  • 응급의학은 여러 가지 질환이나 외상, 사고 등의 원인으로 생명이 위독한 상태에 있는 긴급한 사람을 신속히 진단하고 치료하는것이 주 역할이나, 때로는 사람이 사망하였을경우 이를 다시 살리는 이른바 심폐소생술을 시행함으로써 죽은 사람을 살리는 일도 취급한다. 이러한 행위를 심폐소생술이라 일컫는 이유는, 사람이 죽으면 심장이 멎고 폐를 통한 호흡이 중단되므로 호흡과 심장박동을 인위적으로 다시 소생시켜 주는 시술이기 때문이다. 심폐소생술에는 기본심폐소생술과 전문심폐소생술로 구분한다.

The Effect of a Mechanical Chest Compressions for Out-of-hospital Advanced Cardiac Life Support (병원 전 전문심장소생술을 위한 기계적 가슴압박기의 효과)

  • The purpose of this study is to evaluate the quality of chest compression by conducting comparison research between mechanical chest compressor(LUCAS) and manuale cardiopulmonary resuscitation(CPR) in a out-of-hospital environment and suggest effective advanced cardiac life support using mechanical chest compressors. For this, a out-of-hospital cardiac arrest was simulated with a team of 3 ambulance workers, and manuale CPR and CPR using LUCAS were performed on site and during transport in an ambulance. The research results are as follows: the comparison of manuale CPR between on site and in an ambulance revealed that on-site manuale CPR showed significant differences in the average compression depth, compression rate, and relaxation rate. Second, the comparison between manuale CPR and LUCAS in an ambulance showed significant differences in the average compression depth, compression rate, the number of compression per minute.

  • The aim of this paper was to test the effects of simulation-based learning on knowledge, confidence, critical thinking disposition, and clinical competence to advanced cardiovascular life support of nursing student. This study used one group, pre-post test design. The participants of this research were 35 senior nursing students who underwent training in the medical-surgical nursing education. Data were collected by utilizing the questionnaire in two separate occasions: before and after the simulation-based training. As a result of this paper, the improvements of the knowledge, confidence, critical thinking disposition, and clinical competence to advanced cardiovascular life support were statistically significant. In conclusion, the simulation-based education has a positive effect on nursing students in the area of knowledge, confidence, critical thinking disposition, and clinical competence to advanced cardiovascular life support.

  • This study purpose was to identify the effect of simulation education based on Blended Learning on Nursing students' Knowledge, performance, Learning satisfaction to ACLS. Methods was nonequivalent control group pretest-posttest design. The participants were 88 students 4 grade from the Department of Nursing at C University. The study period was from September 1 to December 31, 2021, and Data were analyzed with X2 test, independent t-test using SPSS 22.0 program. The research results show that the experimental group showed significantly higher performance(t=-9.843, p=.001) and learning satisfaction(t=-3.484, p<.001) for ACLS compared with the control group. In conclusion, it was suggested that ACLS simulation education based on Blended Learning is an effective teaching method to improve nursing students' performance and learning satisfaction.

Out-of-Hospital Resuscitation of Cardiac Arrest by 119 Emergency Medical Service System (119구급대에 의해 소생한 병원 전 심장정지 환자 1례)

  • The aim of this study was to report the out-of-hospital resuscitation of cardiac arrest along with literature consideration. CPR is technology of resuscitating patient by maintaining oxygen supply to organ, by preventing progression from clinical death to biological death, and by recovering heart beat and circulation, through circulatory support of mechanical ventilation. It is not what every patient with cardiac standstill is revived even if being implemented CPR. Patient's survival rate is decided by how quickly and correctly CPR was executed. A patient, who wasn't witnessed the cardiac arrest on the field, was performed 5-cycle CPR for 2 minutes on the field before being transferred to hospital, and was allowed to be used AED. A person, who was observed, is recommended to be used AED immediately, and then is said to be needed ACLS (advanced cardiac life support). In the Out-of-Hospital stage, it is rare in a case that 119 Emergency Medical Service System transfers by being ROSC and in a case of executing ACLS. Cardiac arrest was witnessed on the field, but CPR wasn't executed. First-aid staff executed CPR after arriving, and led to ROSC on the field with ACLS, there by having experienced 1 case of a patient's surviving to leave hospital by being ROSC on the field.

A Case of Resuscitation of an Electrocuted Child by a 119 Paramedic (감전으로 심정지된 소아환자 소생 1례)

  • The purpose of this study was to prevent electric shock to children. children are at high risk of electrocution by accidents. In the present case, a 2-year-old child was electrocuted after inserting metal chopsticks into a home outlet. In this case, professional advanced cardiac life support (ACLS) was properly performed by a 119 paramedic, resulting in the return of spontaneous circulation (ROSC) in the child. There are very few successful cases of resuscitation of patients involved in electric shock accidents in Korea, and we report a case of ROSC and discharge and proper resuscitation by a 119 paramedic.

  • When a pregnant woman experiences cardiac arrest, resuscitation is of the utmost importance. Cardiac arrest in pregnant women differs from cardiac arrest in the general population since both mother and fetus need to be taken into consideration. In the event of cardiac arrest, determining whether to deliver the baby is significant. Cardiopulmonary resuscitation is not always successful, and the survival rate depends on the speed and precision of the procedure. In this study, we focus on the case of a 30-year-old pregnant woman who experienced cardiac arrest and whose family was quick to perceive her condition and call the hospital. A witness performed initial cardiopulmonary resuscitation, while rescue workers performed the advanced procedure. In this case, the patient and baby received proper treatment and left the hospital after six days. It is extremely rare for a pregnant patient to achieve return of spontaneous circulation (ROSC) or receive advanced cardiac life support before reaching the hospital. However, the woman in question in this study achieved ROSC and received both cardiopulmonary resuscitation before reaching the hospital and advanced cardiac life support at the hospital. The specifics of the case are reported in the context of a literature review.

Effects of Simulation-Based Training on Nursing Students' Knowledge and Ability to Perform Advanced Cardiovascular Life Support (시뮬레이션기반 교육이 간호대학생의 전문심폐소생술 지식과 수행능력에 미치는 효과)

  • Purpose: The purpose of this study was to analyze the effects of simulation-based training on nursing students' knowledge and ability to perform advanced cardiovascular life support. Methods: A nonequivalent control group non-synchronized design was used. The participants were, in total, 59 junior college nursing students (experimental group: 30, control group: 29). The data were analyzed with a ${\chi}^2$-test, independent t-tests, and analysis of covariance (ANCOVA) using IBM SPSS Statistics 23 program. Results: The experimental group ($80.8{\pm}8.4$), who received simulation-based advanced cardiovascular life support education, showed a higher level of knowledge of advanced cardiovascular life support than the control group ($75.0{\pm}9.9$), who received lecture-based education. In addition, the experimental group ($106.4{\pm}11.9$) showed a higher level of ability to perform advanced cardiovascular life support than the control group ($86.5{\pm}9.9$). Conclusion: Simulation-based advanced cardiovascular life support education was more effective for increasing the knowledge and performance ability of college students than lecture-based education. Therefore, simulation-based training should be used to improve the knowledge and ability of nursing students and should be applied with reinforced systematic education programs.

Prehospital Care of 119 EMT for Non-traumatic Cardiac Arrest and Improvement to Increase Advanced Care Rate (119 구급대원의 비외상성 심정지 환자의 병원전 처치실태 및 전문 처치율 향상을 위한 개선 방안)

  • This study aimed at evaluating and developing 119 emergency medical technicians' prehospital care for non-traumatic cardiac arrest. Total 322 EMT in Chungnam province and Daejeon city filled out the self-administered questionnaire. The data were analyzed by SPSS 18.0 for descriptive statistics. Among the 322 EMT, 309 (97%) and 169(53%) always or almost performed CPR and AED for nontraumatic cardiac arrest patient, respectively. Among the advanced EMT and nurse, IV were sometimes or not performed at 94.7% and medication including epinephrine which commonly used for survival of cardiac arrest were treated just at 9.3 % (14 person). The reason they did not perform each procedure for airway management, AED or IV was lack of manpower, limit of time or joggle of ambulance and legal restrictions. In conclusion, to increase survival rate of non-traumatic cardiac arrest in out-of-hospital, it is necessary to increase manpower, legal protection of EMS, establishment of standard operating procedure, practice for improvement technique and use of medication for ACLS.

  • Dentists who want to perform sedation must have abilities to control any emergent situation that may occur during treatment procedures. In the 2010 guideline for the use of sedation by dentists, he/she must has valid certifications for basic life support (BLS) and for advanced cardiovascular life support (ACLS). The Korean Association of Cardiopulmonary Resuscitation (KACPR) has made a contract with the American Heart Association for education and certification. From 2004, they have held many courses for BLS and ACLS providers and instructors. The author of this research participated in the above mentioned courses and qualified as a BLS provider, BLS instructor and ACLS provider. The present paper was intended as an introduction to and arrangement of BLS and ACLS courses for health care providers through a year of experience. However, the previous course of the ACLS provider was focused more toward medical doctors. It is necessary to develop a new advanced course for dentists who employ sedation in their medical practices.
