Morphological Analysis of the Sinus Lateral Wall using Computed Tomography (전산화단층촬영법을 이용한 상악동 측벽의 형태학적 분석)

  • The purpose of thise study was to measure the thickness of the sinus lateral wall using computed tomography (CT), and to find the most suitable vertical position for lateral window opening prior to sinus elevation. Thirty patients requiring sinus elevation had CT images taken with Philips Brilliance iCT. The thickness of the sinus lateral wall was measured according to its vertical position against the sinus inferior border, and its mean was calculated through three repeated measurements. When measured 2 mm above the sinus inferior border (SIB+2), the thickness of the sinus lateral wall was observed to be more than 2 mm. When measured 3 mm above the sinus inferior border (SIB +3), the sinus lateral wall was less than 2 mm in thickness. It is recommended that the lateral wall window be made 3 mm above the sinus inferior border when performing sinus elevation using the lateral approach.

A Study on the Appropriate Reconstruction of the CBCT Images of Mandibular Canals (CBCT 영상에서 구치부의 하악관 형태에 따른 재구성 방법 연구)

  • In dental radiography, panoramic views cause distortion and thus may bring about inaccurate results in the process of quantitative analysis. In this connection, there has recently been an increasing use of cone beam computed tomography (CBCT) that is known to provide high-resolution images and positional information. In this study, a dental computed tomography unit, 'DCT-90-P IMPLAGRAPHY (Vatech, Korea)', was applied to 20 patients for 24 seconds respectively, with a tube voltage of 85kVp and a tube current of 7mA. The data of CBCT were three-dimensionally reconstructed by use of a computer program, and were histomorphometrically analyzed. The results showed that the diameter of mandibular canal is less distorted at a certain inclination of the mandibular body. The image tends to seem more distended in proportion to the distance between the subject and film. Also, the image tends to be affected according as it is out of focus. In conclusion, it requires that the image should be reconstructed in light of anatomic position and structure.

Characterization of Deep Learning-Based and Hybrid Iterative Reconstruction for Image Quality Optimization at Computer Tomography Angiography (전산화단층촬영조영술에서 화질 최적화를 위한 딥러닝 기반 및 하이브리드 반복 재구성의 특성분석)

  • For optimal image quality of computer tomography angiography (CTA), different iodine concentrations and scan parameters were applied to quantitatively evaluate the image quality characteristics of filtered back projection (FBP), hybrid-iterative reconstruction (hybrid-IR), and deep learning reconstruction (DLR). A 320-row-detector CT scanner scanned a phantom with various iodine concentrations (1.2, 2.9, 4.9, 6.9, 10.4, 14.3, 18.4, and 25.9 mg/mL) located at the edge of a cylindrical water phantom with a diameter of 19 cm. Data obtained using each reconstruction technique was analyzed through noise, coefficient of variation (COV), and root mean square error (RMSE). As the iodine concentration increased, the CT number value increased, but the noise change did not show any special characteristics. COV decreased with increasing iodine concentration for FBP, adaptive iterative dose reduction (AIDR) 3D, and advanced intelligent clear-IQ engine (AiCE) at various tube voltages and tube currents. In addition, when the iodine concentration was low, there was a slight difference in COV between the reconstitution techniques, but there was little difference as the iodine concentration increased. AiCE showed the characteristic that RMSE decreased as the iodine concentration increased but rather increased after a specific concentration (4.9 mg/mL). Therefore, the user will have to consider the characteristics of scan parameters such as tube current and tube voltage as well as iodine concentration according to the reconstruction technique for optimal CTA image acquisition.

Undifferentiated Pleomorphic Sarcoma of the Thoracic Aorta Presenting with Ruptured Saccular Aneurysm: A Case Report (소낭성 동맥류 파열로 발현된 흉부 대동맥에서 기원한 미분화성 다형성 육종: 증례 보고)

  • Cases of undifferentiated pleomorphic sarcoma of the thoracic aorta are rare, and usually present with embolic events, renovascular hypertension, or back pain. Mural-based undifferentiated pleomorphic sarcomas that present as ruptured saccular aneurysms are extremely rare and are difficult to differentiate from mycotic aneurysms or penetrating atherosclerotic ulcers. Herein, we report a case of histopathologically proven undifferentiated pleomorphic sarcoma arising from the wall of the descending thoracic aorta that manifested as a mass after thoracic endovascular aortic repair for the treatment of a ruptured saccular aneurysm. We present findings obtained by CT and PET to provide helpful information for the accurate diagnosis and appropriate treatment of future cases.

CT and MRI image fusion reproducibility and dose assessment on Treatment planning system (치료계획시스템에서 전산화단층촬영과 자기공명영상의 영상융합 재현성 및 선량평가)

  • Objectives: The aim of this study is to evaluate the reproducibility and usefulness of the images through the fusion of CT(Computed tomography) and MRI(Magnetic resonance imaging) using a self-manufactured phantom. We will also compare and analyze the target dose from acquired images. Materials and Methods: Using a self-manufactured phantom, CT images and MRI images are acquired by 1.5T and 3.0T of different magnetic fields. The reproducibility of the size and volume of the small holes present in the phantom is compared through the image from CT and 1.5T and 3.0T MRI, and dose changes are compared and analyzed on any target. Results: 13 small hole diameters were a maximum 31 mm and a minimum 27.54 mm in the CT scan and the were measured within an average of 29.28 mm 1 % compared to actual size. 1.5 T MRI images showed a maximum 31.65 mm and a minimum 24.3 mm, the average is 28.8 mm, which is within 1 %. 3.0T MRI images showed a maximum 30.2 mm and a minimum 27.92 mm, the average is 29.41 mm, which is within 1.3 %. The dose changes in the target were 95.9-102.1 % in CT images, 93.1-101.4 % in CT-1.5T MRI fusion images, and 96-102 % in CT-3.0T MRI fusion images. Conclusion: CT and MRI are applied with different algorithms for image acquisition. Also, since the organs of the human body have different densities, image distortion may occur during image acquisition. Because these inaccurate images description affects the volume range and dose of the target, accurate volume and location of the target can prevent unnecessary doses from being exposed and errors in treatment planning. Therefore, it should be applied to the treatment plan by taking advantage of the image display algorithm possessed by CT and MRI.

Digital approach integrating 3D facial scan and a virtual mockup for esthetic restorative treatment: A case report (심미보철 수복을 위한 3차원 안면스캔과 가상 보철물 시각화를 이용한 디지털 치료 증례)

  • This clinical case report describes the digital workflow that combines a face scan, cone beam computed tomography and an intraoral scan to visualize the outcome of prosthodontic treatment in the anterior region. This approach improves communication between clinic, laboratory and patients. A patient with healthy general condition came for a restorative treatment to treat post-traumatic central incisors of maxilla. A virtual patient replica was made by incorporating a face scan, cone beam computed tomography and an intraoral scan. Design mockup of definitive restorations was shown to the patient and modified according to the patient's desire. This digital workflow facilitates the fabrication of optimal esthetic restorations, and enhances the predictability of outcome of restorations.

  • 단일광자방출전산화 단층촬영술을 이용한 영상정보를 효과적으로 얻기 위하여 고려되어야 할 사항으로는 1) 조준기(collimator)의 선택, 2) 기질(matrix)의 크기, 3) 회전각의 수 (number of angles), 4) 360도 또는 180도 획득(acquisition), 5) continuous 또는 step& shoot, 6) 원형 또는 비원형회전 등이 있다. 저자들은 비원형회전으로 검체와 검출기 사이의 거리를 단축시킴으로써 직선성, 균일성, 대조도, 해상력에 미치는 영향을 알아보기 위하여 원형회전 방법과 비교하여 다음과 같은 결과를 얻었다. (1) 비원형회전을 하여도 균일성(uniformity)과 직선성(linearlity)을 유지한다. (2) 균일성, 대조도(contrast), 해상력(resolution)들이 비원형 회전을 한 경우에 보다 더 개선되었다. (3) 영상 획득시간은 비원형회전인 경우에 더 소요되었다. (매스캔 당 10분) 따라서 검사자는 영상 화질의 개선효과와 상반되는 보정(calibration)과 설치(set-up)에 소요되는 시간(매스캔당 10분이상)을 비교하여 자료획득(data acquisition) 회전방법을 선택하여야한다.

  • Current advances in CT techniques allow thorough evaluation of the beating heart. The strengths of cardiac CT relative to echocardiography and magnetic resonance imaging are its high availability in most institutions, rapid production of high-quality images, and outstanding delineation of the anatomy. For many normal variants and pathologic conditions, such as thrombi, masses, and congenital abnormalities of the left atrium, CT findings are sufficient to make a presumptive diagnosis. Assessments of the left atrium and left atrial appendage are particularly important for the management of atrial fibrillation, as various catheter-based procedures are aimed at the mechanical and electrical isolation of these structures. CT offers information crucial to a successful catheter-based procedure or surgery. Therefore, a comprehensive review of the geometry (shape, size, and relative position), along with various CT imaging features of pathologic states, should be provided in radiology reports to be of clinical value.

The Usefulness of Cone Beam Computed Tomography in Diagnosis of Temporomandibular Joint Osteoarthritis (측두하악관절 골관절염 환자의 진단에서 Cone Beam 전산화 단층촬영의 유용성)

  • This study is designed to assess Relationship between clinical diagnosis of Temporomandibular Joint Disorder and diagnostic finding of Cone Beam Computed Tomography(CBCT) The author performed clinical examination for TMD Patients who visited Orofacial pain clinic, Jin-ju ooo Dental office. CBCT(Cone beam computed tomography) was taken for 190 joints in 95 subjects. A Oral medicine and Oral radiologist evaluated CBCT each other. then we compared with that result, Condyle bony changes were classified by no bone change, flattening, erosion and osteophyte. The obtained results were as follow: 1. The Kappa index of the diagnosis between oral medicine and oral radiogist were high, the index of diagnosis by degenerative joint disease were more higher. 2. The Kappa index of panoramic view and CBCT was low, more condylar bone chages were observed by CBCT diagnosis 3. Condylar bone changes of the 54.2% of non-DJD group clinicaly was observed by CBCT diagnosis and no bone changes of the 15.3% of DJD group.was observed by CBCT 4. TMJ pain was associated with erosion of condyle bone change of TMJ. Crepitation and longest duration of TMD were associated with osteophytic bone change.

Comparison of 2D and 3D Brachytherapy Planning for Cervical Cancer (자궁경부암 근접방사선치료 시 2차원, 3차원 치료계획 비교평가)

  • To evaluate the usefulness of 3-dimensional brachytherapy(BT) planning technique based on CT in cervical cancer. Patients with cervical cancer underwent 2-D BT treatment planning and then CT scan with HDR intracavitary applicators in place with same positions. Dose was prescribed to Point A with 5Gy per fraction on 2-D BT planning. For 3-D BT planning, and dose was prescribed to the High risk CTV for BT (HR CTV) with 5Gy. The 3-D BT planning goal was to cover at least 90% of the HR CTV with target 5Gy isodose surface while limiting the dose to $2cm^3$ of bladder to less than 7.5 Gy, and $2cm^3$ of rectum to less than 5Gy. In one patient of 10 patients, $D_{2cm3}$ of rectal dose was over 5Gy and 6patients at $D_{2cm3}$ of bladder dose on 2-D BT planning. There was a tendency to underestimate ICRU bladder dose than ICRU rectal dose. CT based 3-D BT planning for cervical cancer will enable evaluation of dose distributions for tumor and critical organs at risk. So, rectal and bladder morbidity as well as geographic miss will be reduced in case of the bulky disease or uterine malposition.
