  • Peritonitis is one of the major complications of CAPD(continuous ambulatory peritoneal dialysis). Recently, multidrug-resistant organisms, such as vancomycin-resistant enterococcus(VRE) have been rarely reported by the pathogen as of CAPD-associated peritonitis. But, there is limited information on choices of effective therapy for VRE peritonitis in patients undergoing CAPD. We present a pediatric case of successful treatment of CAPD-associated peritonitis due to VRE with linezolid, and review of the literature.

Rapid Detection of Vancomycin Resistant Enterococci Using Multiplex Polymerase Chain Reactions (다중 중합효소 연쇄반응을 이용한 반코마이신 내성 장구균의 신속 검출)

  • It is generally difficult, time-consuming, and expensive for the clinical laboratory to detect vancomycin resistant enterococci (VRE). The aim of this study was to develop and evaluate the multiplex polymerase chain reaction (PCR) assay system as a diagnostic tool for the rapid detection of VRE from clinical samples and/or for the identification of VRE from the bacterial strains isolated from clinical specimens. Specific primers, designed from the nucleotide sequences respectively encoding the vanA, vanB, vanC-1, vanC-2/3 genes in enterococci, were coupled in a multiplex PCR assay system. With this multiplex PCR assay system, we investigated the incidence rates and types of VRE isolated from clinical samples. A total of 75 strains of enterococci were isolated in 3 general hospitals in Korea. Of these isolates, 36 strains showed a pattern of high-level vancomycin resistance which associated with vanA gene, whereas 18 strains showed low-level vancomycin resistance associated with vanC-1 or vanC-2/3 gene. Thus, multiplex PCR assay method established in this study could be applied for the rapid detection of VRE.

  • This examination was carried out to evaluate the sanitary quality of slices of raw fish being served in the restaurants. Twenty five kinds of slices of raw fish collected from various restaurants in Busan Korea were examined during the period from March to May in 1971. As the evaluation factors of sanitary quality, the contents of sanitary indicative bacteria such as coliform group, fecal coliform, feral streptococci and enterococci and plate counts were determined. The results obtained are as in below: 1. The numbers of fecal streptococci and enterococci MPN were much greater than those of coliform group and fecal coliform. 2. The median value of coliform group MPN was 3,300 per 100 grams of the sample examined and those of enterococci was 5,400. The median value of plate counts was $1.8\times10^5$ per gram. 3. Fifty-two percent of the samples examined were exceeded fecal coliform MPN 930 per 100 grams. 4. As a sanitary indicative bacteria fecal coliform MPN was more reasonable than enterococci 5. The grade of restaurants was not correlated with the bacterial quality of the foods served. 6. No correlation between the numbers of sanitary indicative bacteria ana plate counts was observed.

Prevalence and Molecular Characterization of Tetracycline Resistance in $Enterococcus$ Isolates from Raw Milk Samples in Korea (원유시료에서 분리한 장구균 속 세균의 tetracycline 내성 유전자형 분석)

  • Antibiotic resistance in animal isolates of enterococci is a public health concern, because of the risk of transmission of antibiotic-resistant strains or resistance genes to humans through the food chain. This study investigated phenotypic and genotypic resistances profile of tetracycline in 245 $Enterococcus$ isolates from bovine milk. A total of 245 enterococci were isolated from 950 milk samples. The predominant strain was $E.$ $faecalis$ (n = 199, 81.2%) and $E.$ $faecium$ (n = 25, 10.2%). $E.$ $avium$ (n = 7, 2.9%), $E.$ $durans$ (n = 6, 2.5%), $E.$ $gallinarum$ (n = 4, 1.6%), and $E.$ $raffinosus$ (n = 4, 1.6%) were also isolated. Of the 245 enterococcal isolates 76.3% (n = 187) displayed tetracycline resistance (${\geq}16{\mu}g/ml$). Of the 187 tetracycline-resistant isolates, 83.4% (n = 156), 16.1% (n = 30), and 26.7% (n = 50) possessed the genes $tet$(M), $tet$(L), $tet$(S) respectively. While 3.2% (n = 6) of the tetracycline-resistant isolates possessed all three genes $tet$(M) + $tet$(L) + $tet$(S), 8.6% (n = 16), 16.0% (n = 30), and 2.7% (n = 5) of them possessed two genes $tet$(M) + $tet$(L), $tet$(M) + $tet$(S), and $tet$(L) + $tet$(S) respectively. The tetracycline resistance pattern investigated in this study was attributable mainly to the presence of $tet$(M).

Investigation of Microbiological and Physiochemical Quality for Irrigation Water used in Napa Cabbage Cultivation (배추 재배에 이용되는 농업용수의 미생물 오염도 조사 및 이화학성분 분석)

  • The purpose of this study was to investigate water quality for irrigation water used in Napa cabbage cultivation. The water samples were analyzed for physiochemical and microbiological quality for a total of 111 samples including surface water (n = 75) and groundwater (n = 36) collected from five different regions where Napa cabbage is massively grown. As a conclusion, the levels of fecal indicators for surface water were higher than those for groundwater. The numbers of coliform from surface water and groundwater were 1.96-4.96 and 0-3.98 log MPN/100 mL, respectively. Enterococci were detected in 95% (72/75) of surface water samples and 22% (8/36) of groundwater samples. Besides, 97% (73/75) of surface water samples were observed being contaminated with Escherichia coli, and 22% (8/36) of groundwater sample was positive for E. coli. In the case of surface water, E. coli and coliform correlate to T-P, and enterococci showed relevance to the suspended solid (SS) and biochemical oxygen demand (BOD). In groundwater, fecal indicator bacteria showed relevance to the SS and chemical oxygen demand (COD). These results could be provided as fundamental date for establishing microbial standard of water used in leafy vegetables cultivation.

Factors Influencing Level of Awareness and Compliance with Vancomycin-Resistant Enterococcus Infection Control among Nurses in Intensive Care Units (중환자실 간호사의 반코마이신 내성 장구균 감염관리에 대한 인지도, 이행도 및 관련요인)

  • Purpose: The purpose of this study was to identify factors influencing Vancomycin-resistant enterococcus infection control among nurses in intensive care units. Method: Data were collected from August 15 to October 14, 2007 from 188 nurses working in intensive care units. The nurses answered a 26 item-questionnaire, which included management of the cohort (14 items), hand washing (6 items) and management of the environment (6 items). Descriptive statistics, t or F test, ANOVA, and multiple regression analysis were used with SPSS PC+ 14.0 to analyze the data. Results: The participant's level of awareness of Vancomycin-resistant enterococcus infection control was 3.87; that of compliance was 3.74. Significant factors influencing the level of compliance with Vancomycin-resistant enterococcus infection control were'the level of the awareness' and 'the type of intensive care unit'. These two variables accounted for 21.0% of variance for compliance with the Vancomycin-resistant enterococcus infection control among the participants. Conclusion: In order to develop a strategy to increase the compliance with Vancomycin-resistant enterococcus infection control, it is necessary to be concerned about 'the level of the awareness', 'the type of intensive care unit;', and 'experiences of caring for patients with Vancomycin-resistant enterococcus'.

Risk Factors and Clinical Outcomes for Vancomycin-Resistant Enterococcus Colonization on Intensive Care Unit Admission (중환자실 환자의 입실시 반코마이신 내성 장구균 집락의 위험요인과 임상적 결과)

  • Purpose: The purpose of this study was to identify vancomycin-resistant enterococcus (VRE) colonization rate in patients admitted to the intensive care unit (ICU), associated risk factors and clinical outcomes for VRE colonization. Methods: Of the 7,703 patients admitted to the ICUs between January, 2008 and December, 2010, medical records of 554 VRE colonized and 503 uncolonized patients were reviewed retrospectively. To analyzed the impact of colonization on patients' clinical outcomes, 199 VRE colonized patients were matched with 199 uncolonized patients using a propensity score matching method. Results: During the study period, 567 (7.2%) of the 7,703 patients were colonized with VRE. Multivariate analysis identified the following independent risk factors for VRE colonization: use of antibiotics (odds ratio [OR]=3.33), having bedsores (OR=2.92), having invasive devices (OR=2.29), methicillin-resistant Staphylococcus aureus co-colonization (OR=1.84), and previous hospitalization (OR=1.74). VRE colonized patients were more likely to have infectious diseases than uncolonized patients. VRE colonization was associated with prolonged hospitalization and higher mortality. Conclusion: Strict infection control program including preemptive isolation for high-risk group may be helpful. Further research needs to be done to investigate the effects of active surveillance program on the incidence of colonization or infection with VRE in the ICU.

Distribution Characteristics and Antibiotics Resistance of Enterococcus spp. in Nakdong River (낙동강에서 엔테로코커스의 분포특성 및 항생제별 내성률)

  • This study was it conducted on effluent or river water close to discharge locations from a treatment plant, which were analyzed for the presence, phenotype and antibiotic resistance rates of Vancomycin Resistant Enterococci (VRE). The test results for isolation and identification of Enterococcus spp. showed that they all were VRE positive, with a total of 32 strains detected. Multiplex PCR was conducted for 32 strains, each of which were identified as E. faecium, and the results indicated that they were all confirmed as VRE, corresponding to the Van A phenotype. The results of E. faecium concentrations measured at various locations indicated that they were, on average, higher at the location of sewage treatment plants. The frequency of positive tests as well as the number of colonies was higher downstream of treatment plants. The minimum inhibitory concentration was inspected for 26 strains of discharged water samples from sewage water treatment plants, and 6 strains of river samples. Out of 19 antibiotics, 14 and 5 species showed resistance and sensitivity, respectively.

  • One hundred and three clinical isolates of enterococci were examined for susceptibility to 8 antibiotics, and transferability and transfer frequency of R plasmid. Ampicillin was the most active, followed in decreasing order by rifampin, amikacin and chloramphenicol, and tetracycline. High-level resistance(${\geq}2,000{\mu}g/ml$) to kanamycin, streptomycin, and gentamicin, known as the most active of the aminoglycosides to enterococcus, was present in 26.2%, 21.4%, and 18.3% of the isolates, respectively. In the drug susceptibility of the species, S. zymogenes was the most resistant and S. durans was the most sensitive to tested antibiotics. We could observed the transferability of enterococcal R plasmid in mixed culture: among the 28 strains which showed multiple drug resistance, 17 strains transferred all or part of their resistance with $2{\times}10^{-4}-2{\times}10^{-6}%$ of transfer frequency to a plasmid-free recipient, S. faecalis strain JH 2-2.

Bacterial Distributions of Escherichia coli and Bacillus cereus etc. Isolated from Dried Seasoned Marine Products in Garak Fishery Wholesale Market in Seoul, 2009 (그대로 섭취하는 수산가공식품 중 조미건어포류에 대한 주요 식중독균류 분포(2009))

  • This study was practiced to survey food-borne bacterial aspects of Escherichia coli and Bacilhus cereus etc. from 210 processed seasoned marine products in Garak fishery wholesale market in Seoul, 2009. Distributions of these bacterial isolates were 28 coliforms, 32 Bacillus spp., 71 Staphylococcus spp., 11 Listeria spp, and 10 Enterococcus spp. in 157 dried squids, 23 dried file fishes, and 20 dried pollacks etc. respectively. Results in 16 kinds of antibiotics susceptibility test by disc diffusion method, special multiple drugs resistance patterns were NOR + LVX + CIP + SAM + VA + S + TE + CF, SAM + C + VA, VA + S + TE, and VA + S in 9 E. coli strains, also, AMC + SAM + CF, SAM + CF, and VA + CF in 21 Bacillus cereus strains respectively. On the basis of the results above, many seasoned dried fishes had multiple drugs resistances, conclusively, we suggest limited guideline and special management on use of antibiotics in floating net cages of fishery farms.
