Treatment of giant iatrogenic pseudoaneurysm of the femoral artery (대퇴동맥에 발생한 의인성 거대 가성동맥류에 대한 치료)

  • The role of angioembolization has increased because of increases in nonoperative treatment for traumatic splenic injury. We report here a case of successful treatment of iatrogenic pseudoaneurysm of the femoral artery by thrombin injection with coil embolization. A 55-year-old female was admitted to our hospital because of blunt trauma. Computed tomography (CT) revealed a grade V splenic injury with contrast extravasation; therefore, angioembolization was performed. Three days after admission, follow-up CT scan revealed rebleeding from the spleen, and repeat angioembolization was performed. Seven days after admission, an approximately $7.0cm{\times}4.0cm-sized$ pseudoaneurysm was found on follow-up CT scan and there was no bleeding from the spleen. Although thrombin was injected into the aneurysmal sac, there was still inflow of blood, as observed on color-doppler ultrasound. Therefore, coil embolization to the neck of the aneurysm was performed. On angiography, there was no contrast filling into the sac. The size of the pseudoaneurysmal sac had decreased on follow-up CT scan, and the patient was discharged to home without complications. We successfully treated a giant pseudoaneurysm of the femoral artery using thrombin and coil embolization.

  • High-grade liver injury is associated with high morbidity and mortality rates. We report successful emergency angioembolization and early interventional radiology support to manage a high-grade liver injury in a 29-year-old man who presented following a fall during parachute training. Upon arrival, his blood pressure was 80/40 mmHg, and emergency ultrasonography showed a liver injury with perihepatic fluid collection. The patient's blood pressure reduced to 60/40 mmHg, and emergency angiography was performed without computed tomography (CT) (door to puncture time 36 min). After angioembolization, his blood pressure returned to 120/77 mmHg. Subsequent CT revealed no additional bleeding or hollow viscus injury. He was admitted to the Intensive Care Unit and discharged without complications 30 days after admission. In this case, emergency angioembolization (without performing CT) could successfully and safely treat a hemodynamically unstable patient with a high-grade liver injury.

Factors Influencing Protective Behavior against Radiation Exposure of Radiological Technologist in Computed Tomography Examination Room (전산화단층촬영검사실 방사선사의 방사선피폭 방어행위에 영향을 미치는 요인 분석)

  • This study was conducted to analyze factors Influencing Protective Behavior against Radiation Exposure using questionnaires for 231 radiological technologists working in Computed Tomography(CT) examination room with high radiation dose in diagnostic radiology field. Statistical analysis of the collected data revealed that the reasons for partially shielding the examination part in the CT scan were the lack of protective equipment, securing of radiation justification, being annoying and maybe not being harm to adults in order. It was also revealed that the variables influencing the protective behavior were protective behavior against radiation harm, self-efficacy, protective environment, organization culture, protective knowledge and protective instrument in order. The higher the radiological protective environment(${\beta}=0.245$) and the lower the radiological protective knowledge(${\beta}=-0.034$), the more influential the protective behavior against radiation harm was. In this study, it was shown that non examination parts were not shielded in the CT scan. Therefore, it is necessary to improve the level of protective environment, to cultivate knowledge to improve the protective behavior against radiation harm and to have an intervention strategy for concrete action.

  • The purpose of this study was to manufacture a wrist brace using a computerized tomography system, clinical design software (MediACE 3D Program), and 3D printer. After acquiring the Dicom file of the upper limb with a computed tomography, the wrist brace was designed using the MediACE 3D Program to create a "stereolithography" file. The designed wrist brace was printed using a 3D printer. To verify the effectiveness of wrist assistive device manufactured by 3D printing technology, the stress distribution of the pressure and orthosis applied to bone and skin is represented by finite element analysis. It is expected that the wrist brace can be manufactured by reinforcing the part where the damage caused by pressure and breakage of the brace frequently occurs with the result of finite element analysis when producing the wrist brace.

  • The purpose of this study was to evaluate the usefulness of the developed air mattress for reducing the deterioration of image quality due to the scattered radiation generated on the computed tomography equipment table. 5 cm and 10 cm thick air mattresses were developed and the image quality was measured by scanning the AAPM phantom according to thickness and thickness. Statistical significance was confirmed by One Way Analysis of Variance(ANOVA) Compared with the AAPM phantom scanned in the standard method, the image with the air mattress did not show any difference, but when the air mattress was not applied, the CT number and uniformity were low and the noise was high, and the spatial resolution Respectively. The developed air mattress has no harmful effect on the diagnostic image, it is very effective in improving the image quality and can increase the CT image quality by simply applying Air Mattress to existing equipment without using the technology applied to the latest and high-cost equipment.

Accuracy of Nodal Staging with Integrated PET/CT Scanning in Non-small Cell Lung Cancer (양전자단층촬영/전산화단층촬영(integrated PET/CT)을 이용한 비소세포폐암의 림프절 병기판정)

  • Background: For staging primary lung cancer, integrated positron emission tomography/computed tomography (PET/CT) imaging is popular. The purpose of this study was to evaluate the accuracy of PET/CT scanning in lymph nodal staging of lung cancer. Material and Method: We studied 48 patients who had received CT, PET/CT and pulmonary resections due to primary non-small cell lung cancer in our hospital between January 2006 and August 2009. Mediastinal lymph nodes were classified as superior mediastinal nodes, aortic nodes, inferior mediastinal nodes, or N1 nodes. We compared the power of CT and PET/CT for diagnosing pulmonary lymph nodes for each of the four types of nodes. Result: PET/CT was more sensitive than CT for all groups except inferior mediastinal nodes. However, the differences were not significant (McNemar's test: superior mediastinal nodes, p=0.109; aortic nodes, p=1.000; inferior mediastinal nodes, p=0.625, N1 nodes, p=0.424). Conclusion: The accuracy of PET/CT is similar to that of CT alone for staging lymph nodes. The two imaging modalities might be used as complementary, cooperative tools. We expect that integrated PET/CT will be found to be significantly mmore sensitive after more trials are done and more data is accumulated.

Stopping Power Ratio Estimation Method Based on Dual-energy Computed Tomography Denoising Images for Proton Radiotherapy Planning (양성자치료계획을 위한 이중에너지 전산화단층촬영 잡음 제거 영상 기반 저지능비 추정 방법)

  • Computed tomography (CT) images are used as the basis for proton Bragg peak position estimation and treatment plan simulation. During the Hounsfield Unit (HU) based proton stopping power ratio (SPR) estimation, small differences in the patient's density and elemental composition lead to uncertainty in the Bragg peak positions along the path of the proton beam. In this study, we investigated the potential of dual-energy computed tomography image-based proton SPRs prediction accuracy to reduce the uncertainty of Bragg peak position prediction. Single- and dual-energy images of an electron density phantom (CIRS Model 062M electron density phantom, CIRS Inc., Norfolk, VA, USA) were acquired using a computed tomography system (Somatom Definition AS, Siemens Health Care, Forchheim, Germany) to estimate the SPRs of the proton beam. To validate the method, it was compared to the SPRs estimated from standard data provided by the National Institute of Standards and Technology (NIST). The results show that the dual-energy image-based method has the potential to improve accuracy in predicting the SPRs of proton beams, and it is expected that further improvements in predicting the position of the proton's Bragg peak will be possible if a wider variety of substitutes with different densities and elemental compositions of the human body are used to predict the SPRs.

Noise Properties for Filtered Back Projection in CT Reconstruction (필터보정역투영 CT 영상재구성방법에서 잡음 특성)

  • The filtered back projection in the image reconstruction algorithms for the clinic computed tomography system has been widely used. Noise of the reconstructed image was examined under the input noise for parallel and fan beam geometries. The reconstruction images of $512{\times}512$ size were carried out under 360 and 720 projection by the Visual C++ for parallel beam and fan beam, respectively, and those agreed with the original Shepp-Logan head phantom very much. Noise was generated because of intrinsic restriction (finite number of projections) for the image reconstruction algorithm, filtered back projection, when no input noise was applied. Because the result noise was rapidly increased under 0.5% input noise ratio, technologies for reducing noise in CT system and image processing is important.
