  • 오늘날 알지네이트 인상재는 다른 어떠한 인상재보다 널리 사용되고 있다. 이는 알지네이트 인상재가 ①사용하기 쉽고, ②환자에게 거부감이 적고, ③특수한 장비의 사용이 필요하지 않으며, ④정확히 사용하면 상당히 정밀한 인상이 가능하기 때문이다. 알지네이트 인상재의 정확한 사용법을 정리한다.

  • 치과용 인상재는 구강내에서 치아나 주위조직을 정확히 인기하여야 하며 조작이 간편하고 유동성이 충분하며 경화시간이 적당하고 값도 저렴해야 한다. 또 구강내에서 제거할 때 영구변형이나 파절되지 않는 충분한 강도와 크기의 정확성도 있어야 하며 자극이나 독성이 없고 인체에 안정성이 있고 모형재와 친화성이 있으며 보관중에도 변질해서는 안된다. 그러나 인상재는 서로 상이한 특성이 있고 장단점이 각각 다르므로 모든 조건에 일치하는 인상재는 거의 없다. 따라서 여러 인상재중에서 가장 적합한 인상재를 선택하는 것이 중요한 과정이다. 즉 인상재의 특성과 응용을 알고 선택할땐 보다 정밀한 모형을 얻을 수 있다. 기록에 의하면 18~19세기경에 최초로 인상용 왁스를 사용한 이래 19세기 중엽에 인상용 석고, 1925년에 agar, 1930년 ZOE, 1945년 앨지네이트, 1957년 실리콘, 1967년 polyether 및 1975년 부가중합형 실리콘이 사용되었으며 인상재가 유연성과 탄성이 있는가 경고하며 비탄성인가 또는 온도변화와 화학반응으로 경화되는 가에 따라서 ZOE paste, 인상용 석고, 인상용 왁스, 인상용 콤파운드, 수성콜로이드(agar, 앨지네이트), polysulfide, 실리콘(축중합형, 부가중합형) 및 polyether 인상재등 10여종으로 분류된다.

Comparative study on properties of commercial polyvinyl siloxane impression materials (시판 폴리비닐실록산 인상재의 물성에 관한 비교연구)

  • The purpose of this study was to compared with the properties of commercial polyvinyl siloxane impression materials on the Korean product and the other country one. The materials used in this study were 5 hydrophilic polvinyly siloxane impression materials of light body automixing type (Perfect-F, Handae Chemical, Korea; Examix, GC, Japan; Contrast, VOCO, Germany; Express, 3M, U.S.A; Extrude, Kerr, U.S.A.). Specimens of each impression material were fabricated from a mold with dimensions identical to that specified in ASTM D624-91 and were subjected to tensile lode at 500 mm/min until failure for tear strength. Properties of consistency, strain in compression, recovery from deformation, detail reproduction and linear dimensional change were tested according to the testing methods of the ISO specification no.4823(1992). From this study, the follow ing results were obtained 1. The tear strength values were high ET(3.4kN/m), EP(3.4kN/m), PF(3.0kN/m), CT(3.0kN/m), but the lowest was EM(2.2kN/m)(p<0.05). 2. In the consistency, CT(47 mm) had the highest value, followed by PF(42.1 mm), ET(41.2 mm), and EM(39.6 mm), EP(39.2 mm)(p<0.05). It means more flow that the consistency value is high. 3. The strain in compression values were high followed by EM(5.8%), PF(52%), CT(4.6%), ET(4.1%), EP(2.9%)(p<0.05). 4. In the recovery from deformation, ET(99.95%) and PF(99.90%) had the highest value, followed by EP(99.75%), EM(99.74%) without statistical signification, CT(99.64%) had the lowest value(p<0.05). 5. Detail reproduction reappeared to line $20{\mu}m$ all products. 6. The linear dimensional change was a little shrinked all products. and there were not statistical signification (p>0.05). All products were satisfied the ISO specification.

A Study on the Physical Properties of Addition Silicone Impression Materials (부가중합형실리콘인상재의 물리적 특성에 대한 연구)

  • This study evaluated the properties of polyvinylsiloxane impression materials: Strain-in-compression, Elastic recovery, Consistency, and Wettability. Four polyvinylsiloxane impression materials(Examixfine, GC, Japan; Aquasilultra, Dentsply, USA; Extrude, Kerr, USA; Perfect-F, Handae, Korea) were used in this study. The measured data were analyzed with SPSSWIN 12.0 program. The results were as follows : 1. The strain-in-compression values showed that Extrude had the lowest value, 4.86% and Examixfine had the highest value, 6.78%. Significant difference between Examixfine and Perfect-F was not found but the other groups had significant differences(p = 0.000). 2. In the elastic recovery test, Extrude had the highest, 99.83% and Perfect-F had the lowest, 96.54%. There is no significant difference between Examixfine and Aquasilultra. The other impression material groups showed significant differences(p = 0.000). 3. The measurement of consistency results that the diameter of Examixfine was the largest, 45.12mm and one of Perfect-F was the smallest, 40.28 mm. No differences were shown between Examixfine and Aquasilultra and between Extrude and Perfect-F(p = 0.001). 4. Aquasilultra had a better hydrophilicity($47.85^{\circ}$) than other materials. Perfect-F had a larger contact angle($94.89^{\circ}$) and was hydrophobic. There were significant differences between groups(p = 0.000). 5. As for correlation among the test types, contact angle correlated with strain-in-compression, elstic recovery and consistency. The strain-in-compression had correlation to contact angle and elastic recovery. The elastic recovery correlated with contact angle and strain-in-compression. And the consistency had correlation to contact angle.

A Study on the Accuracy of Scan by the Standard Model Deformation Depending on the Hand Scanning Method (핸드스캐닝 작업 방법에 따라 표준 모델 변형이 작업 정밀도에 미치는 정밀스캔에 관한 연구)

  • This study was conducted to evaluate the distortion and data accuracy that may occur depending on the methods employed by the oral scanner (intra-oral scanner). Deseutap 3D models employing a plaster model used clinically as a scanner to create a standard scan data using the same model, separated by oral scanners in three different ways (AS Group, ZS group, OS group) How to scan each 5 times made the scan data for each group, it shows the 0.121 mm, 0.172 mm AS group, OS group 0.423 mm accuracy in ZS group. The ZS group showed the highest accuracy, with maximum error values of 0.113 mm, 0.169 mm and 0.246 being observed for the ZS, AS and OS group, respectively. The three scanning methods showed a clear differences in accuracy and reproducibility and also appeared to be meaningful in clinical practice.

Investigation of factors influenced on accuracy of polyvinylsiloxane (일부 시판 폴리비닐실록산 인상재의 정밀도에 영향을 미치는 요인분석)

  • In this study, polyvinylsiloxane impression materials were investigated to examine the characteristics of the impression materials that affect the accuracy of the final restoration. The flow property of impression materials which can reproduce the detail in the oral cavity and accurately duplicate it, and the tear strength and strain-in-compression which can cause problems when it is being removed from the oral cavity were studied. The results are as follows. 1. As for the flow properties of impression materials, Imprint II was 18.24${\pm}$0.30, which was the highest: and Twinz was 8.9${\pm}$0.62, which was the lowest. There was no significant difference among Imprint II, Examixfine, and Genie(p<0.05). 2. As for the tear strength of impression materials, Imprint II had the highest level, while Genie had the lowest value. There was no significant difference between Twinz and Genie(p<0.05). 3. As for the strain-in-compression, there were significant differences by impression material groups(p<0.05); the strain-in-compression of Examixfine was shown to be the highest at 5.56${\pm}$0.56, Twinz and Imprint II followed respectively, and Genie has the lowest at 3.19${\pm}$0.23. 4. Flow showed the correlation with tear strength but no significant connection to strain-in-compression. Also, there was no significant correlation between the tear strength and strain-in-compression (p<0.05). Making impression to reproduce oral tissue and tooth is an important part of making final restoration. The accuracy of impressions is influenced by the methods of taking impression or other condition. However the property itself of impression materials is the most essential and the materials with proper qualities should be selected.

Fabrication of complete denture using digital technology in patient with mandibular deviation: a case report (하악 편위 환자에서 디지털 방식을 이용한 총의치 제작 증례)

  • Recently, digital technology and computer-aided design/computer-aided manufacturing (CAD/CAM) environment have changed the clinician treatment method in the fabrication of dentures. The denture manufacturing method with CAD/CAM technology simplifies the treatment and laboratory process to reduce the occurrence of errors and provides clinical efficiency and convenience. In this case, complete dentures were fabricated using stereolithography (SLA)-based 3D printing in patient with mandibular deviation. Recording base were produced in a digital model obtained with an intraoral scanner, and after recording a jaw relation in the occlusal rim, a definitive impression was obtained with polyvinyl siloxane impression material. In addition, facial scan data with occlusal rim was obtained so that it can be used as a reference in determination of the occlusal plane and in arrangement of artificial teeth during laboratory work. Artificial teeth were arranged through a CAD program, and a gingival festooning was performed. The definitive dentures were printed by SLA-based 3D printer using a Food and Drug Administration (FDA)-approved liquid photocurable resin. The denture showed adequate retention, support and stability, and results were satisfied functionally and aesthetically.

Accuracy of 5-axis precision milling for guided surgical template (가이드 수술용 템플릿을 위한 5축 정밀가공공정의 정확성에 관한 연구)

  • Purpose: The template-guided implant surgery offers several advantages over the traditional approach. The purpose of this study was to evaluate the accuracy of coordinate synchronization procedure with 5-axis milling machine for surgical template fabrication by means of reverse engineering through universal CAD software. Materials and methods: The study was performed on ten edentulous models with imbedded gutta percha stoppings which were hidden under silicon gingival form. The platform for synchordination was formed on the bottom side of models and these casts were imaged in Cone beam CT. Vectors of stoppings were extracted and transferred to those of planned implant on virtual planning software. Depth of milling process was set to the level of one half of stoppings and the coordinate of the data was synchronized to the model image. Synchronization of milling coordinate was done by the conversion process for the platform for the synchordination located on the bottom of the model. The models were fixed on the synchordination plate of 5-axis milling machine and drilling was done as the planned vector and depth based on the synchronized data with twist drill of the same diameter as GP stopping. For the 3D rendering and image merging, the impression tray was set on the conbeam CT and pre- and post- CT acquiring was done with the model fixed on the impression body. The accuracy analysis was done with Solidworks (Dassault systems, Concord, USA) by measuring vector of stopping’s top and bottom centers of experimental model through merging and reverse engineering the planned and post-drilling CT image. Correlations among the parameters were tested by means of Pearson correlation coefficient and calculated with SPSS (release 14.0, SPSS Inc. Chicago, USA) ($\alpha$ = 0.05). Results: Due to the declination, GP remnant on upper half of stoppings was observed for every drilled bores. The deviation between planned image and drilled bore that was reverse engineered was 0.31 (0.15 - 0.42) mm at the entrance, 0.36 (0.24 - 0.51) mm at the apex, and angular deviation was 1.62 (0.54 - 2.27)$^{\circ}$. There was positive correlation between the deviation at the entrance and that at the apex (Pearson Correlation Coefficient = 0.904, P = .013). Conclusion: The coordinate synchronization 5-axis milling procedure has adequate accuracy for the production of the guided surgical template.
