Purpose: This study was designed to know the usefulness of the MRI to distinguish lipoma and well differentiated liposarcoma (WDL). Materials and methods: 47 lipomatous tumors with MRI were reviewed among the 107 lipomatous tumors operated in our department. MRI examinations and their corresponding pathology reports were compared to determine sensitivity, specificity, diagnostic ability, positive predictable value and negative predictable value. Statistical analysis was performed to know the relationship between malignancy of the tumor (WDL) with the age and gender of the patients, and location, depth, size and the enhancement of tumors in MRI. Results: Among 28 lipoma in MRI examinations, 26 were proved as lipoma in pathology, and only 6 were WDL from 19 suspicious lesions in MRI, and others were proved as lipoma variants mostly. The varieties of lipoma variants were fibrolipoma, angiolipoma, spindle cell lipoma, lipoblastoma and angiomyolipoma. The sensitivity, specificity, diagnostic ability, positive predictable value and negative predictable value of MRI were 100%, 68 %, 72%, 31% and 100% in WDL, and 90%, 89%, 89%, 93% and 84% in lipoma. Among the variants to distinguish WDL and lipoma, the size of tumor and enhancement in MRI were significant statistically (p<0.05). Conclusion: MRI was highly sensitive in detection of WDL and highly specific in detection of simple lipoma. The size of tumor and enhancement in MRI were significant variants to distinguish WDL and lipoma. When MRI finding is non-specific, it is more likely to represent one of lipoma variants.

Objectives: This study was conducted to analyze the dental care utilization patterns and related factors of the rural residents. Methods: The data collected by interview and self-administered questionnaire survey of 524 peoples of Seongju county in Gyeongsanbuk-do. The summarized results are as follows. Results: The rate of persons who experienced the oral disease was 52.5% during 1 year and it was at most in the age group of 40-49. The rate of persons who had experienced the oral disease were investigated according to general characteristics, perception of oral health, being of regular treatment facility. Therefore the rate of persons who had experienced the oral disease was significantly higher the younger peoples, worse oral health status and being of the regular treatment source than the other groups. During 1 year period, 64.0% of the cases had treated the perceived oral disease, 36.0% did no action at all during last year. Among respondents, 49.4% had treated their oral disease at dental clinics, 8.0% had treated at community health center or subcenter and remains did not treated at all. The results of logistic regression analysis suggested that statistically significant factors in dental health care utilization were educational level, degree of pain, oral health status and regular treatment facility. Therefore the dental health care utilization rate was higher at groups with the high educational level, serious pain, better oral health status and being of the regular treatment source than other groups. 45.5% of the rural residents did not treat their oral disease immediately due to the no identified need, limitation of time(19.2%), economic limitation(19.2%), and geographical limitation(9.0%). Conclusions: In consideration of above findings, we may conclude that oral health community program to prevent oral diseases should be intensified, oral health education to raise oral health knowledge should be performed periodically.

The ultimate objective of this study is to develop a reliable oxygen-enriched combustion techniques especially for the case of the flue gas recycling in order to reduce the $CO_2$ emissions from practical industrial boilers. To this end a systematic numerical investigation has been performed, as a first step, for the resolution of the combusting flame characteristics of lab-scale LNG combustor. One of the important parameters considered in this study is the level of flue gas recycling calculated in oxygen enriched environment. As a summary of flame characteristics, for the condition of 100% pure $O_2$ as oxidizer without any flue gas recycling, the flame appears as long and thin laminar-like shape with relatively high flame temperature. The feature of high peak of flame temperature is explained by the absence of dilution and heat loss effects due to the presence of $N_2$ inert gas. The same reasoning is also applicable to the laminarized thin flame one, which is attributed to the decrease of the turbulent mixing. These results are physically acceptable and consistent and further generally in good agreement with experimental results appeared in open literature. As the level of $CO_2$ recycling increases in the mixture of $O_2/CO_2$, the peak flame temperature moves near the burner region due to the enhanced turbulent mixing by the increased amount of flow rate of oxidizer stream. However, as might be expected, the flue gas temperature decreases due to presence of $CO_2$ gas together with the inherent feature of large specific heat of this gas. If the recycling ratio more than 80%, gas temperatures drop so significantly that a steady combustion flame can no longer sustain within the furnace. However, combustion in the condition of 30% $O_2/70% $$CO_2$ can produce similar gas temperature profiles to those of conventional combustion in air oxidizer. An indepth analyses have been made for the change of flame characteristics in the aspect of turbulent intensity and heat balance.
Yet this study of a private security company guards who did not apply to most firms - An exchange relationship with the quality of the application of the theory of private security companies and the high turnover and low job satisfaction and find ways to improve the firm - An exchange relationship Job satisfaction and turnover of the quality of the major research variables with a theoretical discussion of the relationship they set and based on this study demonstrate the analysis is attempted. This expansion contributes to the possibility of applying the theory, and private security companies to manage their organizations more effective by presenting the theoretical basis. Was trying to contribute to practical. Therefore, in this study to determine the following three things that it first, in a private security company bosses - men exchange turnover of the members of the quality of the relationship is to help the relationship, secondly, from a private security company boss - An exchange relationship The quality of the work of the members are satisfied with the relationship, and finally the 'private security company's turnover in the member's job satisfaction is associated with a reduction of is whether. To this end, Seoul. Gyeonggi Province located in a private security guard companies and 238 people involved in the actual target and try to analyze these results demonstrate the firm - An exchange relationship to the quality of the turnover of the direct impact of the factors, but showed up as non - Job satisfaction showed a significant impact. Job satisfaction also significantly affect the turnover of the firm - An exchange relationship to the quality parameters according to degree of turnover. Therefore, exchange relationships with men from private security companies want to increase the quality of the firm's efforts to improve the men's job satisfaction indirectly through a reduction in turnover can be affected have been identified. Two-dimensional trend-vector model of sediment transport is first tested in the tidal flat of Garolim Bay, mid-western coast of the Korean Peninsula. Three major parameters of surface sediment, i.e., mean grain size, sorting and skewness, are used for defining the best-fitting transport trend-vector on the sand ridge and muddy sand flat. These trend vectors are compared with the real transport directions determined from morphology, field observation and bedforms. The 15 possible cases of trend vectors are calculated from total sediments. In order to find the role of coarse sediments, trend vectors from sediments coarser than < 4.5 ${\phi}$, (sand size) are separately calculated from those of total sediments. As compared with the real directions, the best-fitting transport-vector model is the "case M" of coarse sediments which is the combined trend vectors of two cases: (1) finer, better sorted and more negatively skewed and (2) coarser, better sorted and more positively skewed. This indicates sand-size grains are formed by simpler hydrodynamic processes than total sediments. Transported sediment grains are better sorted than the source sediment grains. This indicates that consistent hydrodynamic energy can make sediment grains better sorted, regardless of complicated mechanisms of sediment transport. Consequently, both transported vector model and real transported direction show that the source of sediments are located outside of bay (offshore Yellow Sea) and in the baymouth. These source sediments are transported through the East Main Tidal Channel adjacent the baymouth. Some are transported from the subtidal zone to the upper tidal flat, but others are transported farther to the south, reaching the south tidal channel in the study area. Also, coarse sediment grains on the sand ridge are originally from the baymouth, and transported through the subtidal zone to the south tidal channel. These coarse sediments are moved to the northeast, but could not pass the small north tidal channel. It is interpreted that the great amount of coarse sediments is returned back to the outside of the bay (Yellow Sea) again through the baymouth during the ebb tide. The distribution of muddy sand in the northeastern part of study area may result from the mixing of two sediment transport mechanisms, i.e., suspension and bedload processes. The landward movement of sand ridge and the formation of the north tidal channel are formed either by the supply of coarse sediments originating from the baymouth and outside of the bay (subaqueous sand ridges including Jang-An-Tae) or by the recent relative sea-level rise.
With the advent of stage-adapted multimodal regiments for esophageal malignancy, accurate staging has become of utmost importance. Endoscopic ultrasono graphy(EUS) i.: a diagnostic method for the local staging of esophageal cancer, in particular for T and N evaluation. The aim of this study was to examine the diagnostic efficacy of the EUS and to evaluate the relation between prognosis and EUS findings. Material and Method : We reexamined the cases of 88 consecuitive patients who underwent transthroacic esophagectomy and cervical esophagogastrostomy for esophageal cancer from January 1991 to July 2001 We compared EUS findings, histological results and outcomes. Result : There were 83 male(94.3%) and 5 female. Median age was 61.3 years old. Overall staging accuracy were 59.0% for T staging and 76.0% for N staging, In early T staging group, the accuracy was 74.9% except 74 stage. However significant differences were not found in the ability of EUS-determined T-stage and N-Stage to predict survival. Multivariate analysis showed EUS-stage to predict survival. Conclusion : EUS provides a high degree of accuracy in assessing both T and N parameters in staging esophagal cancer. EUS should be performed in all patients with esophageal cancer, not only for staging before therapy, but also as a valuable method of determining prognosis.
Purpose : To identify variable prognostic factors and analyse failure patterns in the uterine cervix cancer after radical operation and adjuvant radio-therapy, a retrospective analysis was undertaken. Materals and Methods : I analysed one hundred and twenty four patients with uterine cervix cancer, FIGO stage IB, IIA and IIB, treated with radical hysterectomy and pelvic lymph node dissection followed by adjuvant radio-therapy between May 1985 and May 1994. Minimum follow up period was 24 months. All of them were treated with full dose external radiotherapy with linear accelerator and/or high dese rate intracavitary radiation. Results : Overall 5 year survival rate and relapse free survival rate were $75.4\%,\;73.5\%$, respectively. Significant prognostic factors by relapse free survival were wall involvement thickness, lymph node location and number, parametrium involvement, tumor size, stage, uterine body involvement, vaginal resection margin involvement. By multivariate analysis, lymph node matastasis. tumor size and vaginal resection margin involvement were significant prognostic factos. Treatment related failure were 33 cases. Locoregional failure were more likely in the stage IIB, lymph node positive or vaginal resection margin positive patients whereas distant failures were relatively more frequent in stage IB, IIA and lymph node, vaginal resection negative patients. In stage IIB, 5 year relapse free survival rate was only $56\%$ and nine of twenty two patients recurred. Conculsion : Postoperative radiotherapy results are good for patients with relatively low risk factor. But the results are poor for patients with multiple, high risk factors or stage IIB. To control recurrence for patients with high risk factors, postoperative adjuvant radiotherapy is not sufficient treatment method. To raise control rate adding other methods such as radiosensitizing agent or chemotherapy is necessary and prospectively randomized study is needed for evaluation of postoperative radiotherapy efficacy and /or other methods. And it is reasonable to treat primary radical radiotherapy for patients with stage IIB cervical cancer instead of radical operation and adjuvant radiotherapy and/or chemotherapy regimen.
Purpose: We have evaluated characteristics of adrenal masses incidentally observed in nonenhanced F-18 FDG PET/CT of the oncologic patients and the diagnostic ability of F-18 FDG PET/CT to differentiate malignant from benign adrenal masses. Materials and Methods: Between Mar 2005 and Aug 2008, 75 oncologic patients (46 men, 29 women; mean age, $60.8{\pm}10.2$ years; range, 35-87 years) with 89 adrenal masses incidentally found in PET/CT were enrolled in this study. For quantitative analysis, size (cm), Hounsfield unit (HU), maximum standardized uptake value (SUVmax), SUVratio of all 89 adrenal masses were measured. SUVmax of the adrenal mass divided by SUVliver, which is SUVmax of the segment 8, was defined as SUVratio. The final diagnosis of adrenal masses was based on pathologic confirmation, radiologic evaluation (HU<0 : benign), and clinical decision. Results: Size, HU, SUVmax, and SUVratio were all significantly different between benign and malignant adrenal masses.(P < 0.05) And, SUVratio was the most accurate parameter. A cut-off value of 1.0 for SUVratio provided 90.9% sensitivity and 75.6% specificity. In small adrenal masses (1.5 cm or less), only SUVratio had statistically significant difference between benign and malignant adrenal masses. Similarly a cut-off value of 1.0 for SUVratio provided 80.0% sensitivity and 86.4% specificity. Conclusion: F-18 FDG PET/CT can offer more accurate information with quantitative analysis in differentiating malignant from benign adrenal masses incidentally observed in oncologic patients, compared to nonenhanced CT.
Purpose: The aim of this study was to evaluate the influence of leptin on biochemical markers of bone metabolism in childhood obesity. Methods: A total of 50 male children (25 obese and 25 controls) were recruited from the pediatric outpatient clinic at the Chosun University Hospital from November 1st 2005 to May 30th 2006. BMI, body fat percentage, serum leptin, bone-specific alkaline phosphatase (B-ALP), C-terminal propeptide of type 1 collagen (CICP), total deoxypyridinoline crosslinks (total DPD) were measured. The correlations of leptin with BMI, body fat percentage, B-ALP, CICP, total DPD were analyzed by Pearson's correlation. In a multiple stepwise regression analysis, leptin after correction for body weight was evaluated if there was a correlation with biochemical markers of bone formation and resorption respectively. Results: The leptin levels of the obese group were significantly higher than those of the control group (p=0.012). In the obese group, the leptin level was significantly positively correlated with the BMI (r=0.551, p=0.01) and the percentage of body fat (r=0.584, p=0.018). In the obese group, of bone markers, B-ALP (r=-0.613, p=0.026) and CICP (r=-0.583, p=0.037) were negatively correlated with leptin. B-ALP (r=-0.728, p=0.007) and CICP (r=-0.684, p=0.014) were negatively correlated with leptin when corrected for body weight. In the control group, bone markers were not correlated with leptin. In the multiple stepwise regression analyses, there was a negative correlation between the leptin and B-ALP (Y=-39.653X+356.341, p=0.026), CICP (Y=-13.437X+ 116.013, p=0.037) respectively in the obese group. Conclusion: Leptin was a significant factor in the bone formation but not in bone resorption in childhood obesity.
Purpose : A new virtual simulation technique for craniospinal irradiation (CSI) that uses a CT-simulator was developed to improve the accuracy of field and shielding placement as well as patient positioning. Materials and Methods : A CT simulator (CT-SIM) and a 3-D conformal radiation treatment planning system (3D-CRT) were used to develop CSI. The head and neck were immobilized with a thermoplastic mask while the rest of the body was immobilized with a Vac-Loc. A volumetric image was then obtained with the CT simulator. In order to improve the reproducibility of the setup, datum lines and points were marked on the head and body. Virtual fluoroscopy was performed with the removal of visual obstacles, such as the treatment table or immobilization devices. After virtual simulation, the treatment isocenters of each field were marked on the body and on the immobilization devices at the conventional simulation room. Each treatment fields was confirmed by comparing the fluoroscopy images with the digitally reconstructed radiography (DRR) and digitally composited radiography (DCR) images from virtual simulation. Port verification films from the first treatment were also compared with the DRR/DCR images for geometric verification. Results : We successfully performed virtual simulations on 11 CSI patients by CT-SIM. It took less than 20 minutes to affix the immobilization devices and to obtain the volumetric images of the entire body. In the absence of the patient, virtual simulation of all fields took 20 min. The DRRs were in agreement with simulation films to within 5 mm. This not only reducee inconveniences to the patients, but also eliminated position-shift variables attendant during the long conventional simulation process. In addition, by obtaining CT volumetric image, critical organs, such as the eyes and the spinal cord, were better defined, and the accuracy of the port designs and shielding was improved. Differences between the DRRs and the portal films were less than 3 m in the vertebral contour. Conclusion : Our analysis showed that CT simulation of craniospinal fields was accurate. In addition, CT simulation reduced the duration of the patient's immobility. During the planning process. This technique can improve accuracy in field placement and shielding by using three-dimensional CT-aided localization of critical and target structures. Overall, it has improved staff efficiency and resource utilization by standard protocol for craniospinal irradiation.
