  • This study identified the perception and coping ability of an emergency involving the elderly for facility directors in charge of services in long-term care facilities, and used it as basic data for developing educational programs and policy data for improving the ability of emergency facilities. The subjects were 192 directors of elderly care facilities and home care centers. Data were collected from March 15 to April 20, 2019 and analyzed using the SPSS WIN 25.0 program. Data analysis was performed using t-test, One-way ANOVA, Pearson's correlation coefficient, Scheffe, and multiple linear regression. The results revealed 97.4% of emergency experience, 6.16 points of emergency perception, and 62% of correct answers, and coping ability of an emergency was 69.61 ± 13.537. The negative correlation between emergency experience and ability to cope with emergencies(r=-.202, p= .005) was the long-term care facility type(β = 8.253, p<.001). Overall, an education program considering the type of long-term care facility is needed when applying emergency education for facility directors.

  • This study aims to explore the meaning and nature of the nursing experience of visiting nurses of home-based long-term care service centers by using van Manen's hermeneutic phenomenological approach. A total of 10 home-visiting nurses working in Seoul and Gyeonggi-do Province were recruited through purposive, convenience, and snowball sampling and participated in the study. Data were collected from January 7, 2017 to October 12, 2018 through in-depth individual interviews with the participants. The analysis resulted in 6 essential themes regarding the lived experience of visiting nurses: 'establising a good rapport with the patient and his or her family caregiver', 'building the foundation of visiting nursing by performing autonomously nursing procedures for the patient', 'facing the reality not to be able to work as much as I want due to the limitations of the support system in performing work as a visiting nurse', 'upset when sincerity could not be delivered', 'following the calling of medical personnel by taking responsibility for 'my patient', and 'pledging oneself to career-long work, feeling reward as a visiting nurse.' The findings of the study help understand the role and importance of visiting nurses and contribute to the preparation of practical measures to improve the quality of home-visiting nursing.

  • The purpose of this research is to analyze the overall problems at the moment of October 2008, and then to find the improvements of home-help services of the Long-Term Care Insurance(LTCI), which has been revealed many problems since it was released in July 2008. The research uses the literature survey which analyzes 2nd-hand materials studied by other people already, and survey research was executed from active social workers in the area of LTCI. Based on the policy analysis framework of Gilbert and Specht, all the data are analyzed in the scopes of client·benefit(service)·finance·transferring system. This research has found the problems in each scope of home-help services of the LTCI. Firstly, the client system has some problems in mismatching between registered and service clients, estimating client number, and judging service levels. Secondly, the service system reveals deficiency in professionality of social workers, service quality lowering by loose qualification criteria on workers, non-reasonable limitation of service time available, and the same fare system applied to visiting-help service in spite of different levels. Thirdly, in financing system, clients need to pay additional money to get extra services such as meal, hair cutting, bathing etc., due to government financial support stopped, some organizations have to reduce services and replace full-time workers to part-time ones, which makes the service quality worse. Lastly, in the transferring system, the management system for service quality is not well prepared. There are too much competion because of allowing too many home-help service organizations and care worker academies. The suggestions that this research has found to improve the policy are as follows. ① It is desirable to make the registered clients the service ones as many as possible in the long term perspective. ② The LTCI organization requires more workers and higher professionality. ③ Many elderly people who are not eligible now require connection system to be more served. ④ Management system and service manual for care worker are to be developed. ⑤ Laws related to the service contents and process should be modified, the proportion of client charge needs to adjust. ⑥ Home-help service organization licensed by the LTCI needs to be financially supported publicly. ⑦ Monitoring system to home-help service organization needs to be strengthened. ⑧ Evaluation tools to home-help service organization and workers is required. ⑨ Specification to open the home-help service organization needs to be more strict.

  • The purpose of this study was to examine the effect of the possibility of job-related risk, emotional labor, and emotional leadership on the job turnover intention of caregivers working at the long-term care facilities in Korea. For the study, survey data of 434 caregivers at senior nursing homes were collected and analyzed using hierarchical multiple regression method. The results showed that high level of caregivers'perception of both the possibility of management risk and employee risk led to increased turnover intention. On the other hand, perception of the possibility of social-disaster risk did not give significant impact on turnover intention. Although the perception of the possibility of service user risk gave statistically significant effect on the turnover intention, the direction was the opposite to the one set at the research hypothesis. Frequent camouflage behavior("surface acting") of caregivers, which is the kind of emotional labor led to increased turnover intention. However, caregivers'behavior consistent with their inner warm heart("deep acting") did not lead to a statistically significant result. Lastly, the more caregivers perceive their superiors'emotionally supportive leadership, the lower possibility of turnover intention. Based on the above result, the researchers suggested several intervention methods in order to prevent turnover of caregivers and improve service quality of long-term care institutions.

  • The purpose of this study is to define the relationship between ethical management, organizational culture, and management performance in long-term care institutes for the elderly while identifying the level of ethical management, organizational culture, and management performance of caregivers and the impact and mediation of the relationships between them based on the hypothesis. The results show that caregivers in the Gyeonggi region scored 2.929 out of 5 in ethical management, 3.393 in organizational culture, 2.925 in organizational transparency and 3.185 in management performance. The levels of ethical management, organizational culture and management performance in caregivers are lower than those of welfare workers, social workers, and nursing assistants. This is due to their relatively low level of education, wages and working environment. This study determined that there is a significant relationship between variables in terms of correlation between ethical management, management performance, organization transparency, and organizational culture. Second, the authors adopted hypotheses that ethical management has a statistical effect on organizational transparency, management performance, and organization culture. Third, the authors concluded that organizational culture has a direct impact on organization transparency and there is a significant mediation effect between ethical management, organizational transparency, and management performance. Based on these results, we propose the following: First, ethical leadership at the core of ethical management. Second, awareness in organizational transparency. • The present study examined the impact of work stress and job satisfaction on intention to leave among staff including social workers, nurses, and care workers. The study subject included 235 staff in a welfare organization that provides long-term care services. Data was analyzed using multiple linear regression. The findings of the study show that work stress and job satisfaction affect intention to leave in the context of welfare organizations. Demographic variables were not the main focus of this study and thus these results are incidental. Staff with higher levels of work stress were more likely to think about leaving, while those with grater job satisfaction were less likely. There were several limitations in this study. Generalizability of the findings are limited to staff working in the province of Jeonnam. The results have important implications for the development of strategies to minimize turnover intention in long-term care. Reducing the intent to leave is desirable for issues of both cost reduction and quality of care. Managers could perhaps start to consider decreasing work overload assigned to staff. This study also provides some insight into the work status of new staff. Clearly this finding needs to be explored in further research studies. A more comprehensive model is likely required to adequately explain intention to leave the job.
