임플란트의 collar design이 변연골과 연조직에 미치는 영향 (Effects of implant collar design on marginal bone and soft tissue)

  • 연구 목적: 임플란트 경부가 잘 설계된 경우 양호한 연조직 반응을 통해 변연골을 보존하는데 도움이 된다. 본 실험에서는 연, 경조직 경계부에 가장 가까이 위치하는 임플란트의 collar design이 변연골 변화와 연조직 반응에 미치는 영향을 동물실험을 통해 알아보고자 하였다. 연구 재료 및 방법: 2마리의 건강한 Beagle dog에 임플란트 collar design만 다른 두 종류의 임플란트(Neobiotech Co. Seoul, Korea)를 식립하였다. Collar에 bevel 을 부여한 군(Bevel군)과 "S"자 형태를 부여한 군(Bioseal 군)으로 나누어 마리 당 7개, 군당 7개, 총 14개의 임플란트를 무작위로 식립한 후 Healing abutment를 즉시 체결하였다. 디지털 표준구내 방사선사진을 이용해 4주 간격으로 총 12주간 근원심 변연골 변화를 관찰하였고, 12주에 희생하여 조직학적 분석을 통해 협설 변연골 흡수 및 임플란트 주변 연조직 반응을 평가하였다. Mann-Whitney test를 통해 동일한 방사선 사진 촬영 시점에서 근원심 변연골 변화량 및 조직계측치를 군 간 비교하였고, Kruskal-Wallis test를 통해 방사선 사진상 근원심 변연골 변화량이 시간에 따른 차이가 있는지 군 내 분석 한 후 Duncan test를 통해 사후 검증하였다(${\alpha}=.05$). 결과: 방사선학적 분석 결과 각 촬영 시점에서 두 군간 근원심 변연골 변화량의 차이를 보이지 않았다(P>.05). 군내에서 시간에 따른 근원심 변연골의 흡수량을 분석한 결과 Bevel 군에서는 시간에 따른 차이를 보이지 않았으나 (P>.05), Bioseal 군에서는 시간에 따른 차이를 보였으며, 4주및 8주와 비교했을 때 12주에서 변연골의 증가를 보였다(P<.05). 조직학적 분석 결과 협설측 변연골 흡수량에서 두 군간 차이를 보이지 않았으나 (P>.05), Bevel 군에 비해 Bioseal 군에서 더 견고한 결합조직부착을 관찰할 수 있었으며, 생물학적 폭경의 값은 두 집단 간 차이를 보이지 않은 반면에 (P>.05), 접합상피부착은 Bevel 군에서 유의하게 길었고, 결합조직부착은 Bioseal 군에서 더 길게 나타났다(P<.05). 결론: Bevel 군에 비해 Bioseal 군에서 결합조직부착은 길게 형성된 반면에 접합상피부착은 더 짧게 나타났으며, 생물학적 폭경과 초기 변연골 흡수에는 차이가 없음을 알 수 있었다. 연조직 반응의 차이가 실제 기능하중 하에서 변연골 변화에 미치는 영향에 대해서 향후 연구가 필요할 것으로 생각된다.

발치 직후 시행된 즉시 임플란트 식립술에 관한 임상적 평가 (Clinical Evaluation about the Immediate Implant Replacement after Tooth Extraction)

  • Background: Immediate implant placement has become an acceptable treatment for the edentulous area. The advantages of the immediate implant placement include considerable decrease in time from tooth extraction to placement of the finial prosthesis, fewer surgical procedures, and better acceptance of the overall treatment plans. But the success is dependent on the quantity and quality of the extraction socket. The purpose of this study is to evaluate the success of the immediate implant placement. Materials and Methods: Twenty-one sites in 16 patients were selected for the evaluation of the immediate implant placement. All of the cases were followed using clinical and radiographic examinations. Criteria of success were the absence of peri-implant radiolucency, mobility, and persistent pain or sign of infection. Results: Of the 21 implants, 13 implants have been succeeded. Of the 13 implants, 10 implants were replaced for the periodontal disease and 3 implants were replaced for the trauma. Conclusion: The criteria of the success in immediate implant placement are as follows. 1) Implants placed into fresh extraction sockets have a high rate of survival. 2) Implant should be placed as close as possible to the alveolar crest. 3) Implant placed into available bone beyond the apex have a high success rate.

상악 전치부에서 단일 치아 발거 후 즉시 임플란트 식립 (Immediate implant placement in areas of aesthetic priority)

  • Implants placed immediately after tooth extraction have been shown to be a successfully predictable treatment modality. Several clinical papers suggest that placing implants immediately after tooth extraction may provide some advantages: reduction of the number of surgical procedures or patient visits, preservation of the dimensions of alveolar ridge, and shortening of the interval between the removal of the tooth and the insertion of the implant supported restoration. In this case report, three patients received single immediate implant placements to replace a maxillary anterior tooth at the time of extraction. As the three cases were somewhat different, treatment protocols had to be modified as follows: Case 1. Immediate implant placement with healing abutment connection. Case 2. Immediate implant placement with immediate provisionalization. Case 3. Immediate implant placement with Guided Bone Regeneration(GBR). Every implant of these cases was placed in proper position buccolingually, mesiodistally and apicocoronally, The procedures following implantation such as immediate provisionalization and GBR were free of problem. Healing of each case was uneventful. In all cases, treatment outcomes were mostly satisfactory and the results maintained during follow-up periods. However, one case (Case 3) showed some papilla loss due to failure in delicate soft tissue handling during surgery. This papilla loss was compromised by prosthetic means. In conclusion, immediate implant placement in the fresh extraction socket can be a valid and successful option of treatment in aesthetic area. Moreover, this treatment protocol seems to maintain the preexisting architecture of soft and hard tissues in most cases.

치근단 수출 시 형성된 골결손 부위를 가진 치아의 발치 후 즉시 식립 임플란트에서 골 접촉률에 대한 흡수성 차폐막의 효과 (Effect of resorbable membrane on immediate placement of implant in extraction socket during periradicular surgery)

  • Purpose: The guided bone regeneration (GBR) technique is widely used in periradicular surgery. However, there is still some controversy regarding the effectiveness of GBR in promoting bone healing after periradicular surgery. The purpose of this study was to evaluate the resorbable membrane on the osteointegration of immediate implants in sites with periradicular lesion that had been removed by periradicular surgery. Materials and methods: Six roots of lower second premolars and 15 roots of lower third and fourth premolars of dogs were used as control and experimental teeth, respectively. Periradicular lesions were induced only in the experimental teeth. Twelve weeks later, the control and experimental teeth were extracted and implants were placed immediately. Periradicular lesions were removed with osteotomy, curettage and saline irrigation. Resorbable membranes were used in experimental group 1 but not in experimental group 2. After 12 week of healing period, the implants were clinically not mobile and showed no signs of infection. Data obtained by histomorphometric analysis were analyzed by Kruskal-Wallis test. Results: The control group showed a significantly higher bone to implant contact (BIC) ($74.14{\pm}16.18$) than experimental group 1 ($40.28{\pm}15.96$) and 2 ($48.70{\pm}17.75$)(p<0.05). However, there was no significant difference between experimental group 1 and 2. Conclusion: Although BIC in experimental groups were lower than in control group, immediate implant can be successfully placed at extraction socket with periradicular lesion and osseous defect. However, the use of resorbable membrane in bony defect created during periradicular surgery was questioned.

  • Supernumerary teeth are extra teeth or tooth-like structures which may have either erupted or unerupted in addition to the 20 deciduous teeth and the 32 permanent teeth. Clinicians sometimes confront unerupted supernumerary teeth where implants would be placed. Many clinicians consider immediate placement of implant into an extracted socket as an effective technique compared to the conventional method of waiting several months for bone healing, because of the advantages such as reduction of treatment period, preservation of alveolar bone and soft tissue, etc. We could hardly find the case of immediate placement of implant after extraction of impacted supernumerary teeth and permanent teeth together. Therefore we report the case of immediate implantation following extraction of impacted supernumerary teeth and poor periodontal conditioned permanent teeth.

심미적 영역에서 즉시 임플란트 식립 및 임시 수복을 통한 단일치의 수복에 대한 증례 보고 (Immediate implant placement and provisionalization of single implant in the esthetic region : Cases report)

  • In the anterior maxillary area, dental implants for tooth replacement are challenging due to the need to satisfy high esthetic level as well as functionality. Immediate implant placement and provisionalization can dramatically reduce the edentulous period, and then fulfill patient's demand for esthetics. The aim of present case report is to demonstrate two cases that successfully restored single tooth with immediate implant placement and provisionalization in the anterior maxillary area. A 47 years old female was scheduled to replace her maxillary right central incisor due to crown-root fracture by trauma. Another 54-year-old female was planned to place dental implant following tooth extraction of maxillary right lateral incisor owing to continuous pus discharge despite repetitive treatments including apicoectomy. In these two cases, surgical and prosthetic procedures progressed in a similar way. After minimal flap elevation, atraumatic tooth extraction was performed. Implant was placed in proper 3-dimensional position and angulation with primary stability. Bone graft or guided bone regeneration for peri-implant bone defect was conducted simultaneously. Provisionalization without occlusal loading was carried out at the same day. Each definitive crown was delivered at 7 and 5 months after the surgery. Two cases have been followed uneventfully for 2 to 5 years of loading time. In conclusion, Immediate implant placement and provisionalization could lead to esthetic outcome for single tooth replacement with dental implant under proper case selection.

치근단 병소를 갖는 발치와에 즉시 임플란트 식립 시 비흡수성 차폐막이 치조골의 흡수에 미치는 영향에 관한 연구 (The effect of non-resorbable barrier membrane on the change of buccal and lingual alveolar bone in immediate implant placement into periapically infected extraction sockets)

  • Purpose: Many researches showed loss of alveolar bone in fresh extraction socket and even in case of immediate implant placement. The aim of this study was to evaluate the effect of non-resorbable barrier membrane on the change of buccal and lingual alveolar bone in immediate implant placement into periapically infected extraction sockets. Materials and methods: Immediate implants were placed into artificially induced periapical lesion of mandibular premolars after complete debridement using buccal bone defect made by a 6mm trephine bur in 4 mongrel dogs. Before flap repositioning, a non-resorbable barrier membrane was placed on the buccal defect in the experimental group. No membrane was placed in the control group. In 12 weeks after placement, the dogs were sacrificed and undecalcified histologic specimens were prepared. The vertical distance from the smooth-rough surface interface(SRI) to gingiva, 1st bone contact and bone crest were measured in buccal and lingual side. The horizontal thicknesses of gingiva and bone at 0, 1, 2 and 3mm below SRI were measured. Results: The buccal bone was resorbed more than lingual bone in both groups and there was statistical significance(p<0.05). The distances from SRI to 1st bone contact were $2.45{\pm}2.35\;mm$ in experimental group and $4.49{\pm}3.10\;mm$ in control group. In all vertical level, lingual bone was thicker than buccal bone(p<0.05). Conclusion: Buccal bone was reduced more than lingual bone in immediate implant placement into periapically infected extraction sockets. Placement of non-resorbable barrier membrane reduced the buccal bone resorption. However there was no statistical significance.

  • Purpose: Although it is generally accepted that patients with controlled diabetes have similar rates of success for dental implants as healthy individuals, the use of dental implants in diabetic patients is controversial. In addition, the impact of diabetes on the healing of bone associated with immediately place dental implants is not completely understood. The purpose of this study was to measure bone response to implants radiologically in uncontrolled and insulin-controlled diabetic rats. Materials and Methods: Twenty rats were divided into control, insulin-treated and diabetic groups. The rats received streptozotocin (60 mg/kg) to induce diabetes; animals in the insulin-treated group also received three units of subcutaneous slow-release insulin. Two titanium implants ($1.2{\times}3$ mm) were placed in the extraction socket of the maxillary first molars of the animals and were harvested at 3 days, 1, 2 and 4 weeks. The bone density was measured by digital radiography using gray-level analysis (histogram) in the regions of interest (ROI) at four points: two mesial and two distal to both sides of the implant. Results: The results showed that the osseointegration of the implants was impaired in the diabetic rats compared to the control and the insulin-treated rats. The radiographic evidence demonstrated marked destruction of bone around the implants in the diabetic group. Both the control and the insulin-treated groups had a significantly higher bone density on radiograph than the diabetic group from the 1 week of the experiment (P<0.05 for each comparison). Conclusion: The present study revealed that the immediate placement of titanium implants in the maxilla of diabetic rat lead to delay in the maturation of bone adjacent to implants. It is expected that the reduced predictability of success of immediate implantation in patient with the uncontrolled diabetes.

한국인에서 상악 전치의 시상 치근 위치에 대한 연구 (A study on sagittal root position of maxillary anterior teeth in Korean)

  • 목적: 본 연구는 한국인에서 상악 전치의 치근 위치를 시상면에서 분석하고, 분류에 따른 빈도를 보고함으로써 즉시 식립 임플란트를 위한 방사선학적 자료를 수집하기 위함이다. 연구 재료 및 방법: 콘빔형 전산화단층영상(cone-beam CT)을 촬영한 환자 중 연구 기준에 적합한 120명(남성 60명, 여성 60명)을 대상으로 후향적 분석을 시행하였다. 축의 방향설정을 시행한 후에, 상악 전치부 치아와 치조골 사이의 관계에 대한 시상 치근 위치를 분석하였다. 치근이 치조골의 협측, 중앙, 구개측으로 위치한 경우 각각 Class I, II, III로 분류하였으며, 치근이 협측과 구개측 모두에서 피질골판에 2/3 이상 닿아있는 경우에는 Class IV로 하였다. 다음으로, 치아의 장축과 치조골의 장축 사이의 각도를 측정하였다. 기술적 분석 및 Kruskal-Wallis 분석을 시행하였으며, 치아의 위치 및 분류에 따른 시상각을 비교하였다. 결과: 상악 전치부의 시상 치근 위치에 대한 빈도분석 결과, Class I은 81.1%, Class II는 10.3%, Class III는 1.9%, 그리고 Class IV는 6.7%로 나타났다. 상악 전치부의 77.5%에서 시상각이 20도 이하로 나타났다. 그러나 견치의 경우, 42.7%에서 20도 이상의 시상각을 보였다. 분류에 따라서는 Class I (16.19)에서 Class II (8.72) 및 Class III (9.93)에 비해 통계학적으로 유의하게 높은 시상각을 보였으며, Class IV (3.79)에서 낮았다. 결론: 본 제한된 연구의 결과를 근거로, 상악 전치의 치근은 일반적으로 협측 치조골에 가깝게 위치하고 있으나, 일부 치아는 매우 얇은 치조골을 가지고 있으며, 30도 이상의 큰 시상각을 보였다. 따라서 적절한 치과 임플란트 치료 계획 수립을 위해 시상 치근 위치 및 시상각에 대한 cone-beam CT 분석이 필요할 것이다.
