Duration of Abstinence after Discharge from Alcoholism Inpatient Treatment - Comparing Voluntarily vs. Involuntarily Admitted Patients - (알코올중독자의 입원동의 여부에 따른 퇴원후 단주기간에 관한 연구)

  • Motivation has been identified as an important factor predicting long-term outcomes of alcohol abuse treatment. Whether a patient agreed on his/her inpatient treatment at time of hospitalization could be an indicator of their motivation for treatment. This study focused on this aspect of treatment motivation and examined whether this factor predicts post-discharge abstinence. A sample of 145 individuals who were hospitalized for alcohol abuse treatment participated in the baseline data collection, of which only 66 successfully completed the 8 month follow-up telephone interview. The findings of survival analysis suggest that voluntarily admitted individuals had significantly longer duration of post-discharge abstinence compared to their counterparts. A Cox proportional hazards regression model revealed that voluntary admission and family support were significant predictors for post-discharge relapse of drinking, after controlling for the effects of marital status, living alone, and working status. Implications for social work practice are discussed.

A Study on the Nutritional Status in Hospitalized Patients with Pulmonary Tuberculosis (폐결핵 환자의 입원기간동안 영양상태 변화 연구)

  • This study was conducted to evaluate the change of nutritional status and to analyze related factors in hospitalized tuberculosis (TB) patients during their hospitalization. The subjects were 398 men patients (mean age: 47.3 ${\pm}$ 14.4 y) who had hospitalized more than 3 months at TB hospital located in Seoul. The anthropometric and blood biochemical indices were measured, and dietary intakes were assessed. At the time of admission the body weight of subjects was about 76% of the average body weight of Korean men with same age, Body mass index (BMI) of subjects was 18.5 kg/$m^2$, and 53.8% of subjects were under weight status. Average level of blood hemoglobin and hematocrit of subjects was lower than the normal value. After 3 months of hospitalization period, the body weight and body mass index were significantly increased compared to admission by 3.9 kg (7.41%) and 1.4 kg/$m^2$ (7.61%)(p < 0.001), respectively. Blood levels of hemoglobin, hematocrit, albumin, and total protein were also significantly increased after 3 months of hospitalization period compared to admission (p < 0.001). The increment in the body weight and blood indices was significantly higher in below 29 years group than over 60 years group (p < 0.05). The increment in the body weight and body mass index was significantly higher in the under-body weight group compared to the normal-body weight group (p < 0.05). In conclusion the body weight and body mass index of subjects were significantly increased after 3 months of hospitalization period, and the age and body weight of subjects at admission were supposed to influence the degree of change in the nutritional status.

The Effectiveness of Antibiotics in Acute Diarrhea of Children (항생제가 소아의 급성 설사에 미치는 영향)

  • The beneficial effects of antibiotics in acute diarrhea of children were analysed 10 assess their effectiveness since many physicians use them to treat acute diarrhea. Searching 124 charts of patients admitted from August 1996 to July 1997, they were divided into two groups, an antibiotic negative (N) group of 28 patients and a positive (P) group of 96 patients. The admission period was significantly shorter in N group than P group (P=0.0057). Duration of fever and diarrhea were also shorter in N group (P=0.0328 and 0.0184 respectively). Although it was not statistically significant. duration of vomit and frequency of diarrheal episodes per day were lower in N group (P=0.7091 and 0.061, respectively). Age, sex, duration of diarrhea before the admission and CRP collected on the day of admission were not statistically different between the two groups (P=0.5866, 0.093, 0.3502 and 0.2628 respectively).

Medication use as a Risk Factor for Falls in Hospitalized Elderly Patients in Korea (입원 노인환자의 의약품 사용과 낙상위험도 연구)

  • 낙상은 노인의 건강을 위협하는 가장 심각한 문제 중의 하나이며, 조기사망, 신체손상, 운동장애, 심리학적 기능장애를 유발하는 원인이기도 하다. 본 연구의 목적은 국내 노인전문 요양병원 입원 환자들의 의약품 사용이 낙상에 미치는 영향을 평가하는데 있다. 후향적으로 원내 의무기록 정보를 이용하여 환자-대조군 연구를 수행하였고, 2008년 1월부터 2010년 12월까지 3년 기간에 입원한 65세 이상을 대상으로 하였다. 입원기간 중 낙상을 경험한 34명의 노인환자들을 환자군으로 선택하였으며 낙상을 경험하지 않은 68명의 노인환자들을 무작위 추출하여 1:2의 환자군:대조군비율로 연구 대상 환자들을 선정하였다. 환자군이 복용한 의약품을 대조군이 복용한 의약품과 비교하였으며 각 계열별 의약품과 낙상위험도 관계를 평가하였다. 두 그룹간의 인구통계학적 특성은 유사하였고 연령, 성별, 복용 의약품수, 고혈압 유무, 혈중 크레아티닌 수치, 혈중 나트륨 수치, 혈압 또는 심박수에 유의한 차이는 없었다. 항히스타민제와 본 연구에서 유일하게 기타 수면보조제로 분류된 졸피뎀이 유의하게 낙상위험도를 증가시켰다.

  • The purpose of this study is to understand and describe the hospitalization stress experience of patients with schizophrenia. The subjects of this study were 15 inpatients who were hospitalized in three psychiatric hospital closure wards. As a result of the study, the contents of hospitalized stress were classified into five categories, 17 sub-categories and 58 meaningful contents. The five top categories were 'missing and guilt for family', 'discomfort due to poor therapeutic environment', 'not respected of rights Patient's, 'anxiety about life after discharge', 'social prejudice and self-perceived stigma. This study has significant implications for the provision of basic data to mediate hospitalization stress of patients with schizophrenia.

Patient Distribution and Hospital Admission Costs in Neonatal Intensive Care Units: Collective Study of 7 Hospitals in Korea during 2006 (2006년도 전국 7개 병원 신생아중환자실 입원 현황 및 입원비용 분석)

  • Purpose: The characteristics of hospitalized patients in neonatal intensive care units (NICUs), including hospitalization costs (HC) and National Health Insurance (NHI) status were studied. Methods: We gathered the following data from 7 hospitals in Korea during 2006: the distribution of patients according to birth weight (BW), and the duration of the hospital stay according to BW and He. Results: The patients who were admitted to the NICU consisted of high-risk neonates, including low birth weight or premature neonates, which comprised 50% of all neonates admitted to the NICU. The duration of hospitalization was 75-90 days for neonates with BW <1,000 g, 45-60 days for neonates with BW between 1,000 and 1,499 g, and approximately 15 days for neonates with BW between 2,000 and 2,499 g. The portion of the HC covered by the NHI was 77.1%, 22.9% of the total HC was not covered by the NHI (19.5% was included in the list, but not covered by the NHI and 3.4% was not listed, but covered by the NHI). The average total HC per person was 4,360,000 won, and the HC covered and not covered by the NHI were 3,677,000 won and 1,007,000 won, respectively. The mean HC were as follows; 35,000,000 won for a BW <500 g, 18,000,000 won for a BW between 500 and 999 g, 16,000,000 won for a BW between 1,000 and 1,499 g, and 4,200,000 won for a BW between 1,500 and 1,999 g. Conclusion: Not only premature, but also ill neonates were under the care of the NICU. The HC increased as the BW decreased and the hospitalization period increased. The proportion of the patient's financial burden is >25% of the total He. For this matter, additional NHI is needed.

Clinical factors causing hyperbilirubinemia in patients with Kawasaki disease (가와사끼병에서 고빌리루빈혈증의 관련인자에 대한 임상적 고찰)

  • Purpose : Hyperbilirubinemia, jaundice and gallbladder hydrops are unusual manifestations of Kawasaki disease (KD). In this case, abdominal pain, anorexia and abdominal distension may follow eventfully. We reviewed the clinical and laboratory data to investigate the causative factors of hyperbilirubinemia in patients with KD. Methods : Two-hundred eighty two children diagnosed and hospitalized with KD were identified by searching patients' charts. Cases were included in the study if diagnosed between January 1997 and December 2004. We reviewed clinical data, age, sex, duration from the onset of fever to admission and duration from start of treatment to defervescence. Clinical and laboratory data were compared between normal (A) and hyperbilirubinemia (B) groups. Results : Thirteen patients (4.6 percent) showed hyperbilirubinemia. In the hyperbilirubinemia group, age and initial ALT value were higher than group A (P=0.003, 0.018 respectively). Duration from the onset of fever to admission and age were relative risk factors in hyperbilirubinemia (P=0.007, 0.003 respectively) in patients with KD. Conclusion : Shorter duration from the onset of fever to admission and older age group were relative risk factors of hyperbilirubinemia in patients with KD.

Comparison of Rehospitalization during the First Year of Life in Normal and Low Birth Weight Infants Discharged from NICU (신생아 집중치료실에서 퇴원한 정상 체중아와 저출생 체중아의 재입원에 관한 비교)

  • Purpose : Although the short- and long-term outcomes of low birth weight(LBW) neonatal intensive care unit(NICU) survivors have been extensively studied, much less information is available for normal birth weight(NBW) infants(greater than 2,500 gm) who require NICU care. Methods : We retrospectively examined the neonatal hospitalizations and one year health status of 302 NBW and 131 LBW admissions to our NICU. Information on the neonatal hospitalization was obtained from a review of medical records. Postdischarge health status was collected by using telephone surveys and medical records. Results : After initial discharge, 21.2% of the NBW infants and 23% of the LBW infants required rehospitalization during the first year of life and there was no significant difference between the two groups. The reasons for rehospitalization of the NBW infants included respiratory disorders (32.1%), G-I problems(26.2%), genitourinary problems(11.9%), surgery(10.7%), cardiac problems(7.1%), and congenital/developmental problems(1.2%). For the LBW infants, the order of frequency was the same, with the percentages slightly different. Neonatal risk factors related to the rehospitalization of the NBW infants included mechanical ventilation, duration of mechanical ventilation, and congenital anomaly. But no positive significant correlation of neonatal risk factors with rehospitalization of LBW infants was found. Conclusion : Low and normal birthweight NICU survivors were rehospitalized at similar rates. The most common cause of rehospitalization was respiratory problems. Neonatal risk factors related to rehospitalization of NBW infants were mechanical ventilation, duration of mechanical ventilation, and congenital anomaly. However, no positive significant correlation of neonatal risk factors with rehospitalization of LBW infants was found. The data suggests that NBW infant survivors, as well as LBW infant NICU survivors, require close follow up.

Risk Factors of Nosocomial Rotavirus Infections in Children (소아 로타바이러스 원내 감염의 위험 인자)

  • Purpose :Rotavirus is one of the most important causes of nosocomial infections among children. The aim of this study is to determine the risk of nosocomial rotavirus infections and to evaluate the effectiveness of breast-feeding and probiotics in the prevention of nosocomial rotavirus infections. Methods : This study was carried out on admitted children without diarrhea between March 1, 2003 and February 29, 2004. Three hundred ninety patients aged 4 days to 13 years during this study were available. We examined the feces of all children for rotavirus by latex agglutination on admission, during hospital, and after discharge, to see whether they developed diarrhea or not. Results : Nosocomial rotavirus infections was significantly increased with children under 12 months of age(P=0.008). The monthly attack rate was great between December and March(P=0.046). Prolonged hospital stay was associated with an increased attack rate of nosocomial rotavirus infections (P=0.003). The risk of nosocomial rotavirus infections was not associated with the number of roommates and whether or not they were breast-fed or fed on probiotics. Conclusion : Nosocomial rotavirus infections are significantly more likely to occur in children under 12 months of age, admitted between December and March, and with prolonged hospital stays. Prompt identification and isolation of children with nosocomial rotavirus infections, even without diarrhea, may decrease rates of nosocomial rotavirus infections.
