Decompression and Medial Epicondyloplasty in Ulnar Nerve Entrapment Syndrome at Elbow (주관절부 척골신경 포착증후군의 수술적 감압술 및 내상과 성형술)

  • As a surgical treatment of ulnar nerve entrapment syndrome includes simple decompression, medial epicondylectomy, and anterior transposition of the ulnar nerve into a subcutaneous or submuscular bed have been widely used. Despite many reports of these surgical procedure, there is little to guide the choice of one surgical technique. The purpose of our study is to analyse clinical and electrodiagnostic result after minimal invasive decompression by decompression and medial epicondyloplasty(deepening of ulnar groove). We have experienced 9 cases of ulnar nerve entrapment syndrome who were treated with decompression and medial epicondyloplasty. Male were five and female were four. The mean age at operation was 36 years ranging from 23 to 47 years. Operative procedure was to incise the medial intermuscular septum and aponeurotic arch of flexor carpiulnaris and to deepen the ulnar groove. Patients are allowed to do range of motion(ROM) exercise on the average 5days. All patient were relieved pain and improved motor and sensory function, and this procedure allows early ROM exercise after operation because the muscle have not been detached.

Comparison of Isokinetic Strength between Stage 1,2 Impingement Syndrome and Rotator Cuff Tear (1, 2단계 충돌 증후군과 회전근 개 파열 환자의 등속성 근력 비교)

  • Purpose: The goal of this study was to evaluate differences in strength deficits between (i) patients with stage 1 or 2 impingement syndrome and (ii) patients with rotator cuff tears. Materials and Methods: We enrolled 43 patients with stage 1 or 2 impingement syndrome (group 1) and 21 patients with rotator cuff tears (group 2). The isokinetic strength of both groups was evaluated at $60^{\circ}/sec$ for external rotation, internal rotation, adduction and abduction. We measured the peak torque, total work, average power of bilateral sides, peak torque relationship to body weight, and the ratio between unilateral agonist and antagonist. Results: The isokinetic strength deficits assessed in ratio of peak torque to body weight for group 1 and group 2 respectively were; $28.48{\pm}23.76%$ and $29.12{\pm}32.81%$ for abduction (p=0.929), $7.20{\pm}13.98%$ and $18.94{\pm}19.52%$; for adduction (p=0.021), $16.88{\pm}13.76%$ and $25.80{\pm}24.07%$; for external rotation (p=0.221), and $14.1{\pm}25.67%$ and $29.02{\pm}35.06%$ for internal rotation (p=0.059). For average power and total work, group 2 showed a significantly greater deficit for adduction and internal rotation than group 1. Conclusion: Those with rotator cuff tears have more isokinetic muscle strength deficits than those with stage 1 or 2 impingement syndrome. Progression from stage 1 and 2 impingement syndrome to stage 3 may result in greatest changes in strength deficits for internal rotation and adduction.

Prevalence and Its Correlates of Night Eating Syndrome in Schizophrenic Outpatients (조현병 외래 환자에서 야간식이증후군의 유병률과 관련요인)

  • Objectives : The aim of study was to examine the prevalence of night eating syndrome(NES) and its correlates in schizophrenic outpatients. Methods : The 14 items of self-reported night eating questionnaire(NEQ) was administered to 201 schizophrenic patients in psychiatric outpatient clinic. We examined demographic and clinical characteristics, body mass index(BMI), subjective measures of mood, sleep, binge eating, and weight-related quality of life using Beck's Depression Inventory(BDI), Pittsburgh Sleep Quality Index(PSQI), Binge Eating Scale(BES) and Korean version of Obesity-Related Quality of Life Scale(KOQoL), respectively. Results : The prevalence of night eaters in schizophrenic outpatients was 10.4%(21 of 201). Comparisons between NES group and non-NES group revealed no significant differences in sociodemographic characteristics, clinical status and BMI. Compared to non-NES, patients with NES reported significantly greater depressed mood and sleep disturbance, more binge eating pattern, and decreased weight-related quality of life. While 'morning anorexia' and 'delayed morning meal'(2 of 5 NES core components in NEQ) were not differed between groups, 'nocturnal ingestions', 'evening hyperphagia', and 'mood/sleep' were more impaired in NES group. Conclusions : These findings are the first to describe the prevalence and its correlates of night eaters in schizophrenic outpatients. These results suggest that NES has negative mental health implications, although it was not associated with obesity. Further study to generalize these results is required.

Simultaneous Anterior and Posterior Interosseous Nerve Syndrome Following Shoulder Arthroscopy in the Lateral Decubitus Position - Case Report - (측와위로 시행한 견관절 관절경 후에 동시에 발생한 전방 및 후방 골간 신경 증후군 - 증례보고 -)

  • We report a case of simultaneous anterior and posterior interosseous nerve syndrome in association with shoulder arthroscopy. Shoulder arthroscopy was performed in a 45-year-old male patient with left shoulder instability. In the right lateral decubitus position, under general anesthesia, traction was applied with elbow extension for 2 hours. One week after surgery, the patient revisited the clinic for weakness of the flexor of the thumb, index finger, and extensor of the fingers. Recovery was not achieved after four months of observation. Therefore, nerve exploration was performed in the anterior and posterior interosseous nerve and hourglass-like fascicular constriction was detected in the posterior interosseous nerve. The area of constriction was removed and epineural neurorrhaphy was performed. Three months after exploration, the extension function of the fingers was recovered. Recovery was achieved gradually, and, five months after nerve exploration, the symptoms were completely recovered. Simultaneous anterior and posterior interosseous nerve syndrome following shoulder arthroscopy is rare. However, it could occur due to the traction and position of the patient. Thus, the operator should be careful of traction and position of the patient.

Acromion Morphology in Coronal and Sagittal Plane; Correlation with Rotator Cuff Syndrome (관상면과 시상면에서의 견봉 형태와 회전근개 파열의 연관성)

  • Purpose: We evaluated the correlation of the anatomic parameters of the acromion those represent on the magnetic resonance image (MRI) of impingement syndrome. Materials and Methods: From June, 2004 to December, 2005, 71 cases were surgically proven to be impingement syndrome, and the anterior acromial hooking angle, the lateral acromial hooking angle (AAHA and LAHA) and the acromial hooking index (AHI: the sum of the AAHA and LAHA) were compared to 16 control cases. At the same period, 55 cases were surgically proven to be partial or full thickness rotator cuff tear, and age, gender and twelve anatomic parameters, including the acromial type, the acromial angle, the anterior covering, the acromial slope, the AAHA, the lateral acromial angle, the acromial torsional angle, the lateral acromial angulation, the LAHA, the lateral covering, the acromiohumeral distance and the AHI were assessed. Results: The AAHA and AHI were increased as impingement syndrome proceeded. The acromial type and acromial angle, and the AAHA, LAHA and AHI showed significant differences between the controls and the rotator cuff tear patients on univariant analysis. On multivariant analysis, gender was most strongly correlated with rotator cuff tear. Age, AAHA and the acromial angle showed similar correlation, respectively. Conclusion: The coronal acromial shape is correlated with rotator cuff tear, and it is important to correct the lateral acromial shape when performing acromioplasty.

Suprascapular Nerve Entrapment Syndrome Caused by Ganglion (결절종에 의한 견갑상신경 포착 증후군)

  • Purpose : To review the operative results and find out better method of a treatment of suprascapular nerve entrapment syndrome caused by ganglion. Materials and Methods : From March of 1996 to May of 2001, thirteen cases of suprascapular nerve entrapment syndrome caused by ganglion were reviewed which were diagnosed using physical examination, EMG and MRI. All of them were treated with open exision of ganglion, nerve decompression and arthroscopic examination. The clinical results were evaluated using subjective recovery of muscle power and ASES (American Shoulder and Elbow Surgeons) score. Results : Nine cases (69$\%$) showed residual symptom such as intermittent pain, insufficient recovery of muscle power, persistent muscle atrophy. Complete recovery in four cases (31$\%$). Mean value of ASES score was 86.4 (70-99.8) points. Postopertively, pain was relieved in 84$\%$ of patients and muscle power was improved in 53$\%$ of the patients but the atrophy persisted. Conclusions : Excision of ganglion is insuffient to relieve the nerve entrapment. Not only excision of ganglion but also treatment of intraarticular lesion, decompression of suprascapular and spinoglenoid notch which attributable to entrapment should be considered for the better clinical results.

Prevalence and Its Correlates of Restless Legs Syndrome in Outpatients with Bipolar Disorders (양극성장애 환자의 하지불안증후군 유병율과 관련 특성)

  • Objectives : This study was to assess the prevalence and its correlates of restless legs syndrome(RLS) in outpatients with bipolar disorder. Methods : A total of 100 clinical stabilized bipolar outpatients were examined. The presence of RLS and its severity were assessed using the International Restless Legs Sydrome Study Group(IRLSSG) diagnostic criteria. Beck's Depression Inventory(BDI), Spielberg's State Anxiety Inventory(STAI-X-1), Pittsburgh Sleep Quality Index(PSQI), Korean version Drug Attitude Inventory(KDAI-10), Subjective Well-Beings under Neuroleptic Treatment Scale-Short Form(SWN-K) and Barnes Akathisia Rating Scale(BARS) were used to evaluate the depressive symptomatology, level of anxiety, subjective quality of sleep, subjective feeling of well-being, drug attitude, presence of akathisia, respectively. Results : Of the 100 bipolar outpatients, 7(7%) were met to full criteria of IRLSSG and 36(36%) have at least one of the 4 IRLSSG criterion. Because of relatively small sample size, non-parametric analysis were done to compare the characteristics among 3 groups(full-RLS, 1 ${\geq}$positive RLS-symptom and Non-RLS). There were no significant differences in sex, age, and other sociodemographic and clinical data among 3 groups. BDI, STAI-X-1 and PSQI are tended to be impaired in RLS and 1 ${\geq}$positive RLS-symptom groups. Conclusions : This is the first preliminary study for studying the prevalence and its correlates of RLS in bipolar disorder. The results shows that relatively small proportion of RLS was present in bipolar disorder patients when compared to patients with schizophrenia. Same tendencies shown in schizophrenic patients were found that bipolar patients with RLS had more depressive symptoms, state anxiety and poor subjective sleep quality. Further systematic studies may be needed to find the characteristics of RLS in bipolar patients.

A Methodology of Measuring Degree of Contextual Subjective Well-Being Using Affective Predicates for Mental Health Aware Service (정신적 건강 서비스를 위한 감성구를 활용한 주관적 웰빙 지수 측정 방법론)

  • The contextual subjective well-being (SWB) of context-aware system users can be very helpful in recommending relevant mental health services, especially for those who struggle with mental illness due to a metabolic syndrome or melancholia. Self-surveying measuring or auto-sensing methods have been suggested to monitor users' SWB. However, self-surveying measuring method is not inappropriate for a context-aware service due to requesting personal data in a manual and hence obtrusive manner. Moreover, auto-sensing methods still suffer from accuracy problem to be applied in mental health services. Hence, the purpose of this paper is to propose a contextual SWB estimation method to estimate the user's mental health in unobtrusive and accurate manners. This method is timely in that it acquires context data from the user's literal responses, which expose their temporal feeling. In particular, we developed a measuring method based on exposed feeling verbs and degree adverbs in chat and other text-based communications which show anger or negative feelings. Based on the proposed contextual SWB degree estimation method, we developed an idea of well-being life care recommendation. From the experiment with actual drivers, we demonstrated that the proposed method accurately estimate the user's degree of negative feelings even though it does not require a self-survey.

Kimura's Disease of the Elbow - A Case Report - (주관절 부위에 발생한 Kimura씨 병 - 증례 보고 -)

  • Purpose: Kimura's disease is an uncommon benign lymphoproliferative inflammatory disorder with an unknown etiology. The recurrence rate after surgical excision is relatively high and renal involvement is its only systemic manifestation. The condition mainly involves the head and neck, and peripheral involvement is extremely rare. Materials and Methods: We encountered the case of a 28-year-old man who had a non-tender mass with mild brownish skin color changes and pruritus around the medial side of the distal arm and elbow. Results: The peripheral blood investigation revealed peripheral eosinophilia and elevated serum IgE levels. Magnetic resonance imaging showed an isointensity signal relative to the muscle on the T1-weighted images and hyperintensity signal relative to the muscle on the T2-weighted images. Conclusion: A marginal resection of the lesion was performed and there was no recurrence at 2 years postoperatively.

The Effect of Metabolic Syndrome Management Program in a Public Health Center (일개 보건소 대사증후군 관리프로그램의 효과분석)

  • Objectives: The aims of this study were to evaluate the effect of a metabolic syndrome management program on participants enrolled in a public health center program in, Seoul, Korea for 6 months and to analyze the changes in their life habits and risk factors for treating their metabolic syndrome. Methods: Participants enrolled in a metabolic syndrome program at a public health center and answered questionnaires. Their waist circumference and, blood pressure were measured and their blood was tested. Based on the results, the participants were classified into 3 groups. Then, the center provided a metabolic syndrome management intervention program for the individual groups and analyzed the changes in participants' life habits and risk factors after 6 months. Results: After the intervention, the active group, showed improved physical activity, increased eating habits by 0.7 points, and decreased drinking and smoking; however, these results were not statistically significant. As for the Participants' overall health status, all groups positively changed a statistically significant result. In regard to the change in risk factors for metabolic syndrome in the active group, systolic and diastolic blood pressure and triglyceride levels were, statistically significantly decreased, meanwhile, an increase was found in HDL cholesterol level, waist measurement, and anteprandial glucose level; but the result were not statistically significant. Their review rate was 19.0%; 29.4% in the active group, 19.0% in motivating group A, and 14.5% in motivating group B. Conclusions: Participants showed changes in some life habits and risk factors, a result still far from the program's goal to improve residents' health status (improved living habits and variations in risk factors); thus, it is necessary to supplement this program for better performance.
