The Detection of Intracranial Calcification by MR : Experimental Model (실험적 모델을 이용한 자기공명영상에서 석회화의 인지)

  • Purporse : It is known that detection of calcification by MRI is difficulty in intracranial calcified lesions, but author tried to evaluate the signal intensity image of calcification by MR with experimental model. Subjects & Methods : Author analyzed and compared with values of calcium carbonate and hydroxyapatite phantoms by each concentration (10, 20, 30, 40, 50%) and size(1-10mm), measured ROI attenuating from CT and MRI(TlWI & T2WI). Results : The high concentration of calcium carbonate is, the lower the signal intensity of calcium carbonate phantom is both T1 & T2WI. For concentration of Hydroxyapatite of up to 30% by weight the signal intensity on standard T1 weighted images increased but subsequently decreased. Hyperintensity does not preclude calcification as a cause of the signal alteration-an observation that all radiologists interpreting MR images need to be aware of. Conclusion: The signal intensity of intracranial calcification is various on MR imaging in concerning with components, concentration, & size of calcification, and especially high signal intensity of intracranial calcification noted differencial diagnosis.

Levoatriocardinal Vein Combined with Pulmonary Venous Varix Mimicking Arteriovenous Malformations: A Case Report (동정맥기형으로 오인되었던 폐정맥정맥류를 동반한 Levoatriocardinal 정맥: 증례 보고)

  • The levoatriocardinal vein is an uncommon pulmonary venous abnormality that connects the left atrium or pulmonary vein with the systemic vein. It is distinct from partial anomalous pulmonary venous return in that the former forms a connection with the left atrium through the normal pulmonary vein whereas the latter involves pulmonary venous drainage to the systemic vein. Herein, we describe a case of the levoatriocardinal vein initially misdiagnosed as a pulmonary arteriovenous malformation using chest radiography and chest CT. The levoatriocardinal vein combined with pulmonary venous varix was confirmed using pulmonary angiography. To the best of our knowledge, this unusual coexistence of the levoatriocardinal vein and pulmonary venous varix has not been reported in English literature.

  • The medial longitudinal fasciculus (MLF) is myelinated composite tract, lying near the midline, ventral to periaqueductal grey matter that plays a key role in coordinating eye movements. A lesion of the MLF results in an ipsilateral adduction deficit and a contralateral abducting nystagmus, referred to as an internuclear ophthalmoparesis. The blended tract with adjacent white matter in pons and midbrain is indistinguishable on brain imaging such as CT and MRI. Until now, to the best of our knowledge, MLF is not delineated on in vivo MRI. We present a case showing the whole connecting courses of MLF lesion on MRI in a patient with inflammatory demyelinating disorder.

Consideration of maxillary sinus bone thickness when installing miniscrews (미니스크류 식립 시 상악동의 골두께에 대한 고려)

  • Objective: Miniscrews are widely used in orthodontic treatment for the purpose of anchorage control. Maximum anchorage can be acquired by the use of miniscrews. Maxillary miniscrew has many clinical advantage for orthodontic treatment. Maxillary sinus, tooth root can be an obstacle for maxillary miniscrew installation. The purpose of this study was to find the safest area and direction of miniscrew insertion in consideration of the maxillary sinus. Methods: The maxillary sinus area of 40 patients (20 male, 20 female) was measured using 3D computed tomography and 3D reconstruction program. Results: The maxillary sinus floor was located most inferiorly between the 1st molar and 2nd molar and located most superiorly between the 1st premolar and 2nd premolar. Buccal bone thickness from the maxillary sinus is significantly thicker between the 1st molar and 2nd molar and significantly thinner between the 1st premolar and 2nd premolar. The area between the 1st premolar and 2nd premolar has a significantly longer vertical distance from CEJ to sinus in consideration of buccal bone thickness. Conclusions: Considering maxillary bone thickness, the posterior area has advantages over the anterior area for installing miniscrews safely and preventing perforation.

Comparison of Sizes of Anatomical Structures according to Scan Position Changes in Patients with Interstitial Lung Disease Using High-Resolution Thoracic CT (고해상도 흉부 전산화단층촬영을 이용한 간질성 폐질환을 가진 환자의 자세에 따른 해부학적 구조물 크기 비교)

  • High-Resolution thoracic CT (HRCT) is a scanning protocol in which thin slice thickness and sharpness algorithm are utilized to enhance image resolution for diagnosis and assessment of interstitial lung disease (ILD). This examination is sometimes performed in both supine and prone position to improve sensitivity to early changes of these conditions. Anatomical structures (the size of lung field and heart and descending aorta) of 150 patients who underwent HRCT were retrospectively compared. HRCT had been conducted in two positions (supine and prone). Data were divided into five groups according to patient body weights (from 40 to more than 80kg, 10kg intervals, 60 patients/each group). Quantitative analysis was utilized in Image J program. In the supine position defined as the control group, the average values of lung fields and heart size and aorta were compared with the prone position defined as the experimental group. The size of the lungs was found to be higher in the supine position, and it was confirmed that there was a statistically significant difference in patients over 70 kg (p<0.05). In addition, both sizes of the heart and descending aorta were larger in prone position, but in the case of the heart, there was no correlation with the presence or absence of ILD disease (p>0.05). Also, the area of prone in the descending aorta was higher than supine position, but there was no statistically significant difference between supine and prone position (p>0.05). In conclusion, when the severity of ILD disease was severe, there was no statistically significant difference in the area difference between supine and prone position, so it is considered that it will be helpful in diagnostic decision.

A Study on Misdiagnosis Rates of Ejection Fraction Associated with Cardiac Computed Tomography: Suggestions and Correction for Improvement (심장 전산화단층촬영을 이용한 박출계수 산출 시 박출계수의 보정을 통한 오진율 개선에 관한 연구)

  • The aim of this study was to compare the cardiac CT and cardiac MRI in calculating and correcting the left ventricle ejection fraction by analyzing the physical and temporal resolution for reducing the misdiagnosis rate. One hundred thirty-eight patients with aortic value regurgitation who underwent both cardiac CT and cardiac MRI were analyzed. Left ventricle ejection fractions calculated from each exam were corrected based on the physical and temporal resolution differences and the reliability test evaluated whether the misdiagnosis rate of cardiac CT was improved after the correction. As a result of the study, the misdiagnosis rate of cardiac CT ejection fraction before correcting the difference in physical and temporal resolution was 38.4%(53 persons). In addition, it can be seen that the corrected cardiac CT ejection fraction confirmed in the Bland-Altman plot was highly consistent with the ejection fraction of cardiac MRI. In conclusion, as the cardiac CT is less well suited for measuring ejection fraction, physical characteristics and the time resolution correction using cardiac MRI is needed and the misdiagnosis rate after correction decreased to 14.5%(20 persons). Therefore, this study appears more appropriate for better prediction of ejection fraction and clinical utility.

Evaluation of the Efficiency of Use of Fixation Instruments in Computed Tomography-Guided Biopsy of Lung Lesions (전산화단층촬영 유도하 폐 병소의 생검시 고정기구 사용의 효용성 평가)

  • Minimizing patient movement during CT-guided lung biopsy is an important factor in the procedure. To minimize movement, a vacuum cushion was used to evaluate its effectiveness. The subjects of this study were 116 patients aged 40 years or older who had good coordination with postural fixation and breathing control. Posture measurements were performed in the supine position, prone position, oblique position, and lateral position according to each position of the lung lesion biopsy lesion. Measurement positions were measured in the anterior, posterior, right, and left positions based on the anatomical posture. In the prone position, the mean difference between the non-use and the use of the posterior was 1.7905, and t=2.913 (p<0.01), and the mean difference between the non-use/use was statistically significant. The difference between the unused and used averages of left was 2.4105, and the difference between the left averages was also significant with t=3.684 (p<0.01). The difference between the unused and used averages of the right was 2.3263, with t=3.791 (p<0.01). The mean difference between unused and used is statistically significant. As a result of statistical analysis, the biopsy of the lung lesion using a fixation device showed less movement in all postures. It is considered that it is meaningful in that it is possible to conduct a more accurate biopsy procedure and minimize the patient's posture movement by using a fixation device during the CT-guided biopsy of the lung lesion.

Comparison of using CBCT with CT Simulator for Radiation dose of Treatment Planning (CBCT와 Simulation CT를 이용한 치료계획의 선량비교)

  • The use of cone-beam computed tomography(CBCT) has been proposed for guiding the delivery of radiation therapy. A kilovoltage imaging system capable of radiography, fluoroscopy, and cone-beam computed tomography(CT) has been integrated with a medical linear accelerator. A standard clinical linear accelerator, operating in arc therapy mode, and an amorphous-silicon (a-Si) with an on-board electronic portal imager can be used to treat palliative patient and verify the patient's position prior to treatment. On-board CBCT images are used to generate patient geometric models to assist patient setup. The image data can also, potentially, be used for dose reconstruction in combination with the fluence maps from treatment plan. In this study, the accuracy of Hounsfield Units of CBCT images as well as the accuracy of dose calculations based on CBCT images of a phantom and compared the results with those of using CT simulator images. Phantom and patient studies were carried out to evaluate the achievable accuracy in using CBCT and CT stimulator for dose calculation. Relative electron density as a function of HU was obtained for both planning CT stimulator and CBCT using a Catphan-600 (The Phantom Laboratory, USA) calibration phantom. A clinical treatment planning system was employed for CT stimulator and CBCT based dose calculations and subsequent comparisons. The dosimetric consequence as the result of HU variation in CBCT was evaluated by comparing MU/cCy. The differences were about 2.7% (3-4MU/100cGy) in phantom and 2.5% (1-3MU/100cGy) in patients. The difference in HU values in Catphan was small. However, the magnitude of scatter and artifacts in CBCT images are affected by limitation of detector's FOV and patient's involuntary motions. CBCT images included scatters and artifacts due to In addition to guide the patient setup process, CBCT data acquired prior to the treatment be used to recalculate or verify the treatment plan based on the patient anatomy of the treatment area. And the CBCT has potential to become a very useful tool for on-line ART.)

Rate of Transformation and Normal Range about Cardiac Size and Cardiothoracic Ratio According to Patient Position and Age at Chest Radiography of Korean Adult Man (한국인 성인 남성의 흉부 방사선영상에서 자세와 연령에 따른 심장 크기 및 심흉비의 정상 범위와 변환율)

  • Purpose of this study is present the normal range of cardiac size and cardiothoracic ratio according to patient position(chest PA and AP) and age of Korean adult male on digital chest X - ray, And to propose a mutually compatible conversion rate. 1,024 males were eligible for this study, among 1,300 normal chest patients who underwent chest PA and low-dose CT examinations on the same day at the 'S' Hospital Health Examination Center in Seoul From January to December 2014. CS and CTR were measured by Danzer (1919). The mean difference between CS and CTR was statistically significant (p<0.01) in Chest PA (CS 135.48 mm, CTR 43.99%) and Chest AP image (CS 155.96 mm, CTR 51.75%). There was no statistically significant difference between left and right heart in chest PA and AP images (p>0.05). CS showed statistically significant difference between Chest PA (p>0. 05) and Chest AP (p<0.05). The thorax size and CTR were statistically significant (p<0.01) in both age and chest PA and AP. Result of this study, On Chest AP image CS was magnified 15%, CTR was magnified 17% compare with Chest PA image. CS and CTR were about 10% difference by changing posture at all ages.

Diagnosis of Location and Size of Lesions using Chest X-ray Image (X-선 영상을 이용한 암의 위치 및 크기 진단)

  • X-ray general radiography is the simplest and most important one to get a lot of information. Nevertheless, current x-ray general radiography does not observation in-depth observation. Information about the anatomy of the human body and changes in disease in x-ray general radiography can be obtained but it is difficult to determine the size and shape of the actual lesion due to the disadvantage of expanding the image. In this study, PA and LAT images were acquired and cancer magnification was calculated in the images by measuring the distance of cancer samples. By adjusting the magnification the actual cancer length and thickness were measured and compared with the CT image and the actual cancer sample size. After the PA and LAT images of the inserted 6.0 mm cancer sample were obtained and the magnification was corrected, the length was 5.9 mm and the thickness was 6.1 mm. This value was measured similarly to the actual. The problem of obtaining the magnification that needs to know the actual length from the detector to the cancer sample was secured by obtaining the magnification through PA and LAT images and it is possible to accurately measure the cancer sample size. X-ray general radiography may provide useful information in situations where CT imaging is difficult.
