Orthodontic protraction of the third molars to the posterior teeth missing area (구치 결손 환자에서 제3대구치의 교정적 활용)

  • The prolonged neglect of the posterior teeth missing area may cause mesial drift, extrusion, unexpected movement of the adjacent teeth and alveolar bone loss with occlusion collapse. Therefore it is recommended to treat that area by the prosthesis as soon as possible after tooth missing. However, if orthodontic treatment is applied to move the remained teeth, it can create improved biomechanical dentoalveolar environment. The use of the third molars in teeth missing area provides advantages as optimizing of prosthesis size. However, crown shape, location, soundness of the third molar and possible of eruption failure should be considered. In this case report, two patients closed a second teeth missing site and reduced the size of the first and second teeth missing area for an implant by protraction of impacted third molars. This case reports the considerations for closing or reducing the posterior teeth space with protracting the third molars by comparing two patients.

Arthroscopic Treatment of Tibial Spine Fracture using Suture Hook and pull-out PDS (Suture Hook과 pull-out PDS를 이용한 경골극 골절의 관절경적 치료)

  • Displaced tibial spine fractures need the anatomical reduction of the displaced bone fragment to achieve normal range of motion and anterior stability of the knee joint. The purpose of this paper is to describe details of arthroscopic technique using suture hook and pull-out PDS and to evaluate the clinical results. We report 7 cases who underwent arthroscopic reduction and internal fixation using suture hook and pull-out PDS. All cases had fresh fractures generated within 3 weeks. The follow up period was at average 16.6 months. The fracture union was achieved at average 7.4 weeks. Knee exercise was started 2 weeks after the operation. One of the patients, who had combined injury of posterior cruciate ligament and lateral meniscus, showed limitation of knee movement. But he was underwent the arthroscopic fibrolysis at one year later, he returned to normal range of motion. Arthroscopic treatment of displaced tibial spine fracture using suture hook and pull-out PDS showed good results including rigid fixation and early mobilization. Therefore it is thought to be one of the effective operative techniques in treatment of the tibial spine fractures.

A FEM study about the initial stress distribution on canine altered by the application point of preangulated TMA T-loop spring (Preangulated TMA T-loop spring의 적용 위치 변화에 따른 견치의 초기 응력 분포에 대한 유한 요소법적 연구)

  • The purpose of this study was to find the difference of stress distribution on canine altered by the application point of preangulated T-loop spring. For this study, the finite element models of upper left canine, upper left second premolar and upper left first molar were made. Also, the finite element models of $0.017{\times}0.025$ inch preangulated, preactivated T-loop spring and $0.018{\times}0.025$ inch stainless steel wire were made. Three types of T-loop spring were made . the middle of activated T-loop is positioned in accordance with the middle position of distance of bracket position of both the canine and first molar, 2mm anterior, 2mm posterior. We compared the forces and the distribution of stress that were generated by the difference of position of T-loop spring. The results were as follows. 1. All of the 3 types of T-loop spring showed the similar retraction forces. 2. All showed the similar amount & pattern of stress distribution. 3. The centers of rotation of canine in 3 types of T-loop spring were same and were positioned between C and D plane. 4. The canine showed the intrusive force by 2mm anterior positioned T-loop spring, but the extrusive force by 2mm posterior positioned T-loop suing. Neverthless, because of the small amount of the forces, the effect of vertical force was not significant.

Presurgical Nasoalverolar Molding in Fraternal Twins with Bilateral Cleft Lip and/or Palate: A Case Report (양측성 구순/구개열을 가진 이란성 쌍둥이를 대상으로 한 술전 비치조정형술: 증례보고)

  • Cleft lip and palate(CLP) is one of the most common craniofacial deformities that requires systemic management involving a multidisciplinary team approach. Although there has been great improvement in the field of cleft surgery, surgical approach alone cannot resolve the various problems in treating cleft lip and palate. Hence the need for presurgical treatment was appreciated and especially, the concept of presurgical nasoalveolar molding was applied to treat unilateral and bilateral cleft lip and palate patients. Presurgical nasoalveolar molding(PNAM) of unilateral cleft mainly aims to recover nasal symmetry while the objectives of pre-surgical nasoalveolar molding in the bilateral cleft are to elongate the columella, to erect the tip of nose, and to reposition the forward displaced premaxillary region. This report covers the case of fraternal twins diagnosed with bilateral cleft. Retraction of the premaxillary region and nasoalveolar molding were conducted for 70 days until cheiloplasty, using elastic bands and nasoalveolar molding appliances. After cheiloplasty, there had been improvements in the length of columella and the position of forward-displaced premaxilla for both patients compared to their initial states. The esthetics was also satisfactory for both the surgery and the parents. In order to maximize the efficacy of the appliance, three components should be in balance; patients' adaptation to the appliance, parents' cooperation and proper selection and careful adjustment of the appliance by the dentist.

MAXILLARY ADVANCEMENT USING RIGID EXTERNAL DISTRACTION(RED) IN CLEFT LIP AND PALATE PATIENT : CASE REPORT (견고 구외 골신장술을 이용한 구순구개열 환아의 치험례)

  • Patients with cleft tip and palate present severe maxillary hypoplasia due to scar of lip and palate, often accompanied by compromised mastication, speech abnormalities. Sometimes maxillary hypoplasia persist even though active orthodontic treatment was done. In theses cases, patients born with cleft lip and palate will be potential candidates for maxillary advancement with bone grafting after growth to correct the functional deformities and improve aesthetic facial proportions. But, maxillary advancement using standard surgical approaches has several limitations : increased relapse tendency after maxillary advancement, necessity of additional bone graft and mandibular setback surgery. Distraction osteogenesis is current treatment modality to overcome these limitations, thus has become popular for treatment of maxillary hypoplasia associated cleft lip and palate, craniosyntosis. Especially, rigid external distraction, contrary to internal device, has advantages : better vector control of osteotomized segment, effective traction of the bony segments, the ease of the application and removal the distraction device. This study showed that relatively successful result could be generated by using rigid external distraction osteogenesis(RED) in the case of cleft lip and palate with severe maxillary hypoplasia, 6 years 7 months old.

MANDIBULAR DIFFERENTIAL PREMOLAR EXTRACTI0N IN GROWING PATIENTS (성장기 환자에서 하악의 차등적 소구치 발치)

  • The extraction lot orthodontic treatment can be adopted for aligning crowded dentition, improving facial esthetics and solving a skeletal discrepancy as alternative for a surgical option. Mandibular second premolar extraction was often selected as treatment plan when there we very little or no space shortage in lower arch or limited retraction of the lower incisors was required. The primary object of this study was evaluate a pretreatment condition and examine the amount of tooth movement ior a mandibular second premolar extraction in growing patients. Pretreatment and posttreatment lateral cephalograms of 14 cases that had their four first premolar extracted (4/4 group), 15 cases with upper first and lower second premolar extraction (4/5 group) were selected. Structural method superimposition was conducted to evaluate a difference of dental change between 4/4 and 4/5 group. The results were as follows, 1. Pretreatment factor for 4/4 extraction or 4/5 extraction choice included maxillary incisor axis to occlusal plane, Class II molar relationship, IMPA and interincisal angle. 2. The amount of molar anterior movement in 4/5 group was greater than that of 4/4 group(p<0.05). 3. There was no significant difference between 4/4 group and 4/5 group in aspects of maxillary tooth movement(p<0.05).

An Analysis about Impact of Smart Home manufacturing and service Industry on National Economy (스마트홈 제조업과 서비스업의 국민경제적 파급효과 분석)

  • This study evaluated its potentials by quantitatively analyzing the national economic impact of the smart home-related industry, which is attracting attention as a core industry of the 4th industrial revolution. For the analysis, the smart home-related industries were classified into manufacturing and service industries through a literature review of the previous studies. Using the 2018 input-output table, this paper analyzed linkage effects between industries as well as spillover effects in the production, value-added, employment and job. As a result, the smart home manufacturing and service sectors showed a higher spillover effect in value-added than other industries in each industrial field. In the smart home industry, the spillover effects of manufacturing sector to service sector are larger than those of service sector to manufacturing sector. Moreover, it was confirmed that smart home industry was highly related to not only the technology-intensive industry, but also the service sector for smart cities, smart cars, Fin-tech, and etc. On the other hand, the smart home manufacturing sector is a final demanding industry with relatively higher backward linkage effect than forward linkage effect. In the smart home service sector, the forward linkage effect was relatively high compared with the backward linkage effect, indicating that it was an industry with a high supply function to other industries.

An Estimating Algorithm of Vehicle Collision Speed Through Images of Blackbox (블랙박스 영상 분석을 통한 차량 충돌 속도 연산 알고리즘에 대한 융복합 연구)

  • The vehicle collision speed in mid and high range can be checked by EDM(Event Driven memory) data recorded when the air bag works. But it's difficult to estimate the low speed of vehicle collision. And estimating the speed is important because the injury level can be changed by the impact speed. The study proposed an estimating algorithm by analysing the images recorded in car blackbox instrument. Low speed rear collision accidents simulated with wire winding motor for various vehicle types. The study estimated the impact speed with the ratio of the distance change between two vehicles and the length change of the number plate of front vehicle. The closer the vehicles are, the larger the plate length is. You can estimate the impact speed with the ratio. The impact speed is calculated with the initial distance for a specific length of number plate in the algorithm. The results can be applied to the linear rear collision because the angle of impact was not considered in this study.

Maxillary Advancement using Distraction Osteogenesis Devices in Cleft Palate Patients (악정형 장치를 이용한 구순구개열 환자의 상악골 전방 견인)

  • For the treatment of cleft palate patients, orthognathic surgery has been used by standard protocol. Predictable results have been achieved with standard techniques in slight to moderate maxillary hypoplasia. However, limited advancement and high relapse rate was reported in severe cases. The purpose of the present study was to review the clinical results of distraction osteogenesis in the patients with cleft lip and palate. Distraction osteogenesis has improved results in these patients by allowing soft tissue relaxation and gradual bone generation. Therefore, greater movement of the craniofacial skeleton is possible in severe cases of maxillary retrusion with lower relapse rates. In conclusion, distraction osteogenesis for the advancement of hypoplastic maxilla of cleft patients has shown successful treatment method.

Nonextraction treatment of Class II division 2 in an adult patient using microimplant anchorage (MIA) (Microimplant Anchorage(MIA)를 이용한 II급 2류 성인 환자의 비발치 치험례)

  • Maxillary anterior teeth were intruded and lingually root torqued with two maxillary anterior microimplants between the lateral incisors and canines. Overerupted maxillary canines were intruded with two other microimplants between the maxillary canines and first premolars. Maxillary posterior teeth and canines were distalized, then the maxillary incisors were retracted with two maxillary posterior microimplants between the first and second molars. The mandibular anterior teeth were intruded and the mandibular posterior teeth were extruded with conventional method such as anterior bite plane, intrusion arch and Class II elastics. The mandible moved slightly forward after the correction of deep bite and retroclination of the upper incisors. Consequently, microimplant anchorage (MIA) provided absolute anchorage for simultaneous correction of Class II canine and molar relationships and deep overbite.
