Comparision of Risk Factors in Lung Cancer Surgery of Above 70-Year Old Patients (70세이상 환자에 대한 폐암 수술의 위험성 비교)

  • Background: Early detection and surgical resection offer the most advantage out of all cures for lung cancer. Elderly patients may fail to benefit maximally from these interventions because of their general condition and residual lung function. To study the impact of age on stages, histology, symptoms, and treatments of the patients with non-small cell lung cancer, we undertook a retrospective review. Material and Method : Two hundred eleven patients with non-small cell lung cancer were operated on at Samsung Seoul hospital between October 1994 and June 1997. Patients were arbitrarily arbitrarily by age less than 70 years(176 patients) and 70 years or more(35 patients), and their medical records were reviewed. Result: There were no differences in pathologic staging and diagnosis. But there were differences in surgical methods, complications, and mortality rates between the two groups. There were much more complications in the 70 years or more group(p=0.02). We chose less invasive surgical methods in the 70 years or more group. Conclusion: More complications were experienced in the 70 years or more group. Although thoracic operation imparts the greatest survival advantage, this benefit is diminished in elderly patients because of their high complications and mortality rate. We recommend serious consideration of surgical indications and operative methods.

Endomyocardial Fibrosis -A case report- (심내막 심근섬유증)

  • Endomyocardial fibrosis(EMF) is an unusual type of cardiomyopathy characterized by a restriction to the ventricular filling and an obliteration of the inflow portion in the ventricular cavity by a fibrosis and often by a thrombus formation. The atrioventricular valve may be involved, resulting in an atrioventricular valvular regurgitation. The only known effective treatments are endomyocardiectomy and replacement of regurgitant AV valves. We report the experience of a case of EMF which required surgical management.

  • In children, the snapping-knee syndrome is strongly suggestive of a diagnosis of discoid menisci. Furthermore the loss of extension of the knee suggests a diagnosis of torn lateral discoid meniscus. Symptomatic lateral discoid meniscus tear in a very young child, especially under the 3-year-old, was very rarely reported and the treatment protocol has not been established. Some therapeutic advancement was achieved in symptomatic lateral discoid meniscus but still technical difficulties exist. We undergone arthroscopic meniscal reshaping in a 26-month-old girl. At the time of postoperatively 2-year 9-month follow up, excellent result has achieved in clinical and functional outcome according to the scale of Ikeuchi without recurrence of symptom or postoperative complication. We reported out experience and results that arthroscopic reshaping procedure was done for management of symptomatic lateral discoid meniscus in a 26-month-old girl.

Clinical Application of Endoscopic Laser Assisted Supraglottic Partial Laryngectomy in Early Supraglottic Cancer (초기 상후두암종에서 레이저를 이용한 내시경하 상후두부분절제술의 적용)

  • Background: Supraglottis and glottis have a different embryologic origin. Supraglottic cancer is characterized by high incidence of cervical lymph node metastasis at initial diagnosis, and favored surgical management of the early supraglottic cancer was partial supraglottic laryngectomy, however the procedure resulted in frequent incidences of postsurgical aspiration and voice disabilities. Objectives: We retrospectively analyzed the problems and the advantages of the endoscopic laser assisted supraglottic partial laryngectomy as a part of surgical management for early supraglottic cancer. Materials and Methods: During the past nine years 25 cases of supraglottic cancer(Tl 10 cases, T2 15 cases) were treated by tracheotomy and laser assisted supraglottic partial laryngectomy(KTP532, 15 Watt, continuous type) and in 10 cases with cervical lymph node metastasis, they were additionally managed by neck dissection one week later, and all cases received postoperative irradiation therapy. Results: At present, 19 cases are alive with no evidence of disease. During the follow up period total of six cases(primary failure: three cases, nodal failure: three cases) were recurred. In relation to tumor staging, One of the 10 Tl cases and two of the 15 T2 cases recurred showing 88% locoregional recurrence rate for early supraglottic cancer. Postoperative com-plication included bleeding in three cases who were controlled by electrocautery under general anesthsia, one case of longstanding aspiration and two cases of laryngeal stenosis as a delayed complication. Conclusion: High control rate suggests that the endoscopic laser assisted supraglottic partial laryngectomy may be a good initial management method for early supraglottic cancer, however it is difficult to determine the resection margin, therefore, accurate tumor staging must be done prior to surgery. In order to prepare for postoperative bleeding, edema and aspiration, the tracheotomy must be performed prior to surgery.

  • Purpose: Management of the stiff elbow by arthroscopic procedure is an effective but technically demanding. Our purpose was to review the specific arthroscopic maneuver which can be useful for the stiff elbow. Materials and Methods: A stiff elbow that is refractory to conservative treatment can be treated surgically to remove soft tissue or bony blocks to motion. The olecranon or coronoid osteophyte and loose bodies have been removed arthroscopically with good results and rare complications. Results and Conclusion: For the successful arthroscopic management of elbow stiffness, it need to knowledge and skills for debride contracted tissue and preserve vital anatomic structure.

Percutaneous Radiofrequency Therapy of Benign Bone Tumors in the Femoral Head (대퇴골두 부위에 발생한 양성 골 종양에 대한 경피적 고주파치료 (증례보고))

  • Purpose: To report two cases of bone tumors other than osteoid osteoma in the proximal femur and treated with percutaneous high frequency radioablation method. Cases: We reviewed two cases with intracortical chondroma and enchondroma in the femoral head retrospectively. The patient with intracortical chondroma was a thirty one year old woman and had suffered right hip pain of 1 year duration. The lesion was located in the head of right femur and treated with CT guided percutaneous high frequency radioablation after needle biopsy under general anesthesia. The symptom was gone immediately after the procedure and was discharged postop. 1 day. 15 months has passed without symptom recurrence. Second case having enchondroma, was 56 year old woman complaining of gluteal area pain for 3 months. Radiologic evaluation showed osteolytic lesion with sclerotic rim on the inferior portion of the left femoral head. She received a same therapy with CT guided radiofrequency ablation following needle biopsy. She reported dramatic pain relief after the procedure and was discharged postop. 1 day. No symptom has occurred for 3 months until now. Conclusion: We present 2 cases of bone tumor occurred in the hip joint area other than osteoid osteoma which were treated with CT guided radiofrequency ablation.

Functional Result of Limb Salvage Surgery with Tumor Prosthesis for Osteosarcoma of Proximal Tibia (근위 경골 골육종의 종양대치물을 이용한 사지 구제술 후의 기능 평가)

  • Purpose : Limb salvage for osteosarcoma of proximal tibia is challenging problem due to difficulties in mobilizing or retracting the main neurovascular structure, inadequate soft tissue coverage, and unsolved problem of patellar tendon reattachment to endoprosthesis. The authors analyzed the functional result of limb salvage using tumor prosthesis with medial gastrocnemius rotation plasty for osteosarcama of the proximal tibia. Materials and Methods : Eleven patients with histologically proven osteosarcoma of the proximal tibia, treated with adjuvant and neoadjuvant chemotherapy and limb salvage operation with tumor prosthesis between January 1992 and December 1998 at our Medical Center, were selected. There were 6 male and 5 female. Age ranged from 15 years to 23.7 years with an average of 23.7 years. Follow-up period ranged from 1 year to 4.5 years with an average of 2.5 years. The final functional result was evaluated using the method by ISOLS, 1993. The factors include pain, functional activities, emotional acceptance, use of external supports, walking ability and gait. Each of the factors has been scored from 0 to 5 depending on the appropriate description or data. The rating score is determined by dividing the individual factor scores into the total score and indicates percentage of normal function. Results : The overall functional result ranged from 53,3% to 86.7% with an average of 68.3% of normal function. In details, the averages were 82.5% for pain, 62.5% for functional activities, 67.5% for emotional acceptance, 77.5% for use of external supports, 62.5% for walking ability, and 57.5% for gait. The average range of motion of the knee joint was $5^{\circ}$ extension and $85^{\circ}$ flexion. Five patients have extension lag ranged from $5^{\circ}$ to $15^{\circ}$ with an average of $10^{\circ}$. Two patients suffered postoperative infection. One was treated with antibiotics injection only, but the other needed removal of the prosthesis and knee fusion. Both of them showed unsatisfactory result. C o n c l u s i o n : The overall functional result after limb salvage using tumor prosthesis with medial gastrocnemius rotational flap for osteosarcoma of the proximal tibia was relatively satisfactory in case of no postoperative infection. The patients were less satisfactory in functional activities, emotional acceptance and gait than pain, use of external supports due to limitation of motion and extension lag. More aggressive postoperative physical therapy and protection with brace for 6~9 months as well as surgical technique is mandatory for more satisfactory result.

Review of osteochondroma: involved in temporomandibular joint (측두하악관절에 발생한 골연골종에 대한 고찰)

  • Osteochondroma is the most common benign bone tumor. The majority of osteochondromas (85%) present as solitary, nonhereditary lesions. In general, osteochondroma has no symptoms,however, facial asymmetry, malocclusion, crossbite and mouth opening can be occurred in case of temporomandibular joint involved. Radiologic analyses are indispensable element to diagnose osteochondroma and pathogenetic analysis showed that hereditary multiple osteochondromas are caused by mutations in either of two genes: exostosis(multiple)-1 (EXT1), which is located on chromosome 8q24.11 - q24.13 or exostosis(multiple)-2 (EXT2), which is located on chromosome 11p11 - 12. Recently, reduced mRNA of EXT1 was described in nonhereditary osteochondromas. The treatment of choice for osteochondroma is surgical unless the skeleton is still immature. Surgery associated with orthodontic treatment can be a valid approach to minimize facial asymmetry and malocclusion in case of temporomandibular with osteochondroma.

Hypertrophy of the Anterior Cruciate Ligament of the Knee (슬관절의 전방 십자 인대의 비후)

  • Purpose : To propose the diagnosis, 'hypertrophy of the anterior cruciate ligament(ACL)', and its clinical, radiological and pathological characteristics. Materials & Method : Since Dec. 1995, we have experienced 10 patients(12 knees) with hypertrophy of the ACL. There were 2 males and 8 females, and the age of the patients ranged from 35 to 67 years. Characteristics of clinical, radiological and pathological findings were analysed. Results : The chief complaint was insidious onset of dull pain on the knee which became worse with activity. The constant physical finding was painful limitation of full extension of the knee. Sagittal MR images showed diffuse swelling of the ACL, similar finding that could be seen in acute ACL tear. Arthroscopically, noted was a marked enlargement of the ACL causing notch impingement. Biopsies of the hypertrophied ACL showed increased collagenous tissue with variable degree of myxoid degeneration. With partial excision of the hypertrophied ACL with or without notchplasty, the symptoms improved in all. Conclusion : Hypertrophy of the ACL, a newly proposed diagnosis, should be considered be determining the cause of the painful knee, and further study should be done about its pathogenesis.
