a t-test, which was carried out with SPSS version 25.0. Results : The application functions that users deemed important for a lifestyle care app were the number of daily steps, physical activity, blood pressure, sleep, nutrition and participation in activity. Interestingly, there was a significant difference in the importance given to the app function of participation in activities between age groups. Conclusion : This study investigated the need and preferences of potential users of health and lifestyle care application for the promotion of health among the elderly. The, findings obtained from this study could be a valuable resource for the development of lifestyle care application.

A Systematic Review on Accelerometer to Measure Activity of Daily Living of Patients with Stroke (뇌졸중 환자의 일상생활활동 평가도구인 가속도계에 대한 체계적 고찰)

  • Objective: The purpose of this study was to systematically review the articles using accelerometer to measure activity of daily living of patients with stroke. Methods: Depending on selection criteria, 13 studies were searched for PubMed, EMBASE, and Cochrane library database from February 2014 to March 2014. A total of 331 papers were searched, and 13 of these were selected. Results: In studies of 13 selected, acute, subacute, and chronic patients with stroke were enrolled. The kind of accelerometer was uniaxial, biaxial, and triaxial, activity monitor. Measurement activities were mainly arm activity, walking activity, and attachment sites were also various depending on the measurement activities. A measured variable was the total number of activities, the movement speed of the patients, ratio between affected and non-affected, and motion analysis. The result indicated that significant correlation with the other assessment tools in all studies. Conclusions: Accelerometer will be applied with a tool for measuring activity of daily living of patients with stroke, depending on activities characteristics. Further, we need accelerometer studies to apply with a variety of assessment in clinical practice or community settings.

The Post Occupancy Evaluation of the Universal Design Project on Geonjisan Forest Trail Jeonju City, South Korea (전주시 건지산 숲길 Universal Design 사업 이용후 평가)

  • The project on Geonjisan Universal Design Forest Trail in Jeonju City was designed and constructed by an NGO called 'Jeonbuk Forest for Life' which was a winner for the '2008 Open Competition by the 'Committee for Greening Society' established in Korea Land Corporation. The purpose of this study is to evaluate the design satisfaction of UD forest trail. Post Occupancy Evaluation (POE) method was applied to measure the user satisfaction and the total number of users. The main result of the study were as follows: the major users of the Geonjisan UD forest trail were people aged over 60 years old(37%) and the main purpose of trail usage were to 'walk and rest'(51.5%) and to 'exercise and rehabilitation' (40.6%). Furthermore, the overall user rating for the UD forest trail design was "satisfied"(3.91 point in 5-point Likert Scale). The three most influencing factors of the overall user satisfaction were facility management, user conflict, and trail width and slope. About forty-five people were found to be End-user in the UD forest trail while six people were found as End-user in non-UD trails. Most importantly, the number of End-user observed on the UD forest trail was greater than the number found in non-UD forest trail. The result implies that the UD forest trail attracts more End-users and provides opportunity for gathering and interaction with the other users. Moreover, the satisfaction rate for the UD forest trail landscape is found to be high in Likert scale, which we can assume that the well-grown existing trees and topographic features as well as appropriately designed wood-paths influence the high satisfaction rate of the users. The POE of UD forest trail revealed the importance of universal design concept due to its convenient uses of the handicapped, old, weak, pregnant woman or children.

The Correlation between Activity Ability and Participation in Children with Cerebral Palsy (뇌성마비 아동의 활동 능력과 참여 사이의 상관관계)

  • This study was performed to search for correlation between activity limitation level and participation restriction of children with cerebral palsy by investigating the activity limitation level through the use of the GMFCS E&R and the MACS and by figuring out participation restriction level through questionnaire survey. This study was performed, from May 1, 2010 to August 31, 2010, on 152 children with cerebral palsy ranging from 4 to 12 years, who are receiving rehabilitation therapy in the hospitals and community clinics in Busan and Gyeongnam province. The levels of activity limitation were assessed by using GMFCS E&R and MACS, and a questionnaire survey was conducted for participation restriction of the participating children. Spearman rank correlation was used for correlation analysis with the statistical software, SPSS 12.0. Majority of the children scored level 5 in both GMFCS E&R and MACS which is high frequency of activity limitation levels. Children in these levels also showed severe or complete participation restriction for mobility, education, and social relations. The analysis showed a significantly positive correlation between activity limitation level and participation restriction. Therefore this study may be useful in assessing the functional movement component of participation in children with cerebral palsy and developing the intervention plan for participation.

Development of Dual-task Based Cognitive Function Screening Test for the Elderly (이중과제를 활용한 노인인지기능 선별검사 개발)

  • Objective : The purpose of this study was to develop a Yonsei dual task cognitive screening test (Y-DuCog) for the elderly. Methods : The reliability and validity test of Y-DuCog (Yonsei Dual Task Cognitive Screening Test) was developed by 229 elderly people aged over 60 years from community organizations at Gyeonggi-do and Chungcheong-do from May 2019 to August 2019. In addition, the criteria for classifying elderly with cognitive impairment were presented. Results : The correlation analysis between MMSE-K, MoCA-K and Y-DuCog were a correlation between the DTE and CRR of Y-DuCog. As a result of internal consistency, Cronbach's-α values of DTE and CRR showed .848 (p<.01) and .916 (p<.01), respectively. The test-retest reliability was high. The screening point showed 88.7% sensitivity and 83.5% specificity at 31.76 seconds in total DTE, and 84.5% sensitivity and 76.6% specificity at 0.38 in total dual-task CRR. Conclusion : This study verified the reliability and validity of Y-DuCog. It was found that the level of education was not a barrier to the undertaking of this test. Furthermore, the test could be performed easily and quickly. It is also expected to be used to evaluate the effectiveness of cognitive function assessment and intervention methods in the elderly.
