Brain Perfusion SPECT Imaging in Sturge-Weber Syndrome: Comparison with MR Imaging (Sturge-Weber 증후군의 뇌관류 SPECT 영상: 자기공명영상과의 비교)

  • The purpose of this study was to evaluate the characteristic perfusion changes in patients with Sturge-Weber syndrome by comparison of the findings of brain MR images and perfusion SPECT images. $^{99m}Tc$-HMPAO or $^{99m}Tc$-ECD interictal brain SPECTs were performed on 5 pediatric patients with Sturge-Weber syndrome within 2 weeks after MR imaging. Brain SPECTs of three patients without calcification showed diminished perfusion in the affected area on MR image. A 3 month-old patient without brain atrophy or calcification demonstrated paradoxical hyperperfusion in the affected hemisphere, and follow-up perfusion SPECT revealed decreased perfusion in the same area. The other patient with advanced calcified lesion and atrophy on MR image showed diffusely decreased perfusion in the affected hemisphere, but a focal area of increased perfusion was also noted in the ipsilateral temporal lobe on SPECT. In conclusion, brain perfusion of the affected area of Sturge-Weber syndrome patients was usually diminished, but early or advanced patients may show paradoxical diffuse or focal hyperperfusion in the affected hemisphere. Further studies are needed for better understanding of these perfusion changes and pathophysiology of Sturge-Weber syndrome.

A Method to Quantify Breast MRI for Predicting Tumor Invasion in Patients with Preoperative Biopsy- Proven Ductal Carcinoma in Situ (DCIS) (유방 자기공명영상법을 이용한 수술 전 관상피내암으로 진단된 환자의 침윤성 유방암을 예측하는 정량적 분석법)

  • Purpose : To determine the quantitative parameters of breast MRI that predict tumor invasion in biopsy-proven DCIS. Materials and Methods: From January 2009 to March 2010, 42 MRI examinations of 41 patients with biopsy-proven DCIS were included. The quantitative parameters, which include the initial percentage enhancement ($E_1$), peak percentage enhancement ($E_{peak}$), time to peak enhancement (TTP), signal enhancement ratio (SER), arterial enhancement fraction (AEF), apparent diffusion coefficient (ADC) value, long diameter and the volume of the lesion, were calculated as parameters that might predict invasion. Univariate and multivariate analyses were used to identify the parameters associated with invasion. Results: Out of 42 lesions, 23 lesions were confirmed to be invasive ductal carcinoma (IDC) and 19 lesions were confirmed to be pure DCIS. Tumor size (p = 0.003; $6.5{\pm}3.2$ cm vs. $3.6{\pm}2.6$ cm, respectively) and SER (p = 0.036; $1.1{\pm}0.3$ vs. $0.9{\pm}0.3$, respectively) showed statistically significant high in IDC. In contrast, E1, Epeak, TTP, ADC, AEF and volume of the lesion were not statistically significant. Tumor size and SER had statistically significant associations with invasion, with an odds ratio of 1.04 and 22.93, respectively. Conclusion: Of quantitative parameters analyzed, SER and the long diameter of the lesion could be specific parameter for predicting invasion in the biopsy-proven DCIS.

Analysis of the Rate of Discrepancy between Preliminary Reports by Radiology Residents and Final Reports by Certified Radiologists for Emergency Radiology: Studies in a University Hospital (대학병원 응급 영상검사에서 영상의학과 전공의 가판독과 전문의 최종 판독 간의 불일치 발생률 분석)

  • Purpose In the adult emergency department of a university hospital, we investigated the frequency of major discrepancies between the preliminary reports by radiology residents and the final reports by certified radiologists. Materials and Methods Based on CT and MRI scans obtained between December 2016 and November 2019, we selected cases with diagnoses or treatment plans that could be changed due to discrepancies between preliminary and final reports and classified them by the type of discrepancy. We also examined the distributions of the major discrepancies and stratified them by residents' working time zone, experience, and subspecialty. Results Based on the 72137 preliminary reports evaluated, 1348 tests (1.9%) showed major discrepancies. Most of the major discrepancies were false negatives (72.0%), followed by misdiagnosis (26.3%) and false positives (1.7%). Acute findings (87.2%) were more common than non-acute findings (12.8%). The major discrepancy rate increased toward the second half of the 24-hour shift, with the highest rate of 2.9% occurring between 2 am and 4 am. The major discrepancy rate did not vary with experience, and it varied from 0.6% to 4.5% for each subspecialty. Conclusion The major discrepancy rate was less than 2%, and it increased with longer working hours during a 24-hour shift.

Relationship between disk displacement of temporomandibular joint and dentofacial asymmetry (측두하악관절 원판 변위와 치열 및 안면부 비대칭의 관계에 대한 연구)

  • The purpose of this study was to determine whether there is an association between disk displacement of the Temporomandibular Joint (TMJ) and dentofacial asymmetry In orthodontic patients. The subjects consisted of 60 female orthodontic patients between the ages of 18 and 38 years (mean age 23.3 years) who had visited the Department of Orthodontics at Seoul National University Dental Hospital from January 2000 to April 2002. On the basis of magnetic resonance imaging (MRI) of their bilateral TMJs, the subjects were divided Into four groups'. bilateral normal group (twenty-one persons); disk displacement of right TMJ group (six persons); disk displacement of left TMJ group (nine persons); and disk displacement of both TMJs group (twenty-four persons) Postero-anterior (PA) cephalograms and diagnostic models which had been taken before orthodontic treatment were measured. In the linear measurements, a line connecting the right and left Latero-Orbitale (Lo) represented the horizontal reference line (H). The vertical reference line (V) was constructed as a line bisecting and running perpendicular to H. One-way analysis of variance (ANOVA) was used to test whether the mean values of measurements between groups were significantly different. In addition, Bonferronil's multiple comparison test was performed at a level of 0.05. The results were as follows; 1 In the diagnostic model analysis, the overjet, nght molar relationship, and left molar relationship were significantly different among the four groups. 2. In the PA cephalometric analysis, differences in the right and left vertical position of the lower first molar and Ag were significantly dissimilar among the four groups. 3. If the disk displacement of TMJ was present on one side, the ipsilateral ramus was shorter, resulting in asymmetry in the vertical position of Ag. This study indicated that dentofacial asymmetry might be related to the disk displacement of TMJ.

Atypical Presentation of Chronic Granulomatous Disease in a Neonate with a Pulmonary Granuloma Mimicking a Tumor: A Case Report (신생아에서 종양으로 오인되는 폐 육아종의 비전형적인 소견을 보인 만성 육아종성 질환: 증례 보고)

  • Chronic granulomatous disease (CGD) is an uncommon primary immune deficiency caused by phagocytes defective in oxygen metabolite production. It results in recurrent bacterial or fungal infections. Herein, we present a case of CGD with a large pulmonary granuloma in a neonate and review the imaging findings. The patient was a 24-day-old neonate admitted to the hospital with fever. A round opacified lesion was identified on the chest radiograph. Subsequent CT and MRI revealed a round mass with heterogeneous enhancement in the right lower lobe. There were foci of diffusion restriction in the mass. Surgical biopsy of the mass revealed chronic granuloma. Finally, the neonate was diagnosed with CGD caused by mutation of the gp91phox gene. Herein, we present the clinical and imaging findings of this unusual case of CGD.

  • Neurogenic tumors are common in posterior mediastinal tumors. In most cases, tumors were accidentally showed on simple chest X-ray. In some cases, they were presented by symptoms which were induced by nerve compression or airway compression. But as in our case, neurogenic tumor with spontaneous hemothorax is very rare. A 45-year-old man admitted to emergency room of other hospital because of acute right chest pain and dyspnea. A chest X-ray showed a right pleural effusion. Hemothorax was diagnosed after closed thoracostomy. Following chest CT showed posterior mediastinal mass. The patient was transferred to our hospital. T spine MRI showed dumbbell shaped mass. Diagostic impression was neurogenic tumor. The pathologic result was neurilemmoma after surgical resection.

  • Although most cases of posterior reversible encephalopathy syndrome (PRES) are reversible, irreversible lesions as a form of hemorrhage or infarction have been described. PRES as a complication of systemic lupus erythematosus (PRES-SLE) is associated with hypertension or use of immunosuppressive agents. We present a case of recurrent atypical PRES-SLE, which showed restricted diffusion in the first manifestation of SLE, resulted in parenchymal hemorrhagic transformations in the recurrent episode.

  • Objective : This study was designed to find out the effect of scolopendrid aquacupuncture on low back pain with or without sciatica showing high intensity zone of magnetic resonance imaging. Methods : The 30 patients who had a diagnosis of high intensity zone by lumbar-MRI and admitted to Gwangju oriental medical hospital in wonkwang university from January 2005 to August 2004 were observed. The symptom of inpatients is low back pain with or without sciatica. We treated 30 patients by scolopendrid aquacupuncture besides the general conservative treatment of oriental medicine. Results and Conclusion : The scolopendrid aquacupuncture treatment led to improvement in the pain and symptom of disability as determined by all efficacy measures. After scolopendrid aquacupuncture treatment, there was improvement in VAS, ROM and SLRT. This results suggest that scolopendrid aquacupuncture is good method for treatment of low back pain with or without sciatica showing high intensity zone of magnetic resonance imaging.

  • Magnetic resonance imaging has a potential to produce clear anatomical as well as functional images of human body. However, the ability to diagnose is limited by signal to noise ratio (SNR) and the resolution of current medical systems. To remove the challenges prevalent due to the use of high field scanners, dedicated radio frequency coils are used. Transverse electromagnetic coils have an advantage of providing homogeneous magnetic field throughout the region but with low signal to noise ratio while surface coils have an advantage of providing higher signal to noise ratio but with low homogeneity. This research combines both the advantage into one by utilizing transmit only transverse electromagnetic radio frequency coils (8 channel) along with receive only surface coils (by varying the number) for better imaging of brain. A 7 Tesla 32-channel close fitting helmet shaped phased-array surface coils along with the combination of 8 channel transmit only transverse electromagnetic coils provided good homogeneity as well as significant SNR improvements throughout the human brain.

Analysis of Magnetic Resonance Characteristics and Images of Korean Red Ginseng (홍삼의 자기공명 특성과 영상 분석)

  • In this study, the feasibility of magnetic resonance techniques for nondestructive internal quality evaluation of Korean red ginseng was examined. Relaxation time constants were measured using various grades of red ginsengs. Solid state magnetic resonance imaging technique was applied to image dried red ginsengs which have low moisture contents (about 13%). A 7 tesla magnetic resonance imaging system operating at a proton resonant frequency of 300 ㎒ was used for acquiring MR images of dried Korean red ginseng. The comparison test of cross cut digital images and magnetic resonance images of heaven grade, good grade with cavity inside, and good grade with white part inside red ginseng suggested the feasibility of the internal quality evaluation of Korean red ginsengs using MRI techniques. A good grade red ginseng included abnormal tissues such as cavities or white parts inside was observed by the signal intensity of MR image based on magnetic resonance properties of proton nucleus. Analysis on an one dimensional profile of acquired MR image of Korean red ginseng showed easy discrimination of normal and abnormal tissues. MR techniques suggested ways to detect internal defects of red ginsengs effectively.
