McBride's Disability Assessment: Why Is It Still Most Used? (맥브라이드 노동능력상실 평가법: 왜 아직도 가장 많이 사용되는가?)

  • The task of defining the concept of 'disability' has proven to be surprisingly difficult. The way disability is assessed in the world suggests that very different conceptions of disability are being presumed. It is very hard to suggest appropriate criteria for assessing a disability that accurately demonstrates the presence and measures the severity of the disability, with proven consistency and validity provided by the assessors consuming the time and cost that the society can afford. When a rigorous framework in disability assessment procedures is used, discretion is then considerably restricted. Limited discretion tends to produce more consistency and validity in the assessment but at the cost of ignoring the individual differences or exceptional circumstances. The disability approach among the categories of the disability assessment is both the oldest and the most commonly used strategy of disability assessment around the world. Most of the criteria of disability assessment in Korea belongs to this. The McBride's criteria for disability evaluations can be used to obtain the ratio of body impairment and work disability without spending too much time and is being applied in court. Nevertheless, there are many errors and controversies, and corrections are required. On the other hand, an improvement plan for a disability evaluation method in Korea is to supplement and use the McBride's disability assessment method, which is currently the most widely used. This review article describes the conception of disability and the assessment of disability, and introduces the models of the assessment of disability to determine if the application of the McBride's criteria of disability evaluation can be justified.

  • The purpose of the study is to identify the usability of OpenCouresWare(OCW) from the perspective of students with visual and hearing impairments. Twelve college students with disabilities(8 visual and 4 hearing impairment) implemented usability testing. Two modules in both edX and KOCW were selected for testing. Students with disabilities implemented usability testing and then each individual case was analyzed through quantitative and qualitative methods. As results, the degree of general usability in both edX and KOCW indicated an average level and the degree of accessibility in edX module was evaluated higher than KOCW. Specifically, the module of edX was rated highly by students with hearing impairments and the usability level of KOCW module was highly appreciated by blind students.

Understanding Assessment for Feeding Disorders in Autistic Spectrum Disorders: A Literature Review (자폐 스펙트럼 장애 섭식장애 평가의 이해: 문헌 고찰)

  • Objective : Children with autism spectrum disorder (ASD) commonly suffer from feeding disorders. Major feeding problems include mealtime behavior problems, picky eating, and a lack of food variety can lead to nutritional problems, developmental and social limitations, and stress for the caregivers. A review of the latest literature was conducted to gain an in-depth understanding of assessment tools for feeding disorders in children with ASD. Method : This study analyzed assessments to identify feeding problems in ASD based on previous studies searched through keywords such as ASD, ASD feeding problem, and ASD feeding evaluation. Results : The ASD feeding disorder assessment was divided into direct and indirect assessments. Indirect assessment, in which caregivers measure a child's situation using questionnaires, is mainly used. The assessment of feeding disorders in children with ASD was divided into 1) mealtime behavior, 2) sensory processing, 3) food consumption, and 4) others. Conclusion : As the main feeding disorder characteristics of children with ASD are very diverse, a comprehensive evaluation is necessary but is still limited. Swallowing rehabilitation experts, such as occupational therapists, should apply comprehensive assessment tools based on a basic understanding of the feeding problems, behaviors, and sensations in ASD.

  • 1. 서론 o 연구배경 05년 4월에는 생손보 공통의 신체장해분류표 개정시행 및 표준약관개정이 있었으며 05년도까지는 어떤 형태로든지 장애인차별금지법안이 입법화될 것이라고 한다. 따라서 장애인의 보험이 거절되었을 경우 입증책임은 보험회사에 있게 될 것이다. 생명보험업계로서는 공 통 인수지침마련을 통해 민원소지 최소화를 위해 노력했으나 오히려 장애인차별이라는 비난을 받게 되었다. 이에 개정신체장해분류표에 대한 연구 및 언더라이팅적 시각에서의 개선방안 모색이 필요하게 되었다. o 연구방향 신체장해제도는 보험회사 뿐만 아니라 각종 법규 및 정부기관에서 다양하게 운영되고 있다. 이들에 대한 연구 및 특히 보험청약시에 주로 접하게 되는 복지장애(장애인복지법하(下))에 대해 주로 연구하여 이와 개정신체장해분류표를 비교분석 하도록 한다. 그리고 개정신체장해분류표에서 향후에 발생가능한 문제에 대해 업계경험을 토대로 개선점을 강구하도록 한다. 2. 신체장애등급의 이론적 배경 o 신체장애제도의 종류 o 국내법상의 신체장애제도 o 신체장애평가제도 근거법규 o 해외주요국의 신체장애평가제도 3. 우리나라의 장애보장제도 현황 o 국가장애등급과 생명보험 실제지급경험의 연구를 통해 신체장해비교를 통해 장애1급의 주요원인이 질병원인에 있으며 재해원인은 상대적으로 낮은 수준을 보이고 있다. 특히 질병장애의 경우 05년 3월까지 등록된 복지장애에서보다 생명 보험 지급경험에서 훨씬 높은 비율을 나타내고 있음이 05년$1{\sim}5$월 생명보험 장애1급 지급 건의 연구결과 나타났다. 문제는 복지장애와 생명보험약관상의 신체장애의 평가기준이 상이하여 등급간 정도와 신체장해물의 정도의 비교에 표준화된 이론적 근거나 tool이 없다는 것이다. 이에 대해 외국사의 경우에는 장애의 결과보다는 원인질병에 의거하여 그에 따른 후유장애로 나뉘어 인수지침을 두고 있다. o 우리나라의 신체장해 평가방법을 보면 각종 법규나 정부기관에서 사용하는 평가방법을 포함하여 대략 신체장애등급방식과 신체장해율 방식으로 나눌 수 있다. 복지장애는 독자적인 신체장애등급방식으로 장애를 평가하고 있으며 생명보험약관은 05넌4월개정시부터 손해보험의 장기보험에서 기(旣)시행중인 신체장해율방식을 쓰고 있으며 이는 미국의 A.M.A법에 근간을 두고 있다. 상호간의 판정기준이 상이한 상황에서 언더라이팅은 장애의 원인은 고려되지 않은 결과물에 해당되는 신체장해율표만 가지고 인수지침을 세우기 어려우므로 A.M.A법에 대한 연구가 필요할 것이다. 또한 손해보험경험에서 이미 문제로 대두되고 있는 한시장해와 기타 제도적 개선방향을 모색해야 할 것이다. 4. 결론 우리나라에서 운영되고 있는 장해는 원인에 대한 고려나 선천성과 후천성의 구별이 없이 운영되고 있다. 또한 신체장해 평가기준이 너무나 다양하게 운영되고 있어서 기초통계축적에도 어려움이 많다. 보험회사의 사회적 책임의 한계도 존재한다. 따라서 장해의 결과보다는 원인에 대한 연구가 더욱 필요할 것이며 이에 대한 국민의 관심 및 공감대형성이 필요할 것이다.

  • o 최근 강박장애의 치료가 발전한 것만은 사실이나 아직도 1차 치료에 반응이 없는 환자가 많다. o 치료반응의 평가는 매우 중요하며 현재 Y-BOCS를 이용하여 주로 치료반응을 평가하고 있으나 Y-BOSC 자체도 많은 문제점을 안고 있다. o 약물치료시 특히 치료반응의 여부를 결정짓는 의사결정은 매우 중요하다. o 국내에서도 강박장애 치료알고리듬이 개발되어 강박장애의 치료 및 평가에 있어 기초가 되어야 할 것으로 기대된다.

  • 악관절장애환자의 임상 평가를 위해서 고안된 여러 지수 중 현재 가장 널리 사용되고 있는 두개하악장애지수와 평가방법을 새로 고안한 변형 두 개하악장애 지수, 혼합두개하악장애지수를 비교하여 환자의 증상변화를 민감하게 감지하는 능력을 알아보고자 본연굴를 시행하였다. 두개하악장애지수의 기준에 따라 단국대학교 치과병원 구강내과에 내원한 악관절장애환자 20명을 대상으로 임상검사를 시행하여 두개하악장애지수, 변형두개하악장애지수, 혼합 두 개하악장애 지수를 산출하였다. 치료전, 치료후 2주, 치료후 4주에 수집된 결과를 각 지수간에 비교분석하여 다음과 같은 결과를 얻었다. 악관절장애환자의 증상평가에서 변형두개하악장애지수가 환자의 증상변화를 가장 민감하게 감지하는 것으로 나타났다(P(0.05). 따라서 사용된 세가지 지수중 변형 두개하악장애지수가 악관절장애환자의 경과를 관찰할 때 증상변화를 관찰하기 위한 가장 효과적인 방법이라고 사료된다.

A Study on Development of the Job Functional Assessment Scale for people with Disabilities (장애인 근로능력평가 척도 개발에 관한 연구 -국민연금장애연금수급자를 중심으로)

  • The purpose of this study was to develop the Job Functional Assessment Scale. This study was involved took third steps. First, 93 preliminary items were developed based on theoretical literature and International Classification of Functioning, Disability and Health (ICF) Model. To verify the content validity, 3 rehabilitation experts rated those items. Second, the scale consisting of 73 items was administered to sample of 880 people with disabilities. Finally, items of the scale were reduced to 58 items. Factor analysis showed evidence of construct validity of the scale. Internal consistency (Cronbach's ${\alpha}$) of total score showed .94 and Cronbach's ${\alpha}$ of 7 subfactor demonstrated .74~.90. Thus, Job Functional Assessment Scale demonstrated satisfactory convergent validity and discriminant validity. In addition, the practical use of the scale was discussed.

Development of Computerized Screening Test Items for Mild Cognitive Impairment (경도인지장애 선별을 위한 전산화 평가 항목 개발)

  • Objective: This study was to develop computerized screening test items for mild cognitive impairment. Methods: Through literature reviews, items from computerized tests for screening mild cognitive impairment were extracted. A panel of professional experts validated that the items were important and fit to screen for mild cognitive impairment. Results: A total 37 items were extracted from 12 computerized tests and 11 new items were added through the first panel review. After that, 18 items were removed via the second panel review. Finally, 16 items were selected by analyzing content validity ratio. 16 items consisted of memory, attention, and executive function areas. Conclusions: A total of 16 computerized test items were developed. It is urgent to validate them to screen mild cognitive impairment. Moreover, standardization studies for this test are required in the future.

Development of Evaluation Indicators for Level of Service on Academic Library Users with Disabilities (대학도서관의 장애인 서비스 수준 측정을 위한 평가지표 개발)

  • The purpose of this study was to identify the service factors of university libraries which aim to improve the educational welfare of the disabled, and to develop an evaluation indicator to evaluate these vital service factors. To proceed with this study, guidelines of public library services for people with disabilities, and previous research and job description of academy library analysed and developed indicators suited for evaluating academic library services for people with disabilities. A pilot survey and main survey about the evaluation factors and indicators conducted with librarians who work for university libraries, and the questions were refined by exploratory factor analysis of the data collected from the survey. As a result, through the process of identifying reliability and validity, it turned out that the evaluation factor and subordinate index of university library services for the disabled consisted of 8 factors and 33 indicators.

The Comorbidity of Attention Deficit Hyperactivity Disorder and the Effect of Methylphenidate on it (주의력 결핍 과잉 운동 장애 환자들의 약물치료 효과 및 Comorbidity에 관한 연구)

  • The objectives of this study were to evaluate the effect of methylphenidate on attention deficit hyperactivity disorder (ADHD) and the comorbidity of the disorder, using child attention problem checklist to 56 (male: 38, female: 18) patients from, March 992 to February 1993. The results were as follows: Among 56 subjects, ADHD alone were 20 (335.71%) subjects, and with one additional diagnosis were 31 (55.35%) subjects and with two additional diagnosis were 5 (8.93%) subjects. There was significant improvement on symptoms in the scores rated by teachers in 7th and 28th day after menthlphenidate administration compared to baseline score (P<0.05) and 28th day score showed significant improvement compared to 7th day score (P<0.05) and 28th day score showed significant improvement compared to 7th day score (P<0.05). There was significant improvement on symptoms in the scores rated by parents in 28th day after methylphenidate administration improvement compared with 7th day score. In single administration of methylphenidate in Sunday morning score compared to afternoon score (P<0.05). In the administration of significant improvement on symptoms compared with the Sunday morning rating score of parents (P<0.05) and the 28th day comparison was also showed significant improvement on symptoms in the scores rated by teachers compared with the scores rated by parents on symptoms (P<0.05). Among group comparison, all groups showed significant improvements (P<0.05) except conduct disorder & oppositional defiant group.
