Risk Factors for the Prevalence of Periodontal Diseases among Adult Workers (성인 근로자의 치주질환 유병 관련 위험요인)

  • This study examined the demographic and oral behavioral risk factors for periodontal disease in adult workers. The research subjects and method were conducted targeting 1,650 Korean adult workers aged 20-64 utilizing the data from the fifth Korea National Health and Nutrition Examination Survey (KNHANES V-3). As a result of this study, the periodontal tissue patients showed the risk factors for gender, age, income level, educational level, systemic disease, sleep hours, oral health level, biting trouble, and chewing difficulty. In terms of the greatest influence, the workers with problems with biting and chewing had a more than 3.5 times higher likelihood of being affected by periodontal disease. The findings of this study show that there are a variety of risk factors for periodontal diseases in Korean workers. To reduce the prevalence rate of periodontal disease, regular checkups including dental checkups and sufficient sleeping will be necessary, and the level of stress should be decreased. In addition, the government should take the required measures to remove the socioeconomic inequality, such as income gap or educational divide.

Radiotherapy Results of Midline Malignant Reticulosis (MMR) (중심선 악성 세망증의 방사선 치료 결과)

  • Purpose : This study was performed to evaluate survival, failure patterns, and prognostic factors of MMR patients after radiation therapy. We also discussed the need for chemotherapy. Materials and Methods : A retrospective analysis was done for 23 patients with MMR who were treated with radiation therapy from June 1985 to November 1992. There were 19 male and 4 female patients, The patients' age ranged from 17 to 71 years (median 39 years). Systemic symptoms including fever, weight loss, or malaise were found in $30\%$ of the patients. The nasal cavity was most frequently involved No patients had nodal involvement at diagnosis. There were 2 patients with distant metastasis at presentation. Radiation therapy was delivered five times a week, 1.8 Gy daily, total $45\~54$ Gy (median 50.4 Gy) using 6 MV X-ray No patients received chemotherapy as initial treatment. Results : Overall 5-year and 10-year survival rates were $52.4\%$ and $44.1\%$, respectively. Seventy Percent(12/17) of the patients achieved complete response to radiotherapy, and $29.4\%$ (5/17) achieved partial response. The Patients with complete response showed a better 5-year survival rate than those with partial response ($66.9\%$ vs. $20\%$. p=0.004). Symptom duration before diagnosis, the presence of systemic symptom, and the number of primary sites had no influence on survival. The patterns of failure were as follows: local failure(1), failure in adjacent site(1), local and distant failure(1) distant metastasis(2). and conversion to malignant lymphoma(1). We could not find factors associated with the patterns of failure. Conclusion : The most important facto associated with survival was the response to radiotherapy. Seventeen percent of the patients had distant metastasis, and the salvage after distant metastasis was not successful. However. about $50\%$ of the patients could achieve long-term survival with local radiation therapy alone. Therefore, chemotherapy of MMR should be done after a prospective randomized study for the factors associated with distant metastasis.

Clinical Features and Correlation With Congenital Missing Teeth of Delayed First Permanent Molar (제1대구치 지연 발육의 임상 양상 및 선천성 결손치와의 연관성)

  • Delayed eruption of the first molar, without a generalized or localized cause, is usually associated with delayed development of the affected tooth. The aim of this study was to investigate the clinical features of the first permanent molar showing delayed development and eruption, and its association with developmental anomalies of other teeth. Panoramic radiographs of 40 healthy children showing delayed development and eruption of first permanent molars were analyzed. The clinical features of affected first molars and developmental anomalies of other teeth (except third molars) were evaluated. Delayed first molars were more frequent in the maxilla. The incidence of bilateral delayed development of first molars was greater than that of unilateral cases in female patients. In contrast, male patients showed unilateral delayed development of the first molar more frequently. A higher incidence of congenitally missing teeth was observed in patients with delayed first molar. In each case, delayed development or congenital absence was observed in the second molar adjacent to the delayed first molar. Overall, delayed first molar seems to be associated with congenital absence of additional teeth. Understanding the developmental mechanisms of this phenomenon requires further studies.

Neighborhood Networks, Identity as a Neighborhood Member, and Volunteering (지역연결망 및 지역성원으로서의 정체성이 자원봉사 참여에 미치는 영향)

  • Volunteering has been defined as a form of altruistic helping behavior directed at improving other's welfare. Volunteering is, however, also identified as a type of collective action for community welfare. In this regard, this study tests whether neighborhood member's network and collective identity are more important determinants to explain participation in volunteer work than altruistic or normative motivation. This study estimates a model in which volunteering is determined by empathy, normative beliefs, neighborhood networks(friendship, contacts, and integration), and identity as a neighborhood member. This study shows that empathy, normative beliefs, and collective identity as a neighborhood member have significant impacts on participation in volunteer work. In addition, this study reveals that neighborhood member's network has an indirect impact on volunteering through identity as a neighborhood member. These results suggest that neighbor-hood community member's ties and collective identity are important sources for community welfare and collective volunteer work.

  • Xerostomia is subjective feeling of dry mouth, a symptom that may or may not be accompanied by hyposalivation, an objective decrease in salivary flow. There are many causes induced xerostomia like drugs, salivary gland diseases, radiation therapy to the head and neck region, Sjogren syndrome, emotional stress etc. Insufficient salivary flow creates complications with oral candidiasis, dental caries, periodontitis, halitosis, dysgeusia. So finally, these complications lead to an overall decline in quality of life. Managements of xerostomia are eliminating or alterating the etiologic factors, relieving symptoms, preventing or correcting the consequences of salivary dysfunction, treating underlying disease and stimulating salivation. One of the salivation stimulation agents studied to treat xerostomia was the pilocarpine muscarinic agonist. Pilocarpine is one of salivation stimulants, a parasympathomimetic drug and non-selective muscarinic receptor agonist. Systemic pilocarpine has been used to stimulate salivary secretion. But systemic administration of pilocarpine has limitations such as increased risk of side effects and contraindications. Side effects of systemic pilocarpine administration are sweating, urinary and gastrointestinal disturbance, risk of cardiovascular and pulmonary disorders. This drug must be used carefully by patients with controlled asthma, chronic bronchitis, pulmonary or cardiac disease. Patient with acute asthma, narrow angle glaucoma, iritis should not use pilocarpine. Like this, systemic pilocarpine has many limitations. So, many investigators also have looked at the effectiveness of topical pilocarpine. Here we present patients with xerostomia which was relieved by pilocarpine mouthwash.

Related Factors to Visual Display Terminal Syndrome in Employees of A General Hospital in one Metropolitan City (한 광역시 종합병원 근로자의 VDT 증후군 자각증상에 대한 관련요인 연구)

  • The existing studies of VDT syndrome have been researched only about for nurse groups without radiological technologists. The target of this study was workers who perform VDT task in general hospital placed in Daejun City such as insurance judgement, patient affairs, medical record management, medical computing service, hospital administration, and radiological sections. This study was conducted at October 15, 2009 to November 2, 2009 using structured questionnaire. Results of group A (age 20-29, less than 5 years experience at clinical site, normal staff, and unmarried group) showed significant skin and psychological conditions among recognition symptoms of VDT syndrome. Group B (age 30-39, 6-10 years experience at clinical site, managing staff, and married group) tends to be similar at orbital, whole body and muscles' frame response among recognition symptoms of VDT syndrome. Results of "Multiple Linear Regression" with dependent variable that is scores of recognition symptoms of VDT syndrome are as follows. The factors which influences for orbital-related problem is gender, health cares and obstacles during daily life. The factors which influences for whole body-related is exercise, stress and recognition for health condition and the factors which influences muscles' frame-related is 6-10 years experience at clinical site, human relationship, health cares and obstacles during daily life. These are proved to be similar by statistic analysis. There is small difference at recognition symptom of VDT syndrome between the radiological technologist and other groups in general hospital. Yet, as the working circumstance of radiological technologist gradually changes to VDT environment, the recognition symptom of VDT syndrome became conscious. This can be prevented by regular job rotation, which will escape the continuous repetition of working pattern.

  • The purpose of this study is to analyze the scale of health risk behaviors among adolescents in Daegu area. The purpose of this study was to develop a health risk behavior measurement item for the adolescent adolescents in Korea and to analyze the validity (CVR) of the existing THI measurement items and then to quantify the health risk behavior Were compared and analyzed. As a result of the analysis, the reliability of the health risk behavior scale of adolescents developed through the factor analysis was verified to derive measurement items such as general condition, respiratory, circulatory, digestive, mental health and other items. The reliability and validity of the developed health risk behavior scale were found to be fairly good. Through these studies, it will be possible to develop a preventive program that can reduce the risk of health related adolescents' youth, and to devise a strategy that can provide basic data for policy formulation.

Fibromyalgia from the Psychiatric Perspective (정신과적 관점에서의 섬유근통)

  • Fibromyalgia is a disorder characterized by the core symptom of chronic widespread pain, along with fatigue, sleep disturbances, mood changes, and cognitive difficulties. The etiology of fibromyalgia involves a combination of biological factors, such as genetic vulnerability, alterations in pain processing and stress response system ; psychological factors, such as anxiety, depression, anger, and perceived stress ; environmental factors, such as infections, febrile diseases, and trauma. Central sensitization, which is amplified in the process of sensory stimulation, has been emphasized as a key etiological factor, as supported by enhanced wind-up, delayed aftersensation, decreased nociceptive flexion reflex threshold and functional imaging studies. Several guidelines recommend that a multimodal approach be used to treat fibromyalgia, including both pharmacological and non-pharmacological treatments, tailored to each individual, and that clinicians should provide an intellectual framework through sufficient education and emphasis on the importance of self-management. The prevalence of mood disorders, anxiety disorders, and other psychiatric problems is 7-9 times higher in patients with fibromyalgia than in the general population ; moreover, the association between fibromyalgia and certain psychopathologies or sleep problems has also been suggested. Since psychiatric problems, with shared vulnerabilities and risk factors, interact with fibromyalgia bidirectionally and also affect the disease course, an integrated management approach is needed to determine the risk of comorbidities.

Demand for Elderly Food Development: Relation to Oral and Overall Health -Focused on the Elderly Who Are Using Senior Welfare Centers in Seoul- (구강 및 전신 건강 기반 고령자용 식품 개발을 위한 요구도 조사 -서울 일부 지역 복지관 이용 노인을 대상으로-)

  • Oral health is a critical factor for diagnosing overall health, as poor oral health has a negative impact on food intake and results in deterioration of nutritional status. The purpose of this study was to research the needs for food developments for the elderly based on their oral and overall health status. Oral status was significantly associated with self-reported health status, and dry mouth (lack of saliva) especially showed a significant relationship with negative self-perception of both oral and overall health. For both elderly men and women, fish and leafy vegetables were the most preferred side dishes. Bread, rice cake, and beverage-type foods were also preferred for snack development. Disease status of the elderly was significantly related with their preference for food development. For instance, those with diabetes were more likely to ask for anti-diabetic diets. The most important factor for the elderly in choosing food was nutritional balance, and less than 3,000 won was cited as an affordable price. The results could be utilized as basic data to develop foods for the elderly.
