Clinical and Symptomatic Correlates of Alexithymia in Schizophrenia (정신분열병의 감정표현 불능증과 관련된 임상 및 증상 요인)

  • Objectives : This study looked at the symptoms of alexithymia in schizophrenia and their association with clinical variables and schizophrenic symptomatology. Methods Consecutive fifty eight inpatients with DSM-IV diagnoses of schizophrenia completed 26item version of Toronto Alexithymia Scale (TAS), Symptom Checklist-90-Revised(SCL-90-R), and Positive and Negative Syndrome Scale(PANSS). Results : Authors did not find any correlation between scores of PANSS and TAS. However, all the subscale scores of SCL-90-R were significantly correlated with total score of TAS. Also, 'difficulty identifying and deistinguishing between feelings and bodily sensations' and 'difficulty describing feelings' significantly correlated with SCL-90-R subscale scores. 'Reduced daydreaming' had mixed findings and 'externally oriented thinking' did not correlate. Multiple regression model included Global Severity Index of SCL-90-R accounting 28.2% of variance for TAS scores. Conclusion : These findings together with discrepancy in results between objective and subjective tests suggest that alexithymia in schizophrenia may have two constructs, 'difficulty to describe and communicate feelings(state)' and 'externally oriented thinking(trait)' Authors suggest further study needs to confirm construct validity of TAS in this population.

Influential Variables on the Husbands' Authoritarian Communication (남아의 권위주의적 의사소통 관련변인)

  • 본 연구는 평등한 부부관계로의 변화를 요구하는 현대사회에서 부인과의 대등하고 민주적인 의사소통을 저해하고, 부인의 정신.신체증상 호소라든가 가족문제를 야기시키는 한국남편의 전형적인 권위주위적 의사소통에 대한 관련변인을 밝혀내어 부부간의 민주적인 관계형성을 도모하고자 하는데 기초자료를 제공하기 위하여 수행되었다. 서울시에 거주하는 328쌍의 부부를 대상으로 남편의 권위주의적 의사소통에 대한 관련변인을 살펴본 결과 개인적 변인으로 자아존중감 성역할태도, 부부간의 변인으로 부부간의 의사결정권, 부부간의 학력차, 부부간의 연령차, 가족환경적 변인으로 남편의 성장가족에서 친부의 권위주위적 행동, 주관적인 사회계층이 통계적으로 유의미하였으며, 개인적 변인, 부부간의 변인, 가족환경적 변인은 남편의 권위주의적 의사소통의 전체분산 중 14.5%, 11.3%, 11.5%를 각각 설명해 주고 있다. 개인, 부부간, 가족환경 변인을 모두 고려하였을 때는 자아존중감, 부부간의 의사결정권 친부의 권위주의적 행동, 성역할 태도, 부부간의 학력차가 통계적으로 유의미하였으며, 이들 변인은 남편의 권위주의적 의사소통의 전체분산 중 25.2%를 설명해 주고 있다.

A study of prostate symptom, sleep quality and quality of life in the community-dwelling korean elderly (남성 노인의 전립선 증상, 수면의 질, 삶의 질에 관한 연구)

  • This study aimed to identify the severity and types of prostate symptom, to identify the relationship of prostate symptom, sleep quality and quality of life of the elderly in korea, so to provide basic data for developing nursing interventions for the elderly's prostate symptom management. This study used a descriptive correlational research design. The subjects of this study were 100 elderly men over 65yr in an urban city. Data were collected through personal interviews using questionnaires from September 2006 to March 2007. The IPPS, PSQI and SF-36-K were administered. The collected data were analyzed by descriptive statistics and Pearson's correlation. The major findings of this study were as follows: 1) 44% of the participants have moderate to severe Prostate symptom(IPSS>7). 2) Prostate symptom has positive relationship with sleep quality(r=.272, p=.006) and negative relationship with health related quality of life(r=-.197, p=.049). The results of this study indicate that prostate symptom is related to sleep quality and quality of life. So it is necessary to give information to the elderly about the effective coping method of prostate symptom and about the life style which have positive effect to prostate symptom.

The Moderate Effect of Social Support on Family Caregivers' Burdens and Health and Quality of Life with Demented Patients (치매환자 가족부양자의 부양부담과 건강 및 삶의 질에 대한 사회지원의 중재효과)

  • As quickly becoming an ageing society in Korea, this study aims to investigate how family caregivers' burden with demented patients and social supports are related to their health and quality of life, and the interaction of caregiving burden and social supports on caregivers' health and quality of life. The participants were 207 family caregivers (106 females) of demented patients lived in Seoul, Kyunggi, and Chungcheong areas, whose average of age were 51.62 ( SD=9.25). The psychological tests used in this research included the following: Multidimensional Caregiver Burden Inventory, Chon's Physical Symptom Questionnaire, Hahn & Jang's Perceived Health Inventory, Negative Affect Self-Statement Questionnaire, and the Korean Version of Cambell's Subjective Well-being Scale, Social Support Survey in Medical Outcomes Study, and Kwon's Questionnaire for Maladjusted Problems of Demented. Results indicated that caregivers' burden with demented patient related to physical symptoms and depression positively, and perceived health and subjective well-being negatively. Social supports showed moderate effects on influences of caregiving burden in perceived health, depression, and subjective well-being of family caregivers with demented patients. It was identified the possibility of their buffering effect on negative results of caregiving burden with demented patients. Since caregiving burden accounted for the 34% of the variances for family caregivers' physical symptoms and depression, researchers discussed the promotion and intervention of their health and quality of life. Beside of different effects of social supports with level of caregiving burden, with previous studies it was discussed some saliant findings such as family caregivers with patients who showed severe dementia symptoms were perceiving even less supports from others.

An Exploration of Somatization among Korean Older Immigrants in the U.S. (신체증후군에 대한 탐색적 연구: 한인 노인 이민자를 중심으로)

  • Knowledge about somatization (somatic manifestation of psychological distress symptoms) among immigrant populations is limited. While several studies have recognized somatization as a culturally distinctive expression of depression amongst older Korean immigrant population, somatization has not been incorporated into the comprehensive empirical model for depression of this population. In order to improve our general understanding of the phenomenon, the objective of this study is to empirically investigate principal contributing factors of somatization as well as inter-relationships among them. Data were collected from a cross-sectional community survey of 234 older Korean immigrants ($$age{\geq_-}55$$) in the New York metropolitan area. The statistical methodology employed a robust hierarchical regression procedure that iteratively downweights outliers. The results indicated that living arrangement, greater numbers of physical illnesses, and depression were significant explanatory factors of somatization. Furthermore, physical illness had a significant joint effect with perception of health on somatization, which confirms that positive perception of health exerts a moderating effect on the relationship between physical illness and somatization. The knowledge obtained from this study will contribute toward extending our knowledge on somatization and implementing more culturally sensitive mental health services for this population.

Prevalence of Musculoskeletal Symptom in Rural Farmers (농촌지역주민의 근골격계 증상 경험률)

  • This study was performed to examine the annual experience rate of musculoskeletal symptom and its related factors in rural farmers. Six-hundred and sixty one rural farmers were investigated by questionnaire survey through interview from July 1 to July 31 in 2000. The results were as follows; The rural farmers who had experienced musculoskeletal symptom during recent 1 year was 82.7% in male, 92.1% in female. The annual experience rate of musculoskeletal symptom by part of the body in male were turned out as back; 42.3% leg and knee; 36.4%, shoulder; 21.4%, arm and hand; 12.7%, elbow; 7.7%, hip; 6.8% neck; 6.4%, ankle and foot; 6.4% in order. The annual experience rate of musculoskeletal symptom by part of the body in female were turned out as back; 59.4%, leg and knee; 48.5%, shoulder; 20.2%, arm and hand; 13.8%, hip; 10.7%, neck; 6.8%, ankle and foot; 6.3%, elbow; 5.0% in order. The annual experience rate of musculoskeletal symptom, back pain, and pain on leg and knee were significantly higher in female. In male, the experience rate of musculoskeletal symptom was different significantly according to educational level, family size, economic status, status of smoking, duration of farming, type of major agricultural affairs. By the multiple logistic regression analysis for experience of musculoskeletal symptom, family size and economic status were the significant variables. It was higher in male with no or one family living together and low economic status. In female, the experience rate of musculoskeletal symptom was different significantly according to body mass index, perceived health status, type of agricultural affairs. By the multiple logistic regression analysis for experience of musculoskeletal symptom, body mass index, perceived health status, and type of major agricultural affairs were the significant variables. It was higher as female farmer's body mass index was increasing and when female farmer's perceived health status was not healthier, and it was lower when female farmer did livestock farming as major agricultural affair. The experience rate of musculoskeletal symptom was higher in the busy season for farmers and treatment methods for musculoskeletal symptom were physical therapy, medication, herb medical treatment, etc. In conclusion, because most of farmers experience musculoskeletal symptom, continuous systematic study on musculoskeletal symptom of farmers and active effort to prevent and manage musculoskeletal symptom of farmers are necessary.

Comparison between Subjective and Actigraphic Measurement of Sleep in Psychiatric Inpatients (주관적 수면평가와 활동기록기를 이용한 수면평가의 비교 - 정신과입원환자를 대상으로 -)

  • Objectives:Assessment of sleep disturbance is an essential part of the diagnostic criteria used for several psychiatric disorders. Change in sleep patterns over time may indicate response to treatment. In clinical practice, sleep is usually evaluated subjectively by patient self-report. This study was aimed to compare subjective sleep assessment with objective sleep measurement by actigraphy in psychiatric inpatients. Methods:A total of 32 psychiatric inpatients were studied. Patients were asked to wear a wrist actigraphy for three consecutive days and nights and to fill out a sleep log each morning. The severity of depression and anxiety was evaluated according to Beck Depression Inventory and State-Trait Anxiety Inventory on the first day of the study. The subjective level of satisfaction with quality of sleep was also evaluated according to visual analog scale. Nurses assessed sleep at one hour interval between 10:00 PM and 6:00 AM for three consecutive nights. Results:There was statistically significant difference of sleep latency between patient's sleep log and acti-graphic measurement. Nursing reports were more consistent with actigraphic measurement than sleep log. Interestingly, subjectively poor sleepers show no significant difference in sleep parameters compared with those of good sleepers. Subjectively poor sleepers report longer sleep latency than that of actigraphic assessment. The discrepancy between subjective and objective assessment of sleep latency was significantly correlated with scores of Beck Depression Inventory and State-Trait Anxiety Inventory. Conclusion:These results show that there are discrepancies between subjective and objective assessment of sleep. The discrepancy of sleep assessment could be influenced by severe depression and anxiety. Especially objective sleep measurement is needed to assess sleep in psychiatric inpatients with severe depression or anxiety and the subjectively poor sleepers for more reliable measurements.

Insomnia in Patients with Chronic Renal Failure on Hemodialysis (혈액투석 중인 만성 신부전증 환자에서의 불면증에 대한 연구)

  • Objectives: The purposes of this study were to investigate 1) the incidence of insomnia, 2) the clinical characteristics of the insomniacs, 3) the correlation of severity of insomnia with somatic complaints and psychological distresses, and 4) the beliefs and attitudes about sleep in patients with chronic renal failure on hemodialysis. Methods: The author evaluated 153 patients, receiving hemodialysis therapy at the four outpatients hemodialysis units in Pusan, Korea. The patients had completed a self-administered questionnaire package, which consisted of basic demographic findings, questions characterizing insomnia, Beck Depression Inventory(BDI), Spielburger's State-Trait Anxiety Inventory(STAI), and visual analogue scales measuring quantitatively the severity of the self-perceived psychological and somatic symptoms. And several laboratory data were collected. Diagnosis of insomnia was made in the base of insomnia criteria of DSM-IV and international classification of sleep disorders. Subjects were dichotomized into those who reported any characteristics of insomnia or those who had no insomnia during the preceding two weeks. Results: Insomnia was found in 100(65.4%) of 153 patients. No statistical differences were found between the patients with and without insomnia in terms of age, gender, education, marital status, mean duration of hemodialysis and all considered laboratory findings except serum albumin. The patients with insomnia had significantly higher BDI score and predialysis systolic blood pressure, and lower serum albumin as compared to non-insomnia group. Significant differences were found between two groups in terms of self-perceived distress such as sadness, anxiety, worry, pruritus, and dysfunction of daily life. The data showed statistically significant correlation between insomnia severity and some variables such as physical dysfunction, pruritus, bone pain, sadness, anxiety, worry, dysfunction of daily life and excessive daytime sleepiness. The patients with insomnia had significantly several dysfunctional beliefs and attitudes about sleep than those without insomnia. Conclusion: These results indicate that insomnia is very common in hemodialysis patients and likely contribute to the impaired quality of life experienced by many these patients. The author suggests that physical and psychological distresses would be reduced and the quality of life could be improved if their sleep disturbances are properly ameliorated in patients on hemodialysis.

A Study on Physical Symptom, Activity of Daily Living, and Health-Related Quality of Life (HRQoL) in the Community-Dwelling Older Adults (재가 노인의 주관적 신체 증상, 일상생활수행능력 및 건강관련 삶의 질)

  • Purpose: This study aimed to identify the relationships among physical symptoms, activities of daily living, and health-related quality of life (HRQoL) in community-dwelling older adults. Methods: A stratified random sampling method was conducted to recruit participants from May 10 to August 17, 2007. Physical symptoms were measured using the Physical Health Questionnaire (PHQ), activities of daily living using the Late-Life Functional and Disability Instrument (LLFDI), and HRQoL using the Medical Outcomes Short-Form Health Survey (SF-36) in 242 community-dwelling elderly Korean people. Results: The HRQoL correlates with the physical symptoms (r=-.31) and the function component (r=.59). Of the two disability parts of the LLFDI, the limitation dimension correlates higher (r=.57) with HRQoL than the frequency dimension (r=.42). The HRQoL is significantly associated with the function component, and disability limitation in capability which explained 44.4% of variance in physical health. Conclusion: These results may contribute to a better understanding of physical symptoms, activities of daily living, and HRQoL in community-dwelling older adults. Therefore, health programs for prompting older adult's health should be planned based on results of the study.

Prevalence and Associated Factors of Depressive Symptoms Among Elderly Individuals in Rural Areas of Jeju Island (제주 농촌 지역 노인들의 우울증상 유병률 및 관련 요인)

  • Objectives : This study aims to explore the prevalence of depressive symptoms among elderly residents in the relatively stable rural areas of Jeju and to examine the relationships between levels of depression, sociodemographic factors, and health habits. Methods : The study site was within rural Jeju, where elderly individuals aged 65 and older were randomly selected from the 'Agricultural Cohort' registered at the Centers for Farmers' Safety and Health Center. Trained interviewers conducted surveys using the Short Form Geriatric Depression Scale (sGDS-K), defining those with scores of 6 or above as experiencing depressive symptoms for the analysis. Other variables such as sex, age, educational level, marital status, annual income, subjective health status, underlying disease, perceived stress levels, smoking, and drinking status were also recorded Results : Out of 533 subjects, the prevalence of depressive symptoms was 35.3%, with 28.5% in male and 45.6% in female (p<0.001). Factors significantly associated with the prevalence of depressive symptoms included marital status (p=0.014), educational level (p<0.001), annual income (p=0.034), subjective health status (p<0.001), perceived stress level (p<0.001), feeling of despair (p<0.001) and suicidal ideas (p<0.001). Multivariate logistic regression analysis revealed that subjective health status, perceived stress level, and feelings of despair were associated with the prevalence of depressive symptoms. Conclusions : The high prevalence of depressive symptoms among the rural elderly in Jeju highlights the need for targeted mental health interventions. Addressing sociocultural factors and improving early detection and intervention strategies can help reduce the socioeconomic impact of depression in this population.
