Incidence and Factors Influencing Oral Mucositis in Patients with Hematopoietic Stem Cell Transplantation (조혈모세포이식 환자의 구강 점막염 발생실태와 영향요인)

  • Purpose: This study was done to examine the incidence of oral mucositis in hematopoietic stem cell transplantation patients and to identify factors influencing oral mucositis and patient outcomes according to severity. Methods: In this retrospective study, data were collected from electronic medical records of 222 patients who had received hematopoietic stem cell transplantation. Oral mucositis was evaluated using WHO's assessment scale. Data were analyzed using Chi-square test, Fisher exact test, Spearman's correlation, Ordinal logistic regression, ANOVA and Kruskal-Wallis test. Results: A total of 69.8% of the patients evaluated developed oral mucositis (grade II and over). As a results of ordinal regression, factors influencing oral mucositis severity were found to be diagnosis, type of transplantation, oxygen inhalation and the number of antiemetics administration before transplantation. The severity of oral mucositis was found to increase the days of hospitalization, days of TPN administration, days of using antibiotics and the number and dosage of analgesics. Conclusion: The results would help predict severity of oral mucositis in hematopoietic stem cell transplantation patients and suggest that provision of appropriate nursing assessment and oral care would improve patient outcomes.

  • Plasma cell mucositis is a very rare benign disease characterized by dense lymphoplasmacytic infiltration in the submucosa layer. It appears as a reddish ulcer on the mucous membrane or as a cobblestone or nodular mass on the affected mucosa. When it involves the pharynx or larynx, the patient presents with dysphagia, voice change and dyspnea. Clinically, it is important to differentiate with malignant diseases such as extramedullary plasmacytoma, amyloidosis and sarcodosis. Several cases of mucositis in the larynx have been reported in English literature, but none have been reported in Korea. We report a case of plasma cell mucositis in the larynx with a review of literature.

The Anti-inflammatory Effects of Golden Apple Snail (Pomacea canaliculata) in Reflux Esophagitis Model (왕우렁이 추출물의 역류성 식도염 억제 효과)

  • Reflux esophagitis (RE) is a common gastrointestinal disease observed at all ages, which seriously affects the quality of life. In this study, we investigated the anti-inflammatory effects of Pomacea canaliculata extract (PCE) using the experimental RE rat model. RE was induced by a surgical procedure. The rats were randomly divided into 4 groups: normal group, RE group, PCE group (RE treated with PCE, 100 mg/kg), positive control group (RE treated with ranitidine, 40 mg/kg). We performed the histological examination and measured the expression of tight junction complex and inflammatory mediators using western blot analysis. The phenotypes of RE were attenuated by PCE treatment. PCE administration significantly reduced esophageal mucosal damage and protected tight junction confirmed by claudin-5. Furthermore, PCE treatment reduced inflammatory reaction by inhibiting the expression of COX-2 and TNF-α. PCE treatment, also, reduced translocation of NF-κB into nuclear and IκB-α phosphorylation at the same time. Our findings indicate that PCE has the potential as a novel therapeutic agent to inhibit RE by protecting mucosal damage and regulating inflammatory reactions mediated by NF-κB signaling.

Protective Effect of Curcumin on Dextran Sulfate Sodium-Induced Ulcerative Colitis in Obese Mice (비만마우스에서 Dextran Sulfate Sodium 유도 궤양성 대장염에 대한 커큐민의 보호효과)

  • Ulcerative colitis is a disease that causes inflammation in the mucosal or submucosal layer of the colon. Previous studies have reported that obesity increases the prevalence of ulcerative colitis and aggravates the progression. This study was therefore undertaken to investigate whether curcumin inhibits the progression of ulcerative colitis caused by obesity. Mice were bred on a high-fat diet to induce obesity, and curcumin was administered with the high-fat diet to confirm the anti-inflammatory effect. To induce ulcerative colitis, dextran sulfate sodium (DSS) was administered orally, and clinical symptoms of colitis were subsequently observed. For histological evaluation of curcumin, the colon, liver and abdominal fat tissue samples were prepared and analyzed by hematoxylin and eosin (H&E) and Alcian blue-periodic acid-Schiff (PAS) staining. Our results confirm that consumption of curcumin resulted in decreasing the score of the disease activity index, and inhibited shortening of the colon length. In addition, inflammatory cell infiltration and mucosal damage were inhibited in the colon tissue of ulcerative colitis exacerbated by obesity. We further confirmed that exposure to curcumin significantly reduced the steatosis area of the liver and adipocytes of abdominal fat. In conclusion, we believe that curcumin can be applied as a therapeutic agent to treat ulcerative colitis, by inhibiting the progression of colitis caused by obesity.

  • 부식성 물질에 의한 양성 식도 협착 환자에서 식도 확장술이 일반적으로 사용되는 술식이나 협착부위가 잔존하여 연하곤란이 발생하므로 식도 재건술이 이용된다. 식도재건술은 대용 식도로 위관, 대장관, 소장관이 이용하여 광범위한 박리, 여러 부위의 절개, 문합부 대용 식도 위치에 따른 문제점 및 협착된 식도를 잔존시킴으로 식도암 발생가능성이 있으며 식도 재건술후 식도 기능의 문제점이 많다. 이에 시고 기능에 이상을 초래하지 않으면서 합병증이 적고 수술하기 쉬운 방법이 요구된다. 저자는 식도 근층만을 절개하고 점막하층을 박리한 후 협착부 점막만 절제하고 점막 단단 문합함으로 좋은 결과를 얻었기에 증례와 더불어 수술 방법을 소개하고자 한다.

후두 및 기관협착증의 유발인자로서의 위산 및 펩신의 역할에 대한 실험적 연구

  • 후두 및 기관지 협착증은 대개가 기도확보를 목적으로 후두 및 기관내에 삽관을 시행하였을 때 삽관튜브에 부착된 cuff내에 압력이 높게 되면 점막에 혈류장애가 초래되고 궤양등의 점막손상이 뒤따르며 여기에 2차감염이 초래되어 육아종과 반흔이 생기면서 결국은 협착이 초래되는 것으로 알려져 있다. 이와 같이 손상된 후두및 기관의 점막에 산도가 강하고 소화효소가 함유된 위액이 역류되어 접촉될 경우 협착이 더욱 조장되리라고 가정할 수 있다. 따라서 위액이 과연 후두 및 기관지 협착증에 관여 한 것인지를 실험적으로 규명해 보고자 하였다. 실험동물은 가토를 대상으로 하여 Urethan마취를 시행하고, 삽관한 다음 합성위액 pH 1.5, 3, 5 짜리 용액을 기관 내에 1시간정도 관류시킨 다음 후두 및 기관을 적출하여 점막상태의 변화를 현미경으로 관찰하였다. 그 결과 산도가 강하고 펩신이 첨가된 합성위액으로 관류한 군에서는 성모의 파괴점막의 궤양, 부종과 염증소견 등이 심하게 관찰되었다. 그러나 산도가 약할수록, 그리고 펩신이 첨가되지 않은 합성위액으로 관류한 군에서는 섬모 및 점막의 손상이나 염증 및 부종등의 소견이 경미하게 관찰되었다. 이러한 결과로 미루어 강한 산도의 위액이 후두 및 기관으로 역류될 경우 협착증의 발생에 깊이 관여하리라고 추측되었다.

Effects of a mixture of Citri Pericarpium and Scutellariae Radix on acute reflux esophagitis in rats (진피-황금 혼합물이 급성 역류성 식도염 흰쥐에 미치는 효과)

  • Purpose: Reflux esophagitis is a disease caused by the reflux of stomach contents and stomach acid etc. into the esophagus due to defect in the lower esophageal sphincter and is currently increasing worldwide. This study was conducted to evaluate the effect of a mixture of Citrus Reticulata and Scutellariae Radix (CS) extract on acute reflux esophagitis in rats. Methods: Rats were divided into five groups for examination: normal group (Normal, n = 8), water-treated acute reflux esophagitis rats (Control, n = 8), tocopherol 30 mg/kg body weight-treated acute reflux esophagitis rats (Toco, n = 8), CS 100 mg/kg body weight-treated acute reflux esophagitis rats (CS100, n = 8), CS 200 mg/kg body weight-treated acute reflux esophagitis rats (CS200, n = 8). The experimental groups were administrated of each treatment compounds and after 90 min, acute reflux esophagitis was induced through surgery. Rats were sacrificed 5 h after surgery. We measured the level of reactive oxygen species (ROS) in serum and analyzed the expression of nicotinamide adenine dinucleotide phosphate, inflammatory, and tight junction-related proteins by western blot in the esophageal tissues. Results: CS administration significantly protected the esophageal mucosal damage due to reflux esophagitis, and the level of ROS in the serum was significantly reduced with CS administration as compared to Control. In addition, CS administration significantly suppressed mitogen-activated protein kinase (MAPK or MAP kinase) and nuclear factor-kappa B (NF-κB) pathways and increased protein expressions of tight junction protein. Conclusion: These results suggest that the CS not only regulates the expression of inflammatory proteins by inhibiting oxidative stress, but also reduces damage to the esophageal mucosa by inhibiting the expression of tight junction proteins.

  • 흔히 여러 가지 형태로 나타나는 구강점막의 염증을 한데 일컬어서 구내염이라고 부르고 있다. 그러나 실지로는 이들 구내염의 원인이 많고 또한 복잡하며 때로는 원인을 잘 모르는 것이 있는가 하면 서로 다른 원인에 의해서 생긴 구내염도 구강 내에 들어온 여러 가지 세균에 의해서 이차적 감염을 일으켰을 때에는 같은 형태나 증상을 나타내는 경우가 있으며 바이러스나 진균에 의해서 또는 전신질환의 분증으로서도 발생되기 때문에 이들을 일률적으로 분류해서 이야기 하기는 어려우나 대체로 임상적인 면에서 다음과 같이 나누어 설명할 수가 있다.

수종 점막 추출액중 메치오닌엔케팔린 및 유사체의 분해 억제

  • 생리활성 펩타이드류의 경점막 수송을 검토하기 위하여 토끼의 비강, 직장, 질 또는 눈 점막 구출액에서의 메치오닌엔케팔린 (Met-Enk) 및 그 합성 유사체인 [D-알라$^2$]-메치오닌엔케팔린아미드 (YAGFM)의 효소적 분해를 억제하고자 수종 효소억제제를 검토하였다. 방법: 토끼의 질, 직장 및 비강 점막을 차례로 적출하여 신속히 Valia-Chien투과셀에 마운팅하고, 눈의 각막은 절취하여 따로 각막 투과셀에 마운팅한 다음 등장 인산염 완충액 3.5ml씩으로 8시간씩 3회 추출하여 점막측 및 장막측 추출액을 제조하였다. 추출 완료 직후 이들 추출액에 Met-Enk 또는 YAGFM 50$\mu\textrm{g}$/ml의 농도로 첨가하고 여러 효소억제제를 단독 또는 혼합하여 첨가한 조건에서 37$^{\circ}C$에서 60 rpm으로 24시간 동안 흔들면서 경시적으로 시료를 취하여 잔존 펩타이드의 양을 HPLC법으로 정량하여 속도론적으로 비교 검토하였다. 이 연구에 사용한 효소억제제로는 아미노펩티다제의 억제제인 amastatin (AM), bestatin (BS). 엔케팔리나제 A의 억제제로 알려진 thiophan (TP), 엔케팔리나제 B의 억제작용이 있는 것으로 밝혀진 thimerosal (TM), metalloenzyme의 억제제인 에데트산나트륨 (EDTA) 등을 검토하였고 또 $\beta$-시클로덱스트린 유도체인 디메칠-$\beta$-시클로덱스트린과 2-히드록시프로필-$\beta$-시클로덱스트린이 펩타이드의 분해억제효과도 함께 검토하였다.
