A Case of Central Pontine Myelinolysis in a Chronic Alcoholic (만성 알콜중독증 환자에서 발생한 중심성 뇌교 수초용해증 치험 1례)

  • Objective : This study was case report to show the good effects of herbal medicine (Daekumeum-ja) and acupuncture complex therapy for central pontine myelinolysis (CPM). Method : A 44-year-old man suffering from CPM was chronic alcoholic without electrolyte disturbance. He showed symptoms of dysarthria, dysphagia, dizziness, weakness of lower extremities and gait disturbance. CPM was diagnosed at his former hospital by MR imaging. He had no positive response to western treatments. We treated the patient with herbal extract (Daekumeum-ja) and acupuncture. Results : After our treatments, there was no change of Brain CT. But overall symptoms of CPM were improved. Conclusion : This study suggests that herbal medicine (Daekumeum-ja) and acupuncture complex therapy is probably effective in the treatment of CPM.

Factors for Intentional Self-harm among the Elderly Patients with Depression (고의적 자해 노인 환자의 우울증 관련 요인)

  • The purpose of this study is to analyze the characteristics of the elderly patients with depression who were admitted to the hospital with intentional self-harm. 3,280 patients were selected from KCDC database(2011-2015) using STATA 12.0. Analysis results show that gender(female), residence(micropolitan city), result of suicide(death), risk factors(financial problems, psychological problems, physical disease, conflicts with family, place(non-residence) method of suicide(poisoning) were statistically significant. The hospital should detect the elderly patient with depression when they admitted.

Sudden death caused by trichlorfon poisoning in Korean native cattle (한우에서 발생한 트리클로르폰 중독증 증례)

  • Two 12-month-old cattle with anthelmintics containing trichlorfon the day before death presented to the Animal and Plant Quarantine Agency for diagnosis. In necropsy, they revealed enlargement of the spleens, redness of mucosa and serosa in stomachs and intestines, and friable kidneys. Histopathologically, hemorrhages in the spleens, omasums, abomasums, and intestines as well as renal tubular necrosis were observed. Trichlorfon was detected at above the lethal dose in the ruminal contents. Based on these findings, we diagnosed this case as death caused by trichlorfon poisoning.

Grayanotoxin poisoning in a black goat (흑염소에서의 철쭉으로부터 유래된 그레이아노톡신 중독증 증례)

  • A 3 year-old black goat was presented to Animal and Plant Quarantine agency for diagnosis in June, 2017. She was intaken feed with Rhododendron schlippenbachii the day before death. The clinical signs included loss of appetite, lethargy, hypersalivation, astasia, yelling. At necropsy, foamy discharge were observed in the airway. Histologically, foreign body, eosinphil and macrophges was observed in alveolar lumen of lung. Grayanotoxin derived from Rhododendrons was detected in ruminal contents. Based on the pathological and toxine examination, we diagnosed this case as grayanotoxin poisoning in a black goat.

A case of treatment on amitraz toxicosis in a Thoroughbred racehorse (Thoroughbred 경주마에서 amitraz 중독증 치료 1례)

  • A 3-year-old female Thoroughbred racehorse was presented following the accidental oral and skin administration of amitraz. This case report describes the clinical signs and the treatment of this horse. Clinical signs of amitraz toxicosis are associated with the stimulation of alpha2-adrenergic receptors. Amitraz is seldom fatal because the effects can be reversed by alpha2-adrenergic antagonists. The horse displayed typical clinical signs of colic, including pawing, small hard drops, tranquillisation, depression, ataxia, muscular incoordination and impaction colic lasting up to 7 days. The syndrome was accompanied by mild dehydration. The horse survived after persistent symptomatic treatment, including the giving of intravenous fluids, antibiotics, multiple doses of mineral oil per os, nonsteroidal anti-inflammatory drugs and dexamethasone intramuscularly and intravenously.

Two Cases of Fatal Paraquat Intoxication by Parenteral Injection (비경구적 투여에 의한 치명적인 파라캇 중독증 2례)

  • Paraquat poisoning is a fatal type of herbicide intoxication. It is characterized by multi-organ failure and pulmonary fibrosis with respiratory failure. Intravenous and intramuscular injection of paraquat is rarely described. However, We encountered two fatal cases of acute poisoning caused by paraquat injection. Two patients were admitted to our emergency unit after intravenous and intramuscular injection of 23.8% paraquat (about 476 mg of paraquat). A 37-year-old man diluted 2 ml of 23.8% paraquat solution with 1 ml of normal saline and injected it both intravenously into his left antecubital fossa and intramuscularly into his abdomen in a suicide attempt. He died 5 days later from respiratory failure and acute renal failure. A 92-year-old man was injected intravenously into his right antecubital fossa by his grandson with 2 ml of 23.8% paraquat solution diluted with 1 ml of normal saline. He died 2 days later from early circulatory collapse and multi-organ failure (metabolic acidosis, acute renal failure, coagulopathy). Intravenous and intramuscular injection with a small quantity of paraquat resulted in fatal toxicity in our patients.

Rapid Determination of Selenium in Foodstuffs by Neutron Activation Analysis (방사화분석법에 의한 식품중의 Se의 정량)

  • The selenium content of a wide variety of Korean food was determined by neutron activation analysis. Most fruits and vegetables contained quantities of selenium less than $0.4{\mu}g/g$. Grain products varied widely in their selenium content with $0.5{\mu}g/g$ and barley cereal as high as $0.7{\mu}g/g$. Dried milk powder sample ranged from $0.7\;to\;0.15{\mu}g/g$. Chicken muscle contained about $0.7{\mu}g/g$. The content of sea food was generally higher, ranging from $0.3\;to\;3.65{\mu}g/g$. These values suggest that a diet well balanced in other nutrients is probably also nutritionally adequate with regard to selenium, although possible effects of cooking and biological availability remain to be investigated.

A Survey on Physical Complaints Related with Farmers' Syndrome of Vinylhouse and Non-vinylhouse Farmers (비닐하우스 재배농민과 일반농민의 농부증 관련 신체증상 호소율 조사)

  • To compare the physical complaints of vinylhouse farmers with those of non-vinylhouse farmers, a personal interviews on 250 vinylhouse and 142 non-vinylhouse farmers were conducted in Sungjoo county in Kyungpook province selected by a random sampling from July 5 to July 10, 1993. Blood pressure of the subjects was also measured. Vinylhouse farmers had a higher average age, larger family size, shorter experience of farming, more working hours per day and working days per year and higher annual income than the non-vinylhouse farmers. The frequency of pesticide spray of the vinylhouse farmers was 3.4 times on the average in June 1993 as compared with 2.0 times of non-vinylhouse farmers, and 16.7 times for the vinylhouse farmers during the last one year while it was 8.3 times for the non-vinylhouse farmers in the same period. While 39.6% of vinylhouse farmers experienced pesticide intoxication symptoms such as headache, nausea, vomiting, dizziness, itching, and skin irritation, etc. during the month of June, 25.4% of non-vinylhouse farmers experienced such symptoms. The most frequent symptoms among eight symptoms that constitute the farmers' syndrome were lumbago, numbness of hand or foot, shoulder pain and dizziness regardless of sex and type of farming. Prevalence of the farmers' syndrome in male and female among vinylhouse farmers were 22.1%, 43.4%, respectively, and the prevalence in non-vinylhouse farmers was 23.2% for male and 50.7% for female. There was no statistically significant difference in the prevalence of farmers' syndrome between vinylhouse and non-vinylhouse farmers. However, the prevalence in female was about 2 times higher than that of male. When the effects of other factors were adjusted by multiple logistic regression for farmers' syndrome, the prevalence in female was 3.0 times higher than that of male. The prevalence of farmers' syndrome was increased as the age of farmers increased in both vinylhouse and non-vinylhouse farmers, and adjusted odds ratio of farmers' syndrome increased by 3% as the age increased by 1 year. Adjusted odds ratio for Farmers' syndrome in farmers who experienced pesticide intoxication during the month of June was 3.1 times higher than that of farmers who did not have such experience. While the prevalence of hypertension in male and female non-vinylhouse farmers were 22.4%, 13.7%, respectively, the prevalence in vinylhouse farmers were 13.5% for male and 12.0% for female. However, there was no association between farmers' syndrome and hypertension. It was found in this study that the vinylhouse farmers are at a high risk of pesticide intoxication, which is associated with tile common physical complaints. To reduce such risk it is necessary to develop farming methods which do not require the pesticide or may use less pesticide, a safer method of pesticide spraying, and the protective equipments which can be worn at a high temperature and have a better protective effect. Also education of farmers for the correct methods of ventilation after pesticide spraying in the vinylhouse and wearing the protective equipments may be considered as a supportive method. Since inappropriate posture at work and intensive labor may cause farmers' syndrome, it is recommended to develop farming tools which reduce physical burden and take a rest and exercise periodically during work. It is necessary to strengthen the hypertension management program of the Kyungpook province, because the prevalence of hypertension was as high as about 15%.

A Study of Mechanism Involved in Cadmium-induced Platelet Aggregation

  • Cadmium (Cd) was administered by a series of weekly intraperitoneal injections at dose of 2mg/kg in rabbits and rats. The levels of malondialdehyde (MDA) and thromboxane $B^2(TXB_2)$ in platelet-rich plasma from Cd-poisoned animals were significantly higher than those of the control group. Furthermore, the inhibition of 6-keto-prostaglandin $F_{1{\alpha}}$, production in Cd-treated aorta ring was inversely related to the enhancement of platelet aggregation. These results suggest that Cd not only inhibits prostacyclin synthesis in the arterial endothelium, but also stimulates the platelet aggregation by enhancing thromboxane AZ production. These findings are assumed to support the evidence of an effect of Cd toxicity on the vascular wall and platelet function in raising arteriall pressure.

Influence of Intrauterine Position on Fetal Weight in Albino Rats Exposed to Carton Monoxide (일산화탄소중독 수태백서의 자궁내 착상위치가 태자의 발육에 미치는 영향)

  • Fetal weight data from 84 litters of Sprague-Dawley rats were statistically analyzed for the effect of position in the uterine horn on fetal weight. The standard score obtained from the mean weight and standard deviation of all fetuses in a uterine horn were studied for position effect. In control group, the heaviest fetus occupied the middle position with a progressive decrease in weights toward the ovarian and cervical ends of the horn. But the effect related to position for the fetal weight was not statistically significant. In contrast, rats acutely or chronically exposed to carbon monoxide showed statistically significant positional differences of fetal weight within the uterine horns. The findings that the intrauterine position effect on fetal weight which is not signicant in control group may act one of confounders at the situation of transplacental toxicological studios.
