  • 성대낭종은 후두미세수술의 발달로 보다 정확한 진단 및 치료가 가능하므로서 관심도가 증가하고 있다. 성대 낭종은 낭종의 내용물 및 점막상태에 따라 저류성 낭종과 유표피성 낭종으로 분류하고 있는데, 저류성 낭종은 점액 분비선의 폐쇄로 점액이 저류되어 발생하며 유표피 낭종은 선천성으로 상피하층에 파묻힌 상피세포의 잔여물이거나 혹은 파묻힌 상피세포 위에 외상(음성남용)으로 손상된 점막이 재생하는 과정에서 발생한다는 설이 있다. (중략)

Pulmonary Mucinous Cystic Tumor of Borderline Malignancy -A case of report- (폐의 저악성 점액성 낭종)

  • Pulmonary mucinous cystic tumor of borderline malignancy is very rare and distinguished from bronchogenic cyst or adenocarcinoma of bronchoalveolar type. We present the case of a 63-year-old woman with a right lower lobe mass, found by chest radiographs. The preoperative diagnosis was made as bronchoalveolar cancer by percutaneous needle aspiration of mass. Right lower lobectomy and lymph node dissections were performed. The lobectomy specimen contained variable sized multilocular cystic mucous masses, filled with mucus. Microscopically, the cystic masses are lined with tall columnar mucinous epithelium but some area contains focal cellular atypism and bronchoalveolar cancer like foci. This foci are lack of cellular atypism consistent with bronchoalveolar cancer cell. After lobectomy the patient has remained free from recurrence and distant metastasis for following 12 months period. Pulmonary mucinous cystic tumor of borderline malignancy appears to have a favorable prognosis and should be distinguished from other lung neoplasms.

  • Medial knee joint pain is a common problem in the field of orthopedics. In these patients, a high resolution ultrasound examination can reveal medial collateral ligament (MCL) bursitis, meniscal cyst, degeneration changes to the MCL and meniscal protrusion etc. Prolotherapy is effective in these patients. The author performed prolotherapy for MCL bursitis of the knee joint, and confirmed the disappearance of the bursitis using high resolution ultrasound.

  • We treated a 37-year-old male with traumatic hematoma in the suprapatellar bursa that had developed in the form of persistent swelling on suprapatellar area of left knee after blunt trauma. Though there were no obvious abnormal findings on plain roentgenographs, an isolated suprapatellar cystic lesion with fluid-fluid level on T2-weighted sagittal image of MRI was noted. We found the suprapatellar plica with complete septum and no synovitis in the knee joint proper by arthroscopy. We incised the plica and found leakage of blood-stained fluid from the suprapatellar bursa. There were no findings of pigmented villonodular synovitis or other tumorous lesions. At 6 months after surgery, the patient felt symptom-free and there was no recurrence.

  • Mucinous adenocarcinoma of the kidney is an extremely rare cystic malignant tumor with a poor prognosis that occurs in the pelvicalyceal system. Pre-opeartive diagnosis is very difficult because the tumor's clinical and imaging features are nonspecific. Here we report a case of primary mucinous adenocarcinoma arising from the horseshoe kidney in a 69-year-old male, focusing on CT findings. The tumor was a complex cystic mass with irregular wall thickening, multifocal calcifications, and septa and progressed to pseudomyxoma peritonei postoperatively.

  • Dentigerous cysts generally encompass the crown of an unerupted tooth. These cysts are usually solitary. They are the second most common odontogenic type of cysts following radicular cysts, and are frequently associated with impacted mandibular third molars or maxillary canines. Most multiple cysts found in the jaw are odontogenic keratocysts associated with the nevoid basal cell carcinoma syndrome, mucopolysaccharidoses and cleidocranial dysplasia. Although a single dentigerous cyst is well documented in the medical literature, including the prevalence, treatment and prognosis, multiple dentigerous cysts without any systemic symptoms is unusual. Furthermore, cases involving both the maxilla and mandible are especially rare. We present the case of an 11-year-old boy with nonsyndromic multiple dentigerous cysts associated with a mandibular second premolar and a maxillary canine. The treatment was conservative and included marsupialization and eruption guidance. Further follow up is planned to rule out additional problems and the possible identification of a syndrome.

  • A 66-year-old woman described a 6-month history of hoarseness after upper respiratory infection. She was a house wife and referred from an outside clinic under a diagnosis of vocal fold nodules. Strobovideolaryngoscopy revealed bilateral vocal fold lesions, and decreased mucosal wave of both vocal folds. She was brought to the operating room for microsuspension laryngoscopy. Under general anesthesia, dual intracordal cysts on left vocal fold were completely resected with microflap technique. The lesion on the right vocal fold turned out to be a reactive fibrous mass, which was also resected. Dual intracordal cysts were confirmed histopathologically. The one was an epidermoid cyst lined with squamous epithelium, and the other was a mucus retention cyst lined with cuboidal epithelium. Postoperative voice was acceptable by the patient and the mucosal vibration has much improved after the surgery.

  • Odontogenic myxoma is a rare tumor which occurs almost exclusively in the jaws, and seems to be of odontogenic origin. Clinically this tumor tends to appear in the second and third decades of life, and most of the lesions are located in the premolar - molar region. It is characterized grossly by mucoid or gelatinous gray-whitish tissue that replaces the cancellous bone and expands the cortex. It is invasive locally and has a high recurrence rate ranging from 10% to 33%. Radiographically, the appearance of this tumor is varied, but usually appears as a unilocular or multilocualr radioluscency of varying size. We experienced a rare case of odontogenic myxoma 12 years old patient related to mandibular bilateral impacted canines in the mandible, so we report this case with literature.

  • Primary retroperitoneal mucinous cystadenocarcinoma is a very rare malignancy. To date, 51 cases have been reported, including 3 in pregnant women. Herein, we report magnetic resonance findings of a 31-year-old Korean woman (15 weeks and 3 days pregnant) with primary retroperitoneal mucinous cystadenocarcinoma. On abdominal magnetic resonance imaging (MRI), a mass was identified in the retroperitoneal area with a nodular lesion showing heterogeneous signal intensity and focal wall thickening on T1- and T2-weighted images. Exploratory laparotomy and tumor excision were performed. Histological examination revealed primary retroperitoneal mucinous cystadenocarcinoma. The patient subsequently underwent total hysterectomy, bilateral salpingo-oophorectomy, and omentectomy for metastatic mucinous cystadenocarcinoma of both ovaries 15 months after her initial surgery.

A Case of Mucocele in the Left Frontal Sinus (전두동 점액낭종)

  • Authors was experienced a case of mucocele in the left frontal sinus. A 24-years-old Korean male soldier was admitted to C.A.F.G.H. on 16th May 1980, with chief complaints of dull headache, exophthalmos, visual disturbance and intermittent epistaxis on the left side. On physical examination, left turbinates and septum were revealed hyperemic middle turbinate with blood tinged spots and deviated slightly to right side, and felt round, smooth, rubbery painless swelling within the orbit at the left inner canthus. The left orbital contents was displaced laterally producing proptosis with diplopia. On the radiological examination, Caldwell and Water's view showed hazy density in medial side of left orbital and left frontal sinus. Tomography of orbit showed hazy increased mass density with rather sharply defined outer margin of left bony orbit probably due to compressive erosion. The case was treated with surgical removal of the Lynch frontal approach, so present this case with a brief review of the literature.
