Quantitative Comparison of Motion Artifacts in PET Images using Data-Based Gating (데이터 기반 게이팅을 이용한 PET 영상의 움직임 인공물의 정량적 비교)

  • PET is used effectively for biochemical or pathological phenomena, disease diagnosis, prognosis determination after treatment, and treatment planning because it can quantify physiological indicators in the human body by imaging the distribution of various biochemical substances. However, since respiratory motion artifacts may occur due to the movement of the diaphragm due to breathing, we would like to evaluate the practical effect by using the a device-less data-driven gated (DDG) technique called MotionFree with the phase-based gating correction method called Q.static scan mode. In this study, images of changes in moving distance (0 cm, 1 cm, 2 cm, 3 cm) are acquired using a breathing-simulated moving phantom. The diameters of the six spheres in the phantom are 10 mm, 13 mm, 17 mm, 22 mm, 28 mm, and 37 mm, respectively. According to maximum standardized uptake value (SUVmax) measurements, when DDG was applied based on the moving distance, the average SUVmax of the correction effect by the moving distance was improved by 1.92, 2.48, 3.23 and 3.00, respectively. When DDG was applied based on the diameter of the phantom spheres, the average SUVmax of the correction effect by the moving distance was improved by 2.37, 2.02, 1.44, 1.20, 0.42 and 0.52 respectively.

  • Most Hepatic hemangiomas are asymptomatic and small in size, making them difficult to find by pathological examination. Therefore, radiological diagnosis is essential for the early finding and diagnosis of Hepatic hemangioma. Three-phase method using contrast medium in computed tomography, T1, T2-weighted imaging in magnetic resonance imaging, dynamic magnetic resonance imaging using contrast medium, echo planar imaging method, diffusion-weighted imaging method, blood pool scan using 99mTc-labeled red blood cells in nuclear medicine, we looked at the color doppler method In ultrasound, and it is important to accurately understand the imaging findings of hepatic hemangioma and perform the examination in order to make an accurate diagnosis. most hepatic hemangioma are benign tumors, care should be taken not to confuse them with malignant tumors such as hepatocellular carcinoma to prevent unnecessary procedures. Therefore, in order to make an accurate diagnosis, it is important to accurately understand the imaging findings of hemangioma and perform the examination.

A Case Report of Intraductal Carcinoma Detected in a Patient Undergoing Surveillance for Benign Breast Mass (유방 양성 종괴 추적 관찰 환자에게서 발견된 관상피내암 증례 보고)

  • Breast ductal carcinoma in situ(DCIS) refers to cases in which breast epithelial cells have become malignant but are still limited to normal ducts, and has been increasing rapidly in recent years. In this case, a two-year follow-up revealed findings on mammography and ultrasonography that indicated a small mass classified as BI-RADS Category 3, However far from typical malignant tumor these findings were. In the second year of follow-up, a hypoechoic mass with an indistinct boundary of the right breast in the 6 o'clock direction, on mammography appeared to be about 2.1×1.3 cm in size, and biopsy diagnosed it as ductal carcinoma. Since ductal endothelial cancer has no characteristic clinical findings and can show positive clinical and imaging findings in the early stages, regular follow-up is considered important for early diagnosis, and detection of ductal endothelial cancer through mammography and ultrasound is important for improving the prognosis of all breast cancer patients. During the initial examination conducted four years ago, we reported cases of intra ductal cancer in which asymmetric shading, microcalcification, and branched mass, indicative of intra ductal cancer, were observed during follow-up. It is advisable to concurrently explore methods for reducing X-ray dosage to mitigate potential side effects of contrast medium.

The Clinical Significance of Follow Up SCC Levels in Patients with Recurrent Squamous Cell Carcinoma of the Cervix (재발성 자궁경부 편평상피암 환자들에서 Squamous Cell Carcinoma 항원의 유용성)

  • Purpose : To investigate the clinical usefulness of a follow-up examination using serum squamous cell carcinoma antigen (SCC) for the early detection of recurrence in patients treated for conical squamous cell carcinoma. Materials and Methods : 20 patients who were treated for recurrent cervical squamous cell carcinoma between 1997 and 1998, who had experienced a complete remission after radiotherapy and who underwent an SCC test around the time when recurrence was detected, were included in this study. The levels of SCC were measured from the serum of the patients by immunoassay and values less than 2 ng/mL were regarded as normal. The sensitivity of the SCC test for use in the detection of recurrence, the association between the SCC values and the recurrence patterns and the tumor size and stage, and the temporal relation between the SCC increment and recurrence detection were evaluated. Results : The SCC values were above normal in 17 out of 20 patients, so the sensitivity of the SCC test for the detection of recurrence was $85\%$, and the mean and median of the SCC values were 15.2 and 9.5 ng/mL, respectively. No differences were observed in the SCC values according to the recurrence sites. For 11 patients, the SCC values were measured over a period of 6 months before recurrence was detected, and the mean and median values were 13.6 and 3.6 ng/mL, respectively. The SCC values of 7 patients were higher than the normal range, and the SCC values of the other 4 patients were normal but 3 among them were above 1.5 ng/mL. At the time of diagnosis, the SCC valuess were measured for 16 of the 20 recurrent patients, and the SCC values of the patients with a bulky tumor $(\geq4\;cm)$ or who were in stage IIb or III were higher than those of the patients with a non-bulky tumor or who were in stage Ib or IIa. Conclusion : The SCC test is thought to be useful for the early detection of recurrence during the follow up period in patients treated for cervical squamous cell carcinoma. When an effective salvage treatment is developed in the future, the benefit of this follow-up SCC test will be increased.

Literature Analysis of Radiotherapy in Uterine Cervix Cancer for the Processing of the Patterns of Care Study in Korea (한국에서 자궁경부알 방사선치료의 Patterns of Care Study 진행을 위한 문헌 비교 연구)

  • Purpose: Uterine cervix cancer is one of the most prevalent women cancer in Korea. We analysed published papers in Korea with comparing Patterns of Care Study (PCS) articles of United States and Japan for the purpose of developing and processing Korean PCS. Materials and Methods: We searched PCS related foreign-produced papers in the PCS homepage (212 articles and abstracts) and from the Pub Med to find Structure and Process of the PCS. To compare their study with Korean papers, we used the internet site 'Korean Pub Med' to search 99 articles regarding uterine cervix cancer and radiation therapy. We analysed Korean paper by comparing them with selected PCS papers regarding Structure, Process and Outcome and compared their items between the period of before 1980's and 1990's. Results: Evaluable papers were 28 from United States, 10 from the Japan and 73 from the Korea which treated cervix PCS items. PCS papers for United States and Japan commonly stratified into $3\~4$ categories on the bases of the scales characteristics of the facilities, numbers of the patients, doctors, Researchers restricted eligible patients strictly. For the process of the study, they analysed factors regarding pretreatment staging in chronological order, treatment related factors, factors in addition to FIGO staging and treatment machine. Papers in United States dealt with racial characteristics, socioeconomic characteristics of the patients, tumor size (6), and bilaterality of parametrial or pelvic side wail invasion (5), whereas papers from Japan treated of the tumor markers. The common trend in the process of staging work-up was decreased use of lymphangiogram, barium enema and increased use of CT and MRI over the times. The recent subject from the Korean papers dealt with concurrent chemoradiotherapy (9 papers), treatment duration (4), tumor markers (B) and unconventional fractionation. Conclusion: By comparing papers among 3 nations, we collected items for Korean uterine cervix cancer PCS. By consensus meeting and close communication, survey items for cervix cancer PCS were developed to measure structure, process and outcome of the radiation treatment of the cervix cancer. Subsequent future research will focus on the use of brachytherapy and its impact on outcome including complications. These finding and future PCS studies will direct the development of educational programs aimed at correcting identified deficits in care.

Physical Characteristics Comparison of Virtual Wedge Device with Physical Wedge (가상쐐기와 기존쐐기의 물리적 특성 비교)

  • Purpose : We have compared the characteristics of Siemens virtual wedge device with physical wedges for clinical application. Materials and Methods : We investigated the characteristics of virtual and physical wedges for various wedge angles (15, 30, 45, and 60$^{\circ}$) using 6- and 15MV photon beams. Wedge factors were measured in water using an ion chamber for various field sizes and depths. In case of virtual wedge device, as upper jaw moves during irradiation, wedge angles were estimated by accumulated doses. These measurements were performed at off-axis points perpendicular to the beam central axis in water for a 15cm${\times}$20cm radiation field size at the depth of loom. Surface doses without and with virtual or physical wedges were measured using a parallel plate ion chamber at surface. Field size was 15cm H20cm and a polystyrene phantom was used. Results : For various field sizes, virtual and physical wedge factors were changed by maximum 2.1% and 3.9%) , respectively. For various depths, virtual and physical wedge factors were changed by maximum 1.9% and 2.9%, respectively. No major difference was found between the virtual and physical wedge angles and the difference was within 0.5$^{\circ}$ . Suface dose with physical wedge was reduced by maximum 20% (x-ray beam :6 MV, wedge angle:45$^{\circ}$, 550: 80 cm) relative to one with virtual wedge or without wedge. Conclusions : Comparison of the characteristics of Siemens virtual wedge device with physical wedges was performed. Depth dependence of virtual wedge factor was smaller than that of physical wedge factor. Virtual and physical wedge factors were nearly independent of field sizes. The accuracy of virtual and physical wedge angles was excellent. Surface dose was found to be reduced using physical wedge.

Pretreatment Prognostic Factors in Carcinoma of the Uterine Cervix (자궁경부암에 있어서의 치료전 예후인자)

  • To identify pretreatment prognostic factors in carcinoma of the uterine cervix, a retrospective analysis was undertaken of 510 patients treated with curative radiation therapy in Seoul National University Hospital during the 7 year period, from March 1979 through December 1986. According to FIGO classification,35 patients were stage I B,89 were stage IIA, 232 were stage IIB,8 were stage IIIA, 134 were IIIB, and 12 were stage IVA. Five year locoregional control (LRC) rates in stage I B, II A, II B, IIIA, IIIB, and IVA were $79\%,78\%,70\%,58\%,51\%\;and\;27\%,$ respectively. Five year disease free survival (DFS) rates were $76\%,67\%,60\$,57\%,40\%,\;and\;25\%,$ respectively. Overall survival (OS) rates at five years were $82\%,72\%,67\%,67\%,51\%,\;and\;33\%,$ respectively. In univariate analyses, stage, age, initial hemoglobin level, type of histology, tumor size, and several CT findings including pelvic lymph node (LN) status, paraaortic lymph node (PAN) status, extent of parametrial invasion, bladder invasion, and rectal invasion were significant factors in terms of LRC. All these factors and elevation of BUN or creatinine were associated with DFS. In terms of overall survival, stage, initial hemoglobin level, type of histology, tumor size, elevation of BUN or creatinine, and five CT findings associated with LRC were prognostically significant. In multivariate analysis excluding CT findings, stage IV disease, non-squamous histology, and tumor size $\ge$4 cm were associated with poor LRC and DFS. Stage IV disease and tumor size significantly affected OS. in multivariate analysis including CT findings, histology, tumor size, and pelvic LN status on CT were uniformly significant in terms of LRC, DFS, and 05, PAN status on CT affected overall survival only.

Peripheral Giant Cell Granuloma in a Dog (개에서 거대세포 치은종의 증례)

  • A mass was detected in the oral cavity from a 18-year-old female miniature poodle dog. Grossly, the mass was soft to hard, red to purple, and $1.5{\times}1.5{\times}1cm$ in size. Histopathologically, the mass was composed of hyperplastic gingival epithelium, well-vascularized stroma, closely packed pleomorphic cells, and numerous giant cells with multiple nuclei and abundant eosinophilic cytoplasm. Immunohistochemically, tumor cells were positive for alkaline phosphatase and cytokeratin 7, but not positive for CD68 unlike in human. The mass was diagnosed to peripheral giant cell granuloma in oral cavity through typical clinical and histopathological features.

  • Ameloblastic fibro-odontoma is a rare benign odontogenic tumor that mainly occurs at mandibular molar area. Radiography usually shows a well-defined radiolucent area containing various amounts of radiopaque material of irregular size and form. In histiologic aspect, Ameloblastic fibro-odontoma composed of connective tissue characteristic of an ameloblastic fibroma and calcined tissue identifying the tumor as a complex odontoma. Thirteen years two months aged boy of this case visited with chief complaint of delayed eruption of mandibular left canine. After enucleation of lesion, biopsy was done and diagnosed by ameloblastic fibro-odontoma. In continous follow-up check, canine erupted normally after operation.

Fine Needle Aspiration Cytology of Adenomyoepithelioma of the Breast - Comparison with Typical Fibroadenoma - (유방 선근상피종의 세침흡인 세포학적 소견 - 섬유선종과의 비교 -)

  • Adenomyoepithelioma is an uncommon benign tumor of the breast. We present the fine needle aspiration cytologic features of adenomyoepithelioma in a 23 year-old Korean women, initially diagnosed as fibroadenoma. Aspiration cytologic findings of the left breast mass revealed high cellularity, small to medium sized, less cohesive epithelial clusters, rich naked cells and amorphous materials on background. The epithelial cells were round and uniform with no cytologic atypia or mitosis. Myoeplthelial cells were conspicuous with peripheral rimming along the epithelial clusters. Small amount of fibrotic stromal tissues were observed. Distinguishing features from typical fibroadenoma are less tight epithelial clusters, dyscohesive epithelial cell aggregates, more abundant naked cells and scant stromal tissue fragments.
