충남장애인구강진료센터에서 시행된 전신마취 및 진정법에 관한 실태조사 (A SURVEY OF GENERAL ANESTHESIA, SEVOFLURANE SEDATION AND INTRAVENOUS SEDATION IN CHUNGNAM DENTAL CLINIC FOR THE DISABLED)

  • 전신마취 및 진정법은 소아 및 장애인에게 성공적 치과치료를 위해서 필수불가결하다. 이 연구의 목적은 충남장애인구강진료센터에 치과치료를 위해 시행된 전신마취와 진정법의 실태를 평가하기 위함이다. 2011년 1월부터 2012년 9월까지 전신마취, 세보플루란진정 및 정주진정을 받은 소아와 장애인 426명을 대상으로 조사분석하였으며 389명이 전신마취, 20명이 세보플루란진정 그리고 17명이 정주진정하에서 치과진료를 받았다. 전신마취의 비율이 두 진정법에 비해 높았다. 세보플루란진정은 소아환자의 외상처럼 짧고 간단한 치료에 유용하게 사용되었다. 정주진정은 공포와 불안을 가진 치과적장애환자에게 선택되었다. 세보플루란과 정주진정은 점차적으로 전신마취의 사용을 줄이고 치과적장애환자를 위한 유용한 방법이었다.

  • Objectives: The purpose of this study was to identify the factors of satisfaction with regional oral health centers for people with disabilities in Korea. Methods: A survey was conducted with 784 patients who visited regional oral health care centers in Korea. A self-developed questionnaire (29 items) was adapted with a consent form and IRB (Institutional Review Board). All collected data were analyzed for statistics using SPSS version 25.0. Results: The study participants were 452 (57.7%) male patients, 207 (26.4%) aged 20-29 years old, and mostly had physical, developmental, and mental disabilities. The patients were introduced by their acquaintances, and the major reason for visit was specific dental care for the disabled. Most patients used their cars and took half to one hour to commute. The common factor affecting the comprehensive satisfaction was the facility satisfaction. Conclusions: A more comprehensive and standardized questionnaire should be developed for the annual evaluation of all centers. This study presents the basic data that can aid to activate the operation of the four oral health care centers for the disabled, which are scheduled to be implemented in the future, as well as 10 currently operated centers.

한국 내 장애인 구강진료센터의 공간특성에 관한 연구 (A Study on the Spatial Characteristics of Public Dental Healthcare Center for the Disabled in Korea)

  • Purpose: The characteristics of site plan and space configuration of public dentistry through examining the public dental healthcare centers for the disabled in Korea and comparison between them are necessary for the development of planning of the dental healthcare system. This study has been started to provide basic informations such as nationwide distribution, site relationship, and space configuration for the planning of public dental hospital architecture. Methods: Literature review of publicness and public dental healthcare and investigation on current status of public dental healthcare center for the disabled in Korea have been conducted. The site plan and space configuration of eight public healthcare centers for the disabled have been analyzed. Results: The result of this study can be summarized in three points. The first one is that public dentistry in Korea are distributed public dental hospital for the disabled in Seoul and public dental healthcare centers for the disabled in eight provinces. The second one is that the types of the dental healthcare center for the disabled are divided with remodeling type which is diverted from existed dental or medical out patient clinic space or independent building type which is planned with a new and exclusive usage for the disabled. The third one is that the space configuration of dental healthcare center for the disabled is needed more required programs, larger treatment unit space, and more private clinic space than typical dental treatment plan. Implications: This study is the starting point for the research of public dentistry and it is necessary to analyze the dental prevention and dental public policy to develop the public dental healthcare system.

  • Background: Dental treatment under outpatient general anesthesia is an option for disabled person, children and dentally disabled person who have the difficulty of cooperating voluntary with dental care. The aim of this study was to access the patients who had dental management under general anesthesia at Chungnam dental clinic for the disabled. Methods: This survey is consisted of 114 severely disabled and dentally disabled person who undergone dental treatment under general anesthesia from January to June 2011. Data were collected by individual dental and anesthesia records. Results: Of the 114 patients studies, the 45 patients was the dentally disabled and the 69 patients was the severely disabled. Rate of dental treatment among male patients was higher than among female patients. The dentally disabled was an similar age distribution. In the severly disabled, above 13-year-old was higher than below 12-year-old. The most popular induction method of general anesthesia was sevoflurane inhalation after induced sedative effect of nitrous oxide inhalation. The severly disabled who had definitely negative behavior was applied inhalation of high concentration sevoflurane. Conclusions: Chungnam Dental clinic for the disabled provide dental management for the disabled and the dentally disabled.

일부지역 장애인구강진료센터 이용 만족도에 미치는 요인 (Factors affecting the satisfaction of persons with disabilities at community oral care centers)

  • Background: To identify the factors that affect the current status and satisfaction of people with disabilities at community oral care centers. Methods: A structured self-administered survey, including five questions on facility environment, five on usage procedure, four on medical skill, four on care cost, three on friendliness, and three on satisfaction, was administered to 218 residents of the G-disabled community care center. It comprised a Likert 5-point scale (strongly agree, 5 points; agree, 4 points; moderate, 3 points; disagree, 2 points; not at all, 1 point). The reliability of the measurement tool was 0.932 for Cronbach's α. Results: The evaluation of community oral care centers for the disabled showed that the environment was hygienic (4.42±0.73), reservation system was well maintained (4.18±0.95), and the dentist-in-charge was satisfied with the treatment (4.37±0.62). The participants agreed that the details were sufficiently explained (4.29±0.71). However, in terms of medical expenses, the score of "have fully heard the explanation of medical expenses and reductions" was 3.88±0.92. The factors affecting satisfaction were sex, final educational background in the facility environment, usage procedure, and medical skill. Conclusion: To increase the satisfaction of people with disabilities at community oral care centers, it is necessary to establish a facility environment and service according to the patients' need and increase the reduction or exemption benefits between different treatment cost categories. Oral health management policies for the disabled should be developed based on these factors, so that the oral care of vulnerable groups in blind spots can be maintained.

전남대학교 치과병원 소아치과와 광주장애인구강진료센터에서 시행된 진정법과 외래전신마취에 대한 실태조사 (A Survey of the Sedation or Outpatient General Anesthesia in Department of Pediatric Dentistry, Chonnam National University Dental Hospital and Gwangju Dental Clinic for the Disabled)

  • Background: Sedation or outpatient general anesthesia is a necessary method for a successful dental treatment for children and the disabled who have a difficulty of cooperation. The aim of this study was to assess the patients who had dental treatment under sedation or outpatient general anesthesia at department of pediatric dentistry, Chonnam national university dental hospital and Gwangju dental clinic for the disabled. Methods: 875 patients who had sedation from January 2009 to September 2013 and 88 patients who had outpatient general anesthesia after May 2011 were assessed for this study. Patient's distribution and treatment patterns were examined. Results: The number of patients has been increasing in both sedation and outpatient general anesthesia. In case of sedation, the proportion of male patients was higher and 4-6 years age group was the highest. Most patients have resided in Gwangju buk-gu and the percentage of restorative treatment was the highest. In case of outpatient general anesthesia, the proportion of male patients was higher than that of sedation. Two groups, 10-19 and 20-29 years age, consisted most of patient. Most patients have resided in Gwangju buk-gu and Jeollanam-do, and the percentage of periodontic and preventive treatment was the highest. Conclusions: Sedation or outpatient general anesthesia for dental treatment has been increasing for children and the disabled who have a difficulty of cooperation. Therefore, it is important to improve treatment environment under sedation and general anesthesia through continuous research and studies.

우리나라 권역장애인구강진료센터 현황과 전망 (A report on the regional oral health centers for the disabled in Korea)

  • Purpose : The purpose of this study was to survey the present status of the regional oral health centers for the disabled. Material and methods : Face to face interviews with oral healthcare professionals in eight regional oral health centers and literature reviews were conducted among eight regional oral health centers for the disabled during 2, July to 30, August 2017. Results : It is necessary to establish more regional oral health centers for the disabled in area where the number of disabled person as percentage of whole region population is high. Also, all the centers appeared a lack of professional manpower including dentist, dental hygienist and anesthesiologist, equipment and facilities for the demands of patients and their guardians. It is essential for the government to improve the support system to regional oral health centers for the disabled. Conclusions : This study can be used as a reference for the countermeasure to make a new policy and activate the accessibility of dental care services in the public for the disabled.
